Abstract

Training deep neural networks for 3D segmentation tasks can be challenging, often requiring efficient and effective strategies to improve model performance. In this study, we introduce a novel approach, DeCode, that utilizes label-derived features for model conditioning to support the decoder in the reconstruction process dynamically, aiming to enhance the efficiency of the training process. DeCode focuses on improving 3D segmentation performance through the incorporation of conditioning embedding with learned numerical representation of 3D-label shape features. Specifically, we develop an approach, where conditioning is applied during the training phase to guide the network toward robust segmentation. When labels are not available during inference, our model infers the necessary conditioning embedding directly from the input data, thanks to a feed-forward network learned during the training phase. This approach is tested using synthetic data and cone-beam computed tomography (CBCT) images of teeth. For CBCT, three datasets are used: one publicly available and two in-house. Our results show that DeCode significantly outperforms traditional, unconditioned models in terms of generalization to unseen data, achieving higher accuracy at a reduced computational cost. This work represents the first of its kind to explore conditioning strategies in 3D data segmentation, offering a novel and more efficient method for leveraging annotated data. Abstract

A critical step in the fight against COVID-19, which continues to have a catastrophic impact on peoples lives, is the effective screening of patients presented in the clinics with severe COVID-19 symptoms. Chest radiography is one of the promising screening approaches. Many studies reported detecting COVID-19 in chest X-rays accurately using deep learning. A serious limitation of many published approaches is insufficient attention paid to explaining decisions made by deep learning models. Using explainable artificial intelligence methods, we demonstrate that model decisions may rely on confounding factors rather than medical pathology. After an analysis of potential confounding factors found on chest X-ray images, we propose a novel method to minimise their negative impact. We show that our proposed method is more robust than previous attempts to counter confounding factors such as ECG leads in chest X-rays that often influence model classification decisions. In addition to being robust, our method achieves results comparable to the state-of-the-art. Abstract

The article concerns the problem of lateral stabilization of a tricycle with variable front wheel track. The vehicle can operate in two modes: with the front wheels folded out and connected. A bicycle designed for people with special needs. The element that requires elaboration is the stabilization while driving with the front wheels unfolded. In this mode, a metastable state is created. When the lateral angle of the slope exceeds the limit value, the lateral force causes the lateral tilt. This phenomenon should be treated as a disadvantage as it makes it difficult to smoothly tilt the bicycle, which can lead to disorientation of the rider and difficulty in maintaining balance. The article presents mathematical simulations that allow for the analysis of factors influencing the discussed phenomenon.

11. Changes in the course of constant loading consolidation in soil with modeled granulometric composition contaminated with petroleum substances

Abstract

The effect of contamination with petroleum substances (A1 jet fuel and 15W40 mineral oil) on the compressibility and consolidation of mixtures of ice-dammed clay and aeolian sand was determined in a Barden–Rowe consolidometer adapted for liquid-phase contamination tests. The deterioration of the filtration capacity of the contaminated soil medium relative to the non-contamination condition is reflected by the mobilization of higher values of pore pressure u b and pore water pressure parameter C CL in the course of consolidometer tests. As a consequence, lower values of the compressibility modulus M 0 and the consolidation coefficient c v are obtained in relation to the results of uncontaminated samples. This generally indicates changes in the consolidation parameters of the soil medium in the presence of the tested contaminants. Methodologically, a delay in the onset of the steady-state phase of constant loading (CL) consolidation was observed in contaminated soils. This provides a rationale for programming the tests at lower values of the CL rate. At the same time, the unified model characterization of C CL–T CL allows us to assess the reliable nature of the results. This allows one to recommend CRL (constant rate of loading) tests as a reliable tool for determining the consolidation properties of cohesive soils in the design of soil isolation barriers.

14. Triaxial creep tests of glacitectonically disturbed stiff clay – structural, strength, and slope stability aspects

Abstract

This study concerns the creep impact on strength parameters of the selected very cohesive soils (PI = 30–70%). The analysis refers to Neogene clays characterized by a complex structure, resulting directly from a complicated load history in the geological time scale and identified glacitectonic deformations. In the process of samples’ preparation for strength tests as well as during the interpretation of the post-failure state, particular attention was paid to the soil structure. The imaging methods (X-ray densitometry and computer microtomography) enabled the comparison of the soil structure and the selection of samples with similar characteristics. The completed program of strength tests consisted of two series of tests in the triaxial stress state, differentiated by the occurrence of the initial creep stage, preceding the typical strength test scheme under undrained conditions. This study allowed to obtain a quantitative assessment of the influence of the creep process on the strength parameters of tested soils. Constant stress lower than 60% of the shear stress deviator leads to the deceleration creep course (m parameter 0.64–0.89). As a result, higher values of internal friction angle (20% increase comparing to triaxial tests without creep stage) and cohesion reduction are obtained from triaxial creep tests. Creep parameter m is found to be a valuable indicator for differentiation of landslide activity trend. The tests proved low values of axial strains (1–5%) at failure, which was associated with lithogenesis. By the implementation of obtained strength parameters into the 3D finite element model of the slope, the potential influence of the creep process on the stability of an exemplary cross section of the Warsaw slope could be determined.

Abstract

Background/Objectives. Iron deficiency is one of the most common nutritional deficiencies worldwide and is the leading cause of anemia in the pediatric population (microcytic, hypochromic anemia due to iron deficiency). Moreover, untreated iron deficiency can lead to various systemic consequences and can disrupt the child's development. Methods/Results. Therefore, a team of experts from the Polish Pediatric Society, the Polish Society of Pediatric Oncology and Hematology, the Polish Neonatology Society, and the Polish Society of Family Medicine, based on a review of the current literature, their own clinical experience, and critical discussion, has developed updated guidelines for the diagnosis, prevention, and treatment of iron deficiency in children from birth to 18 years of age. Abstract

This article presents the results of the analysis of data from 500 vehicle control stations concerning periodic tests of passenger cars in the number of 2 million. Based on the collected data, a database of vehicles has been prepared to cover technical information, such as model, mass, engine capacity and maximum power. This information has been summarized with the exploitation data such as age of the car, distance traveled, the area in which the vehicle was most frequently used, the use for business or private purposes, and intensity of operation that was measured by the ratio of kilometers traveled to the age of the car. This information constituted a set of factors affecting the reliability of the car. Information about reliability itself was the results of control tests, including occurring defects and their significance. The occurring defects were codified, which allowed their allocation to particular categories describing car systems, such as steering, braking and driving. While the significance of the fault resulted from the test result: positive (minor defects) or negative (important defects). This article presents a factor and the correlation analysis of the database, which assesses which construction and operating factors have a significant impact on damages occurring in individual car systems.

Abstract

Organic molecules have recently revolutionized ways to create new spintronic devices. Despite intense studies, the statistics of tunneling electrons through organic barriers remains unclear. Here we investigate conductance and shot noise in magnetic tunnel junctions with PTCDA barriers a few nm thick. For junctions in the electron tunneling regime, with magnetoresistance ratios between 10 and 40\%, we observe superpoissonian shot noise. The Fano factor exceeds in 1.5-2 times the maximum values reported for magnetic tunnel junctions with inorganic barriers, indicating spin dependent bunching in tunneling. We explain our main findings in terms of a model which includes tunneling through a two level (or multilevel) system, originated from interfacial bonds of the PTCDA molecules. Our results suggest that interfaces play an important role in the control of shot noise when electrons tunnel through organic barriers.

Abstract

This article presents the results of measurements of the braking efficiency of vehicles adapted to be operated by drivers with motor dysfunctions. In such cars, the braking system is extended with an adaptive device that allows braking with the upper limb. This device applies pressure to the original brake in the car. The braking force and thus its efficiency depend on the mechanical ratio in the adapting device. In addition, braking performance depends on the sensitivity of the car’s original braking system and the maximum force that a disabled person can exert on the handbrake lever. Such a person may have limited power in the upper limbs. The force exerted by the driver can also be influenced by the position of the driver’s seat in relation to the handbrake lever. This article describes the research aimed at understanding the influence of the above-mentioned factors on the car braking performance. As a part of the analysis of the test results, a mathematical function was proposed that allows a parametric description of the braking efficiency index on the basis of data on the braking system, adaptation device, driver’s motor limitations, and the position of the driver’s seat. The information presented in this article can be used for the preliminary selection of adaptive devices to the needs of a given driver with a disability and to the vehicle construction.

Simple Summary: Acute lymphoblastic leukemia (ALL) is the most common childhood malignancy. Survival rates for children with ALL have improved, and today, more than 90% of children with ALL can be successfully treated. Acute pancreatitis (AP), which develops during chemotherapy for ALL can be a serious and fatal complication. It is a widely known toxicity, and several reports indicate that AP occurs with an incidence of 2–10% in patients treated for leukemia. It is mainly, but not exclusively, associated with the administration of asparaginase (ASP), which is one of the key components of multiagent chemotherapy used in ALL. The development of AP usually delays subsequent courses of chemotherapy and is the most common reason for the discontinuation of ASP, thus increasing the potential risk of leukemia relapse in children with ALL. Therefore, both the pathogenesis of AP in ALL and its impact on long-term outcomes remain to be thoroughly investigated. Purpose: This study aimed to identify the risk factors for acute pancreatitis (AP) and its impact on outcomes in Polish children treated for ALL. Methods: The study group included 2303 children receiving intensive chemotherapy for ALL. The group was divided into patients with at least one episode of AP and those who did not develop AP after treatment for ALL. Results: The cumulative incidence of AP in the study group was 4.08%. Older age was an independent risk factor for the development of AP (OR = 1.05; 95%CI = 1.006–1.098; p = 0.03). The overall mortality associated with AP was 2.13%. The probabilities of disease-free survival (p-DFS) and event-free survival (p-EFS) in both subgroups were 0.84 vs. 0.86, log-rank p = 0.65 and 0.75 vs. 0.80, log-rank p = 0.12, respectively. A total of 22 out of 94 patients (23.4%) with AP were re-exposed to asparaginase (ASP) during the subsequent treatment phases. Only one patient re-exposed to ASP (4.5%) developed a second episode of AP. There were no significant differences in p-DFS and p-EFS between patients re-exposed and not re-exposed to asparaginase (0.78 vs. 0.86, log-rank p = 0.27 and 0.63 vs. 0.79, log-rank p = 0.09, respectively). Conclusions: The incidence of AP in children with ALL is low and related to patients' age. The development of AP does not seem to influence p-DFS and p-EFS in children with ALL. Recurrence of AP after re-exposure to asparaginase in patients with ALL and a history of AP is low (4.5%). Abstract

Background: Klebsiella pneumoniae is a nosocomial pathogen that causes severe infections in immunocompromised patients. The aim of the study was to conduct a microbiological and clinical analysis of K. pneumoniae infections in children with malignancies or undergoing hematopoietic cell transplantation in Poland. Methods: We conducted a retrospective, multicenter study including children and adolescents under 19 years old treated between 2012 and 2021. We analyzed patients' characteristics, microbiological data, and the outcomes of antibiotic therapy. Results: A total of 9121 newly diagnosed children were treated for malignancy and 1697 pediatric patients underwent hematopoietic cell transplantation. K. pneumoniae infections were diagnosed in 527 patients. Their overall incidence was 4.86% in pediatric hematology and oncology patients and 4.95% in patients who underwent hematopoietic cell transplantation. The incidence of infection was higher in patients with acute leukemia than with solid tumors (7.8% vs. 4.1%; OR = 2.0; 95% CI = 1.6–2.4; p < 0.0001). The most frequent source of infection was in the urinary tract at 55.2%. More than 57% of K. pneumoniae strains were extended-spectrum β-lactamase-positive and almost 34% were multidrug-resistant. Infections with K. pneumoniae contributed to death in 3.22% of patients. Conclusions: K. pneumoniae is one of the most critical pathogens in children suffering from malignancies or undergoing hematopoietic cell transplantation. Abstract

Formation of varved clays is characterized by specific compressibility and consolidation features, which are difficult to assess. The construction of an expressway through the area of varved, glacilimnic sediments (Vistula glacial period) required careful analysis of the soil reaction to the increasing load caused by growing embankment. The settlement analyses conducted in relation to the schedule of load increase during construction allowed to verify the deformability assessment of the compressible clays. In order to quantify the compressibility and consolidation parameters of clays, an iterative calculation model was created. The method of the “inverse solution” was used to define optimized values of deformability parameters. The observed delayed reaction of the soil to applied load allowed to assess the nature of consolidation. Comparison of the parameters obtained from the model with the results of laboratory and field tests allowed to evaluate drainage characteristics during consolidation of varved clays as well as to introduce correlation coefficients for interpreting compressibility parameters on the basis of CPT tests.

Introduction: Immune thrombocytopaenia (ITP), previously known as idiopathic thrombocytopaenic purpura, is an acquired autoimmune disorder characterised by immune-mediated platelet destruction. It has been observed that some paediatric patients with ITP treated with intravenous immunoglobulin (IVIG) developed neutropaenia. The aim of the study was to investigate the association between IVIG therapy and neutropaenia in paediatric ITP. Material and methods: The retrospective cohort study involved 123 children (79 girls, 44 boys) with immune thrombocytopaenia, aged 8.03 ±4.55 (0.8-17.9) years (mean ± standard deviation; range) who underwent IVIG in the Department of Haematology and Paediatric Oncology in Zabrze between April 2014 and December 2021. Correlations between age, sex, weight of patients, total IVIG dose, and neutrophil/platelet counts on administration day and 1, 2, and 3 days after IVIG treatment from official medical records were analysed. Results: The mean total dose of IVIG was 1.7 ±0.48 g/kg BW A significant increase in platelet level was observed usually on the first (67.5%) or second (20.3%) day after initial administration of intravenous immunoglobulins. After the course of IVIG, neutropaenia was observed in 50 subjects (40.7%). The neutropaenia occurred mostly (54%) on the first day after administration of IVIG. There was a positive correlation between the onset of neutropaenia and lower age of patients (p < 0.05). What is more, the decrease in the neutrophil count was more distinct in the group of subjects with neutropaenia. Conclusions: Intravenous immunoglobulin therapy in children with ITP can lead to neutropaenia. However, patients benefit noticeably from IVIG therapy, and neutropaenia in this case tends to be a transient, self-limiting condition. [ABSTRACT FROM AUTHOR]
