Treatment of chronic prostatitis is a vital and complicated problem, in which a large number of stamps and "stereotyped" approaches often result in uncured patients. The increasing use of intracellular microorganisms in prostatitis etiology requires a modification in the standard approaches. TAURUS study shows high efficacy of doxycycline (Unidox Solutab®) and/or josamycin (Wilprafen®) in chronic prostatitis. Therapy, studied in this program, according to physicians, was effective in 93.2% of patients. Treatment failure was observed in 1.3% of all patients, another 5.5% of patients had insufficient data for assessment. Low incidence of adverse reactions was observed. In the study population, adverse reactions occurred in 2.6% of patients, of them serious adverse events were registered in 0.7% of patients. The most common adverse event in all treatment groups was diarrhea.

High-frequency intravesical electroneurostimulation was made simultaneously with magnetophoresis of the mixture based on oxibutinin. Compared to effects of oral intake of oxibutinin, such method was two times more effective in relation to overactive bladder symptoms relief in shorter duration of the treatment and reduced side effects. Laser Doppler flowmetry discovered that the above combined treatment corrects microcirculation in bladder mucosa.

Combined use of dynamic magnetotherapy of local and central action in combination with antibacterial endourethral therapy and rectal administration of vitaprost arrests symptoms of chronic prostatitis. Combination of local and total dynamic magnetotherapy with application of AMUS-01-INTRAMAG unit improves erection quality in 81.8% males with psychogenic form of erectile dysfunction. There was also improvement of spermogram parameters and relieve of asthenovegetative syndrome.

Aim: To evaluate effectiveness and safety of terazosin vs tamsulosin in patients with benign prostatic hyperplasia (BPH); to elucidate effectiveness of the use of tamsulosin followed by terazosin and vice versa., Material and Methods: Group 1 of BPH patients received terazosin (5 mg/day) followed in a month by tamsulosin (0.4 mg/day). Group 2 received the same drug in the opposite sequence. The trial included 60 patients with symptoms of the lower urinary tracts due to BPH. The results were assessed on the treatment day 5-7, 14, 30 and 60 one month after the treatment termination., Results: Terazosin and tamsulosin relieved clinical symptoms, reduced QOL and Qmax. One month after the treatment the symptoms partially returned to baseline, Qmax returned completely. In cross-over, positive trends were weaker., Conclusion: Clinical efficiency of both drugs was comparable, there was no cross effect. A month interval between the treatments in this case was insufficient. This should be taken into account when planning further studies.

Aim: As baseline urodynamic parameters do not correlate well with response to medical treatment for lower urinary tract symptoms (LUTS), this study was designed to asertain whether urodynamic parameters correlate better with response to alpha 1-adrenoblockers., Methods: 23 men (mean age 64.6 +/- 9.3 years) with LUTS have undergone urodynamic examination prior to initiating therapy with terazosin titrated to 5 mg and followed for 2.5 months. Parameters of evaluation included IPSS symptom score, peak flow rate (Qmax), maximal detrusor pressure (Pmax), detrusor pressure at maximal flow (Pdet) and detrusor contraction duration (DCD). The correlation and predictive value of therapeutic response and baseline urodynamic parameters were assessed., Results: For the entire group, IPSS decreased from 13.25 +/- 3.39 to 7.38 +/- 3.21 (44.6%), Qmax increased from 9.75 +/- 1.95 to 11.00 +/- 3.23 ml/s (21%), Pmax decreased from 109.75 +/- 35.72 to 93.88 +/- 20.91 cm. H2O (14.4%), Pdet decreased from 84.00 +/- 23.71 to 74.00 +/- 15.53 cm. H2O (16.8%), DCD increased from 127.63 +/- 53.99 to 130.13 +/- 45.41s (2.7%). There was a weak correlation between either Pmax or Pdet and therapeutic response (defined as a IPSS decrease of 40% and an increase in Qmax of 30%). There was a significant correlation with therapeutic response of DCD > 60 s (p < 0.05, r = 0.56)., Conclusion: The pilot data suggest that DCD may be a useful urodynamic parameter to predict and optimize therapy with alpha 1-adrenoblockers.
