Water Management Digital Knowledge Repository presented in this article is unique in the sense that it helps to preserve the heritage of water profession and facilitates access to the latest information for a wider public. This is particularly important in a country where water-related disasters have not only threatened the people in the past but unfortunately, they need to be prepared for such events in the future, too. Therefore, the digital knowledge repository, based on six pillars (eLearning, Photos, Videos, Journals, Textbooks, Glossary), plays a valuable and important role in Hungary.

In light of the demographic shift towards an aging population, there is an increasing prevalence of dementia among the elderly. The negative impact on mental health is preventing individuals from taking proper care of themselves. For individuals requiring hospital care, those receiving home care, or as a precaution for a specific individual, it is advantageous to utilize monitoring equipment to track their biological parameters on an ongoing basis. This equipment can minimize the risk of serious accidents or severe health hazards. The objective of the present research project is to design an armband with an accurate location tracking system. This is of particular importance for individuals with dementia and Alzheimer’s disease, who frequently leave their homes and are unable to find their way back. The proposed armband also includes a fingerprint identification system that allows only authorized personnel to use it. Furthermore, in hospitals and healthcare facilities the biometric identification system can be used to trace periodic medical or nursing visits. This process improves the reliability and transparency of healthcare. The test results indicate that the armband functions in accordance with the desired design specifications, with performance evaluation of the main features including fall detection, where a hit rate of 100% was obtained, a fingerprint recognition test demonstrating accuracy from 88% to 100% on high-quality samples, and a GPS tracking test determining position with a difference of between 1.8 and 2.1 m. The proposed solution may be of benefit to healthcare professionals, supported housing providers, elderly people as target users, or their family members.

Background/Objectives: In Sjögren’s syndrome, exocrine glands are destructed in an autoimmune-mediated process. Obesity is known to influence a wide range of diseases. This study aimed to examine whether obesity has an impact on the disease course of our patients with Sjögren’s syndrome. Methods: Out of the regularly followed-up patients, 125 were grouped based on their body mass index (BMI). Below a BMI of 25, they were listed as “non-obese” (n = 45), whereas above a BMI of 25, they were categorized as “obese” (n = 80). Demographic, laboratory, and immunological parameters; Sjögren’s syndrome disease activity index; certain extraglandular manifestations; and treatment modalities were compared using biostatistical methods. Results: Among the examined cardiovascular and cerebrovascular co-morbidities, type 2 diabetes and hypertension were significantly more frequent in the obese group. Considering the associated further autoimmune disorders and extraglandular manifestations, in our patients, there were no significant differences between the two groups. Among laboratory parameters, gamma glutamil transferase, alanine transaminase, hemoglobin, hematocrit, lymphocyte rate, triglyceride, and c3 and c4 complement levels were significantly higher in the obese group, while the proportion of rheumatoid factor positivity and the neutrophil granulocyte rate were significantly lower. Immunoglobulin G, A, and M levels did not differ significantly between the two subsets. Obese patients needed steroid therapy significantly less frequently; however, statin therapy was remarkably more frequent in that group. Furthermore, the European League Against Rheumatism (EULAR) Sjögren’s syndrome disease activity index (ESSDAI) was significantly lower in the group of overweight patients. Conclusions: Our results suggest that several immunological parameters of obese patients are more favorable compared to those with normal body weight. Behind that, we might suspect either the beneficial effect of statin therapy and/or the obesity paradox.

This short essay is an introduction to the study of the Mahāmudrātilaka (‘An Ornament of the Great Seal’), an important Buddhist tantric scripture of the Hevajra cycle. The text is a so-called uttaratantra (ancillary scripture) of the Hevajratantra. This cult emerged around ca. 900 CE in Eastern India and quickly rose to a position of prominence. In order to illustrate this point, first I discuss some historical references to the Hevajra cult: a lexicographical work, inscriptions, and testimony in Śaiva exegesis. I then contextualise the Mahāmudrātilaka among the Hevajra ancillary scriptures and share some notes on the purpose of such texts. I argue that such scriptures were meant to update a cult’s ritual and doctrinal palette in order to keep up with developments seen (and thought desirable to have) in rival schools. In the next section, I present the only known Sanskrit manuscript of the Mahāmudrātilaka, Staatsbibliothek zu Berlin, Preußischer Kulturbesitz Orientabteilung Hs. or. 8711, a late Nepalese copy dated 1827 CE, which can be shown to be a copy of a Vorlage dating to 1204 CE. Next, I compare the text transmitted therein to the Tibetan translation, Tōh. 420, and identify them as two recensions. I then proceed to discussing exegesis on the Mahāmudrātilaka, the works of *Gambhīravajra and *Prajñāśrīgupta; these texts are for now available only in their Tibetan translation. I also identify some testimonia of the Mahāmudrātilaka. Using all this evidence, I argue that the text cannot be much earlier than the late 11th century. Next, I present an overview of the text by means of examining selected passages and their most significant features, with special focus on the differences and similarities with the Hevajratantra, the internal references to other scriptures, and the text’s significant parallels with the Vajramālābhidhāna. I argue that the text is unapologetically antinomian and gnostic. In the second half of the paper, styled as an appendix, I select five blocks of verses, which I edit and translate: the first deals with the relationship between initiating master and disciple, the second provides some insights into the attitudes of tantric practitioners towards orthodox Buddhists, the third contains detailed instructions on how to gather the antinomian substances known as ‘nectars’, the fourth deals with communal worship in a rite known as the gaṇacakra, and the final one describes a somewhat rare and rather gruesome ritual meant to bestow the power of flight.

Alterations to intestinal microbiota are assumed to occur in the pathogenesis of inflammatory bowel disease (IBD). This study aims to analyze the association of fecal microbiota composition, body composition, and lipid characteristics in patients with Crohn’s disease (CD). In our cross-sectional study, patients with CD were enrolled and blood and fecal samples were collected. Clinical and endoscopic disease activity and body composition were assessed and laboratory tests were made. Fecal bacterial composition was analyzed using the shotgun method. Microbiota alterations based on obesity, lipid parameters, and disease characteristics were analyzed. In this study, 27 patients with CD were analyzed, of which 37.0% were obese based on visceral fat area (VFA). Beta diversities were higher in non-obese patients (p < 0.001), but relative abundances did not differ. C. innocuum had a higher abundance at a high cholesterol level than Bacillota (p = 0.001, p = 0.0034). Adlercreutzia, B. longum, and Blautia alterations were correlated with triglyceride levels. Higher Clostridia (p = 0.009) and B. schinkii (p = 0.032) and lower Lactobacillus (p = 0.035) were connected to high VFA. Disease activity was coupled with dysbiotic elements. Microbiota alterations in obesity highlight the importance of gut microbiota in diseases with a similar inflammatory background and project therapeutic options.

Virtual commissioning (VC), defined as the simulation and testing of systems in a virtual environment before physical implementation, plays a key role in addressing the challenges of integrating and validating complex systems efficiently and effectively. This paper focuses on the topic of virtual commissioning, summarizing and organizing existing research in the field. The paper provides a comprehensive overview of various design methods and technologies currently in use. A case study of virtual commissioning is also presented within the area of the Cyber-Physical Manufacturing Systems Laboratory of the Széchenyi István University, detailing the solution steps taken. Drawing on both research and practical experience, the paper proposes a novel framework to support virtual commissioning design, referred to as the “Virtual Commissioning House” (VCH). The methodology is evaluated through comparisons with existing virtual commissioning solutions, demonstrating its effectiveness.

Introduction The role of spirituality in health and disease is a complex and emerging area of research. Incorporating spirituality into the bio-psycho-social model of health and disease leading to the bio-psycho-social-spiritual model provides a more comprehensive framework. In this context, chronic disorders like primary Sjögren’s syndrome (pSS) are of interest due to their intricate interactions between biological, psychological, and spiritual factors. Objectives To study possible relationships between spirituality, immune parameters, and disease activity in pSS patients. Methods Patient recruitment for the study took place at the Autoimmune Sjögren specialty clinic, University of Debrecen, resulting in 112 patients. Assessing spirituality of the patients happened through 4 direct questions and the Sprituality Transcendence Scale (24 items). Besides, clinical data of the patients were involved in the study including blood cell counts, rheumatoid factor, immunoglobulin G, Sjögren-specific autoantibodies and disease activity scores (semi-objective and patient reported,). The statistical analysis was conducted applying group comparisons between spiritual and non-spiritual groups, and linear and logistic regression analyses adjusted for sex, age, disease duration, settlement type, education, living in partnership and smoking. Out of the 112 patients 4 gave incomplete response, and therefore got excluded from the analysis, resulting in a total sample size of 108. Results Semi-objective disease activity score (ESSDAI) and perceived vaginal dryness was significantly lower in the non-spiritual group. Spirituality was proven as a significant predictor of anti-SSB autoantibody serum activity and ESSDAI, while engaging in prayer/meditation and its duration predicted significantly anti-SSA autoantibody serum activity, perceived skin and tracheal dryness. Concerning logistic regression analysis, we found that an increase of one unit in spirituality reduces the probability with 81.6% of having a detectable, semi-objective disease activity at all. Significant associations were found between the duration of prayer/meditation and both semi-objective and patient reported disease activity scores and autoantibody anti-SSB with an inverse ratio based on logistic regression model. Conclusions Spirituality is associated with immune parameters and disease activity in pSS. Patients with spiritual attitude are less likely to have increased disease activity. Besides being spiritual, engagement in individual spiritual activities, such as prayer/meditation has beneficial disease modifying effect. These changes are supposedly due to psychoneuroimmunological pathways. In addition to the biologically measurable variables, the alleviation and aggravation of perceived symptoms (e.g. dryness) are important outcomes of spiritual engagement and practice.
