We use seismic waveform data from the AlpArray Seismic Network and three other temporary seismic networks, to perform receiver function (RF) calculations and time-to-depth migration to update the knowledge of the Moho discontinuity beneath the broader European Alps. In particular, we set up a homogeneous processing scheme to compute RFs using the time-domain iterative deconvolution method and apply consistent quality control to yield 112 205 high-quality RFs. We then perform time-to-depth migration in a newly implemented 3D spherical coordinate system using a European-scale reference P and S wave velocity model. This approach, together with the dense data coverage, provide us with a 3D migrated volume, from which we present migrated profiles that reflect the first-order crustal thickness structure. We create a detailed Moho map by manually picking the discontinuity in a set of orthogonal profiles covering the entire area. We make the RF dataset, the software for the entire processing workflow, as well as the Moho map, openly available; these open-access datasets and results will allow other researchers to build on the current study.

The wide-angle reflection and refraction (WARR) SHIELD’21 profile carried out in 2021 crosses from SW to NE the main tectonic of Ukraine. The SHIELD’21 targeted the structure of the Earth’s crust and upper mantle of the southwestern margin of the East European Craton with overlying Neogene Carpathian Foredeep and Vendian-Paleozoic Volyn-Podolian Monocline, Archaean and Paleoproterozoic segments of Ukrainian Shield and Late Paleozoic Dnieper-Donets Basin. The ~650 km long profile is an extension of realized in 2014 RomUkrSeis line in Romania and Ukraine from Apuseni Mountains to southwestern Ukrainian Shield. The field work performed in 2021, included the deployment of autonomous seismic stations and drilling-explosive works. A total of 264 seismic receivers were deployed (160 DATA-CUBE and 104 TEXAN stations) with the average spacing between the observation points about 2.65 km. The sampling interval for all stations was 0.01 s. Seismic energy was generated by 10 shot points with ~50 km of distance between them and total charge in all wells 5775 kg. The SHIELD’21 experiment using TEXAN and DATA-CUBE short-period seismic stations provided high-quality seismic records. The main recorded seismic waves are the refractions of P- and S-waves in sediments, basement, crust and upper-most mantle, and reflections from crustal boundaries, Moho interface and boundaries in the uppermost mantle. Correlation of travel time arrivals of seismic waves provides calculation of the velocity model for both P- and S-waves. The main objective of the SHIELD’21 project is to get new seismic data that increase our knowledge on the lithosphere structure as well as geodynamics of the oil-and-gas-bearing and ore regions of Ukraine.

Přehledová studie pojednává o mezinárodních srovnávacích výzkumech v oblasti vzdělávání realizovaných v České republice v posledních dvaceti letech. Jejím cílem je informovat o tom, jak jsou výsledky mezinárodních výzkumů zpracovávány v domácí odborné literatuře a k jakým závěrům vedou. V první části studie stručně popisujeme, jakých mezinárodních výzkumů se Česká republika účastnila. V druhé části se zaměřujeme na témata, která byla v návaznosti na výsledky českých žáků zpracována ve vybraných domácích odborných časopisech a monografických pracích. Je možné konstatovat, že výsledky českých žáků v mezinárodních výzkumech lze považovat za poměrně konzistentní, i když v průběhu času dochází k určitým výkyvům. Závěrečná část přehledové studie naznačuje několik cest, jak by bylo možné s daty a výsledky mezinárodních výzkumů dále pracovat. Mezinárodní srovnávací výzkumy představují pro Českou republiku bohatý rezervoár dat, jehož potenciál by mohl být více využíván pro didaktiku v zájmu rozvíjení produktivní kultury vyučování a učení.

Studie se zabývá komunikací ve školní třídě. Konkrétně je zaměřena na používání angličtiny jako cílového jazyka a češtiny jako mateřského jazyka ve výuce angličtiny na základních školách v České republice. Představuje výsledky analýzy 89 vyučovacích hodin anglického jazyka zaměřené na proporci mezi používáním angličtiny a češtiny učiteli a žáky a také na specifické situace, ve kterých jsou oba jazyky míšeny. Analýzy ukázaly, že učitelé ve vyučování vyřkli více českých slov než anglických, zatímco žáci naopak vyřkli více anglických slov než českých. Analýzy využívající časové kódování ukázaly, že angličtina a čeština se ve výuce objevují vyváženě. Alternativní perspektivou na vztah cílového a mateřského jazyka je pohled skrze příležitosti k procvičování jednotlivých jazykových dovedností, jež učitelé ve výuce vytvářejí. V souladu s jinými realizovanými výzkumy byly mezi jednotlivými zkoumanými učiteli pozorovány podstatné rozdíly v tom, do jaké míry využívají ve výuce mateřský jazyk žáků. V závěru studie je představeno pět typických situací, ve kterých ve výuce dochází k míšení jazyků.

Předložený text je pokusem o analytickou studii s programovým vyústěním. Jeho cílem je teoreticky vymezit a empiricky analyzovat „problém“ kurikulární reformy a koncipovat její vztah ke kultuře vyučování a učení. V úvodu autor zasazuje současnou českou kurikulární reformu do širšího kontextu vzdělávací transformace a zamýšlí se nad předmětem a charakterem reformy. V návaznosti na to s využitím tzv. ekvalizéru governance rozebírá problém načasování reformy, a to v kontextu analýzy širších mechanismů fungování a ovládání školského systému. Za klíčové lze považovat autorovo tvrzení, že současná česká kurikulární reforma stěží pozvedne kvalitu výuky, neboť její implementace vyústila v nezvladatelný formalismus. Cesta ke kvalitě výuky podle autora spočívá v podpoře produktivní kultury vyučování a učení. Realizovat změnu tudíž znamená propracovat novou – produktivní – kulturu vyučování a učení. V závěru autor předkládá argumenty pro potřebu „programu“ spočívajícího v rozvinutí kurikulární reformy do koncepce podpory škol a profesního rozvoje učitelů s ideou produktivní kultury vyučování a učení na horizontu.

Text je koncipován jako teoretická studie vycházející v návaznosti na český překlad knihy Linking practice and theory: The pedagogy of realistic teacher education od F. A. J. Korthagena et al. Výklad je ve studii rozvržen do dvou částí. V první části jsou představeny vůdčí (programové) myšlenky Korthagenovy koncepce; text je převážně popisný, nemá ambici kritické analýzy. O to jde v druhé části studie, kde se autoři vracejí k vybraným myšlenkám Korthagenovy koncepce, rozebírají je a podrobují je diskusi v širším kontextu novějších domácích i zahraničních přístupů a poznatků. Zaměřují se na tři související problémové okruhy: (1) Povaha vztahu teorie a praxe – nedocenění Teorie s velkým T? (2) Význam reflexe v učitelově profesním rozvoji – vyprázdněnost s ohledem na učivo? (3) Podpora profesního učení – redukované pojetí supervize? V závěru studie je otevřena otázka, jak prezentovanou koncepci využít v kontextu učitelského vzdělávání v České republice.

Přehledová studie uvádí do českého odborného diskursu profesní vidění jako jeden z perspektivních pojmů využitelných při úvahách na téma (učitelské) profesionalizace. V úvodu textu se autoři zmiňují o vybraných pokusech o vymezení znaků profesí a představují pohled na problematiku učitelské profesionality, který se opírá o tři dimenze – profesní vidění, profesní vědění a profesní jednání. Dále naznačují vztahy mezi těmito třemi dimenzemi, a to v rámci modelu pedagogického uvažování a jednání (Shulman, 1986) a modelu reflexe ALACT (Korthagen, 2011). V dalším textu autoři rozebírají profesní vidění u profesí obecně a poté specificky u učitelů. Představují existující konceptualizace tohoto pojmu a na základě souvisejících teoretických i empirických studií se propracovávají k vlastnímu pojetí profesního vidění učitelů. Dále naznačují vztah profesního vidění a reflexe na spirále pedagogického uvažování a jednání. V závěru nastiňují využití konceptu profesního vidění v učitelském vzdělávání, konkrétně při tvorbě elektronického učebního a diagnostického (video)prostředí pro (budoucí) učitele.

V předkládané studii jsou prezentována hlavní výzkumná zjištění a doporu čení vztahující se k zavádění kurikulární reformy na gymnáziích. Autoři postupně rozebírají celkem deset problémových okruhů kurikulární reformy, jak byly podchyceny ve výzkumu Kvalitní škola: (1) Chybí (sdílené) porozumění vůdčím idejím a pojmům reformy; (2) Diskuse o reformě: aktéři reformy a jejich hlasy a nehlasy; (3) Problém jazyka – problém po-rozumění; (4) Nejasnosti v tom, o reformu čeho se jedná; (5) Když se zaváděné má stát zavedeným: implementace reformy jako problém koordinace; (6) Podmínky, v jakých se implementuje; (7) Rozporuplné vnímání a přijímání kurikulární reformy učiteli; (8) Dvoustupňové kurikulum: RVP a tvorba ŠVP jako klíčový prvek reformy; (9) Vstupování učitelů do role tvůrců kurikula: rozpaky a zdrženlivost; (10) Realizace kurikula: formalizmus reformy, nebo cesta k nové kultuře vyučování a učení. V závčěu studie autoři prezentují souhrn doporučení pro aktéry, kteří mají s kurikulární reformou na různých úrovních co dočinění. Vedle toho naznačují výhledy pro další výzkum v této oblasti.

Současné pojetí cizojazyčného vzdělávání vychází z komunikačního přístupu a jeho cíle v podobě komunikační kompetence. Otázkou je, do jaké míry se cíle cizojazy čného vzdělávání promítají do reálné výuky. Cílem předkládané empirické studie bylo zjistit, do jaké míry poskytuje výuka anglického jazyka na 2. stupni základních škol příležitosti k rozvíjení řečových dovedností poslechu, mluvení, čtení a psaní a jaká je míra integrovanosti těchto příležitostí. Výzkumné šetření bylo realizováno formou videostudie. Výzkumným nástrojem byl kategoriální systém vytvořený na základě konceptu řečových dovedností. Výsledky poukázaly na dominantní zastoupení recepce nad produkcí řeči žáky. Nejvíce zastoupenými řečovými dovednostmi bylo čtení a poslech, relativně méně měli žáci příležitosti k mluvení a psaní. Pokud jde o integraci řečových dovedností, bylo zjištěno, že receptivní řečové dovednosti (poslech a čtení) se ve výuce častěji než dovednosti produktivní (mluvení a psaní) uplatňují samostatně. Převážná část učebních aktivit zahrnovala integraci dvou řečových dovedností; komplexnější učební aktivity integrující tři nebo čtyři řečové dovednosti byly ve výuce relativně méně zastoupeny

Cílem přehledové studie je hodnotit stav českého pedagogického výzkumu a naznačit možné perspektivy jeho vývoje. Studie je rozvržena do tří částí. V první části je čtenář uveden do širšího kontextu problému, jímž je reforma (financování) vědy a výzkumu v České republice. Poukazuje se na upozad’ování pojmu „věda“ v současné diskusi, o sporech kolem základního a aplikovaného výzkumu, o marném (zdá se) úsilí obhájit institucionální financování výzkumu a o různých dočasně aktuálních metodikách hodnocení výzkumu a vývoje apod. V druhé části studie je rozbírán současný stav českého pedagogického výzkumu – ve zkratce jsou shrnuty závěry dřívějších analýz J. Průchy, J. Mareše, E. Walterové a analýz vypracovaných Centrem pedagogického výzkumu PdF MU. Ve třetí části studie jsou rozbírány výhledy českého pedagogického výzkumu. Přednostní pozornost je věnována jedné za všechny, a to tvorbě poznatkové základny věd o edukaci. Po rozboru povahy a předmětu vědění, které má výzkum produkovat, je prezentována diferenciace pojmů pedagogický výzkum a výzkum vzdělávání. V návaznosti na to jsou rozebírány relevantní disciplinární přístupy, předmětné oblasti a druhy výzkumných poznatků.

Jednou z funkcí školy, která se jeví jako klíčová, a to navzdory proměnám, jimiž škola prochází, je funkce enkulturační. Škola je jedinou relativně systematizovanou formou seznamování členů společnosti s kulturními obsahy. Ty zde figurují v podobě učiva systematizovaného prostřednictvím kurikulárních dokumentů do vzdělávacích oblastí, oborů, předmětů, průřezových témat apod. V přehledové studii zabýváme vztahem mezi obory a školními vyučovacími předměty. V souvislosti s tím autoři rozebírají některé problémy kurikula, zejména problém (ne)korespondence mezi kurikulem projektovaným, realizovaným a dosahovaným. Následně se zamýšlejí nad možnými příčinami (ne)úspěchu českých žáků v mezinárodně srovnávacích studiích TIMSS a PISA. V závěru zdůvodňují potřebu chápat pedagogický výzkum mj. v roli „kritického přítele“ probíhající kurikulární reformy.

Učební úlohy představují soubor specifických požadavků kladených na žákovo učení. Z hlediska didaktiky je klíčovou otázka, jaký potenciál mají učební úlohy pro procesy učení. Předkládané výzkumné sdělení je zaměřeno právě tímto směrem. Na základě systematické analýzy videozáznamů 27 vyučovacích hodin fyziky na 2. stupni základní školy je hledána odpověď na otázky: Kolik učebních úloh je řešeno ve vyučovací hodině a v jakém časovém zastoupení? Kolik času zaujímají fáze zadávání, řešení a kontroly úloh? Kdo je řešitelem úloh? Jaký typ řešení úlohy vyžadují? Výsledky ukazují, že práce s učebními úlohami zaujímá 63 % všech posuzovaných sekvencí, což odpovídá přibližně 6 úlohám za jednu vyučovací hodinu. Dále bylo zjištěno, že nejčastěji řeší úlohy učitel v interakci se žáky, nejvíce úloh vyžaduje slovní řešení a nejméně jsou zastoupeny úlohy řešené experimentálně.

Téma pedagogických znalostí bylo do českého odborného prostředí uvedeno koncem 90. let, a to zejména studiemi Vlastimila Švece a jeho spolupracovníků. Cílem předkládané přehledové studie je zmapovat, jak se koncept „pedagogické znalosti“ (v pojetí V. Švece) etabloval v oblasti teorie, praxe a výzkumu učitelského vzdělávání v České republice.

Studie představuje vybrané výsledky výzkumu, v němž byly zkoumány subjektivní teorie učitelů fyziky na 2. stupni základní školy. Na pozadí aktuálních proměn kurikula české základní školy autor věnuje pozornost kategorii „cílů výuky“. Výzkum byl realizován ve snaze získat představu o tom, k jakým cílům by měla podle názoru učitelů směřovat výuka fyziky a jak se s těmito cíli ve výuce pracuje. Do tohoto výzkumu, jenž volně navazuje na CPV videostudii fyziky (Janík a Miková, 2006), se zapojilo jedenáct učitelů, s nimiž bylo vedeno olostrukturované interview. Výroky učitelů byly následně kódovány s využitím kategoriálního systému, který postihoval různé aspekty jejich subjektivních teorií (cíle výuky, pojetí vyučovacího předmětu, vyučování a učení, žákovské představy, role experimentů). Na tomto místě představujeme pouze výsledky vztahující se k cílům výuky. Nabízíme odpovědi na dvě otázky: 1) K jakým cílům by podle učitelů měla výuka fyziky směřovat? 2) Jakým způsobem učitelé s cíli ve výuce fyziky pracují? Výsledky naznačují, že učitelé za nejdůležitější cíle považují vedení žáků k uvědomění si významu fyziky pro porozumění každodenním problémům a k porozumění základním fyzikálním pojmům a principům. Co se práce s cíli ve výuce týče, nejčastěji se objevuje explicitní práce s cíli, přičemž kategorie cílů je ve většině případů ztotožňována s kategorií obsahů. K práci s cíli, která by podporovala reflexivitu a vedla žáky k uvědomění si jejich učební situace, učitelé odkazovali pouze ojediněle.

GPA33 is a promising surface antigen for targeted therapy in colorectal cancer (CRC). It is expressed almost exclusively in CRC and intestinal epithelia. However, previous clinical studies have not achieved expected response rates. We investigated GPA33 expression and regulation in CRC and developed a GPA33-targeted cellular therapy. We examined GPA33 expression in CRC cohorts using immunohistochemistry and immunofluorescence. We analyzed GPA33 regulation by interference with oncogenic signaling in vitro and in vivo using inhibitors and conditional inducible regulators. Furthermore, we engineered anti-GPA33-CAR T cells and assessed their activity in vitro and in vivo. GPA33 expression showed consistent intratumoral heterogeneity in CRC with antigen loss at the infiltrative tumor edge. This pattern was preserved at metastatic sites. GPA33-positive cells had a differentiated phenotype and low WNT activity. Low GPA33 expression levels were linked to tumor progression in patients with CRC. Downregulation of WNT activity induced GPA33 expression in vitro and in GPA33-negative tumor cell subpopulations in xenografts. GPA33-CAR T cells were activated in response to GPA33 and reduced xenograft growth in mice after intratumoral application. GPA33-targeted therapy may be improved by simultaneous WNT inhibition to enhance GPA33 expression. A significant unmet need remains for patients with Hodgkin lymphoma (HL) who fail to respond to first-line treatment or experience an early relapse. Tinostamustine, a novel alkylating deacetylase inhibitor, inhibits tumor cell growth and slows disease progression in models of hematological malignancies and solid tumors. This was a Phase I, multicenter, open-label, two-stage trial investigating the safety and efficacy of tinostamustine in patients ≥ 18 years with relapsed/refractory (R/R) hematological malignancies, including HL. Stage 1 involved dose-escalation to determine the maximum tolerated dose (MTD) of tinostamustine, optimal infusion time and recommended Phase II dose (RP2D). Stage 2 confirmed the safety and efficacy of the RP2D in expansion cohorts of selected R/R hematological malignancies. Ten patients with heavily pre-treated HL entered dose-escalation, with nine patients experiencing treatment-emergent adverse events (TEAEs) considered to be related to study treatment-primarily hematological toxicities. MTD was 100 mg/m 2 tinostamustine over 60 min and signals of efficacy were observed for patients with HL. In Stage 2, all 20 patients with HL experienced ≥ 1 TEAE, which were principally hematological or gastrointestinal. There were no tinostamustine-related deaths in either stage of the study. Overall response rate in Stage 2 was 37% (2 complete responses, 5 partial responses; 95% confidence interval [CI]: 16%, 62%) and median progression-free survival 3.8 months (95% CI: 2.2-9.4 months). Tinostamustine is a promising new therapeutic approach for the treatment of patients with R/R classical HL with limited options. Background: We have recently constructed a DNA methylation classifier that can discriminate between pancreatic ductal adenocarcinoma (PAAD) liver metastasis and intrahepatic cholangiocarcinoma (iCCA) with high accuracy (PAAD-iCCA-Classifier). PAAD is one of the leading causes of cancer of unknown primary and diagnosis is based on exclusion of other malignancies. Therefore, our focus was to investigate whether the PAAD-iCCA-Classifier can be used to diagnose PAAD metastases from other sites., Methods: For this scope, the anomaly detection filter of the initial classifier was expanded by 8 additional mimicker carcinomas, amounting to a total of 10 carcinomas in the negative class. We validated the updated version of the classifier on a validation set, which consisted of a biological cohort (n = 3579) and a technical one (n = 15). We then assessed the performance of the classifier on a test set, which included a positive control cohort of 16 PAAD metastases from various sites and a cohort of 124 negative control samples consisting of 96 breast cancer metastases from 18 anatomical sites and 28 carcinoma metastases to the brain., Results: The updated PAAD-iCCA-Classifier achieved 98.21% accuracy on the biological validation samples, and on the technical validation ones it reached 100%. The classifier also correctly identified 15/16 (93.75%) metastases of the positive control as PAAD, and on the negative control, it correctly classified 122/124 samples (98.39%) for a 97.85% overall accuracy on the test set. Purpose: Immune checkpoint inhibitors (CPIs) have been widely adopted in a number of early and advanced malignancies. Histone deacetylase inhibitors (HDACis) and alkylating agents (AAs) have been suggested to potentiate the actions of CPIs on tumor cells. We conducted a comprehensive literature review to explore the potential synergistic activity between CPIs, AAs, and HDACis., Patients and Methods: Clinical and non-clinical studies describing outcomes in patients with cancer receiving CPIs and either concomitant or sequential (pre- or post-CPI) AAs or HDACis were identified in PubMed using pre-defined search strings. Manual searches of key oncology congresses were similarly performed. All relevant articles and abstracts were manually screened for relevance, classified according to the specific anticancer agents used (CPIs, AAs, or HDACis), tumor entity, and whether treatment was concomitant or sequential., Results: Overall, 227 unique clinical studies across a range of tumor types, both solid tumors and hematological malignancies, were identified. One hundred and fifty-nine publications on Phase I and II clinical studies together with 41 publications on Phase III studies were examined. The most commonly investigated tumor types were melanoma, triple-negative breast cancer, non-small cell lung cancer, and Hodgkin lymphoma. The randomized clinical studies identified, all of which reported on the combination of a CPI with an AA, demonstrated superior outcomes in the combination arm compared with CPI or AA monotherapy. Similarly, combination therapy with CPIs and HDACis demonstrated promising activity., Conclusion: Sequential or concomitant administration of a CPI with an AA or an HDACi may improve outcomes for patients with a range of tumor types. The nature of the lower crust and the crust-mantle transition is fundamental to Earth sciences. Transformation of lower crustal rocks into eclogite facies is usually expected to result in lower crustal delamination. Here we provide compelling evidence for long-lasting presence of lower crustal eclogite below the seismic Moho. Our new wide-angle seismic data from the Paleoproterozoic Fennoscandian Shield identify a 6-8 km thick body with extremely high velocity (Vp ~ 8.5-8.6 km/s) and high density (>3.4 g/cm 3 ) immediately beneath equally thinned high-velocity (Vp ~ 7.3-7.4 km/s) lowermost crust, which extends over >350 km distance. We relate this observed structure to partial (50-70%) transformation of part of the mafic lowermost crustal layer into eclogite facies during Paleoproterozoic orogeny without later delamination. Background: To evaluate the outcome and complication rate in a single institution experience using the two most commonly used techniques of ureteroenteric anastomosis, the Bricker and Wallace anastomosis., Methods: A total of 137 patients underwent ileal conduit for bladder cancer. Ureters were anastomosed by two experienced surgeons, one performing a Bricker and the other, a Wallace anastomosis. Stricture was identified during clinical follow-up., Results: Seventy-five patients underwent a Bricker anastomotic, and 65 received a Wallace anastomosis. The average age was 70 in both groups, males were predominant (66% Bricker, 70% Wallace). Follow up period was 36.5 months in Bricker group and 17 months in Wallace group. In both groups, the body mass index (BMI) was similar (26.1 kg/m 2 Bricker and 26.4 kg/m 2 Wallace). We observed that the stricture rate after performing the Bricker anastomosis technique was 25.3% (19/75) as compared to 7.7% (5/65) after Wallace anastomosis technique, which was statistically significant (p = 0.001). In the Bricker group, patients with strictures had higher BMI (28.3 vs. 25.7 kg/m 2 , p = 0.05). On average it took 8.5 months in the Bricker group and three months in the Wallace group (p = 0.6) to develop stricture., Conclusions: The stricture rate was significantly higher when Bricker technique was applied. Although the BMI was not different in both groups, patients with a higher BMI were more likely to develop stricture. We believe that the approach of the separate and refluxing technique of Bricker anastomosis especially in obese patients poses a higher risk for anastomotic stricture formation.

Background: Oral isotretinoin (ISO) is the standard of care for severe inflammatory acne and a threshold dose of 120-150 mg/kg is widely regarded as increasing remission potential., Objective: Our objective was to evaluate the evidence underlying ISO dosing of 120-150 mg/kg in acne remission., Methods: A systematic literature search was performed using keywords "acne," "isotretinoin," "efficacy," "dosing," "relapse," and "remission.", Results: Definitions for acne clearance, relapse/remission, and treatment endpoint vary widely across studies. Only 2 studies explicitly evaluated the cumulative dose of 120-150 mg/kg for induction of acne remission-both low grade., Conclusion: The threshold dose of 120-150 mg/kg for oral ISO is based on past parameters of treatment duration and prior studies used vague or inconsistent definitions of clearance and remission. The outcome of a cutaneous immune response is critically dependent upon the ability of dendritic cells (DC) to migrate from skin to the draining lymph nodes - a process that is influenced by the cutaneous tissue microenvironment. Here, the role of fibroblasts - a major component of the dermal microenvironment - on the migratory capacity of monocyte-derived DC (MoDC) was investigated in a 3D collagen I matrix. Indeed, dermal fibroblasts supported the migration of pre-activated MoDC through a 3D collagen I matrix. Activation of human MoDC resulted in the release of TNFα and IL-1β that in turn stimulated MMP-1 (human collagenase) and PGE2 secretion by human dermal fibroblasts. Transmigration assays confirmed the importance of fibroblast-derived MMP-1 and PGE2 for the migration of MoDC through a 3D collagen I matrix. Finally, in mice initiation of inflammation by induction of an irritant contact dermatitis or a psoriasis-like skin inflammation, the expression of the PGE2 generating cox-2 and the mouse collagen I degrading enzyme matrix metalloproteinases (MMP)-13 was strongly up-regulated. Our study indicates that MoDC are able to instruct dermal fibroblasts resulting in enhanced migratory capability of MoDC, thus highlighting the role of a crosstalk of DC with their stromal microenvironment for the control of cutaneous immune responses., (© 2015 John Wiley & Sons A/S. Background: Liver transplantation (LT) using organs donated after cardiac death (DCD) is increasing due, in large part, to a shortage of organs. The outcome of using DCD organs in recipients with hepatits C virus (HCV) infection remains unclear due to the limited experience and number of publications addressing this issue., Objective: To evaluate the clinical outcomes of DCD versus donation after brain death (DBD) in HCV-positive patients undergoing LT., Methods: Studies comparing DCD versus DBD LT in HCV-positive patients were identified based on systematic searches of seven electronic databases and multiple sources of gray literature., Results: The search identified 58 citations, including three studies, with 324 patients meeting eligibility criteria. The use of DCD livers was associated with a significantly higher risk of primary nonfunction (RR 5.49 [95% CI 1.53 to 19.64]; P=0.009; I2=0%), while not associated with a significantly different patient survival (RR 0.89 [95% CI 0.37 to 2.11]; P=0.79; I2=51%), graft survival (RR 0.40 [95% CI 0.14 to 1.11]; P=0.08; I2=34%), rate of recurrence of severe HCV infection (RR 2.74 [95% CI 0.36 to 20.92]; P=0.33; I2=84%), retransplantation or liver disease-related death (RR 1.79 [95% CI 0.66 to 4.84]; P=0.25; I2=44%), and biliary complications., Conclusions: While the literature and quality of studies assessing DCD versus DBD grafts are limited, there was significantly more primary nonfunction and a trend toward decreased graft survival, but no significant difference in biliary complications or recipient mortality rates between DCD and DBD LT in patients with HCV infection. There is insufficient literature on the topic to draw any definitive conclusions.

Objective: Interactions of immune cells, such as activated T helper cells, with fibroblast-like synoviocytes (FLS) play a crucial role in the joint destruction during human rheumatoid arthritis (RA). This study was undertaken to investigate the expression of the proinflammatory cytokine macrophage migration inhibitory factor (MIF) by T helper cells, and to assess the role of MIF in overexpression of matrix metalloproteinases (MMPs) in cocultures of FLS from arthritic mice with either Th1 or Th2 cells., Methods: MIF expression by in vitro-polarized murine Th1 and Th2 cells was determined using 2 different generation protocols. FLS were isolated from the inflamed joints of mice with antigen-induced arthritis. MMP expression was analyzed in cocultures of the FLS with T helper cell subsets. Effects of MIF were blocked by a neutralizing anti-MIF antibody. In addition, analyses were performed on cocultures of either Th1 or Th2 cells with FLS from MIF-deficient mice., Results: Both Th1 and Th2 cells expressed high quantities of MIF. MMPs were overexpressed by FLS after coculture with both Th1 and Th2 cells. Activated T helper cells were more effective than resting cells. Neutralization of MIF by an anti-MIF antibody led to a marked reduction in MMP expression in Th1- and Th2-stimulated FLS. T helper cells generated from MIF-deficient mice exhibited a T helper cell-specific cytokine profile comparable with that in wild-type cells, except in the expression of MIF, but showed an impaired ability to stimulate MMP expression in FLS., Conclusion: MIF is an important Th1 and Th2 cell-derived proinflammatory cytokine that stimulates MMP expression in FLS from arthritic mice, and therefore inhibition of MIF might be a promising target for novel therapeutic strategies in human RA.

Aims: Coxsackievirus B3 (CVB3)-induced chronic myocarditis in mice is accompanied by severe fibrosis and by sustained elevation of platelet-derived growth factor (PDGF)-A, -B, and -C levels in the cardiac tissue. To test if PDGF stimulation of resident fibroblasts causally contributes to fibrosis, we employed inhibition of PDGF receptor signalling with the orally available kinase inhibitor Imatinib., Methods and Results: Chronic myocarditis was induced by CVB3 infection of major histocompatibility complex (MHC) class II knockout (B6Aa(0)/Aa(0)) mice. The mice were treated with 100 mg/kg Imatinib or vehicle, respectively, twice daily for 34 days. Expression of PDGF-C and of inflammatory cytokines were analysed by semi-quantitative RT-PCR. PDGFalpha receptor phosphorylation was detected by immunoblotting of cardiac tissue extracts and in situ by immunohistochemistry. Fibrosis formation was analysed by Sirius-Red staining and hydroxyproline (HP) determination. Fibronectin, and tenascin expression was analysed by RT-PCR and immunohistochemistry. Matrix metalloproteinase (MMP) activity was assessed with collagen, synthetic peptides, and gelatine as substrates. Imatinib significantly inhibited the myocarditis-related PDGFalpha receptor activation in the heart tissue. The virus titres in the hearts, inflammatory infiltrations, and elevated PDGF levels were unaffected by the Imatinib treatment. A significant attenuation of fibrosis occurred in Imatinib-treated animals. The Sirius Red-stained fibrotic area was reduced from 5.30 +/- 0.50 to 3.21 +/- 0.35%, and the HP content was reduced from 362 +/- 43 to 238 +/- 32 microMol/10 mg dry weight vs. 190 +/- 27 in uninfected controls. The expression of fibronectin, EIIIA+ fibronectin, and tenascin C were likewise reduced. The diminished matrix protein deposition was not caused by elevated MMP activity, since MMP activity was not changed or even reduced under Imatinib., Conclusion: The data suggest a causal role for elevated PDGF expression and PDGF receptor activity in the pathogenesis of cardiac fibrosis.

Objective: Cathepsin K is believed to have an eminent role in the pathologic resorption of bone. However, several studies have shown that other proteinases also participate in this process. In order to clarify the contribution of cathepsin K to the destruction of arthritic bone, we applied the human tumor necrosis factor (hTNF)-transgenic mouse model, in which severe polyarthritis characterized by strong osteoclast-mediated bone destruction develops spontaneously., Methods: Arthritis was evaluated in hTNF-transgenic mice that were either wild-type for cathepsin K (CK(+/+)), heterozygous for cathepsin K (CK(+/-)), or deficient in cathepsin K (CK(-/-)). Arthritic knee joints were prepared for standard histologic assessment aimed at establishing a semiquantitative score for joint destruction and quantification of the area of bone erosion. Additionally, microfocal computed tomography was performed to visualize bone destruction in mice with the different CK genotypes. CK(+/+) and CK(-/-) osteoclasts were generated in vitro, and their proteinase expression profiles were compared by complementary DNA array analysis, real-time polymerase chain reaction, and activity assays., Results: Although the area of bone erosion was significantly reduced in hTNF-transgenic CK(-/-) mice, the absence of cathepsin K did not completely protect against inflammatory bone lesions. Several matrix metalloproteinases (MMPs) and cathepsins were expressed by in vitro-generated CK(-/-) osteoclasts, without marked differences from CK(+/+) osteoclasts. MMP activity was detected in CK(-/-) osteoclasts, and MMP-14 was localized by immunohistochemistry in inflammatory bone erosions in hTNF-transgenic CK(-/-) mice, suggesting MMPs as potential contributors to bone destruction. Additionally, we detected a reduction in osteoclast formation in cathepsin K-deficient mice, both in vitro and in vivo., Conclusion: The results of our experiments raise doubts about a crucial role of cathepsin K in arthritic bone destruction.

Starting in 1991, the Medical College of Wisconsin's (MCW) primary care-focused faculty development programs have continuously evolved in order to sustain tight alignment among faculty members' needs, institutional priorities, and academic reward structures. Informed by literature on the essential competencies associated with academic success and using educational methods demonstrated to achieve targeted objectives, MCW's initial 1.5-day per month comprehensive faculty development programs prepared faculty as clinician-researchers, leaders, and educators. As institutional priorities and faculty roles shifted, a half-day per month advanced education program was added, and the comprehensive faculty development program transitioned to its current half-day per month program. Using a modular approach, this program focuses exclusively on clinician-educator competencies in curriculum, teaching, leadership, evaluation, and learner assessment. Instructional methods combine interactive, face-to-face sessions modeling a range of instructional strategies with between-session assignments now supported through an e-learning platform. All participants complete a required project, which addresses a divisional or departmental need, meets standards associated with scholarship, and is submitted to a peer-reviewed forum. To date, over 115 faculty members have enrolled in MCW's faculty development programs. Program evaluation over the 15-year span has served to guide program revision and to provide clear evidence of program impact. A longitudinal evaluation of comprehensive program graduates from 1993 to 1999 showed that 88% of graduates' educational projects were implemented and sustained more than one year after program completion. Since 2001, each participant, on average, attributes more than two peer-reviewed presentations and one peer-reviewed publication to program participation. Based on 15 years of evaluation data, five tenets associated with program success are outlined.

Objectives: To evaluate reliability of a simple instrument, the flexicurve, in determining cervical sagittal skin contour., Methods: This study obtained repeated random measurements involving 3 investigators and 30 subjects once per day over a 2-day trial period. Thirty normal subjects were examined for cervical spine skin contour twice by 3 separate investigators with a 1-day delay. With subjects in a neutral standing position, investigators placed the flexicurve on the posterior portion of the subject's neck from the external occipital protuberance to the vertebral prominens and traced the flexicurve shape onto paper. The tracings were divided into 6 equal arcs and digitized. Statistical computation was performed on the depth at 5 points, arc angle, and arc radius of curvature. Interexaminer and intraexaminer correlation coefficients (ICCs) were calculated to determine reliability., Results: All interexaminer correlation coefficients were in the poor range (<0.40). For the arc radius, arc angle, depth at top one third, and depth at bottom two thirds, the intraexaminer correlation coefficients were in the poor range. For the 3 deepest depths, the intraexaminer correlation coefficients were in the fair range (0.4-0.50)., Conclusion: The flexicurve showed marginal reliability with most (12/16) ICCs in the poor range (ICC <0.40) and 4 values in the fair range (0.4 < ICC < 0.5).

Objective: To investigate the reliability of a new radiographic measurement of axial rotation and lateral bending on anterior-posterior cervical views by using a computer and sonic digitizer., Design: A blind, repeated-measure design was used. Anteroposterior cervicothoracic radiographs were presented to each of 3 examiners in random order. Each film was digitized, and 1 week later the films were randomized for a second run., Setting: Private, primary-care chiropractic clinic., Main Outcome Measures: The interclass and intraclass correlation coefficients (ICC) for intraexaminer and interexaminer reliability were calculated from measurements on radiographs for determining axial rotations (Ry) and lateral bending (Rz) of C3 to T3., Results: When the new axial rotation method was applied to small rotations of a C3 plastic model, the average error was less than 1 degrees. For the calculations of axial rotation (Ry), the ICC values were in the good to excellent range. For axial rotation, the intraclass correlation coefficients were ICCs > or =0.78, and the interclass correlation coefficients were ICCs > or =0.67. For lateral flexions (Rz) of C3 to T3, all intraclass and interclass correlation coefficients were in the excellent range (ICCs > 0.87)., Conclusions: Methods of calculating axial rotations in the spine have been reported for large angles (5 degrees to 30 degrees ) but not for smaller angles. A new method for determining axial rotations of the cervical segments on AP views, based on the chord across the arc displaced by the spinous-lamina junction, had reliability (ICC values) in the good to excellent range. Compared with measured rotations of a C3 model (-5 degrees to +5 degrees ), the new method had an average error of less than 1 degrees and approximately 11.5%. The reliability for the axial rotation measurements was in the good to excellent range, and the lateral bending measurements were all in the excellent range.

Objective: To investigate the reliability of a radiographic measurement procedure that uses a computer and sonic digitizer to determine projected spinal displacements from an ideal, normal position., Design: A blind, repeated-measure design was used. Anteroposterior cervicothoracic spine radiographs were presented in random order to each of 3 examiners. Each film was digitized, and the films were randomized for a second examination., Setting: Private, primary care chiropractic clinic., Main Outcome Measures: Intraclass correlation coefficients for intraexaminer and interexaminer reliability for measures on radiographs comparing the perpendicular distance (T(x)) from a vertical axis line drawn through the center of T4 and the center of C2, the linear distance (vertebra(apex)) from the center of the vertebra most displaced from a line connecting the centers of C2 and T4, the angle (Rz) formed by the intersection of the vertical axis line and the upper thoracic line, and the angle of intersection (CDA) between the upper thoracic line and the cervical line., Results: Intraexaminer reliability for T(x) distance was 0.99 to 1.00, with confidence intervals from 0.98-1.00; for vertebra(apex) was 0.96 to 0.97, with confidence intervals from 0.92-0.98; for Rz was 0.94 to 0.98, with confidence intervals from 0. 89-0.99; and for CDA was 0.92 to 0.95, with confidence intervals from 0.84-0.97. Interexaminer reliabilities for the 3 examiners ranged from 0.97 to 0.99., Conclusions: Measures similar to those described in this study are commonly used to quantify and categorize spinal displacements from true vertical alignment (i.e., scoliosis measurements). Intraclass correlation coefficient values >0.70 are considered accurate enough for use in clinical and research applications. The measures tested here would fit within these guidelines of reliability. Establishing reliability is an important first step in evaluating these measures so that future studies of validity may be undertaken.
