Abstract

Current techniques and tools for automated termination analysis of term rewrite systems (TRSs) are already very powerful. However, they fail for algorithms whose termination is essentially due to an inductive argument. Therefore, we show how to couple the dependency pair method for TRS termination with inductive theorem proving. As confirmed by the implementation of our new approach in the tool AProVE, now TRS termination techniques are also successful on this important class of algorithms.

7. Clinician-initiated written communication for families of patients at a long-term acute care hospital.

Abstract

Objective: To assess the experience of families and clinicians at a long term acute care hospital (LTACH) after implementing a written communication intervention., Methods: Written communication templates were developed for six clinical disciplines. LTACH clinicians used templates to describe the condition of 30 mechanically ventilated patients at up to three time points. Completed templates were the basis for written summaries that were sent to families. Impressions of the intervention among families ( n  = 21) and clinicians ( n  = 17) were assessed using a descriptive correlational design. Interviews were analyzed using thematic content analysis., Results: We identified four themes during interviews with families: Written summaries 1) facilitated communication with LTACH staff, 2) reduced stress related to COVID-19 visitor restrictions, 3) facilitated understanding of the patient condition, prognosis, and goals and 4) facilitated communication among family members. Although clinicians understood why families would appreciate written material, they did not feel that the intervention addressed their main challenge - overly optimistic expectations for patient recovery among families., Conclusion: Written communication positively affected the experience of families of LTACH patients, but was less useful for clinicians., Innovation: Use of written patient care updates helps LTACH clinicians initiate communication with families., Competing Interests: No author has a relevant competing interest that might lead to bias., (© 2023 The Authors.)

Abstract

Purpose: The authors discuss measurement methods and instrumentation useful for the characterization of the gray tracking performance of medical color monitors for diagnostic applications. The authors define gray tracking as the variability in the chromaticity of the gray levels in a color monitor., Methods: The authors present data regarding the capability of color measurement instruments with respect to their abilities to measure a target white point corresponding to the CIE Standard Illuminant D65 at different luminance values within the grayscale palette of a medical display. The authors then discuss evidence of significant differences in performance among color measurement instruments currently available for medical physicists to perform calibrations and image quality checks for the consistent representation of color in medical displays. In addition, the authors introduce two metrics for quantifying grayscale chromaticity consistency of gray tracking., Results: The authors' findings show that there is an order of magnitude difference in the accuracy of field and reference instruments. The gray tracking metrics quantify how close the grayscale chromaticity is to the chromaticity of the full white point (equal amounts of red, green, and blue at maximum level) or to consecutive levels (equal values for red, green, and blue), with a lower value representing an improved grayscale tracking performance. An illustrative example of how to calculate and report the gray tracking performance according to the Task Group definitions is provided., Conclusions: The authors' proposed methodology for characterizing the grayscale degradation in chromaticity for color monitors that can be used to establish standards and procedures aiding in the quality control testing of color displays and color measurement instrumentation.

Abstract

The global epidemic of infection by HIV has created an urgent need for new classes of antiretroviral agents. The potent ability of small interfering (si)RNAs to inhibit the expression of complementary RNA transcripts is being exploited as a new class of therapeutics for a variety of diseases including HIV. Many previous reports have shown that novel RNAi-based anti-HIV/AIDS therapeutic strategies have considerable promise; however, a key obstacle to the successful therapeutic application and clinical translation of siRNAs is efficient delivery. Particularly, considering the safety and efficacy of RNAi-based therapeutics, it is highly desirable to develop a targeted intracellular siRNA delivery approach to specific cell populations or tissues. The HIV-1 gp120 protein, a glycoprotein envelope on the surface of HIV-1, plays an important role in viral entry into CD4 cells. The interaction of gp120 and CD4 that triggers HIV-1 entry and initiates cell fusion has been validated as a clinically relevant anti-viral strategy for drug discovery. Herein, we firstly discuss the selection and identification of 2'-F modified anti-HIV gp120 RNA aptamers. Using a conventional nitrocellulose filter SELEX method, several new aptamers with nanomolar affinity were isolated from a 50 random nt RNA library. In order to successfully obtain bound species with higher affinity, the selection stringency is carefully controlled by adjusting the conditions. The selected aptamers can specifically bind and be rapidly internalized into cells expressing the HIV-1 envelope protein. Additionally, the aptamers alone can neutralize HIV-1 infectivity. Based upon the best aptamer A-1, we also create a novel dual inhibitory function anti-gp120 aptamer-siRNA chimera in which both the aptamer and the siRNA portions have potent anti-HIV activities. Further, we utilize the gp120 aptamer-siRNA chimeras for cell-type specific delivery of the siRNA into HIV-1 infected cells. This dual function chimera shows considerable potential for combining various nucleic acid therapeutic agents (aptamer and siRNA) in suppressing HIV-1 infection, making the aptamer-siRNA chimeras attractive therapeutic candidates for patients failing highly active antiretroviral therapy (HAART).

Abstract

Background/aims: Pregnancy complicated by diabetes is associated with increased risk of unfavorable obstetric outcomes. A common abnormality in diabetes is endothelial dysfunction resulting in an altered pattern of vasoactive substance production by the endothelial cells. The aim of study was to assess serum endothelin-1 (ET-1) and cyclic guanosine monophosphate (cGMP) in pregnant women with pregravid (PGDM) or gestational diabetes (GDM)., Methods: At the time of delivery, serum ET-1, cGMP, glycated hemoglobin (A1c), fructosamine and non-fasting glucose were measured in 19 PGDM, 23 GDM and 18 controls., Results: ET-1 and cGMP were similar in all groups. In GDM there was a positive association between A1c and ET-1 (r = 0.437; p < 0.05) and cGMP (r = 0.542; p < 0.02). In the controls, but not in PGDM and GDM, we found a positive correlation between ET-1 and cGMP (r = 0.634; p < 0.005). In women with diabetes, an optimal (A1c <6%) or inadequate (A1c >6%) metabolic control of diabetes did not influence ET-1 or cGMP levels., Conclusions: In women with PGDM and GDM, serum ET-1 and cGMP were similar to the levels observed in healthy pregnant women. Objectives: The aim of the study was to assess the relationship between maternal and umbilical cord plasma concentrations of endothelin 1 (ET-1) and cyclic guanosine monophosphate (cGMP) in women with diabetes mellitus (DM)., Material and Methods: The study was performed on 19 neonates of women with pregestational DM, 23 neonates of women with gestational diabetes (GDM), and 18 neonates of healthy uncomplicated pregnancies., Results: We found that umbilical and maternal ET-1 and cGMP concentrations in pregestational DM or GDM women were not changed in comparison with the controls. In women without DM, positive correlations between maternal (r=0.64; p<0.005) and umbilical cord plasma (r=0.60; p<0.009) ET-1 and cGMP concentrations were found. However in pregnancies complicated by pregestational DM or GDM such associations were not observed., Conclusions: Umbilical and maternal plasma concentrations of ET-1 and cGMP in pregnant women with pregestational diabetes as well as gestational diabetes were not changed in comparison with the non-diabetic women. In women without DM maternal and umbilical cord plasma ET-1 and cGMP concentrations showed positive correlations. In pregnancies complicated by pregestational DM or GDM maternal and umbilical cord plasma ET-1 and cGMP concentrations were not interdependent.

Abstract

Aortic aneurysms occurs very rarely in young women. They usually accompany congenital heart diseases, Marfan's syndrome, bicuspid aortic valve and coarctation. What is more, they may be the consequence of syphilis and trauma, however less frequently. Despite their rare incidence, aortic aneurysms seem to be an important problem due to high maternal mortality during gestation. Hemodynamic changes which occur in pregnancy favour the appearance of this pathology, deteriorating the prognosis even further--half of the cases of aortic ruptures in women aged below 40 years are associated with pregnancy. Until recently only a few cases of aneurysm repairs during pregnancy have been reported. Also, only one pregnancy and one delivery in a patient after the repair of aortic aneurysm have been described in the literature. In our research we wish to report the successful management of a pregnancy, ended by vaginal route, 8 years after the replacement of descending thoracic aorta with a synthetic graft for aneurysm.

Abstract

Objective: To analyse the relationship between intrapartum fetal heart rate disturbances and electrocardiographic changes and umbilical venous troponin-I (T-I), concentrations as well., Material and Methods: 14 fetuses were continuously surveyed by CTG and ECG recordings in the first and second stage of labor, using STAN S-21 analyser. After birth, umbilical venous blood samples were collected for determination of acid-base balance, base excess and troponin-I concentrations., Results: From among monitored fetuses, in 8 cases the CTG and FECG patterns were correct. Vaginally delivered neonates were born in good clinical status, with normal acid-base balance, base excess and T-I < 0.3 ng/ml. In 3 cases abnormal CTG patterns were observed with early decelerations but FECG patterns were correct. Vaginally delivered neonates were born in good clinical status, with normal acid-base balance, base excess and T-I < 0.3 ng/ml. In 2 cases abnormal CTG patterns were observed with variable decelerations but FECG patterns were correct. Vaginally delivered neonates were born in good clinical status, with normal acid-base balance, base excess and T-I < 0.3 ng/ml. In one case abnormal CTG pattern were observed with late decelerations but FECG patterns was correct. The pregnancy was terminated by caesarean section because of fetal distress. The neonate was born in good clinical status with normal acid-base balance and base excess but T-I concentration was increased (1.5 ng/ml).
