Introduction. In patients receiving cardiac resynchronization therapy (CRT), failure rate to implant the left ventricular (LV) lead by the traditional trans-venous approach is 4-8%. Surgical epicardial implantation is considered as an alternative, but this technique is not without morbidity. Evidence from case documentation and from small trial batches demonstrated the viability of endocardial LV lead implantation where surgical epicardial lead placement is not applicable.

Introduction: During the pandemic years in Hungary the completed suicide rates hasrisen significantly. Suicide rates had been decreasing until 2019 since 1986. In 2019, 1550 people dead by completed suicide, in 2020 this number increased to 1705, in 2021,1561 cases were registered. Violent suicide attempts represent the majority of completed suicides. Objectives: In our study we were analysing the number of alteration of violent suicide attempts between 2016-2021, focusing on the trend in the first two years of the pandemic outbreak. 228 inpatients (65,4% male, 34,6% female) gone under medical treatment due to violent suicide attempts between 2016-2021 at Dr. Manninger Jenő National Traumatology Center, Budapest, Hungary. Methods: We used an interrupted time-series analysis with Prais-Winsten regression, controlling autoagressive and seasonal effects, to estimate the effect of the pandemic years on the violent suicide attempt rates in our sample. Demographic features, risk factors for suicidal behaviours, motivation and methods were analysed by Chi-square test and cross tabulation. Results: Comparing to the previous years, in the first two pandemic years significantly has risen the number of inpatients treated because of violent suicide attempts. After the rapid change in 2020, decreasing numbers could be observed in 2021. Conclusions: Analyzing the numbers of violent suicide attempts between 2016 and 2021, an increase inthe number of attempts was observed during the first two pandemic years. Introduction: In Hungary, in contrast to most other countries, suicide deaths increased significantly during the first year of the COVID-19 epidemic (March to December 2020). Globally, the burden of emergency care in the healthcare system tended to decrease during the first period of the epidemic. Objectives: Our research aimed to evaluate the changes in the number of intentional suicide attempts by violent and non-violent means during the first two years of the epidemic, compared to the trend before March 2020 in the Budapest metropolitan area and Pest County. Methods: We analyzed psychiatric assessment reports of self-poisoning patients admitted to Péterfy Hospital's Emergency Department and Clinical Toxicology from Jan 2019 to Dec 2021 to estimate non-violent suicide attempt trends. We analyzed patient data for violent suicide attempts treated at Dr. Manninger Jenő Trauma Centre from 2016-2021, focusing on trends during the first two years of the pandemic. Negative binomial regression estimates were used for interrupted time series analysis with Prais-Winsten regression, controlling for time and seasonal and autoregressive effects. We used change-point detection to examine the leveling of trends. The Institutional Review Board approved the research in both institutions. Results: The number of male non-violent suicide attempts decreased by 16.6% compared with the pre-epidemic period (p<0.001). A similar and significant decrease was observed in females and in the total population (Image 1). The female and total population trends, i.e., the decrease, were reversed by August 2020, and the male trends were reversed by October 2020. The total number of patients treated for violent suicide attempts increased significantly (p<0.05) during the first two years of the pandemic (Image 2). There was a slight increase in violent attempts in men and a small decrease in women, but these changes are not statistically significant. Image: Image 2: Conclusions: We hypothesize that those who tried to end their life through non-violent drug use were less inclined to seek assistance because they were concerned about being hospitalized during the COVID-19 outbreak. The surge in violent attempts is striking, as it correlates with the rise in suicide fatalities documented in Hungary during the initial year of the outbreak. Our data was obtained from two prominent public hospitals in Budapest, enabling us to conduct a more concentrated and thorough examination of the circumstances in the capital. Cloud based computing is among the top present-day research areas. Datacenters serving as the backend for cloud solutions have to satisfy the demands set by the time-critical applications emerging in our rushing world. In order to provide physical redundancy, the datacenters are equipped with redundant interserver connections that are left idle during normal operation via the traditional TCP protocol, e.g. with regards to link-capacity aggregation. Our paper provides a comparison between two technologies (MPTCP and MPT-GRE) that support multipath operation and could prove useful in datacenter environments by increasing efficiency and enhancing user experience in the cloud.

Multipath communication technologies is one of the current research fields for infocommunications. No better proof for this than having the specification of the MPTCP protocol, considered the field’s mothership, integrated into the operating systems of multiple corporations (e.g. Apple, Cisco) shorty after being released. The MPTCP multipath solution is practically the extension of the classic TCP protocol with the application of TCPsubflows. Besides its numerous advantages, this multipath protocol has a big drawback as well, namely that it supports data transfer over TCP only. However, for transmitting multimedia traffic, the UDP protocol is the practical choice. Stemming from this idea, the Networks Research Group at the Faculty of Informatics, University of Debrecen, has started development of a multipath communication technology (MPT-GRE) that aims to fulfil this need, providing an alternate solution besides MPTCP. In this paper we provide a comparison based on performance measurements between the MPTCP as a reference and the MPT solution developed by our research group, illustrated via numerous scenarios implemented over quad-path Gigabit Ethernet and IPv4/IPv6 connections.

Coronary artery disease represents one of the leading reasons of death worldwide, and acute coronary syndromes are their most devastating consequences. It is extremely important to identify the patients at risk for developing an acute myocardial infarction, and this goal can be achieved using noninvasive imaging techniques. Coronary computed tomography angiography (CCTA) is currently one of the most reliable methods used for assessing the coronary arteries; however, its use in emergency settings is sometimes limited due to time constraints. This paper presents the main characteristics of plaque vulnerability, the role of CCTA in the assessment of vulnerable plaques, and automatic segmentation techniques of the coronary artery tree based on CT angiography images. A detailed inventory of existing methods is given, representing the state-of-the-art of computational methods applied in vascular system segmentation, focusing on the current applications in acute coronary syndromes.

Subclavian artery puncture and dilation with 9 Fr dilator during pacemaker implantation is a rare complication. We present a case, where this complication occurred and the injury was sealed with collagen-based closure device without any complications. Long term follow-up and the control angio-CT revealed no further late vascular complications. Therefore, use of a collagen based closure device for treating accidental iatrogenic subclavian artery injury can be a simple, successful alternative therapy when surgical direct vascular repair is not possible or not available.
