The beta regression model is commonly used when analyzing data that come in the form of rates or percentages. However, a problem that may encounter when analyzing these kinds of data that has not been investigated for this model is the multicollinearity problem. It is well known that the maximum likelihood (ML) method is very sensitive to high inter-correlation among the explanatory variables. Therefore, this paper proposes some ridge estimators for the beta regression model to remedy the problem of instability of the traditional ML method and increase the efficiency of estimation. The performance of ridge estimators is compared to the ML estimator through the mean squared error (MSE) and the mean absolute error (MAE) criteria by conducting a Monte-Carlo simulation study and through an empirical application. Objectives: The purpose of this study was to investigate the effects of additional 6-week scapular stabilization training in patients with nonspecific neck pain (NNP). Materials and Methods: A total of 30 patients with NNP were randomly allocated to the study. Fifteen participants in the intervention group received neck-focused exercise and scapular stabilization training, whereas 15 participants in the control group received neck-focused exercise training. All groups were evaluated at baseline and after 6 weeks of rehabilitation. The pain intensity on the neck was measured with the visual analog scale (VAS). The self-reported disability status was measured with the neck disability index (NDI). Three-dimensional scapular kinematics were recorded during dynamic shoulder elevation trials using an electromagnetic tracking device, and data were further analyzed at 30°, 60°, 90°, and 120° of humerothoracic elevations. Results: Comparisons revealed that, regardless of the received treatment, after 6 weeks of training both groups showed significant improvements in VAS (P <.001) and NDI (P <.001) scores. Both VAS and NDI outcomes have a large effect size (r =.618 and r =.619, respectively). For scapular kinematics, there were no group differences, especially for scapular upward–downward rotation and anterior–posterior tilt (P >.05). However, in the intervention group, the scapula was more externally rotated at 120° humerothoracic elevation (P =.04). Conclusion: Findings of this study showed that both manual therapy and active interventions, including neck-focused exercise and scapular stabilization training, are effective in decreasing pain and disability level in patients with NNP. Introduction. Shwachman-Diamond Syndrome (SDS) is an autosomal-recessive disorder characterized by neutropenia, pancreatic exocrine insufficiency, skeletal dysplasia, and an increased risk for leukemic transformation. Biallelic mutations in the SBDS gene have been found in about 90% of patients. The clinical spectrum of SDS in patients is wide, and variability has been noticed between different patients, siblings, and even within the same patient over time. Herein, we present two SDS siblings (UPN42 and UPN43) carrying the same SBDS mutations and showing relevant differences in their phenotypic presentation. Study aim. We attempted to understand whether other germline variants, in addition to SBDS, could explain some of the clinical variability noticed between the siblings. Methods. Whole-exome sequencing (WES) was performed. Human Phenotype Ontology (HPO) terms were defined for each patient, and the WES data were analyzed using the eVai and DIVAs platforms. Results. In UPN43, we found and confirmed, using Sanger sequencing, a novel de novo variant (c.10663G > A, p.Gly3555Ser) in the KMT2A gene that is associated with autosomal-dominant Wiedemann–Steiner Syndrome. The variant is classified as pathogenic according to different in silico prediction tools. Interestingly, it was found to be related to some of the HPO terms that describe UPN43. Conclusions. We postulate that the KMT2A variant found in UPN43 has a concomitant and co-occurring clinical effect, in addition to SBDS mutation. This dual molecular effect, supported by in silico prediction, could help to understand some of the clinical variations found among the siblings. Background: Diabetic retinopathy (DR) is a complication associated with uncontrolled DM. It is a leading preventable cause of visual impairment in the world and a cause of blindness in those under 75 years old in developing countries. We aimed to explore the prevalence and associated risk factors of DR among diabetic patients in the West Bank. Materials and Methods:A quantitative multicenter cross-sectional study was conducted in all West Bank cities. Nearly, 385 patients underwent a comprehensive eye examination in addition to blood and urine tests. A previously validated questionnaire for ocular examination classification was used together with a socio-demographic and past medical history information sheet. Results: The prevalence of all DR in the West Bank was 41.8%. The prevalence of non-proliferative diabetic retinopathy (NPDR) was 50.3% (38.5% for mild NPDR, 10.6% for moderate NPDR and 1.2% for severe NPDR). The prevalence of proliferative diabetic retinopathy (PDR) was 9.9% and 39.7% for diabetic macular edema (DME) (17.4% for mild, 15.5% for moderate and 6.8% for severe DME). The prevalence of vision-threatening PDR and DME was 49.7% for both. In a univariate analysis, DR was significantly associated with body mass index; BMI (p = 0.035), DM duration (p = 0.002), Low-density lipoprotein (LDL) (p = 0.034), glutamic-oxaloacetic transaminase (GOT) level (p = 0.016) andblood urea (BU) (p = 0.044). A multivariate analysis showed a strong significant association between DR andpatients who had DM for 10-19years (adjusted odds ratio; AOR (95%CI); 1.843 (1.05–3.22)), abnormal levels of LDL (AOR (95%CI); 0.50 (0.30–0.83)), abnormal levels of GOT (AOR (95%CI); 0.49 (0.27–0.89)), and overweight (AOR (95%CI); 0.39 (0.19–0.80)). Conclusions: We found that the prevalence of DR in Palestine was higher than the global prevalence. Referral coordination between ophthalmologists and internal physicians is necessary to better follow up with DR patients. The strawberry has a very short postharvest life due to its fast softening and decomposition. The goal of this research is to see how well a starch-silver nanoparticle (St-AgNPs) coating affects the physical, chemical, and microbiological qualities of strawberries during postharvest life. Additionally, the effect of washing with running water on silver concentration in coated strawberry fruit was studied by an inductively coupled plasma-optical emission spectrometer (ICP-OES). Furthermore, the shelf-life period was calculated in relation to the temperature of storage. Fourier transform infrared-attenuated total reflectance (FTIR-ATR), UV-Visible, and Transmission Electron Microscopic (TEM) were used to investigate the structure of starch-silver materials, the size and shape of AgNPs, respectively. The AgNPs were spherical, with an average size range of 12.7 nm. The coated samples had the lowest weight loss, decay, and microbial counts as compared to the uncoated sample. They had higher total acidity and anthocyanin contents as well. The washing process led to the almost complete removal of silver particles by rates ranging from 98.86 to 99.10%. Finally, the coating maintained strawberry qualities and lengthened their shelf-life from 2 to 6 days at room storage and from 8 to 16 days in cold storage. [ABSTRACT FROM AUTHOR]

To translate and culturally adapt the shoulder instability-return to sport after injury (SI-RSI) scale into Turkish (SI-RSI-Tr) and examine the psychometric properties of the Turkish version of athletes following a traumatic shoulder instability. The SI-RSI was translated into Turkish using Beaton guidelines. Sixty-nine patients with shoulder instability completed the translated SI-RSI, Western Ontario Shoulder Instability Index (WOSI), the Tampa Scale of Kinesiophobia (TSK), and the Walch-Duplay Scores. We analysed the internal consistency, agreement, reliability, and validity of the SI-RSI-Tr. The SI-RSI-Tr demonstrated excellent internal consistency (Cronbach's alpha = 0.92), test–retest reliability (ICC = 0.95), and feasibility with no ceiling or floor effect. SI-RSI-Tr correlated with WOSI total score (r = −0.824, p < 0.001), its subscales: WOSI-physical (r = −0.683, p < 0.001), WOSI-sports (r = −0.832, p < 0.001), WOSI-lifestyle (r = −0.739, p < 0.001), and WOSI-emotions (r = −0.734, p < 0.001) respectively), Walch-Duplay (r = 0.840, p < 0.001) and TSK (r = −0.828, p < 0.001) scores. The Turkish SI-RSI is a reliable, internally consistent, and valid tool for athletes with shoulder instability. Researchers and clinicians could safely use the SIRSI-Tr to evaluate the shoulder-specific psychological factors on return to sports following an episode of shoulder instability. The prediction of fault type in transformers at an early stage is an important aspect for power system reliability. Checking the transformer status begins with the dissolved gas analysis (DGA) test. Rogers' four-ratio and IEC 60599 standard methods are commonly DGA-based techniques that used to detect the transformer faults. These methods are very easy to implement, but their diagnostic accuracies are very poor. The target of this work is to build a new PSO-FS optimization platform identifying the optimal limits of gas ratios and corresponding rules for each fault type to enhance the diagnostic accuracies of Rogers' four-ratio and IEC 60599 code methods. In the proposed platform, PSO optimization technique specifies the gas ratio limits and corresponding rules where the fuzzy system identifies the fault types. Based on the results of the optimization process, the new codes that used to diagnose the fault types were developed. The performance of the proposed approach investigated using 481 collected datasets. The overall accuracies of the Rogers' four-ratio and IEC code models enhanced from 47.19 and 55.09 to 85.65 and 85.03, respectively. However, when datasets due to mixtures of faults (samples with undetermined or misidentified faults) are removed from the calculations, the accuracies of the PSO-FS methods are not significantly better than when using existing DGA methods. Although it is thought that there is a relationship between neck pain and scapular dysfunction, there are controversial results and no clear information in the literature regarding this issue. This study aimed to investigate the alterations in the 3-Dimensional scapular orientation on patients with non-specific neck pain. Thirty-four patients with chronic non-specific neck pain [age, 29.1 (5) years; height, 165.3 (6.1) centimeters; weight, 62.1 (9.6) kilogram] and 29 healthy controls [age, 28.2 (3.3) years; height, 166.8 (8.1) centimeters; weight, 60.9 (8.5) kilogram] were included in the study. 3-Dimensional scapular kinematics were obtained during arm elevation and lowering trials in the sagittal, scapular, and frontal planes. Compared to healthy controls, the patients with neck pain demonstrated significantly reduced upward scapular rotation on both the dominant and non-dominant sides in all three planes. Patients with neck pain had increased internal rotation in the sagittal and frontal planes on the dominant side and increased external rotation in the frontal plane on the non-dominant side. No difference was observed between groups considering posterior scapular tilt. There are alterations in the 3-Dimensional scapular orientations in patients with chronic non-specific neck pain compared to healthy controls. Background: Poultry feather waste has a potential for bioenergy production because of its high protein content. This research explored the use of chicken feather hydrolysate for methane and hydrogen production via anaerobic digestion and bioelectrochemical systems, respectively. Solid state fermentation of chicken waste was conducted using a recombinant strain of Bacillus subtilis DB100 (p5.2). Results: In the anaerobic digestion, feather hydrolysate produced maximally 0.67 Nm3 CH4/kg feathers and 0.85 mmol H2/day.L concomitant to COD removal of 86% and 93%, respectively. The bioelectrochemical systems used were microbial fuel and electrolysis cells. In the first using a microbial fuel cell, feather hydrolysate produced electricity with a maximum cell potential of 375 mV and a current of 0.52 mA. In the microbial electrolysis cell, the hydrolysate enhanced the hydrogen production rate to 7.5 mmol/day.L, with a current density of 11.5 A/m2 and a power density of 9.26 W/m2. Conclusions: The data indicated that the sustainable utilization of keratin hydrolysate to produce electricity and biohydrogen via bioelectrical chemical systems is feasible. Background: The aim of this study was to investigate the scapular kinematics during dynamic humeral movements in patients with arthroscopic anterior capsulolabral repair of the shoulder along with the potential biomechanical corrective effects of scapular stabilization taping.Methods: Twenty patients with unilateral traumatic anterior shoulder instability and arthroscopic anterior capsulolabral repair participated in the study. Dynamic shoulder kinematics were assessed during the scapular plane shoulder elevation for both the operated and non operated shoulders and also under two conditions: no-taping and taping. Statistical analysis to compare sides and conditions was performed with analysis of variance models.Results: The scapula was more internally rotated position in operated shoulders than in non operated shoulders. Furthermore, the scapula was less internally rotated and more downwardly rotated at 120° of elevation in the taping condition.Conclusions: Operated shoulders demonstrated kinematics alterations when compared to non operated shoulders underwent arthroscopic anterior capsulolabral repair. Additionally, changes in the scapular orientation with the taping was very small but followed a pattern, which would be suggested to be an orientation that potentially produce more scapular stability and to increase stress on the inferior glenohumeral ligament. Therefore, this work presents a new version of grey wolf optimiser (GWO) to solve the problem of optimal reactive power resources sizing for power system operation enhancement. The proposed method which called improved grey wolf optimiser (IGWO) can be derived by modifying the exploration-exploitation balance in the conventional GWO to enhance its rate of convergence. Also, the weighted distance strategy is employed in the proposed IGWO to overcome the drawback of the conventional GWO. Optimal reactive power resources sizing problem is nonlinear and non-convex optimisation problem. To solve this problem, different objective functions are used. These objective functions are minimisation of generating cost, minimisation of transmission power loss and voltage profile improvement. The validity and superiority of the proposed IGWO method are tested using three standard IEEE systems for normal and contingency conditions. Then the results are compared with those obtained from other recently published algorithms. There is very limited information about the changes in shoulder kinematics in patients with reverse shoulder arthroplasty. The aim of the study was to investigate the changes in the scapulohumeral rhythm and shoulder kinematics over time after the reverse shoulder procedure. Nineteen patients with reverse shoulder arthroplasty (age: 65.8 ± 10.3 years) were included to the study. During arm elevation in the sagittal and scapular planes, operated shoulder kinematics (humerothoracic elevation, glenohumeral elevation, scapulohumeral rhythm, and scapular rotations) were assessed using an electromagnetic tracking system at the postoperative 3rd, 6th, and 18th months. Asymptomatic shoulder kinematics were also assessed at the postoperative 18th month. Shoulder function was assessed using The Disabilities of the Arm Shoulder and Hand score at the postoperative 3rd, 6th, and 18th months. Maximum humerothoracic elevation increased from 98° to 109° over the postoperative period (p = 0.01). The scapulohumeral rhythm was similar on the operated and asymptomatic shoulders at the final follow-up (p = 0.11). Both the operated and asymptomatic shoulder demonstrated similar scapular kinematics at the postoperative 18th month (p > 0.05). The Disabilities of the Arm Shoulder and Hand score decreased over time in the postoperative period (p < 0.05). Shoulder kinematics may be improved after reverse shoulder arthroplasty in the postoperative period. Objective There is no consensus about the optimal frequency of patient visits during the rehabilitation program after arthroscopic rotator cuff repair (ARCR). This study aimed to investigate the short- and long-term effects of high-frequency (HF) and low-frequency (LF) visits of the patients in the first 12 weeks of rehabilitation after ARCR. Methods This was a quasi-randomized study with 2 parallel groups. Forty-seven patients with ARCR were included in 2 different patient visit frequency protocols (HF = 23, LF = 24) in 12 weeks of postoperative rehabilitation. Patients in the HF group visited the clinic twice a week, whereas patients in the LF group visited once every 2 weeks for the first 6 weeks and once a week for the following 6 weeks. Both groups performed the same exercise protocol. Outcome measurements were pain and range of motion measured at baseline; at the 3rd, 5th, 8th, 12th, and 24th weeks; and at 1-year follow-up. Shoulder function was assessed at the 12th and 24th weeks and at 1-year follow-up with an American Shoulder and Elbow Surgeons score. Results There was a significant group × time interaction in pain intensity during the activity between the groups. The activity pain intensity was higher in the LF group (4.2 points) at 8 weeks post surgery than in the HF group (2.7 points) (mean difference: 1.5 points, P  < .05), whereas it was similar in both groups at other time periods. The interaction term was not significant between the groups for pain intensity during rest and night through the 1-year follow-up. No group × time interactions were observed in shoulder range of motion and American Shoulder and Elbow Surgeons score over the postoperative period. Conclusion Both rehabilitation programs at different visit frequencies after ARCR showed similar clinical results in the long term. A supervised, controlled rehabilitation program with LF visits in the first 12 weeks after surgery can be sufficient to achieve optimal clinical results and reduce rehabilitation-related costs after ARCR. Impact This study highlights that LF treatment protocols under the supervision of the therapist can be adopted after the arthroscopic rotator cuff repair to achieve successful results while decreasing the treatment costs. Physical therapists should plan the treatment sessions efficiently for the compliance of the patients to the exercise treatment. Lay Summary If you are a patient with arthroscopic rotator cuff repair, a supervised, controlled rehabilitation program with low-frequency visits in the first 12 weeks after surgery could help you achieve the best outcome and help lower the costs of rehabilitation. In this study, a conditional probability scheme is designed to diagnose transformer incipient faults based on the percentage of five dissolved gases (hydrogen (H2), methane (CH4), ethane (C2 H6), ethylene (C2 H4), and acetylene (C2 H2)) with respect to their summation. Based on the fault features, the point probabilities of each fault type occurrence and non‐occurrence are computed. Then, the conditional probability of certain fault occurrence is estimated to specify the probabilistic indication of each fault type occurrence. Multivariate normal probability density function is considered to point out the conditional probability of the fault occurrence. It is implemented in different scenarios to define the fault type (partial discharge, low energy discharge, high energy discharge, low thermal, medium thermal, and high thermal) where the best scenario is selected. The proposed technique has the merits of simplicity and ease of implementation. It is assessed through the analysis of 403 dissolved gas sample dataset collected from the Egyptian electric utility as well as from credited literatures. The proposed probabilistic technique is evaluated in comparison with other methods in literatures. Also, it is validated against uncertainty in DGA data up to 20%. In this study, an evolutionary strategy is presented to get the optimal grounding grid design to minimise the design cost, the touch and step voltages of the grounding grid to ensure the economical constraint as well as the safety conditions. The proposed technique is used for one and two‐layer soil. This technique is based on the comparison between the quality factors of the initial grid dimension according to the cost, touch and step voltages with the other quality factors obtained from new generations (offspring) of the grid dimensions. Effects of top layer thickness, number of grid meshes, vertical rods number and length, and the soil resistivity of the top and bottom layers on grounding resistance (Rg) and earth surface potential are studied. Different case studies are introduced to evaluate the performance of the proposed technique and to provide the knowledge about the grid dimensions that is substantial for the economical design of the grounding grids. Dissolved Gas Analysis (DGA) is one of the most common techniques to detect the incipient faults in the oil-filled power transformers. In this paper, a new approach of DGA technique is proposed to overcome the conflict that takes place in the traditional interpretation techniques for transformer fault diagnosis. The new approach is based on the analysis of 386 dissolved gas samples data set that collected from the Egyptian electric utility chemical laboratory as well as from credited literatures. These data sets are used to build the technique model and also as a tested data set to get the technique’s accuracy. The new approach DGA diagnoses the transformer fault types based on the gas concentration percentage limit of the sum of main five gases (Hydrogen (H 2 ), Methane (CH 4 ), Ethan (C 2 H 6 ), Ethylene (C 2 H 4 ), and Acetylene (C 2 H 2 )) and some suggested gases ratios depending on the sample data set analysis. The validation of the proposed approach of DGA technique is satisfied by comparing its results with the results of the IEC Standard Code, Duval triangle and Rogers methods for the collected data set. Partial discharge (PD) diagnostics in Gas Insulated Switchgear (GIS) is important for reliable and secure operation of electrical utilities. Different techniques were used for PD diagnosis in GIS. In this work, PD diagnosis in GIS is proposed based on PD pulse sequence. PD pulse sequence only requires the measurement of PD phase appearance and its corresponding instantaneous voltage. The PD diagnosis of various defect types is implemented using five optimized machine learning classification techniques: decision tree classification, ensemble methods, k-nearest neighbouring, Discriminant analysis, and Naïve Bayes classification. The features used for PD pulse sequence are the voltage change and phase angle change between successive PD pulses. Three scenarios are proposed for predicting the defect types in GIS. The first scenario is built based on the extracted features for two successive PD pulses, the second scenario is built based on the extracted features for three successive PD pulses, while the last scenario is built based on the extracted features for four successive PD pulses. The results illustrate the superior detecting accuracy of the second scenario with the proposed five ML classification techniques. The optimized ML classification techniques are implemented and carried out based on MATLAB software package. This paper focuses on a Smart Fault Diagnostic Approach (SFDA) based on the integration among the output results of recognized dissolved gas analysis (DGA) techniques. These techniques are Dornenburg method, Electro-technical Commission standard (IEC) Code, the Central Electricity Generating Board (CEGB) Code based on Rogers' four ratios, Rogers method given in IEEE-C57 standard, and the Duval triangle. The artificial neural networks (ANN) model is constructed to monitor the transformer fault conditions and trained for each technique individually. The fault decision of each ANN model supplies the proposed integrated SFDA. The integration between these DGA approaches not only improves the fault condition monitoring of the transformers but also overcomes the individual weakness and the differences between the above methods. Toward a better diagnostic scheme, a new SFDA is developed based on the integration of the most three appropriate DGA methods. Further gas concentrations have been considered as raw data (California State University Sacramento (CSUS) as an example) to enhance the proposed SFDA performance. The variable performance of the thyristor-controlled series compensator (TCSC) system represents a vital challenge for conventional distance relays. Most existing algorithms are based on the measured data at two ends of the TCSC-compensated lines. However, some of single-end algorithms need to modify the distance relay for the series capacitor (SC) only. This paper introduces a generalized algorithm to estimate the voltage drop across the TCSC. The distance relay uses the estimated TCSC voltage drop for terminal voltage compensation. The second objective of the proposed algorithm is to estimate the conduction angle of the thyristor-controlled reactor (TCR) branch under different modes of operation. This algorithm is effective for different TCSC modes such as the capacitive or the inductive modes, in addition to the method of firing angle of synchronization current or voltage synchronization modes. The main benefit of the proposed algorithm is presenting a parameter-less distance scheme for TCSC-compensated transmission line. A 400-kV overhead transmission line test system is simulated in MATLAB/SIMULINK package® and the numerical modeling is implemented in the MATLAB® script in order to evaluate the proposed algorithm effectiveness. The purpose of this study was to examine the association of breast cancer-related lymphedema on shoulder girdle kinematics and upper extremity function. The study included 67 breast cancer survivors with and without unilateral lymphedema. Individuals were divided into non-lymphedema, moderate and severe lymphedema groups according to the volumetric measurement difference between the affected and unaffected upper extremities. A three-dimensional motion monitor-electromagnetic system was used to analyze scapular movements during the elevation and depression phases of the upper extremity elevation in the scapular plane. Shoulder range of motion was assessed with a digital inclinometer. Upper extremity function was assessed with the 'Disabilities of the Arm, Shoulder, and Hand Questionnaire–Short Form (Quick-DASH)'. The scapular upward rotation was lower for the severe lymphedema group than for the non-lymphedema group in the 90–60–30° depression phases of arm elevation (p <.05). The scapular anterior tilt was higher for the severe lymphedema group than for the non-lymphedema group in the 30° depression phase of arm elevation (p <.05). Shoulder abduction range of motion was the lowest in the severe lymphedema group (p <.05). The non-lymphedema group had the lowest quick-DASH score and the severe lymphedema group had the highest score (p <.05). There were statistically significant moderate associations between the quick-DASH scores and scapular movements in all groups (p <.05). The development, presence and/or severity of lymphedema were associated with impaired shoulder-girdle kinematics and decreased upper extremity function. The use of corrugated webs increases web shear stability and eliminates the need for transverse stiffeners in steel beams. Optimised regression learner techniques (ORLTs) are rarely used for calculating shear capacity in steel beam research. This study proposes a new approach for calculating the maximum shear capacity of steel beams with trapezoidal corrugated webs (SBCWs) by using ORLTs. A new shear model is proposed using ORLTs in accordance with plate buckling theory and previously developed formulas for predicting the shear strength of SBCWs. The proposed ORLT models are implemented using the regression learner toolbox of MATLAB software (2020b). The available data of more than 125 test results from different specimens prepared by previous researchers are used to create the model. In this study, web geometry and relevant web steel grades determine the shear capacity of SBCWs. Four regression methods are adopted. Results are compared with those of an artificial neural network model. The model output factor represents the ratio of the web vertical shear stress to the normalised shear stress. Shear capacity can be estimated on the basis of the resulting factor from the model. The proposed model is verified using two methods. In the first method, a series of tests are performed by the authors. In the second method, the results of the model are compared with the shear values obtained experimentally by other researchers. On the basis of the test results of previous studies and the current work, the proposed model provides an acceptable degree of accuracy for predicting the shear capacity of SBCWs. The results obtained using Gaussian process regression are the most appropriate because its recoded mean square error is 0.07%. The proposed model can predict the shear capacity of SBCWs with an acceptable percentage of error. The recoded percentage of error is less than 5% for 93% of the total specimens. Background: We aimed to analyze in-hospital timing and risk factors for mortality in a level 1 trauma center. Methods: This is a retrospective analysis of all trauma-related mortality between 2013 and 2018. Patients were divided and analyzed based on the time of mortality (early (≤48 h) vs late (>48 h)), and within different age groups. Multivariate regression analysis was performed to predict in-hospital mortality. Results: 8624 trauma admissions and 677 trauma-related deaths occurred (47.7% at the scene and 52.3% in-hospital). Among in-hospital mortality, the majority were males, with a mean age of 35.8 ± 17.2 years. Most deaths occurred within 3–7 days (35%), followed by 33% after 1 week, 20% on the first day, and 12% on the second day of admission. Patients with early mortality were more likely to have a lower Glasgow coma scale, a higher shock index, a higher chest and abdominal abbreviated injury score, and frequently required exploratory laparotomy and massive blood transfusion (P <.005). The injury severity scores and proportions of head injuries were higher in the late mortality group than in the early group. The severity of injuries, blood transfusion, in-hospital complications, and length of intensive care unit stay were comparable among the age groups, whereas mortality was higher in the age group of 19 to 44. The higher proportions of early and late in-hospital deaths were evident in the age group of 24 to 29. In multivariate analysis, the shock index (OR 2.26; 95%CI 1.04-4.925; P =.04) was an independent predictor of early death, whereas head injury was a predictor of late death (OR 4.54; 95%CI 1.92-11.11; P =.001). Conclusion: One-third of trauma-related mortalities occur early after injury. The initial shock index appears to be a reliable hemodynamic indicator for predicting early mortality. • Present thermodynamic model to identify the severity of the fault. • Construct NPR model to detect the transformer fault type with its severity. • High diagnosing accuracy is observed using NPR. Dissolved gas analysis (DGA) is used to diagnose power transformer fault based on the concentration of dissolved gases and the ratios between them. These gases are generated in oils as a result of electrical and thermal stresses, but these DGA techniques cannot identify the severity of the fault types. In IEEE Standard C57.104, the maintenance action is taken based on the total dissolved combustible gases, which is not sufficient because it ignores the importance of the gas type and its change rate. Thermodynamic theory using different starting decomposing materials, namely, n-octane (C 8 H 18) and eicosane (C 20 H 42), is used to estimate the severity of transformer fault types. Two scenarios are suggested with different data transformation techniques to enhance neural pattern-recognition (NPR) method accuracy for predicting transformer fault types and their severity classes. The proposed scenarios are built based on 446 samples collected from the laboratory and literature. Results refer to the role of the starting decomposing material on the severity of the transformer fault and illustrate that the proposed model has a higher accuracy (92.8%) compared with other DGA methods for diagnosing transformer fault types and superior accuracy (99.1%) to predict fault severity class. BACKGROUND AND OBJECTIVE: Earlier studies on the treatment of congenital melanocytic nevi have reported the efficacy of Q-switched ruby laser and Nd:YAG lasers. In this study we evaluated the clinical and the histological effects of a single treatment of normal mode (3 msec) alexandrite laser (755 nm) on small melanocytic nevi. MATERIALS AND METHODS: Fourteen patients having 40 nevi entered into the study. Twenty six nevi were clinically followed up after laser treatment for 4 months. Fourteen nevi were biopsied after laser treatment: 8 nevi immediately, 3 nevi after 10 days and 3 nevi after 1 month. RESULTS: At the end of 4 months, 20 (76.92%) nevi attained moderate to significant lightening of color, 5 (19%) nevi had their color matching that of the surrounding skin, one nevus (3.85%) showed very mild hypopigmentation, and 18 (69%) nevi had a surface level similar to the surrounding skin. One nevus healed with keloid formation. Histologically, immediately following laser, there was necrosis of nevus cells in the papillary and uppermost reticular dermis. Nevus cells in the deeper layers of the dermis were not affected. Ten days following laser, the upper dermis showed granulation tissue. One month after laser, there was mild fibrosis in the upper dermis and persistence of nevus cells in the middle and deep dermis. CONCLUSION: These results suggest that normal mode alexandrite laser may clear or cosmetically improve small melanocytic nevi whether congenital or acquired. Nowadays, water pumping systems based on photovoltaics as a source of electricity have widely increased. System cost and efficiency still require enhancement in order to spread their application. Perovskite solar cells (PSCs) are the most hopeful third-generation photovoltaic for replacing the silicon-based photovoltaic thanks to their high power conversion efficiency, reaching 25.8%; tunable band-gap; long diffusion length; low fabrication temperature; and low cost. In this work, for the first time, we proposed a high-power-density hybrid perovskite solar cell thermoelectric generator (TEG) array for feeding a synchronous reluctance motor (SynRM) driving a water pump for use in an irrigation system. A control technique was used to achieve two functions. The first function was driving the motor to obtain the maximum torque/ampere. The second was harvesting the maximum perovskite solar cell array output power on the basis of the maximum power point tracking (MPPT) algorithm using the perturbation and observation approach. Thus, the proposed hybrid perovskite solar cell–thermoelectric generator feeds the motor via an inverter without DC–DC converters or batteries. Accordingly, the short life problems and the high replacement cost are avoided. The proposed complete system was simulated via the MATLAB package. Moreover, a complete laboratory infrastructure was constructed for testing the proposed high-power-density hybrid perovskite solar cell–TEG array for the water pumping system. The results revealed that using the high-power-density hybrid perovskite solar cell–TEG array, both the motor's output power and the pump's flow rate were improved by 11% and 14%, respectively, compared to only using the perovskite solar cell array. Summary: This paper investigates the interruption capability with the aid of a parallel capacitor branch and exploits the single current direction through a series thyristor with the DC system. Two DC current interruptions are introduced by providing superimposed oscillatory currents. The first one is that the capacitor branch (capacitor and inductor) is continuously connected in parallel with the system where the capacitor is charged from the system during the normal operation. Due to fault occurrence, the capacitor branch including an inductor produces an oscillatory current that can make zero crossing through the series thyristor. However, the interrupting current is unfortunately after the second oscillatory peak current that the fault resistance can damp the oscillatory current. By changing the charged capacitor polarity, the second breaking concept is introduced depending on a precharged capacitor by the system voltage and then injecting the oscillatory current using a self‐triggered additional thyristor. Stenotrophomonas maltophilia is an emerging pathogen that can cause several disease manifestations such as bacteremia, meningitis, respiratory tract infections and others. More seriously, this pathogen has a highly evolving antibiotic resistance profile. Antibiotic misuse is further aggravating the situation by inducing the development of multi- and even pan-resistance. Thus, employing diverse strategies to overcome this increasing antibiotic resistance is of paramount importance. In general, vaccination is one of these strategies that prevents the onset of infection, provides long term protection against infection, and most importantly diminishes the antibiotic consumption, thus, resulting in controlling resistance. Unfortunately, vaccine research concerning S. maltophilia is very scarce in the literature. Ax21 protein is an outer membrane protein implicated in several virulence mechanisms of S. maltophilia such as quorum sensing, biofilm formation, and antibiotic resistance. Our computational analysis of Ax21 revealed its potential immunogenicity. In the current study, Ax21 protein of S. maltophilia was cloned and heterologously expressed in Escherichia coli. Mice were immunized with the purified recombinant antigen using Bacillus Calmette-Guérin (BCG) and incomplete Freund's adjuvant (IFA) as immune-adjuvants. Enzyme-linked immunosorbent assay (ELISA) revealed significant antigen-specific IgG1, IgG2a and total IgG levels in immunized mice which reflected successful immune stimulation. Immunized mice that were challenged with S. maltophilia showed a substantial reduction in bacterial bioburden in lungs, liver, kidneys, and heart. In addition, liver histological examination demonstrated a remarkable decrease in pathological signs such as necrosis, vacuolation, bile duct fibrosis and necrosis, infiltration of inflammatory cells, and hemorrhage. Whole cell ELISA and opsonophagocytic assay confirmed the ability of serum antibodies from immunized mice to bind and facilitate phagocytosis of S. maltophilia, respectively. In this paper, maintenance process for on-load tap changer (OLTC) would be enhanced using nanofluid that developed by adding nanoparticles (titanium dioxide, TiO2) to the insulating oil. The occurrence of the internal arcs inside OLTC resulted in the formation of carbon atoms on the inner bonding points during its operation. Over time, these points became unusable since the contact resistance value was high. To increase the efficiency of the maintenance process, infrared radiation was used to excite the additive TiO2 atoms to attract carbon atoms. Electrical tests were performed before and after the proposed maintenance process according to IEC standards. The results showed significant differences in the maintenance process with and without nanoparticles. A prediction model to identify the OLTC state was constructed based on support vector machine classifier. Enterococcus faecium is evolving as a multi-resistant pathogen causing infections with high morbidity and mortality. A protective vaccine against E. faecium is lacking up till now. ATP-binding cassette (ABC) transporter proteins have important functions in bacteria to maintain survival and homeostasis. In the present study, we evaluated the basic membrane protein (BMP) family ABC transporter substrate-binding protein, designated herein as BMP, as a potential vaccine candidate against E. faecium. Recombinant BMP of E. faecium was expressed in Escherichia coli, and purified by metal affinity chromatography. Swiss albino mice were immunized with the recombinant BMP combined with Bacillus Calmette–Guérin (BCG) and/or alum as adjuvants. Mice immunized with BMP combined with alternating BCG and alum developed BMP-specific IgG and were protected against E. faecium challenge as evidenced from organ bioburden and histopathological examination. Furthermore, serum from immunized mice showed enhanced opsonophagocytic activity and protected mice against E. faecium challenge by passive immunization. Bioinformatic analysis revealed appreciable degrees of homology between E. faecium BMP and proteins from other pathogens which suggests BMP could be a useful vaccine against multiple pathogens. The aim of this study is to investigate the effects of scapular taping on scapular kinematics by three-dimensional electromagnetic system during shoulder elevation in facioscapulohumeral muscular dystrophy patients. A total of 11 patients with facioscapulohumeral muscular dystrophy were included in the study. Scapular anterior-posterior tilt, upward-downward rotation, and internal-external rotations were evaluated using the three-dimensional electromagnetic system during the elevation of the upper limbs in the scapular plane before and after kinesio taping. For maximum humerothoracic elevation, there were no differences between the patients before and after taping on both dominant (p = 0.72) and non-dominant sides (p = 0.64). For scapular internal rotation, upward rotation, and posterior tilt, there were no differences between patients before and after taping during humerothoracic elevation on both dominant and non-dominant sides (p > 0.05). These results showed us that the excessive and abnormal movements of the scapula observed during the humeral elevation in facioscapulohumeral muscular dystrophy patients cannot be supported with flexible methods like kinesio taping. Therefore, we recommend to evaluate the scapula position by applying flexible and rigid taping to the patients who can reach over 90o in humerothoracic elevation in future studies. Pomegranate is one of the most important and widely distributed trees. Boron and zinc are important nutrients for plant growth and fruit quality. Nanotechnology has emerged as one of the most innovative scientific fields in agriculture. This study was conducted to describe the changes in the physiochemical characteristics (weight, diameter, length, firmness and color), as well as the phytochemicals attributes (total phenolics, total flavonoids, ascorbic acid, anthocyanin and antioxidant %) and minerals contents, of pomegranates fruits of the 'Wonderful' cultivar as a result of spraying pomegranate trees using nanomaterials (zinc oxide (ZnONPs) and boron oxide (B2O3NPs)). In three successive developmental stages (full bloom, 6 weeks after full bloom and one month before harvest time), the trees were sprayed with 0.25, 0.5 and 1 g/L ZnONPs, as well as 0.25, 0.5 and 1 g/L B2O3NPs during the 2021 and 2022 seasons. The application of ZnONPs and B2O3NPs influenced the qualitative characteristics of the fruits in the studied seasons. The highest marketable % was observed for the 0.50 and 1 g/L ZnONPs and 1 g/L B2O3NPs compared to the other treatments. Also, a positive effect was recorded for the ZnONPs and B2O3NPs on the fruits' physical properties. All of the ZnONP and B2O3NP treatments resulted in increasing the total phenolic, flavonoid, anthocyanin and ascorbic acid contents and the antioxidant activity in the pomegranate juices. In conclusion, our results suggest that spraying pomegranate trees with ZnONPs and B2O3NPs improves the marketable fruit, enhances the fruit quality and increases the bioactive components and antioxidant activity. Purpose: The aim of this study was to investigate the effects of concentric and eccentric cross-education (CE) on quadriceps strength and knee function recoveries after anterior cruciate ligament (ACL) reconstruction.Methods: Forty-eight patients (age: 29.5 ± 6.8 years, body mass index: 26.1 ± 3.2 kg/m2) who had undergone ACL reconstruction with hamstring tendon autograft were included in the study. The patients were randomly divided into three groups when they reached four weeks post surgery: (1) concentric CE (n = 16); (2) eccentric CE (n = 16); and (3) control (n = 16). All groups followed the same post-surgical rehabilitation program for their reconstructed limb. Additionally, the two experimental groups followed eight weeks of isokinetic training for the uninjured knee at 60°/s for 3 days per week. Quadriceps maximum voluntary isometric strength (MVIC) was measured during the 4th week (pre-training), 12th week (post training), and 24th week post surgery. The single-leg hop distance and International Knee Documentary Committee (IKDC) scores were also evaluated during the 24th week post surgery. Analysis of variance was used for statistical analysis.Results: Group-by-time interaction was significant for quadriceps MVICs for reconstructed and healthy limbs (p = 0.02). Quadriceps strength of both knees was greater in concentric and eccentric CE groups compared to control group during the 12th- and 24th weeks post surgery (p < 0.05). Strength gain was 28% and 31% in concentric and eccentric CE groups, respectively, when compared with the control group. Concentric and eccentric CE had similar effects on quadriceps strength recovery (n.s.). IKDC score, and single-leg hop distances were not significantly different among groups (n.s.).Conclusions: Concentric and eccentric quadricep strengthening of healthy limbs in early phases of ACL rehabilitation improved post-surgical quadriceps strength recovery of the reconstructed limb. The development of a new dissolved gas analysis (DGA) method often requires a comparative study to assess the accuracy of the proposed technique. This is faced with the following challenges: (i) the time and effort required to implement and validate the implementation of existing DGA methods, adds to the comparative study cost; (ii) the output states of different DGA methods are not similar, which makes it difficult to put methods side by side in a comparative study; and (iii) the availability of test data is limited. In this study, a user‐friendly graphical user interface software package, DGALab, is developed to overcome these challenges. DGALab implements a unified DGA diagnosis framework to map the output states of DGA methods to uniform specifications. DGALab includes a library implementing most common DGA techniques, and includes a repository for input datasets available in the literature and collected directly from laboratories. DGALab simplifies the addition of new DGA techniques written in virtually any programming language. As a result, the process of developing a new DGA technique is greatly simplified using DGALab. Background/Aims: Ankylosing spondylitis mainly affects the vertebral column, meaning that the assessment of the physical function of the upper extremity and scapulothoracic region in ankylosing spondylitis is often overlooked. Therefore, the aim of this study was to investigate shoulder muscle strength, shoulder active range of motion and three-dimensional scapular motion in patients with ankylosing spondylitis. Methods: The study included 18 men with ankylosing spondylitis, and 16 age-matched typically healthy men as the control group. Muscle strength, active range of motion, and three-dimensional scapular motion were assessed with a digital hand-held dynamometer, an inclinometer, and an electromagnetic tracking device respectively. Results: A decrease in shoulder flexion and abduction muscle strength on both the dominant side (P<0.001, P=0.001) and non-dominant side (P<0.001, P<0.001) was found in the ankylosing spondylitis group compared to the control group. A decrease in shoulder flexion, abduction, and external and internal rotation active range of motion on both the dominant side (P=0.001, P<0.001, P=0.015, P<0.001, respectively) and non-dominant side (P=0.001, P<0.001, P=0.017, P<0.001 respectively) was observed in the ankylosing spondylitis group. There was an increase in the upward rotation of the scapula on the non-dominant side at 30, 60, and 90° humeral elevations in the ankylosing spondylitis group compared to the control group (P=0.018, P=0.003, P=0.001 respectively). The other parameters of the scapular motion did not differ between groups (P>0.05). Conclusions: This study showed that shoulder muscle strength and active range of motion were lower, and the upward rotation of the scapula was greater in men with ankylosing spondylitis compared to typically healthy men. Objectives. The objective of this study is to explore the gender discrepancy in patients with traumatic brain injury (TBI). Methods. A retrospective analysis of Qatar Trauma Registry (QTR) was conducted among patients (age ≥14y) who were hospitalized with TBI. Data were collected and analyzed based on the gender and age. Results. Over 5 years (2014-2019), 9, 309 trauma patients (90% males and 10% females) were admitted to the trauma center. Of these, 1, 620 (17.4%) patients were hospitalized with TBI (94% males and 6% females). Motor vehicle crash was the main mechanism of injury (MOI) in females, and fall from height was predominant among males. Subdural hematoma (SDH) was the more frequent type of TBI in both genders, but it was more prevalent in male patients ≥55 years. Injury severity score, Glasgow coma scale, and head abbreviated injury score were comparable between males and females. The length of stay in the ICU and hospital and mortality were similar in both genders. However, mortality was higher among males ≥55 years when compared to 14-54 years within the same gender (21% vs. 12%, p = 0.002). The crude and adjusted odds ratio did not show that gender is a significant predictor of mortality among TBI patients. Conclusions. Summary: During the last years, the electricity networks worldwide have rapidly developed, especially with integrating many types of renewable energy sources (RESs). The optimal operation is an opportunity to increase the penetration level of stochastic RESs into the power grid to maximize energy efficiency. Generally, the optimal power flow (OPF) problem is a highly complex, non‐convex, and non‐linear optimization problem. The complexity of the OPF problem is further increased as stochastic RESs are incorporated into the network. This paper presents an effective solution to the OPF problem for a traditional power generation with stochastic RESs. For solving this problem, chaotic Bonobo optimizer (CBO) is proposed in this paper based on the Chaos Theory to avoid the stuck in the local minimum by applying the original Bonobo optimization (BO). The performance of BO is enhanced using the chaotic maps sequences technique to enhance its global search capability and prevent getting stuck into local solutions. Uncertainty of the output power generated by RESs is forecasted based on probabilistic models. To minimize the total operating cost, the direct, underestimation, and overestimation costs of RESs are considered. Three different objective functions are considered, minimizing total operating cost, emissions, and power losses. Moreover, a carbon tax is incorporated in the objective function problem to minimize carbon emissions. The proposed OPF model and CBO technique are verified on the modified IEEE‐30 and IEEE‐57 bus test systems to confirm the superiority and effectiveness of the proposed CBO to achieve the optimal solution. Apricots are a fragile fruit that rots quickly after harvest. Therefore, they have a short shelf-life. The purpose of this work is to determine the effect of coatings containing chitosan (CH) as well as its nanoparticles (CHNPs) as thin films on the quality and shelf-life of apricots stored at room (25 ± 3 °C) and cold (5 ± 1 °C) temperatures. The physical, chemical, and sensorial changes that occurred during storage were assessed, and the shelf-life was estimated. Transmission electron microscopy was used to examine the size and shape of the nanoparticle. The nanoparticles had a spherical shape with an average diameter of 16.4 nm. During the storage of the apricots, those treated with CHNPs showed an obvious decrease in weight loss, decay percent, total soluble solids, and lipid peroxidation, whereas total acidity, ascorbic acid, and carotenoid content were higher than those in the fruits treated with CH and the untreated fruits (control). The findings of the sensory evaluation revealed a significant difference in the overall acceptability scores between the samples treated with CHNPs and the other samples. Finally, it was found that CHNP coatings improved the qualitative features of the apricots and extended their shelf-life for up to 9 days at room temperature storage and for 30 days in cold storage. While much attention was given in the literature to improve the fault ride through (FRT) capability of a doubly-fed induction generator (DFIG)-based wind energy conversion system (WECS) during various faults at the grid side, not much attention was given to investigate and improve the performance of the DFIG-WECS under ferroresonance conditions. This paper investigates the impact of ferroresonance on the overall performance of a DFIG-WECS. The paper also presents a new technique to improve the high voltage ride through capability of the investigated system under such condition. The proposed technique relies on incorporating a superconducting coil within the dc-link of the DFIG grid-side and rotor-side converters. Energy exchange between the coil and the system is controlled through regulating the duty cycle of the dc-chopper interfacing the coil with the dc-link. In this regard, a simple but yet effective proportional-integral (PI) controller is employed to regulate the dc-chopper duty cycle. Parameters of the PI controller are optimized using elephant herding optimization algorithm. Background: Shoulder pain is a common symptom for non-traumatic shoulder pathologies and affects 4.7-66.7% of entire population. Even with the latest technological advantages, palpation still is a cost and time efficient tool to evaluate the shoulder pathologies. Our aim was to investigate and compare palpation findings in different shoulder pathologies.Method: We used first assessments of impingement syndrome (IS) (n = 205), rotator cuff tear (RCT) (n = 185) and frozen shoulder (FS) (n = 210) patients who received treatment between 2010 and 2019 years. Two experienced physiotherapists palpated predefined points: long head of biceps brachii, pectoralis minor, lateral intermuscular septum, proximal tendons of extensor group. These points categorized into "proximal", "mid-proximal", "mid-distal", "distal", "irregular". We also grouped patients by their BMIs.Results: We found RCT patients had higher BMI than IS and FS patients (p < 0.001, p = 0.001); more tender intermuscular septum points and night pain than IS patients (p = 0.001, p = 0.003) and more extensor group proximal tendon points than IS and FS patients (p < 0.001, p = 0.001). "No tenderness" group had lesser resting pain than distal group (p < 0.001) and lesser activity and night pain than middle-proximal, middle-distal, and distal groups (all ps < 0.002). Distal group had higher night pain than middle-proximal group (p = 0.003). Morbidly obese patients had higher night pain than normal and overweight patients (p = 0.003 and p = 0.009).Conclusions: Tender point distribution varies depending on the pathology with biceps brachii being the most common. RCT patients had higher night pain related to more distal tender points. Patients with higher night pain had increased tenderness in septum intermuscular and extensor muscles' proximal tendons. This paper proposes a new simple technique to detect and discriminate the abnormal states of the grid-connected photovoltaic (PV) solar system based on the rate of change of voltage and current trajectory. The design of the PV system is developed by implementation of only one diode in every PV string. The diode prevents the reverse direction of the fault current in case of faulty strings. On the other hand, the installation of a diode reduces the ability of faults detection and diagnosis depending on currents in each string. The proposed technique depends on the rate of change of voltages and current trajectory during the fault transient period. The proposed algorithm can detect various fault cases such as cell-to-cell and string to string faults in addition to partial and full shadow faults. Also, the algorithm differentiates between high and low fault cases for each fault types. Moreover, the proposed technique is characterized by high sensitivity for internal abnormal states within certain array with high security level for the external abnormal conditions and normal load changing. The proposed technique is applied for a four-array PV system, which has a power rating of 400 kW connected to an AC grid. The simulation results and validation of the proposed technique are implemented by MATLAB/Simulink toolbox. Worldwide, the emerging trend of hybrid renewable energy sources integration in modern power systems is increasing due to privileged prices and clean electricity supply. However, the optimal planning of rural hybrid systems is a challenging and complex task, especially when different alternatives and sustainability aspects are considered. This paper develops an integrated decision-making approach for the optimal planning of a 100% renewable energy supply system comprising solar, wind, hydro, and biomass sources in a rural area located in Pakistan. An hourly-based design optimization analysis of twelve on/off-grid electrification alternatives is performed. The optimization model simultaneously addresses five sustainability criteria related to economy, reliability, ecology, society, and topography aspects. Furthermore, a novel hybrid decision-making model has developed to identify the unique best configuration with on-grid and off-grid options. The proposed model combines fuzzy analytic hierarchy process, multi-objective optimization based on ratio analysis technique for order of preference by similarity to an ideal solution, and evaluation based on distance from average solution methods. The results reveal that the solar-hydro-biomass battery with a life cycle cost of 10.9 M$ is the top-ranking off-grid system. When the hybrid system is connected to the grid, the solar-hydro-battery has found the most appropriate design with a life cycle cost of 12.96 M$. Both scenarios have a negligible capacity shortage of 0.09%. Ecologically, the optimal off-grid system produces only 408.37 kg/yr of CO 2 due to the significant energy share of solar and hydro sources (99.3%). The optimal on-grid system produced the minimum CO 2 with 29,177.89 kg/yr compared to other alternatives. Also, employing the optimal on/off-grid designs require land area and jobs of 96.6 m2, 14 jobs, and 118 m2, 15 jobs, respectively. Overall, the developed approach with the presented case study offers a valuable benchmark and guidelines for investors and stakeholders to create realistic investment plans for the energy industry looking to push efficient inducements to encourage the high dissemination of renewables. Background: The constellation of the initial hyperglycemia, proinflammatory cytokines and severity of injury among trauma patients is understudied. We aimed to evaluate the patterns and effects of on-admission hyperglycemia and inflammatory response in a level 1 trauma center. We hypothesized that higher initial readings of blood glucose and cytokines are associated with severe injuries and worse in-hospital outcomes in trauma patients. Methods: A prospective, observational study was conducted for adult trauma patients who were admitted and tested for on-admission blood glucose, hemoglobin A1c, interleukin (IL)-6, IL-18 and hs-CRP. Patients were categorized into four groups [non-diabetic normoglycemic, diabetic normoglycemic, diabetic hyperglycemic (DH) and stress-induced hyperglycemic (SIH)]. The inflammatory markers were measured on three time points (admission, 24 h and 48 h). Generalized estimating equations (GEE) were used to account for the correlation for the inflammatory markers. Pearson's correlation test and logistic regression analysis were also performed. Results: During the study period, 250 adult trauma patients were enrolled. Almost 13% of patients presented with hyperglycemia (50% had SIH and 50% had DH). Patients with SIH were younger, had significantly higher Injury Severity Score (ISS), higher IL-6 readings, prolonged hospital length of stay and higher mortality. The SIH group had lower Revised Trauma Score (p = 0.005), lower Trauma Injury Severity Score (p = 0.01) and lower GCS (p = 0.001). Patients with hyperglycemia had higher in-hospital mortality than the normoglycemia group (12.5% vs 3.7%; p = 0.02). A significant correlation was identified between the initial blood glucose level and serum lactate, IL-6, ISS and hospital length of stay. Overall rate of change in slope 88.54 (95% CI:-143.39–33.68) points was found more in hyperglycemia than normoglycemia group (p = 0.002) for IL-6 values, whereas there was no statistical significant change in slopes of age, gender and their interaction. The initial IL-6 levels correlated with ISS (r = 0.40, p = 0.001). On-admission hyperglycemia had an adjusted odds ratio 2.42 (95% CI: 1.076–5.447, p = 0.03) for severe injury (ISS > 12) after adjusting for age, shock index and blood transfusion. Conclusions: In trauma patients, on-admission hyperglycemia correlates well with the initial serum IL-6 level and is associated with more severe injuries. Therefore, it could be a simple marker of injury severity and useful tool for patient triage and risk assessment. Background: Two oral antifungal agents, griseofulvin and terbinafine, have regulatory approval but it is unknown whether one has superior overall efficacy. Genus-specific differences in efficacy are believed to exist for the two agents. It is not clear at what doses and durations of treatment these differences apply. Purpose: The purposes of this meta-analysis were to determine whether a statistically significant difference in efficacy exists between these agents at a given dose and duration of each in tinea capitis infections overall and to determine whether a genus-specific difference in efficacy exists for these two treatments at a given dose and duration of each. We performed a literature search for clinically and methodologically similar randomized controlled trials comparing 8 weeks of griseofulvin (6.25-12.5 mg/kg/day) to 4 weeks of terbinafine (3.125-6.25 mg/kg/day) in the treatment of tinea capitis. A meta-analysis was performed using the Mantel-Haenszel method and random effects model; results were expressed as odds ratios with 95%. Results: Meta-analysis of randomized controlled trials did not show a significant difference in the overall efficacy of the two drugs at the doses specified, but specific efficacy differences were observed based on the infectious species. For tinea capitis caused by Microsporumspp., griseofulvin is superior (p = 0.04), whereas terbinafine is superior for Trichophyton spp. infection (p = 0.04). Conclusion: Our results support species-specific differences in treatment efficacy between griseofulvin and terbinafine and provide a clinical context in which this knowledge may be applied.
