Aim: Albumin-to-globulin ratio (AGR) is emerged as a marker of impaired prognosis. We determined the predictive value of AGR in patients with heart failure with reduced ejection fraction (HFrEF)., Methodology: 999 patients with HFrEF were enrolled. Rates of 1-year all-cause mortality were compared between AGR quartiles (Q). Moreover, multivariate survival analysis in Cox's regression model and receiver operating characteristic analyses were performed., Results: 90-day and 1-year mortality was the highest in AGR Q1. AGR was an independent predictor of 90-day and 1-year mortality. Receiver operating characteristic analysis revealed moderate diagnostic value in predicting 90-day (AGR cutoff <1.2) and 1-year (AGR cutoff <1.38) mortality., Conclusion: AGR had a good prognostic value and remained an independent predictor of mortality in HFrEF patients.

INTRODUCTION    An increasing number of ambulatory patients are placed on orthotopic heart transplantation (OHT) waiting lists, which results in an extended waiting time and a higher mortality rate. OBJECTIVES    The aim of this study was to identify the factors associated with reduced survival during a 1‑year follow‑up in patients with end‑stage heart failure listed for an OHT. PATIENTS AND METHODS    We retrospectively analyzed the data of 221 adult patients, who were accepted for OHT in our institution over a 2‑year period between 2013 and 2014. RESULTS    The mean (SD) age of the patients was 54.7 (9.6) years, and 90.1% of them were male. The mortality rate during the follow‑up period was 43.3%. The modified Model for End‑Stage Liver Disease (modMELD) score (odds ratio [OR], 1.70; P <0.001), as well as the plasma levels of high‑sensitivity C‑reactive protein (hs‑CRP; OR, 1.10; P <0.01), sodium (OR, 0.74; P <0.001), and uric acid (UA; OR, 1.003; P <0.05) were independent factors affecting death. The receiver‑operating characteristic (ROC) analysis indicated that a modMELD cut‑off of 10 (area under the ROC curve [AUC], 0.868; P <0.001), hs‑CRP cut‑off of 5.6 mg/l (AUC, 0.674; P <0.001), plasma sodium level cut‑off of 135 mmol/l (AUC, 0.778; P <0.001), and a plasma UA cut‑off of 488 μmol/l (AUC, 0.634; P <0.001) were the most accurate factors affecting death. CONCLUSIONS    In conclusion, although limited to a single center, our study demonstrated that an elevated modMELD score, incorporating a combination of renal and hepatic laboratory parameters, as well as plasma sodium, UA, and hs‑CRP levels at the time of listing are associated with reduced survival in ambulatory patients with end‑stage heart failure, accepted for OHT.

Background: Even though coronary angiography (CAG) underestimates coronary allograft vasculopathy (CAV) development, especially in the distal parts of arteries, it remains a frame of reference for International Society for Heart and Lung Transplantation (ISHLT) CAV classification. A retrospective analysis was performed to assess the prognostic value of CAG findings., Methods: Among 310 orthotopic heart transplantation (OHT) recipients with at least 2 CAGs at 2-year intervals, we identified 197 (146 men and 41 women; 55 ± 13 years) without lesions (Group 0), 27 (15 men and 12 women; 58 ± 8 years) in whom mild changes remained in consecutive CAGs (Group 1), 28 (24 men and 4 women; 58 ± 10 years) in whom mild lesions decreased in consecutive CAGs (Group 1REG), and 58 (53 men and 5 women; 56 ± 10 years) in whom the stenosis criteria of ISHLT CAV 2 or 3 were covered (Group 2). We compared survival and other clinical variables among the groups., Results: The average follow-up was 10 ± 4 years. Forty-one (21%) deaths occurred in Group 0, 15 (56%) in Group 1 (p = 0.002), 9(31%) in Group 1REG (p = NS), and 26 (46%) in Group 2 (p = 0.004, chi-square test). Time free from all-cause death was significantly shorter in Group 1 (T1/2 = 8 years) than in Group 0 (T1/2 = 15.5 years; p = 0.00072, log-rank test). Time free from cardiovascular death was significantly shorter in Groups 1 and 2, as was time free from CAV-related death in Groups 1, 1REG, and 2. Multivariate analysis, using a Cox proportional hazards model, revealed that Group 1 inclusion criterion of CAG findings is an independent predictor of all-cause death, cardiovascular death, and CAV-related death., Conclusions: Persistent mild coronary lesions, observed in consecutive CAG, predicted shorter survival of OHT recipients., (© 2013 International Society for Heart and Lung Transplantation Published by International Society for the Heart and Lung Transplantation All rights reserved.)

Many studies have suggested an imbalance between proinflammatory and anti-inflammatory markers in acute myocardial infarction (AMI). Interleukin 4 (IL-4) shows mainly anti-inflammatory activities. Interferon-gamma (IFN-γ) is a proinflammatory cytokine secreted by Th1 lymphocytes. Severe systolic left ventricular (LV) dysfunction following AMI is one of the major risk factors for poor prognosis. The aim of this study was to evaluate whether IL-4 and IFN-γ concentrations can serve as the correlates of impaired left ventricular function. Fifty-three patients with AMI were enrolled and divided into two groups depending on their ejection fraction (EF): group 1 with EF ≤ 30% (N = 10) and group 2 with EF >30% (N = 43). All patients underwent coronary angiography followed by percutaneous coronary intervention (PCI). Blood samples were taken (1) before, (2) immediately after, (3) 2 h after, and (4) 72 h after PCI. A receiver-operating characteristic (ROC) analysis was planned to identify possible cutoff values to predict LV dysfunction. There was no sex difference between the two groups. The rate of hypertension and diabetes mellitus was also similar. Median concentrations for IL-4 (pg/ml) were: (1) 13.4 versus 17.2 (p = 0.0001), (2) 16.8 versus 18.6 (p = 0.01), (3) 17.2 versus 17.6 (p = NS), and (4) 17.6 versus 17.2 (p = NS). Median concentrations for IFNγ (pg/ml) were: (1) 0.3 versus 1.9 (p = 0.00001), (2) 2.1 versus 1.6 (p = NS), (3) 0.4 versus 0.9 (p = NS), and (4) 0.9 versus 1.1 (p = NS). The area under the ROC curve (AUC) analysis is presented in the table below. Reported results suggest the high diagnostic value of IL-4 measurements before and immediately after PCI as the correlates of impaired LV dysfunction, whereas only IFN-γ measurement before PCI had a high diagnostic value. Measurements performed later on have no predictive value. Aims: Today's knowledge concerning the progression of coronary artery disease (CAD) is mainly derived from randomised clinical studies performed in patients with stable CAD. Data on atherosclerosis progression (AP) after myocardial infarction are limited. The aim of this study was to assess the AP in such a population of patients., Methods and Results: A total of 186 patients were analyzed. After exclusion of patients not suitable for quantitative coronary angiography analysis, the final group was comprised of 154 patients. AP was recognized in 93 (60.4%) patients. A multivariate regression analysis revealed that the risk of AP in patients increases with lack of adequate microvascular perfusion, as assessed by the thrombolysis in myocardial infarction (TIMI) myocardial perfusion grade (TMPG) score (odds ratio [OR] 4.13; 95% confidence interval [CI] = 1.17-14.53), non-smoking status and lower baseline cholesterol value (OR = 2,74; 95% CI 1.19-6.31 and OR = 1.44; 95% CI = 1.04-1.98, respectively). Moreover, the risk of progression of a pre-existing coronary lesion was enhanced the most when the baseline minimal lumen diameter was greater (OR = 2.34; 95% CI = 1.83-3.00), and a lack of adequate microvascular perfusion (OR = 3.12; 95% CI = 1.67-5.85) was the strongest predictor of risk of developing new coronary lesions., Conclusions: Atherosclerosis progression concerns over 60% of patients 12 months after acute myocardial infarction. Poor myocardial perfusion, as assessed by TMPG score, may be a simple tool for the identification of patients at greater risk of atherosclerosis progression, especially the development of new lesions.

Background: Surgical ventricular reconstruction (SVR) is used in conjunction with coronary artery bypass graft surgery (CABG) to improve left ventricular function and clinical outcomes in selected patients with ischemic heart failure. The impact of SVR on quality of life (QOL) and medical costs is unknown., Methods: We compared CABG plus SVR with CABG alone in 1,000 patients with ischemic heart failure, an anterior wall scar, and a left ventricular ejection fraction or=92% complete. Cost data were collected on 196 (98%) of 200 patients enrolled in the United States., Results: Heart-failure-related QOL outcomes did not differ between the 2 treatment strategies out to 3 years (median Kansas City Cardiomyopathy Questionnaire scores for CABG alone and CABG plus SVR, respectively: baseline 53 versus 54, P = .53; 3 years 85 versus 84, P = .89). There were no treatment-related differences in other QOL measures. In the US patients, total index hospitalization costs averaged over $14,500 higher for CABG plus SVR (P = .004) due primarily to 4.2 extra postoperative, high-intensity care days in the hospital., Conclusions: Addition of SVR to CABG in patients with ischemic heart failure did not improve QOL but significantly increased health care costs.

Objective: The efficacy of percutaneous and surgical revascularization in acute coronary syndromes without ST-segment elevation is similar. Therefore, other factors, like health-related quality of life, should play an important role in choosing the revascularization method., Methods: We compared 12-month health-related quality of life for high-risk patients with acute coronary syndromes without ST-segment elevation assigned to percutaneous coronary intervention (group A) versus coronary artery bypass graft surgery (group B). Patients had an episode of rest angina within 24h prior to admission and had to fulfil at least one of the criteria: (1) ST-segment depression (>or=0.05mV), (2) transient (<20min) ST-segment elevation or T-wave inversion (>or=0.1mV), (3) positive serum cardiac markers. Four hundred and ninety-six (91.34%) of 543 patients alive 12-months after index hospitalization completed a Short Form-36 (SF-36) health status survey. Group A comprised 392 patients and group B comprised 104 patients. We compared mental component summary (MCS) and physical component summary (PCS) scores from the SF-36 survey between analyzed groups., Results: There were no significant differences in MCS scores (47.21+/-12.30 vs. 46.60+/-11.3 in group A and group B, respectively, NS). PCS scores were lower in group A (38.30+/-11.10 vs. 42.64+/-9.76; p=0.003). Patients of group A had a higher rate of unstable angina (22.45 vs. 5.77%, p=0.0002) and repeated revascularization (12.76 vs. 1.92%, p=0.001) at 1 year. Patients of group A also had higher systolic and diastolic blood pressure during follow-up (138.17+/-20.41 vs. 133.47+/-19.21, p=0.04 and 82.48+/-11.32 vs. 77.25+/-16.17, p=0.0003, respectively). Systolic blood pressure was inversely associated with PCS scores in group A (Spearman's R= -0.18 p=0.0007)., Conclusions: This study has shown that there is a significant difference in health-related quality of life 12-months after percutaneous coronary intervention and coronary artery bypass graft surgery. This difference arises from better physical function (physical component summary) for coronary artery bypass graft surgery patients compared with percutaneous coronary intervention patients. Despite impairment of the physical health status (physical component summary), the mental health status (mental component summary) remained similar in both groups.
