Abdominal tuberculosis is defined as infection of the peritoneum, hollow or solid abdominal organs with Mycobacterium tuberculosis. The peritoneum and the ileocaecal region are the most likely sites of infection and are involved in the majority of the cases by haematogenous spread or through swallowing of infected sputum from primary pulmonary tuberculosis. Pulmonary tuberculosis is apparent in less than half of the patients. Patients usually present with abdominal pain, and the cause is usually identified through a combination of radiologic, endoscopic, microbiologic, histologic and molecular techniques. Anti-microbial treatment is the same as for pulmonary tuberculosis. Surgery is occasionally required.

Supernumerary breast is an uncommon disease seen in Asian population at an incidence of 2–6 % of general population. The pathology despite being present since birth remains asymptomatic till hormonal stimulation viz. puberty, pregnancy, and lactation. The management for such benign ectopic breast tissue is excision. However, one should always rule out malignancy in such lumps.

We present the case of an 18 year old female with tuberculous mesenteric lymphadenopathy who came to hospital with peritonitis and acute mesenteric ischemia. An emergency exploratory laparotomy with resection of gangrenous small bowel and right colon with end-to-end anastomosis was performed. Subsequent evaluation for etiology of bowel gangrene was essentially normal. By exclusion, the most likely cause of acute mesenteric ischemia in this patient was of unknown etiology which is indeed rare. However, tuberculosis as a cause of mesenteric vasculitis and bowel gangrene needs to be explored.

MPNST is a rare soft tissue neoplasm that accounts for less than 10 % of all soft tissue sarcomas. These tumors often affect males between 20 to 50 years, often with NF1, These with predilection for buttocks, thigh, brachial plexus, upper arm and paraspinal region. MPNST spread perineurally or hematogenously to lungs. Lymph node metastasis is seen in less than 5% cases of all sarcomas. This is a case of 46 year old male with NF-1 with MPNST posterior abdominal wall. The patient had metastatic pleural effusion in the absence of lung metastasis, along with widespread axillary, retroperitoneal and inguinal lymph node metastasis. Thus, although lymph node metastasis from MPNST is rare, this case expands our knowledge of the clinical behavior of MPNST.
