Abstract

A growing literature shows an improvement of chronic hepatitis C virus (HCV)-related depression after successful treatment with direct-acting antivirals. However, depression after HCV cure remains insufficiently documented in people living with HIV (PLWH) and HCV, a population with specific mental health challenges. This study aimed to (i) document the prevalence of moderate-to-severe depression (PHQ-9 score ≥10) across different age classes in HCV-cured PLWH; (ii) identify associated socio-behavioral correlates.Descriptive analyses were performed on data collected during a cross-sectional survey (February 2018 - May 2019) nested in a prospective, multicenter cohort of individuals living with HIV and HCV (ANRS CO13 HEPAVIH). Socio-behavioral correlates of moderate-to-severe depression were identified using logistic regression.Among the 398 HCV-cured individuals in the study sample (median age [IQR]: 56 [53-59] years; 73.1% men), 23.9% presented with moderate-to-severe depression (PHQ-9 score ≥10). Depressive symptom prevalence rates were as follows: anhedonia: 52.3%; feeling 'down' or feelings of hopelessness: 48.3%; sleeping problems: 65.7%; lack of energy: 70.3%; eating disorders: 51.2%; lack of self-esteem: 34.3%; difficulty concentrating: 34.9%; sluggishness (in movement and voice) or restlessness: 24.6%; suicidal ideation: 17.1%. No significant difference was detected across age classes. Female sex, unhealthy alcohol use, sedentary lifestyle, and unhealthy eating behaviors were associated with increased odds of moderate-to-severe depression.Depressive symptoms were common in this sample of HCV-cured PLWH. Unlike findings for the French general population, the prevalence of depression did not decrease with age class. Mental health remains a key issue for HIV-HCV-coinfected individuals, even after HCV cure, especially in women and in individuals with unhealthy behaviors.Despite potential improvements in mental health after successful treatment with direct-acting antivirals, many people living with HIV (PLWH) and hepatitis C virus (HCV) - even in older age classes - still face depressive symptoms after HCV cure. In this population, women and people reporting unhealthy alcohol use, sedentary lifestyle, or unhealthy eating behaviors are more prone to report depressive symptoms after HCV cure. Mental health and lifestyle-related issues should be integrated in a global care model for PLWH living with or having a history of hepatitis C.

Abstract

Introduction Alors qu’en France, la possession et la consommation de cannabis est une infraction penale, la legislation espagnole autorise la consommation et la culture privee du cannabis. Meme si certains produits pharmaceutiques a base de cannabis ont une autorisation de mise sur le marche dans les deux pays, seuls les patients en Espagne y ont acces. Malgre ces differentes legislations, les patients atteints de maladies chroniques utilisent le cannabis sous differentes formes, naturelle, transformee, autorisee ou non, a des fins therapeutiques. Objectifs Les objectifs de cette etude sont : – d’identifier les caracteristiques des patients atteints de sclerose en plaques (SEP) qui utilisent deja le cannabis pour des objectifs therapeutiques ; – d’evaluer l’effet du cannabis sur les symptomes de la SEP percus par les patients ; – de comparer les resultats entre les deux pays ayant des legislations differentes : France et Espagne. Methodologie Un questionnaire en ligne de 31 questions a ete propose, entre avril et juillet 2019, aux patients adultes (≥ 18 ans) atteints de SEP, vivant en Espagne ou en France, inscrits sur Carenity.com, une plateforme de patients en ligne permettant de generer des donnees en vie reelle. Un modele logistique sera utilise pour identifier les facteurs associes a l’utilisation therapeutique du cannabis. Pour evaluer l’effet du cannabis therapeutique sur les symptomes de la SEP, nous allons utiliser des modeles de regression lineaire ou logistique pour chacun des domaines evalues : troubles du sommeil, spasticite, troubles de la sensibilite, fatigue, troubles de la marche, niveau de douleur. A travers ces analyses nous allons comparer les produits naturels aux produits pharmaceutiques a base de cannabis, en ajustant pour la frequence d’utilisation du cannabis, le type de SEP et sa severite, et d’autres facteurs potentiels (sexe, âge, anteriorite du diagnostic de SEP). L’effet pays sera pris en compte dans toutes les analyses comme un facteur de stratification ou d’ajustement. Resultats Un total de 641 patients atteints de SEP ont repondu a l’enquete, 70 % vivent en France et 30 % en Espagne. Les participants ont une moyenne d’âge de 47 ans, une anteriorite moyenne de diagnostic de 12 ans et 73 % sont des femmes. Respectivement 23 % et 20 % des repondants francais et espagnols ont consomme du cannabis a des fins therapeutiques. Ils en consomment depuis 3,6 ans en moyenne, 70 % quotidiennement, 18 % hebdomadairement et 12 % occasionnellement. Les produits naturels (herbe sechee ou resine) (47 %) et les produits a base de cannabidiol (45 %) sont les types de cannabis les plus couramment utilises par les patients atteints de SEP, suivis par un traitement a base de cannabis (13 %). Les patients commencent a utiliser le cannabis principalement pour soulager la douleur (64 %), traiter la spasticite (62 %) et les troubles du sommeil (48 %). Pres de trois quarts (74 %) des patients considerent que la douleur liee a la SEP a diminue grâce au cannabis, 76 % decrivent un effet positif du cannabis sur la spasticite, 76 % sur les troubles du sommeil et 60 % sur les troubles de la sensibilite. Les analyses de modelisation sont en cours de realisation et seront presentees a la conference. Conclusion Independamment des restrictions du cannabis liees a la legislation du pays, le niveau d’utilisation et la perception de l’efficacite sont similaires. Dans ce contexte, il est essentiel d’aider les patients a obtenir des produits de qualite controlee et de les accompagner dans leur utilisation pour la gestion des symptomes.

Due to the nature of gravity, non-linear effects are left imprinted in the quasi-normal modes generated in the ringdown phase of the merger of two black holes. We offer an analytical treatment of the quasi-normal modes at second-order in black hole perturbation theory which takes advantage from the fact that the non-linear sources are peaked around the light ring. As a byproduct, we describe why the amplitude of the second-order mode relative to the square of the first-order amplitude depends only weakly on the initial condition of the problem.

Nuestros ojos nos indican donde ir y nuestros pies nos llevan. Disfunciones subyacentes pueden, por lo tanto, influir en la funcionalidad misma de este arco inferior. No hay que olvidar que alteraciones de la entrada visual, del sistema vestibular, de la propiocepcion del cuello y de la columna, incluso del aparato masticador, pueden tener una importante repercusion sobre la marcha y la posicion en bipedestacion. El pie, contacto con el suelo, vector de la sensibilidad plantar y de la propiocepcion podal, es una herramienta formidable e indispensable en la gestion de la estabilidad durante el movimiento. Esta intrinsecamente relacionado con el miembro inferior y la pelvis, que sirve de base a la columna vertebral, mediante el esqueleto y el juego articular y muscular. Vehicula y transmite tambien informaciones sensoriales hasta los centros nerviosos. Cualquier alteracion en uno de estos sistemas produce una repercusion global sobre la dinamica y requiere adaptaciones. Se trataran aqui los pies y los miembros inferiores.

8. Amélioration de marqueurs de santé physique chez des patients en programme de maintien d’abstinence post sevrage pour trouble d’usage d’alcool sévère en hôpital de jour : une étude prospective observationnelle

Abstract

Introduction Le Trouble lie a l’Usage d’Alcool (TUA) est associe a une augmentation de la morbi-mortalite. Il est connu que l’abstinence chez des patients avec TUA severe ameliore les biomarqueurs hepatiques, mais on en sait peu sur l’evolution temporelle d’autres marqueurs de sante (Rahhali et al 2015). Objectif Nous avons voulu decrire l’evolution de plusieurs marqueurs de sante chez des patients avec TUA severe beneficiant d’un programme de maintien d’abstinence post sevrage en Hopital de Jour (HDJ) Methode Nous avons suivi de maniere prospective 52 patients a partir de leur entree en HDJ. Les mesures cardiovasculaires (pression arterielle, frequence cardiaque), nutrition (IMC, taux de lipides dans le sang, albumine), hepatiques et cognitives (test de depistage du MoCA) ont ete evaluees a deux reprises : a l’entree du programme et apres un mois d’abstinence documentee. Les scores moyens ont ete compares entre l’entree et a un mois avec un test de Wilcoxon. Resultats 25 des 52 patients inclus ont poursuivi le programme d’HDJ et maintenus une abstinence documentee jusqu’a un mois. Ils etaient âges de 49 ± 6.5 ans, etaient majoritairement des hommes (71.7 %) et etaient dependants de l’alcool a l’âge de 34 ans ± 10. Des ameliorations significatives ont ete observees entre l’entree et le suivi a un mois sur le score au Montreal Cognitive Assessment (MoCA : 24,9 ± 4,9 versus 26,9 ± 2,2 p = 0,004) ainsi que sur deux biomarqueurs: le volume moyen de globulaires (VGM : 96.3 UI/L ± 10,3 versus 95,2 UI/L ± 5,3 p = 0,042) et les gamma-glutamyl transferases (GGT : 170,3 UI/L ± 272,7 versus 59,1 UI/L ± 61,7 p = 0,023) Conclusion L’abstinence chez des patients avec TUA severes en HDJ ameliore significativement des un mois des marqueurs hepatiques mais egalement hematologiques et cognitifs.

9. Genetic diversity and structure of Slovenian native germplasm of plum species (P. domestica L., P. cerasifera Ehrh. and P. spinosa L.)

Abstract

Slovenia has particular climatic, soil, geographic and historical conditions that lead to long tradition of plum cultivation and use. In this work, a set of 11 SSR and three universal cpDNA markers, as well as flow cytometry, were used to (1) evaluate the genetic diversity of 124 accessions of the three Prunus species (P. domestica L., P. cerasifera Ehrh., and P. spinosa L.), (2) investigate the possible involvement of P. cerasifera and P. spinosa species in P. domestica origin, (3) study the genetic relationships and variability among the most typical P. domestica accessions present in Slovenia. Ten haplotypes of cpDNA were identified and clustered into three groups according to the Neighbor-Joining analysis (NJ). All 11 SSR primer pairs were polymorphic, revealing 116 unique genotypes. A total of 328 alleles were detected with an average value of 29.82 alleles per locus, showing relatively high diversity. Bayesian analysis of genetic structure was used to identify two ancestral populations in the analyses of all three species as well as in a separate set consisting of P. domestica material only. Principal Coordinate Analysis (PCoA) showed that accessions clustered largely in agreement with Bayesian analysis. Neighbor-Joining analysis grouped 71 P. domestica accessions into three clusters with many subgroups that exhibited complex arrangement. Most accessions clustered in agreement with traditional pomological groups, such as common prunes, mirabelle plums and greengages. In this study, the analyses revealed within P. domestica pool valuable local landraces, such as traditional prunes or bluish plums, which seem to be highly interesting from a genetic point of view. Moreover, complementary approaches allowed us to distinguish between the three species and to gain insights into the origin of plum. The results will be instrumental in understanding the diversity of Slovenian plum germplasm, improving the conservation process, recovering local genotypes and enriching existing collections of plant genetic resources.

11. Identifying the axillary nerve during shoulder surgery: an anatomic study using advanced imaging

Abstract

Background: The axillary nerve (AXN) is one of the more commonly injured nerves during shoulder surgery. Prior anatomic studies of the AXN in adults were performed using cadaveric specimens with small sample sizes. Our research observes a larger cohort of magnetic resonance imaging (MRI) studies in order to gain a more representative sample of the course of the AXN and aid surgeons intraoperatively. Methods: High-resolution 3T MRI studies performed at our institution from January 2010 to June 2019 were reviewed. Four blinded reviewers with musculoskeletal radiology or orthopedic surgery training measured the distance of the AXN to the surgical neck of the humerus (SNH), the lateral tip of the acromion (LTA), and the inferior glenoid rim (IGR). Intraclass correlation coefficient was calculated to assess reliability between reviewers. The nerve location was assessed relative to rotator cuff tear status. Results: A total of 257 shoulder MRIs were included. Intraclass correlation coefficient was excellent at 0.80 for the SNH, 0.90 for the LTA, and 0.94 for the IGR. All intraobserver reliabilities were above 0.80. The mean distance from the AXN to SNH was 1.7 cm (range, 0.7-3.1 cm; interquartile range, 1.38-2.00) and that from the AXN to IGR was 1.6 cm (range, 0.6-2.6 cm; interquartile range, 1.33-1.88). The mean AXN to LTA distance was 7.1 cm, with a range of 5.2-9.0 cm across patient heights; there was a large effect size related to the LTA to AXN distance and patient height with a correlation of r = 0.603 (P < .001). Rotator cuff pathology appears to affect nerve location by increasing the distance between the AXN and SNH (P = .027). Discussion/Conclusion: The AXN is vulnerable to injury during both open and arthroscopic shoulder procedures. This injury can be either a result of direct trauma to the nerve or secondary to traction placed on the nerve with reconstructive procedures that distalize the humerus. Our study demonstrates that the AXN can be found as little as 5.6 mm from the IGR and 6.9 mm from the SNH. In addition, we illustrate the relationship between patient height and the LTA to AXN distance and complete rotator cuff tears and the SNH to AXN distance. Our study is the first to demonstrate the nerve's proximity to important surgical landmarks of the shoulder using a large sample size of high-resolution images in living human shoulders.

Abstract

The divertor target plates are the most thermally loaded in-vessel components in a fusion reactor where high heat fluxes are produced on the plasma-facing components (PFCs) by intense plasma bombardment, radiation and nuclear heating. For reliable exhaust of huge thermal power, robust and durable divertor target PFCs with a sufficiently large heat removal capability and lifetime has to be developed. Since 2014 in the framework of the preconceptual design activities of the EUROfusion DEMO project, integrated R&D efforts have been made in the subproject ‘Target development’ of the work package ‘Divertor’ to develop divertor target PFCs for DEMO. Recently, the first R&D phase was concluded where six (partly novel) target PFC concepts were developed and evaluated by means of non-destructive inspections and high-heat-flux fatigue testing. In this paper, the major achievements of the first phase activities in this subproject are presented focusing on the design rationales of the target PFC concepts, technology options employed for small-scale mock-up fabrication and the results of the first round high-heat-flux qualification test campaign. Stradwin is a new software-tool, with demonstrated sub-voxel accuracy in assessing cortical bone properties from routine CT. Abstract

The design of plasma facing components (PFCs) requires knowledge of the charged particle heat load in the scrape-off layer (SOL). Ray-tracing codes like PFCFlux can model this heat load assuming that particles follow the magnetic field lines. Calculations on limiter equilibria underestimate the heat load significantly. In fact, not all the power circulating in the SOL is reported on the wall, with 80% of the total power circulating in the SOL missing in the worst cases. This paper explains why some power is missing in this case, and presents different ways to rescale the heat load to recover all the power coming from the SOL. The maximum heat load on the limiter for a given magnetic configuration can change from 1 MW/m2 without rescaling to values from 3.5 MW/m2 to 21.7 MW/m2 depending on the rescaling method. Abstract

Recently, an integrated program of conceptual design activities for the European DEMO reactor was launched in the framework of the EUROfusion Consortium, where reliable power handling capability was identified as one of the most critical scientific as well as technological challenges for a DEMO reactor. The divertor is the key in-vessel plasma-facing component being in charge of power exhaust and removal of impurity particles. The DEMO divertor target will have to withstand extreme thermal loads where the local peak heat flux is expected to reach up to 20 MW/m2 during slow transient events in DEMO. To assure sufficient heat removal capability of the divertor target against normal and transient operational scenarios under expected cumulative neutron dose of up to 13 dpa is one of the fundamental engineering challenges imposed on target design. To develop the design of the DEMO divertor and related technologies, an R&D work package ‘Divertor’ has been set up in this consortium. The subproject ‘Target Development’ is devoted to the development of the conceptual design and the core technologies of the plasma-facing target. Devising and implementing novel structural heat sink materials (e.g. W/Cu composites) to advanced target design concepts is one of the major objectives of this subproject. In this paper, the underlying design requirements imposed by the envisaged power exhaust goal and the prominent material-design interface issues are discussed. In addition, the candidate design concepts being currently considered are presented together with the related material issues. Finally, the first results achieved so far are presented.

Abstract

With prostate and colorectal malignancies being the most common cancers in men, elevated prostate specific antigen (PSA) in patients without rectal access due to prior surgery poses a diagnostic dilemma. We report the first use of CT-guided biopsy in combination with prebiopsy MRI in 2 patients with a clinical suspicion of prostate cancer and no rectal access. In both cases, a diagnostic multiparametric MRI of the prostate was performed to detect and to localize a potential suspicious lesion. The localization served as a cognitive map for guiding needle placement using a CT-guided transgluteal approach.

Abstract

International audience; BACKGROUND: Thanks to direct-acting antivirals, hepatitis C virus (HCV) infection can be cured, with similar rates in HCV-infected and HIV-HCV co-infected patients. HCV cure is likely to foster behavioral changes in psychoactive substance use, which is highly prevalent in people living with HIV (PLWH). Cannabis is one substance that is very commonly used by PLWH, sometimes for therapeutic purposes. We aimed to identify correlates of cannabis use reduction following HCV cure in HIV-HCV co-infected cannabis users and to characterize persons who reduced their use. METHODS: We used data collected on HCV-cured cannabis users in a cross-sectional survey nested in the ANRS CO13 HEPAVIH cohort of HIV-HCV co-infected patients, to perform logistic regression, with post-HCV cure cannabis reduction as the outcome, and socio-behavioral characteristics as potential correlates. We also characterized the study sample by comparing post-cure substance use behaviors between those who reduced their cannabis use and those who did not. RESULTS: Among 140 HIV-infected cannabis users, 50 and 5 had reduced and increased their use, respectively, while 85 had not changed their use since HCV cure. Cannabis use reduction was significantly associated with tobacco use reduction, a decrease in fatigue level, paying more attention to one's dietary habits since HCV cure, and pre-HCV cure alcohol abstinence (p = 0.063 for alcohol use reduction). CONCLUSIONS: Among PLWH using cannabis, post-HCV cure cannabis reduction was associated with tobacco use reduction, improved well-being, and adoption of healthy behaviors. The management of addictive behaviors should therefore be encouraged during HCV treatment.

We evaluated the association between psychoactive substance use and both HCV and non-HCV mortality in HIV/HCV co-infected patients in France, using Fine and Gray's competing-risk model adjusted for socio-demographic, clinical predictors and confounding factors, while accounting for competing causes of death. Over a 5-year median follow-up period, 77 deaths occurred among 1028 patients. Regular/daily cannabis use, elevated coffee intake, and not currently smoking were independently associated with reduced HCV-mortality (adjusted sub-hazard ratio [95% CI] 0.28 [0.10-0.83], 0.38 [0.15-0.95], and 0.28 [0.10-0.79], respectively). Obesity and severe thinness were associated with increased HCV-mortality (2.44 [1.00-5.93] and 7.25 [2.22-23.6] versus normal weight, respectively). Regular binge drinking was associated with increased non-HCV-mortality (2.19 [1.10-4.37]). Further research is needed to understand the causal mechanisms involved. People living with HIV/HCV co-infection should be referred for tobacco, alcohol and weight control interventions and potential benefits of cannabis-based therapies investigated.

Abstract

International audience; OBJECTIVES:Although common among patients coinfected with HIV and hepatitis C virus (HCV), sleep disturbances (SD) are still poorly documented in this population in the HCV cure era. This longitudinal study aimed at analysing SD in HIV-HCV coinfected patients and identifying their clinical and sociobehavioural correlates.METHODS:We used 5-year annual follow-up data from 1047 participants in the French National Agency for Research on Aids and Viral Hepatitis Cohort 13 'Hépatite et VIH' (ANRS CO13 HEPAVIH) cohort of HIV-HCV coinfected patients to identify clinical (medical records) and behavioural (self-administered questionnaires) correlates of SD (mixed-effects logistic regression). SD were identified using one item documenting the occurrence of insomnia or difficulty falling asleep (ANRS 'Action Coordonnée 24' self-reported symptoms checklist), and two items documenting perceived sleep quality (Center for Epidemiologic Studies Depression and WHO Quality of Life HIV-specific brief scales).RESULTS:Seven hundred and sixteen (68.4%) patients with completed self-administered questionnaires reported SD at their most recent follow-up visit. In the multivariable model, hazardous alcohol consumption (Alcohol Use Disorders Identification Test-Consumption score≥4 for men, ≥3 for women) (adjusted odds ratio=1.61; 95% confidence interval: 1.09-2.36), depressive symptoms (6.78; 4.36-10.55) and the number of other physical and psychological self-reported symptoms (1.10; 1.07-1.13) were associated independently with SD after adjustment for sex, age and employment status. HCV cure was not associated significantly with SD.CONCLUSION:SD remain frequent in HIV-HCV coinfected patients and are associated with a series of modifiable behavioural risk factors. Independent of HCV cure, improved screening and comprehensive management of alcohol use, physical and psychological self-reported symptoms and depression are essential in this population. Closer investigation of these risk factors of SDs may both increase sleep quality and indirectly improve patients' clinical outcomes.

Abstract

Cannabidiol (CBD) has a wide spectrum of potential therapeutic use and is growing in popularity. However, there is a need to characterize users, as associated benefits and risks may differ depending on the pattern of use. This study explored the characteristics of French and Italian CBD users according to their primary reason to use it. We analyzed data from an online survey of French ( n  = 1164) and Italian ( n  = 2608) cannabidiol users. We compared their characteristics according to their primary reason to use CBD, and identified correlates of therapeutic use with logistic regressions. In both countries, users' characteristics differed according to their reasons to use CBD. Reporting therapeutic use was associated with daily CBD use and nonsmoking routes of administration in both countries. User profiles differed according to their primary reason to use it. However, similarities in characteristics were found between both countries for users who used CBD for therapeutic reasons. Accordingly, analyzing the reasons to use CBD would appear to be a pertinent way to characterize users and identify their unmet needs in terms of self-care.

Abstract

Although Hepatitis C virus (HCV) infection can be cured with direct-acting antivirals (DAA), some cured patients face a serious risk of advanced liver damage and early mortality. In order to avoid these two negative health outcomes, it is important to identify and assess related risk factors. Little is currently known about socioeconomic and behavioural factors in this context. Using data from the ANRS CO22 Hepather cohort, we tested for associations between socioeconomic and behavioural factors and (i) advanced liver fibrosis (defined as an FIB-4 > 3.25) assessed longitudinally using a mixed-effects logistic regression model (both the whole population and stratified on advanced liver fibrosis status at the time of HCV cure) and (ii) all-cause mortality (Cox proportional hazards model), during post-HCV cure follow-up. Among 5833 participants cured of HCV, living in poverty was associated with postcure advanced liver fibrosis in participants without this diagnosis at the time of HCV cure (population attributable fraction-PAF-of 8.6%) and with mortality in the whole study population (PAF of 10.6%). The detrimental effects of unhealthy alcohol use and heavy tobacco smoking, as well as the beneficial effect of living with a stable partner were also highlighted. We highlighted the major role of poverty and behavioural factors in advanced liver fibrosis and all-cause mortality in patients cured of HCV. Encouraging linkage to social support services and healthy behaviours after successful DAA treatment could limit morbidity and increase survival in this population. 27. Acceptability of and attitudes to the therapeutic use of cannabis and cannabidiol in people with Parkinson's disease: A French survey.

Introduction: Cannabis and cannabidiol (CBD) may potentially alleviate symptoms and improve the quality of life of people with Parkinson's disease (PD), although clinical results to date have provided conflicting evidence. In France, cannabis use is illegal outside the current restricted medical cannabis experimental framework which does not include PD as an eligible condition. In contrast, CBD products are legal and are easily available. We aimed to evaluate the acceptability of therapeutic cannabis and CBD use, and to assess cannabinoid-related attitudes among people with PD in France, with a view to assessing the potential inclusion of medical cannabinoids in PD treatment options., Methods: We conducted a French nationwide online survey among people with PD. Cannabis and CBD acceptability levels were derived from the answers to four questions. Logistic regressions were performed to identify factors associated with these levels. We also collected data on knowledge, information-seeking, and barriers to self-medication., Results: Of 1136 participants, acceptability levels of medical cannabis and CBD use were 81.7% and 87.4%, respectively. For both substances, acceptability was associated with the presence of anxiety symptoms, greater knowledge about cannabinoids, seeking information on medical cannabis, and considering the risk of cannabis dependence to be low. A fear of dependence was one of the main barriers to using either substance; healthcare providers were rarely mentioned as sources of information on medical cannabis., Conclusions: Acceptability levels of cannabis and CBD were high. Acceptability was associated with knowledge and perceptions of cannabinoids. Abstract

Dermoid cysts are embryological derivatives from soft tissues. They result from an ectodermic inclusion in the mesoderm, during the early embryonic development. These cysts have a slow development and are mostly encountered in the pediatric population. Eighty percent of them are located in the head and neck area, and most of them are on the frontozygomatic suture. They are superficial or deep on clinical examination. The diagnosis is mostly easy, sometimes with the help of radiological examination. The confirmation is histological. 29. Limited Impact of Cannabidiol on Health-related Quality of Life of People With Long-term Controlled HIV: A Double-blind, Randomized, Controlled Trial.

Background: People with HIV (PWH) with undetectable HIV viral load still have an impaired health-related quality of life (HRQoL). Cannabidiol (CBD) is a nonintoxicating cannabis-derived cannabinoid that holds promise for the treatment of many ailments. In the present study, we tested whether oral CBD-rich medication could significantly improve PWH's HRQoL., Methods: Eighty participants with undetectable HIV viral load were randomized to either a placebo or full-spectrum CBD (1 mg/kg twice a day) arm for 12 weeks plus a 4-week follow-up period. HRQoL was assessed at baseline, week 12, and week 16 using the 36-Item Short Form Health Survey questionnaire (SF-36). Primary outcomes were physical and mental component summary scores; secondary outcomes were the 8 SF-36 subscale scores. Treatment effects on outcomes were estimated using generalized estimating equations., Results: We found no effect of CBD intake on the summary score for either component. However, CBD intake was associated with a higher physical functioning score at week 12 only (regression coefficient [95% confidence interval], 7.72 [0.55-14.89]; P = .035). No significant main effect of CBD intake on the other HRQoL subscale scores was observed. Furthermore, there was no difference in self-reported adverse effects between the 2 arms., Conclusions: Twice-daily CBD full-spectrum oil at 1 mg/kg had no major effect on virologically suppressed PWH's HRQoL but had a positive effect on physical functioning. Abstract

Introduction: Cannabidiol (CBD) is a non-intoxicating cannabis compound found in diverse commercial products worldwide. However, its use may not be fully harmless. Accordingly, it is important to document the prevalence of CBD use and user characteristics in the general population., Methods: We conducted a nationwide survey from a random sample of adults living in France using computer-assisted telephone interviews between 2 March and 9 July 2022. We estimated the prevalence of CBD awareness and CBD use, and explored the different routes of administration. We also performed logistic regression models to identify factors associated with past-year CBD use., Results: Based on data from 3229 participants, we estimated that 71.0% (95% confidence interval) (69.0-73.0) of the French adult population had heard of CBD, and 10.1% (8.7-11.4) had used it in the previous year. Past-year CBD use was associated with younger age, a higher educational level, not living in a middle-sized urban unit, tobacco consumption and e-cigarette use. The most common route of administration was smoking (56.1%)., Discussion and Conclusion: Past-year CBD use prevalence in France appeared to be as high as that for cannabis. Proper prevention, regulation and control of CBD products is necessary to ensure that people have access to safe and high-quality products. Reliable information on CBD should be sought and disseminated, especially regarding the harms associated with smoking the compound., (© 2024 The Authors. Abstract

Many cannabis users report therapeutic benefits from cannabis consumption, even when not recommended by a physician. To date, few data on therapeutic users of cannabis in France are available. Using a cross-sectional survey held in 2020, we collected sociodemographic, health and substance use data from 4150 daily cannabis users in France. We used multivariable logistic regression to assess factors associated with exclusive therapeutic use of cannabis. Approximately 10% ( n  = 453) of the participants reported using cannabis exclusively for therapeutic purposes. Exclusive therapeutic users of cannabis differed from non-exclusive (i.e. recreational and mixed) users, especially regarding age (aOR [95%CI] = 1.01 [1.00-1.02]), employment (aOR = 0.61 [0.47-0.79]), urban area of residence (aOR = 0.75 [0.60-0.94]), physical (aOR = 2.95 [2.34-3.70]) and mental health condition (aOR = 2.63 [1.99-3.49]), mode of cannabis administration (non-smoked, aOR = 1.89 [1.22-2.95); smoked with little tobacco, aOR = 1.39 [1.09-1.76]), frequency of cannabis use (aOR = 1.04 [1.01-1.06]), home cultivation (aOR = 1.56 [1.13-2.15]), at-ridsk alcohol use (aOR = 0.68 [0.54-0.84]), and previous-month opiate use (aOR = 1.67 [1.22-2.30]). A greater understanding of the distinct profiles of regular cannabis users could inform harm reduction strategies and care access for this population. Further studies are needed to better understand the boundaries between therapeutic and recreational use.

Abstract

Introduction: The risk of mortality in people with a history of injection drug use (PHID) is high, as is the prevalence of hepatitis C virus (HCV) infection. Although direct-acting antivirals (DAA) are effective in this population in terms of sustained virological response, it is not known whether PHID benefit as much as people with no history of injection drug use from DAA-related HCV cure in terms of reduced all-cause mortality., Methods: Using Cox proportional hazards models based on the ANRS CO22 Hepather cohort data (n = 9735), we identified factors associated with all-cause mortality among HCV-infected people. We tested for interaction effects between drug injection status, HCV cure and other explanatory variables., Results: DAA-related HCV cure was associated with a 66% (adjusted hazard ratio [95% confidence interval]: 0.34 [0.29-0.39]) lower risk of all-cause mortality, irrespective of drug injection status. Detrimental effects of unhealthy alcohol use on mortality were identified in PHID only., Discussion and Conclusions: DAA-related HCV cure led to comparable benefits in terms of reduced mortality in PHID and people with no history of injection drug use. Abstract

Cannabis use is being increasingly liberalized worldwide, and an increasing prevalence of cannabis-use disorder (CUD) is observed. The few current therapeutic options for CUD are only modestly effective. Mindfulness-based interventions offer promising prospects for the management of substance-use disorders. However, despite proliferating literature on mindfulness and substance use, few studies have explored mindfulness in terms of cannabis use and CUD. There are many possibilities for the implementation of mindfulness-based interventions for cannabis use reduction, especially for younger users, who are more vulnerable to cannabis-related harms. 34. Screening for alcohol use in primary care: assessing French general practitioner practices.

Abstract

Alcohol use is a leading risk factor for premature death and disability. To tackle this issue, more systematic and accurate screening for at-risk consumption is needed in healthcare systems, especially by general practitioners (GPs). We assessed the frequency of at-risk consumption screening by GPs in France. We also identified characteristics associated with more frequent screening and greater use of validated screening tools by these healthcare providers. A cross-sectional survey was conducted among a representative sample of French GPs. Multinomial logistic regressions were used to identify factors associated with more frequent screening and greater use of validated screening tools. Response rate was of 73%. Of the 2412 participants, 42.8% screened all their patients systematically and repeatedly, while 48.0% never used standardized tools to screen potentially at-risk patients. Among other characteristics, being aware of and using the "early identification and brief intervention" screening strategy, and feeling absolutely comfortable talking with patients about reducing or stopping their alcohol use, were both associated with more frequent screening and use of standardized tools. Our results on at-risk alcohol use screening highlight an improvement over data from previous studies. Nevertheless, better training of French GPs in good alcohol screening practices-specifically, increased screening frequency and greater use of standardized tools-may improve identification of at-risk patients., (© The Author(s) 2023. Abstract

Aims and Objectives: We aimed to identify correlates of cannabinoid-based products (CBP) use in patients with multiple sclerosis (MS) in France and Spain., Background: MS is responsible for a wide range of symptoms, including pain. Access to CBP differs according to local legislation. The French context is more restrictive than the Spanish one, and no data regarding cannabis use among MS patients has yet been published. Characterizing MS patients who use CBP constitutes a first step toward identifying persons most likely to benefit from them., Design: An online cross-sectional survey was submitted to MS patients who were members of a social network for people living with chronic diseases and were living in France or Spain., Methods: Two study outcomes measured therapeutic CBP use and daily therapeutic CBP use. Seemingly unrelated bivariate probit regression models were used to test for associations between the outcomes and patients' characteristics while accounting for country-related differences. STROBE guidelines were followed in reporting this study., Results: Among 641 study participants (70% from France), the prevalence of CBP use was similar in both countries (23.3% in France vs. 20.1% in Spain). MS-related disability was associated with both outcomes, with a gradient observed between different degrees of disability. MS-related pain level was associated with CBP use only., Conclusions: CBP use is common in MS patients from both countries. The more severe the MS, the more participants turned to CBP to alleviate their symptoms. Easier access to CBP should be ensured for MS patients in need of relief, especially from pain., Relevance to Clinical Practice: This study highlights the characteristics of MS patients using CBP. Introduction: Alcohol use disorder (AUD) is associated with a significant disease burden in France, where alcohol use is deeply rooted in culture. However, the treatment gap is large because of several barriers, including stigmatisation and drinkers' apprehension about total abstinence. However, standardised and evidence-based interventions based on controlled-drinking for people with AUD are lacking. We aimed to assess the effectiveness of a novel community-based French therapeutic patient education (TPE) program for people with AUD named Choizitaconso., Methods: A before-after non-randomised quasi-experimental study, named ETHER, was designed and implemented with people living with AUD, over a period of 6 months. The primary outcome was percentage change in the number of alcohol-related harms experienced. Secondary outcomes were percentage changes in psycho-social patient-reported and community-validated outcomes. Participants in the intervention group (n = 34) benefited from the 10-week TPE program Choizitaconso, while the comparison group (n = 58) received standard care. The Kruskall-Wallis and chi-squared or Fisher's exact tests were used to compare before-after changes in variables in both groups. Linear regression models were used to test for the effect of study group on each outcome and to test for the effect of alcohol consumption as a confounder., Results: At 6 months, all outcomes but one either remained stable or numerically improved in both groups. Internalised stigma significantly improved in the intervention group (p = 0.026) but not in the comparison group (p = 0.207), with a significant group effect (p = 0.014)., Discussion and Conclusions: This study demonstrates the effectiveness of the Choizitaconso TPE program on community-validated outcomes, especially internalised stigma., (© 2023 The Authors. Abstract

Obesity and nonalcoholic fatty liver disease (NAFLD) constitute global and growing epidemics that result in therapeutic dead ends. There is an urgent need for new and accessible treatments to improve and widen both preventive and curative approaches against NAFLD. The endocannabinoid system (ECS) is recognized as a complex signaling apparatus closely related to metabolic disorders and is a key target for treating NAFLD. Despite a lack of conclusive clinical trials, observational and pre-clinical studies highlight putative benefits of phytocannabinoids on liver steatosis through multiple pathways. Owing to both its safety profile and its diversity of active compounds acting primarily (although not exclusively) on the ECS-and its expanded version, the endocannabinoidome, the Cannabis plant should be considered a major prospect in the treatment of NAFLD. However, seizing this opportunity, and intensifying clinical research in this direction, will require overcoming both scientific and nonscientific barriers.

Background & Aims: A growing literature shows an improvement of chronic hepatitis C virus (HCV)-related depression after successful treatment with direct-acting antivirals. However, depression after HCV cure remains insufficiently documented in people living with HIV (PLWH) and HCV, a population with specific mental health challenges. This study aimed to (i) document the prevalence of moderate-to-severe depression (PHQ-9 score ≥10) across different age classes in HCV-cured PLWH; (ii) identify associated socio-behavioral correlates., Methods: Descriptive analyses were performed on data collected during a cross-sectional survey (February 2018 - May 2019) nested in a prospective, multicenter cohort of individuals living with HIV and HCV (ANRS CO13 HEPAVIH). Socio-behavioral correlates of moderate-to-severe depression were identified using logistic regression., Results: Among the 398 HCV-cured individuals in the study sample (median age [IQR]: 56 [53-59] years; 73.1% men), 23.9% presented with moderate-to-severe depression (PHQ-9 score ≥10). Depressive symptom prevalence rates were as follows: anhedonia: 52.3%; feeling 'down' or feelings of hopelessness: 48.3%; sleeping problems: 65.7%; lack of energy: 70.3%; eating disorders: 51.2%; lack of self-esteem: 34.3%; difficulty concentrating: 34.9%; sluggishness (in movement and voice) or restlessness: 24.6%; suicidal ideation: 17.1%. No significant difference was detected across age classes. Female sex, unhealthy alcohol use, sedentary lifestyle, and unhealthy eating behaviors were associated with increased odds of moderate-to-severe depression., Conclusions: Depressive symptoms were common in this sample of HCV-cured PLWH. Unlike findings for the French general population, the prevalence of depression did not decrease with age class. Mental health remains a key issue for HIV-HCV-coinfected individuals, even after HCV cure, especially in women and in individuals with unhealthy behaviors., Lay Summary: Despite potential improvements in mental health after successful treatment with direct-acting antivirals, many people living with HIV (PLWH) and hepatitis C virus (HCV) - even in older age classes - still face depressive symptoms after HCV cure. In this population, women and people reporting unhealthy alcohol use, sedentary lifestyle, or unhealthy eating behaviors are more prone to report depressive symptoms after HCV cure. Abstract

Background: Opioid agonist therapy (OAT) is associated with reduced injection, reduced HCV transmission, and more opportunities to initiate hepatitis C virus (HCV) treatment in people who use drugs (PWUD). We aimed to study the extent to which adherence to OAT was predictive of increased uptake of direct-acting antivirals (DAA) in PWUD with chronic HCV infection., Methods: Using the French national healthcare system database, we targeted PWUD (i.e. with a history of OAT) who had chronic HCV infection and were eligible for DAA during 2014-2016. Adherence to OAT was computed as a time-varying variable expressing the proportion of days covered by OAT receipt, over any six-month interval before DAA receipt. We used a Cox proportional hazards model to estimate the association between adherence to OAT and the rate of DAA uptake after adjustment for age, sex, alcohol use disorder, socioeconomic status, and liver disease severity., Results: Among the 22,615 persons included in the ANRS FANTASIO study, 3438 (15.2%) initiated DAA during the study period. After multivariable adjustment, adherence to OAT was associated with a higher rate of DAA initiation. However, this association was not linear, and only individuals on OAT for 20% or more of the time in the previous six-month period had a higher rate of DAA initiation (adjusted hazard ratio [95% confidence interval]: 1.28 [1.18-1.38]). Other variables associated with DAA initiation were male sex, older age, cirrhosis or liver cancer, and higher socioeconomic status., Conclusions: Adherence to OAT is a major predictor of DAA initiation in PWUD living with chronic HCV infection in France. Our results also suggest that even moderate adherence to OAT can facilitate DAA uptake. Adequate HCV training for OAT prescribers together with interventions to ensure adherence to OAT will help improve DAA initiation rates and reach HCV elimination goals., (© 2022. The Author(s).)

Background and Aims: Hepatitis C virus (HCV) infection is associated with the onset of metabolic disorders which constitute risk factors for liver disease progression. Their impact may persist after the HCV infection has been cured. Cannabis use is associated with a lower risk of obesity and diabetes in both general and HCV populations. The associations between cannabis use and both dyslipidemia and hypertension have not yet been studied in persons with chronic HCV infection., Methods: Using cross-sectional data from the French ANRS CO22 Hepather cohort, we used regression models to test for an inverse relationship between cannabis use and (i) dyslipidemia, (ii) hypertension, and (iii) the total number of metabolic disorders., Results: Among the 6364 participants in the study population, both former and current cannabis use were associated with a lower risk of hypertension and fewer metabolic disorders. These results were independent of central obesity. Cannabis use was not associated with dyslipidemia., Conclusions: In people chronically infected with HCV, cannabis use was associated with a lower risk of hypertension and a lower number of metabolic disorders. Post-HCV cure studies are needed to confirm these findings using longitudinal data and to test whether they translate into reduced mortality in this population.

Background: Chronic hepatitis B virus (HBV) infection may evolve into cirrhosis and hepatocellular carcinoma, and this progression may be accelerated by specific risk factors, including overweight and obesity. Although evidence for a protective effect of cannabis use on elevated body weight has been found for other populations, no data are available for HBV-infected patients. Aims: We aimed to identify risk factors (including cannabis use) for overweight and obesity in patients with HBV chronic infection. Methods: Using baseline data from the French ANRS CO22 Hepather cohort, we performed two separate analyses, one using "central obesity" (based on waist circumference) and the other "overweight" and "obesity" (based on body mass index) as outcomes. Logistic and multinomial regressions were used to model central obesity and overweight/obesity, respectively. Results: Among the 3706 patients in the study population, 50.8% had central obesity, 34.7% overweight, and 14.4% obesity. After multivariable adjustment, current cannabis use was associated with a 59% lower risk of central obesity compared with no lifetime use (adjusted odds ratio [95% CI]: 0.41 [0.24 to 0.70]). It was also associated with a 54% and 84% lower risk of overweight (adjusted relative risk ratio [95% CI]: 0.46 [0.27 to 0.76]) and obesity (0.16 [0.04 to 0.67]), respectively. Conclusions: Cannabis use was associated with lower risks of overweight and obesity in patients with HBV chronic infection. Future studies should test whether these potential benefits of cannabis and cannabinoid use translate into reduced liver disease progression in this high-risk population.

Background: Cannabidiol (CBD), a safe, non-intoxicating cannabis component, is growing in popularity in Europe and worldwide. However, CBD EU regulation is blurry, and consequent labelling and product quality issues may have implications for public health. There is therefore a need to assess the prevalence and perceived harmfulness of CBD use in EU countries, as well as to characterise CBD users. We aimed to do so in the French population., Methods: In December 2021, an online survey was conducted in a sample respecting the French adult population structure for key demographic variables. Sociodemographic, behavioural and CBD perception data were collected. Three separate regressions were performed to identify correlates of i) having heard of CBD, ii) using CBD, iii) perceived harmfulness of CBD. A hierarchical classification was also performed to identify profiles of CBD users., Results: The study sample comprised 1969 adults, of whom 69.2% had heard of CBD and 10.1% used it. Less than half (46.8%) of the former considered it harmful. Having heard of CBD was associated with younger age, being born in France, tobacco use, and cannabis use. CBD use was associated with younger age, tobacco use, cannabis use, poor self-reported general health status, and positive perception of alternative medicines. Cluster analysis revealed four different CBD user profiles based on socio-demographics and behavioural characteristics., Conclusion: Ten percent of the adults in this French study used CBD, and several user profiles emerged. Abstract

Overweight is increasingly prevalent in people living with HIV (PLWH), and is a high risk factor for metabolic disorders in this population. PLWH co-infected with hepatitis C virus (HCV) have a higher risk of metabolic disorders than their mono-infected counterparts. The putative relationship between cannabis use and body weight found in the general population has never been documented in HIV-HCV co-infected people. We tested whether cannabis use is associated with body mass index (BMI), overweight, and underweight in HCV co-infected PLWH ( N = 992). Mixed-effects linear and logistic regression models were used to study the association between cannabis use and the three outcomes over time. After multivariable adjustment, cannabis use was inversely associated with BMI. Cannabis use was associated with a lower and higher risk of overweight and underweight, respectively. Cannabis use should be assessed and taken into account in the clinical management of the HIV-HCV co-infected population.
