The Southern Ocean is responsible for approximately 40% of oceanic carbon uptake through biological and physical processes. In the Southern Ocean, phytoplankton growth is limited by low iron (Fe) and light supply. Climate model projections for the Southern Ocean indicate that temperature, underwater irradiance and Fe supply are likely to change simultaneously in the future due to increasing anthropogenic carbon dioxide emissions. The individual effects of these environmental properties on phytoplankton physiology have been extensively researched, and culturing studies using Southern Ocean phytoplankton have shown that temperature and Fe will play a key role on setting growth under future conditions. To explore the potential responses of Southern Ocean phytoplankton to these environmental changes, we cultured the haptophyte Phaeocystis antarctica and the diatoms Chaetoceros flexuosus, Proboscia inermis, and Thalassiosira antarctica under two light and iron combinations and over a range of temperatures. Our study revealed that the thermal response curves of key Southern Ocean phytoplankton are diverse, with the highest growth rates measured at 5°C (the annual temperature range at the isolation sites is currently 1–4°C). Warming had species-specific effects on the photochemical efficiency of photosystem II (PSII; Fv/Fm), the functional absorption cross-section of PSII (σPSII), carbon:nitrogen ratio and cellular Chlorophyll a concentrations. Iron availability increased species’ ability to tolerate warmer conditions by increasing the upper limit for growth and subsequently increasing the thermal niche that each species inhabit.

Purpose: To evaluate the diagnostic performance of lung ultrasound (LUS) in screening for SARS-CoV-2 infection in patients requiring surgery., Methods: Patients underwent a LUS protocol that included a scoring system for screening COVID-19 pneumonia as well as RT-PCR test for SARS-CoV-2. The receiver operator characteristic (ROC) curve was determined for the relationship between LUS score and PCR test results for COVID-19. The optimal threshold for the best discrimination between non-COVID-19 patients and COVID-19 patients was calculated., Results: Among 203 patients enrolled (mean age 48 years; 82 males), 8.3% were COVID-19-positive; 4.9% were diagnosed via the initial RT-PCR test. Of the patients diagnosed with SARS-CoV-2, 64.7% required in-hospital management and 17.6% died. The most common ultrasound findings were B lines (19.7%) and a thickened pleura (19.2%). The AUC of the ROC curve of the relationship of LUS score with a cutoff value >8 versus RT-PCR test for the assessment of SARS-CoV-2 pneumonia was 0.75 (95% CI 0.61-0.89; sensitivity 52.9%; specificity 91%; LR (+) 6.15, LR (-) 0.51)., Conclusion: The LUS score in surgical patients is not a useful tool for screening patients with potential COVID-19 infection. Extramedullary hematopoiesis is rare outside the setting of significant primary hematologic disease. We describe this phenomenon in an exuberant pyogenic granuloma in an otherwise healthy man. We postulate that this vascular lesion provided a suitable milieu for homing and proliferation of stem cells.

Objectives: To examine the pharmacokinetics and the relation between plasma concentrations of the new potassium channel blocker dofetilide and QTc prolongation on the surface electrocardiogram after oral and intravenous administration., Methods: Ten healthy volunteers received a single dose of 0.5 mg dofetilide orally and intravenously (over 30 minutes) in a randomized crossover study. The QTc interval versus dofetilide plasma concentration was analyzed by use of pharmacokinetic and pharmacodynamic modeling techniques., Results: Dofetilide absolute bioavailability and systemic clearance were 92% +/- 9% and 0.35 +/- 0.05 L/hr/kg, respectively. Mean maximum increase in QTc interval duration was 99 msec (27%) and 61 msec (16%) after intravenous and oral administration, respectively. A counterclockwise hysteresis loop between dofetilide plasma concentrations and QTc interval duration was observed after intravenous infusions in all subjects, whereas direct linear relationships were observed after oral administrations in eight of 10 subjects. Pharmacokinetic-pharmacodynamic modeling showed the consistency of the effect versus concentration relationships obtained with the two routes of administration. With use of a maximum effect (Emax) model and data obtained after intravenous infusion, mean maximum QTc prolongation (Emax) was 121 +/- 57 msec and mean dofetilide plasma concentration associated with half the maximum effect (EC50) was 2.2 +/- 0.6 ng/ml. Pharmacokinetic-pharmacodynamic modeling was useful in detecting the maximum effect and in describing the plasma concentration versus effect relationship during intravenous infusion of dofetilide but was otherwise not superior to analyses performed with postdistribution data., Conclusion: We conclude that dofetilide prolongs QTc interval duration in a concentration-dependent manner in normal volunteers during sinus rhythm and that pharmacokinetic-pharmacodynamic modeling is useful for examination of maximum QTc prolongation induced by dofetilide.

This investigation is a review of the injuries due to fireworks seen in five casualty departments in Denmark over a period of four years. These casualty departments have a total catchment population of 675,000 (13% of the population of Denmark). The review is based on data from the Danish section of EHLASS-project. (Europea Home and Leisure Accident surveillance system). During the period of observation, 358 injuries due to fireworks were observed. 60% of the injuries occurred in persons under the age of 18 years. In 51%, the mechanism of the injury consisted of direct heat while 33% of the patients were hit by flying fireworks and 10% had small foreign bodies in the eye. The regions most frequently injured were the hands/fingers (40%) or the eyes (24%). 6% of the patients required admission to hospital. The estimated number of injuries due to fireworks in Denmark is 690 annually. The majority of injuries due to fireworks were due to incorrect use of fireworks of absence of protective measures.
