1. Integration of clinical parameters, genotype and epistaxis severity score to guide treatment for hereditary hemorrhagic telangiectasia associated bleeding

Abstract

Abstract Background Hereditary Hemorrhagic Telangiectasia (HHT) is a rare inherited disorder characterized by development of mucocutaneous telangiectases and visceral organ arteriovenous malformations, which can lead to recurrent, spontaneous bleeding and development of iron deficiency anemia. The primary objective of this study was to ascertain the relationship between epistaxis severity scores (ESS), laboratory values, genotype, and phenotype in HHT. Our secondary objective was to assess efficacy of systemic antifibrinolytic therapy in reducing ESS in HHT. Methodology We conducted a retrospective review of patients seen at the UNC HHT Center from January 1, 2009 to February 28, 2015. ESS, demographics, and results of genetic testing were abstracted from the medical record. Response to antifibrinolytic therapy was evaluated by comparing pre-post ESS. Results One hundred and forty nine patients were eligible with 116 having genetic testing and 33 without. Age, hemoglobin and ferritin levels were predictive of ESS. Of the 116 patients that underwent genetic testing: 63 had an ACVRL1 mutation, 40 had an ENG mutation, 2 had a SMAD4 mutation, and 11 patients had no pathologic HHT genetic variation detected. Compared to patients without a detectable HHT-associated genetic variation, patients with a HHT-associated genetic variation had higher ESS scores (p 4 were started on antifibrinolytic therapy (tranexamic acid or aminocaproic acid) and had a post-treatment ESS recorded. All patients had a decrease in ESS of > 0.71 (minimal meaningful difference), but patients taking antifibrinolytics displayed larger decreases. No patients on antifibrinolytics experienced a VTE with median follow up of 13 months. Conclusions We demonstrate that the ESS correlates with age, hemoglobin and ferritin. Additionally, we demonstrate that HHT patients with genetic mutations have higher ESS scores. Our data demonstrate that antifibrinolytics are effective in decreasing epistaxis severity and safe with long-term use in HHT patients.

Abstract Background While warfarin is the most commonly prescribed medication to prevent thromboembolic disorders, the risk of adverse drug reactions (ADR) poses a serious concern. This prospective study evaluated how primary care providers (PCP) and cardiologists at our Institution managed patients treated with warfarin with the goal of decreasing the number of warfarin ADRs. Methods A multidisciplinary anticoagulation task force was established at our Institution in 2014 to standardize warfarin monitoring and management. Between 2013 and 2017, we analyzed patients who were prescribed warfarin by their PCP or cardiologist upon hospital discharge and in the ambulatory setting to determine the international normalized ratio (INR) within 5, 10, and 30 days after discharge, time in therapeutic range (TTR), number of severe warfarin ADRs, and total and average cost reduction of all severe warfarin ADRs to determine whether there was an organizational cost savings following the implementation of standardized warfarin care. Abstract

Computational models and simulations often involve representations of decision-making processes. Numerous methods exist for representing decision-making at varied resolution levels based on the objectives of the simulation and the desired level of fidelity for validation. Decision making relies on the type of decision and the criteria that is appropriate for making the decision; therefore, decision makers can reach unique decisions that meet their own needs given the same information. Accounting for personalized weighting scales can help to reflect a more realistic state for a modeled system. To this end, this article reviews and summarizes eight multi-criteria decision analysis (MCDA) techniques that serve as options for reaching unique decisions based on personally and individually ranked criteria. These techniques are organized into a taxonomy of ratio assignment and approximate techniques, and the strengths and limitations of each are explored. We compare these techniques potential uses across the Agent-Based Modeling (ABM), System Dynamics (SD), and Discrete Event Simulation (DES) modeling paradigms to inform current researchers, students, and practitioners on the state-of-the-art and to enable new researchers to utilize methods for modeling multi-criteria decisions.

Abstract

The purpose of this study was to examine the effects of listening to preferred and non-preferred warm-up music on upper-body resistance exercise performance. Resistance-trained males (ages 18–24) participated in two separate bench press trials each with a different warm-up music condition: preferred warm-up music (PREF) or non-preferred warm-up music (NON-PREF). In each trial, participants listened to PREF or NON-PREF music during a standardized bench press warm-up. Following the warm-up, motivation to exercise was measured using a visual analog scale followed by two sets × repetitions to failure (RTF) at 75% of 1-RM separated by 1 min of rest. A linear position transducer was used to measure mean barbell velocity. Rate of perceived exertion (RPE) was obtained after each set. RTF, velocity, RPE, and motivation were analyzed. RTF were significantly higher during the PREF versus NON-PREF trail (p = 0.001) while mean barbell velocity remained unchanged (p = 0.777). RPE was not significantly different between PREF and NON-PREF trials (p = 0.735). Motivation to exercise was significantly higher during the PREF versus NON-PREF trial (p < 0.001). Findings show that listening to PREF music during a warm-up improves subsequent RTF performance during bench press exercise. However, barbell velocity was largely unaffected. While perceived exertion was similar between trials, motivation to exercise was markedly increased during the PREF warm-up music trial. These findings suggest that competitors listening to warm-up music before giving maximal effort during resistance exercise could optimize performance by ensuring self-selection of their own preferred music.

Abstract

Objective: Prescription opioid misuse represents a social and economic challenge in the United States. We evaluated Schedule II opioid prescribing practices by primary care providers (PCPs), orthopedic and general surgeons, and pain management specialists. Design: Prospective evaluation of prescribing practices of PCPs, orthopedic and general surgeons, and pain management specialists over 5 years (October 1, 2014-September 30, 2019) in an outpatient setting. Methods: An analysis of Schedule II opioid prescribing following the implementation of federal and state guidelines and evidence-based standards at our institution. Results: There were significantly more PCPs, orthopedic and general surgeons, and pain management specialists with a significantly increased number who prescribed Schedule II opioids, whereas there was a simultaneous significant decline in the average number of Schedule II opioid prescriptions per provider, Schedule II opioid pills prescribed per provider, and Schedule II opioid pills prescribed per patient by providers. The average number of Schedule II opioid prescriptions with a quantity >90 and Opana/Oxycontin prescriptions per PCP, orthopedic surgeon, and pain management specialist significantly decreased. The total morphine milligram equivalent (MME)/day of Schedule II opioids ordered by PCPs, orthopedic and general surgeons, and pain management specialists significantly declined. The ages of the providers remained consistent throughout the study. Conclusions: This study reports the implementation of federal and state regulations and institutional evidence-based guidelines into primary care and medical specialty practices to reduce the number of Schedule II opioids prescribed. Further research is warranted to determine alternative therapies to Schedule II opioids that may alleviate a patient’s pain without initiating or exacerbating a potentially lethal opioid addiction.

Abstract

Successful use of prognostics involves the prediction of future system behaviors in an effort to maintain system availability and reduce the cost of maintenance and repairs. Recent work by the National Institute of Standards and Technology indicates that the field of prognostics and health management is vital for remaining competitive in today’s manufacturing environment. While prognostics-based maintenance involves many traditional operations researchcentric challenges for successful deployment such as limited availability of information and concerns regarding computational efficiency, the authors argue in this paper that the field of prognostics and health management, still in its embryonic development stage, could benefit greatly from considering soft operations research techniques as well. Specifically, the authors propose the use of qualitative problem structuring techniques that aid in problem understanding and scoping. This paper provides an overview of these soft methods and discusses and demonstrates how manufacturers might use them. An approach combining problem structuring methods with traditional operations research techniques would help accelerate the development of the prognostics field.

Abstract

Floodplain restoration is popular to address excess nutrients, but its ability to enhance photolysis of emerging contaminants has not been evaluated. We used the numerical model MIKE-21 to simulate photolysis reactions within the inundated surface water of restored floodplains along a mid-size river. We examined both "high" and "low" floodplain scenarios where inundation occurs 5% (storms) and 50% (baseflow) of the year, respectively. We simulated photolysis of the pharmaceuticals morphine, codeine, and methamphetamine and, for context, compared it with nitrate removal (denitrification and plant uptake). Pollutant removal due to floodplain restoration was greater for the low floodplain (e.g., 18.8% for morphine) than for the high floodplain (5.6% for morphine) due to greater water exchange relative to channel flow. The fastest- and slowest-reacting pollutants (morphine and methamphetamine, respectively) were always transport- and reaction/kinetics-limited within floodplain surface water, respectively. Yet, those with intermediate decay-rate constants switched from reaction limitation to transport limitation as the floodplain length increased, and removal leveled off at an optimum length of ∼1000 m. However, as the floodplain width increased, the required floodplain length for 30% removal decreased. Optimal restored floodplain conditions for photolysis would maximize light exposure, which may differ from those for nutrients.

Abstract

The transient storage model (TSM) is a popular solute transport model among hydrologists and ecologists for characterizing solute retention potential of small headwater streams. TSM parameters are most commonly estimated in an inverse modeling framework using tracer breakthrough data. A key challenge in interpreting TSM parameters is the inherent difficulty in the estimation process and relating these parameters with the physical characteristics of streams. While often recognized that multiple sources of uncertainty impact transient storage modeling, rigorous evaluation of TSM parameter uncertainty is rare. Stream flow magnitude has been identified as a key variable in characterizing stream solute transport dynamics, yet no study has shown the impact of flow magnitude on TSM parameter uncertainty. In this paper we study the impact of flow magnitude on TSM parameter uncertainty by using tracer data from repeated tracer studies with changing stream flow magnitudes, and introduce a software tool to perform Markov chain Monte Carlo based parameter estimation and uncertainty evaluation. The software tool developed is capable of performing thousands of model runs within a few hours, which is necessary for Monte Carlo based parameter estimation, and provides a potentially attractive framework for model testing and improvement. TSM parameters were found to be relatively less important during higher flows with large uncertainty bounds and clear evidence of parameter correlations. The importance of TSM parameters on solute transport seemed to increase as flow decreased with seasonal baseflow recession. Our findings highlight the need for improved field methodologies to reduce parameter uncertainty and the need for further improvement in the model structure for successful stream characterization.

Results: A statistically significant increase in the total number of PCPs and PCPs who prescribed Schedule II opioids was observed, while there was a concurrent significant decrease in the average number of Schedule II opioid pills prescribed per PCP, Schedule II opioid prescriptions per PCP, Schedule II opioid pills prescribed per patient by PCPs, Schedule II opioid prescriptions with a quantity > 90 per PCP, and Opana/Oxycontin prescriptions per PCP. A statistically significant decline in the average morphine equivalent daily dosage of Schedule II opioids per PCP was noted. Conclusions: This study reports the benefit of incorporating federal and state regulations and institutional evidence-based guidelines into primary care practice to decrease the number of Schedule II opioids prescribed. Further preventive measures include selecting alternative treatments to opioids and reducing the rates of opioid nonmedical use and overdose while maintaining access to prescription opioids when indicated.

Abstract

Surface coal mining alters landscapes including creating waste-rock fills or dumps. In Appalachia USA, mines fill valleys with waste rock, constructing valley fills that affect water quality and aquatic ecology downstream. Total dissolved solids (TDS) in mine effluent are elevated from exposure of mineral surfaces to weathering. Understanding TDS variability requires understanding valley fill internal structure and its effect on hydrology, yet prior studies focused on point measurements or did not address patterns among fills. Here we investigated subsurface structure and hydrologic flowpaths in two dimensions within four valley fills using electrical resistivity imaging (ERI). We used artificial rainfall to investigate the location and transit time of preferential flowpaths through the fills. We corroborated our ERI interpretations using borehole logs, downhole video, and shallow soil excavation. ERI results indicated variability in substrate type and widespread presence of preferential flowpaths. We estimated an average preferential flowpath vertical length of 6.6 m, average transit time of water along the flowpath of 1.4 h, and average minimum water velocity of 5.1 m/h (0.14 cm/s). These rates are higher than typical for undisturbed lands, and resemble highly preferential flow in karst terrain. ERI successfully distinguished fills using conventional loose-dump construction from experimental controlled-material compacted-lift construction. Conventional fills exhibited finer particles that retain water at the surface, with larger rocks and larger voids at depth. Conventional fills had greater ranges of subsurface resistivity (i.e. substrate types) and greater interior accumulation of water during artificial rainfall, indicating more quick/deep preferential infiltration flowpaths. We show experimental construction significantly alters hydrologic response, which in combination with use of low-TDS waste rock, may affect downstream water quality relative to conventional loose-dump methods. Our soil boring and pits corroborated ERI interpretation, thus demonstrating ERI to be a robust non-invasive technique that provides reliable information on valley fill structure and hydrology.

Abstract

Many of the world's greatest challenges are complex socio-environmental problems, often framed in terms of integrated assessment, resilience or sustainability. To resolve any of these challenges, it is essential to elicit and integrate knowledge across a range of systems, informing the design of solutions that take into account the complex and uncertain nature of the individual systems and their interrelationships. To meet this scientific challenge, we propose a tiered, system-of-systems modeling framework with these elements: a component-based, software framework that couples a wide range of relevant systems using a modular, system-of-systems structure; a tiered structure with different levels of abstraction that spans bottom-up and top-down approaches; the ability to inform robust decisions in the face of deep uncertainty; and the systematic integration of multiple knowledge domains and disciplines. We illustrate the application of the framework, and identify research and education initiatives that are needed to facilitate its development and implementation.

Abstract

Autor predočuje projekt urbane revitalizacije koji je izvanredno prihvaćen u SAD-u jer je omogućio gospodarski oporavak maloga gradića na umoru, uz istodobno očuvanje načina života i pejzažnih vrijednosti omiljenih mjesnom stanovništvu. Autor ocjenjuje način na koji je to urađeno, tehnike planiranja i rezultate. Rezultati istraživanja upućuju na uspješnost tehnike u uočavanju najvažnijih mjesta u gradu, tzv. svete strukture grada, koja su inspirirala sam razvojni projekt. Ta se tehnika može djelotvorno primijeniti i u drugim mjestima i gradovima suočenim s oprekom između čuvanja kulturnog naslijeđa i gospodarskog razvoja.

Abstract

The purpose of this study was to examine the effects of listening to preferred and non-preferred warm-up music on upper-body resistance exercise performance. Resistance-trained males (ages 18–24) participated in two separate bench press trials each with a different warm-up music condition: preferred warm-up music (PREF) or non-preferred warm-up music (NON-PREF). In each trial, participants listened to PREF or NON-PREF music during a standardized bench press warm-up. Following the warm-up, motivation to exercise was measured using a visual analog scale followed by two sets × repetitions to failure (RTF) at 75% of 1-RM separated by 1 min of rest. A linear position transducer was used to measure mean barbell velocity. Rate of perceived exertion (RPE) was obtained after each set. RTF, velocity, RPE, and motivation were analyzed. RTF were significantly higher during the PREF versus NON-PREF trail (p = 0.001) while mean barbell velocity remained unchanged (p = 0.777). RPE was not significantly different between PREF and NON-PREF trials (p = 0.735). Motivation to exercise was significantly higher during the PREF versus NON-PREF trial (p < 0.001). Findings show that listening to PREF music during a warm-up improves subsequent RTF performance during bench press exercise. However, barbell velocity was largely unaffected. While perceived exertion was similar between trials, motivation to exercise was markedly increased during the PREF warm-up music trial. These findings suggest that competitors listening to warm-up music before giving maximal effort during resistance exercise could optimize performance by ensuring self-selection of their own preferred music.

Abstract

Riparian zones are key gateways for solutes in watersheds, including nutrients and pollutants moving toward the stream network. In human‐dominated landscapes, they are widely used as buffers to remove pollutants at substantial cumulative cost yet vary widely in their effectiveness, and much is unknown about the detailed processes involved and their controls. Preferential flow is widespread in riparian zones, oriented both horizontally toward the channel (often bypassing beneficial reactions in the soil during baseflow) or vertically (enhancing beneficial reactions by infiltrating surface flow during storms). Preferential flow thus contributes to widespread variability of riparian zone/buffer function, with implications for legacy nutrients stored in upland soils and aquifers. This creates a disconnect between functional riparian definitions based on flow and transport processes and common operational ones based on buffer width. Enhanced field characterization of preferential flow path spatial distribution and connectivity, together with developments in simulating preferential solute transport in soils, would allow better prediction of spatial and temporal variation in riparian zone function. Such prediction in turn would allow improved buffer function by tailoring buffer design to site specific conditions, thereby reconciling functional and operational viewpoints.

Abstract

We present direct photon-hadron correlations in 200 GeV/A Au+Au, d+Au, and p+p collisions, for direct photon pT from 5–12 GeV/c, collected by the PHENIX Collaboration in the years from 2006 to 2011. We observe no significant modification of jet fragmentation in d+Au collisions, indicating that cold nuclear matter effects are small or absent. Hadrons carrying a large fraction of the quark's momentum are suppressed in Au+Au compared to p+p and d+Au. As the momentum fraction decreases, the yield of hadrons in Au+Au increases to an excess over the yield in p+p collisions. The excess is at large angles and at low hadron pT and is most pronounced for hadrons associated with lower momentum direct photons. Comparison to theoretical calculations suggests that the hadron excess arises from medium response to energy deposited by jets.
