BackgroundThe mechanisms and outcomes in coronavirus disease (COVID-19)-associated stroke are unique from those of non-COVID-19 stroke.ObjectiveTo describe the efficacy and outcomes of acute revascularization of large vessel occlusion (LVO) in the setting of COVID-19 in an international cohort.MethodsWe conducted an international multicenter retrospective study of consecutively admitted patients with COVID-19 with concomitant acute LVO across 50 comprehensive stroke centers. Our control group constituted historical controls of patients presenting with LVO and receiving a mechanical thrombectomy between January 2018 and December 2020.ResultsThe total cohort was 575 patients with acute LVO; 194 patients had COVID-19 while 381 patients did not. Patients in the COVID-19 group were younger (62.5 vs 71.2; P < .001) and lacked vascular risk factors (49, 25.3% vs 54, 14.2%; P = .001). Modified thrombolysis in cerebral infarction 3 revascularization was less common in the COVID-19 group (74, 39.2% vs 252, 67.2%; P < .001). Poor functional outcome at discharge (defined as modified Ranklin Scale 3-6) was more common in the COVID-19 group (150, 79.8% vs 132, 66.7%; P = .004). COVID-19 was independently associated with a lower likelihood of achieving modified thrombolysis in cerebral infarction 3 (odds ratio [OR]: 0.4, 95% CI: 0.2-0.7; P < .001) and unfavorable outcomes (OR: 2.5, 95% CI: 1.4-4.5; P = .002).ConclusionCOVID-19 was an independent predictor of incomplete revascularization and poor outcomes in patients with stroke due to LVO. Patients with COVID-19 with LVO were younger, had fewer cerebrovascular risk factors, and suffered from higher morbidity/mortality rates.

Purpose: The purpose of this study is to investigate the cross-cultural training (CCT) provided to European expatriate executives in New Zealand, and consequently add to the knowledge for human resource management in the Australasian region. Design/methodology/approach: The present qualitative research study is based on open-ended and in-depth interviews with 30 European expatriate executives who had been residing in New Zealand for the past year or more. Findings: The results indicate that at least some CCT was provided to all interviewees; however, the training content seemed to be strongly centered around professional work, with little focus on the culture of the host country. Moreover, there was no follow-up to the pre-departure CCT and very few expatriates in the sample received any training in New Zealand. Similarly, the family is considered a fundamental factor for the success of expatriates; however, in the sample, neither spouses nor children received any training before or after their arrival in New Zealand. Research limitations/implications: The research is limited by the sample small size. The study's inconclusive highlights the need for further research to explore the influences of local residents, colleagues and neighbors on expatriates' adjustment. Similarly, further studies are also required to ascertain the effectiveness of CCT in helping support expatriates' performance and adjustment in New Zealand. Practical implications: The present study suggests that opportunities do exist for multinational companies (MNCs) to better prepare their expatriates for assignments by integrating more effectively issues related to cultural awareness into their CCT. Specifically, experimental CCT methods that emphasize the host country's culture are most valuable. The CCT technique should be tailored to the cultural distance between the host nation and expatriates' country of origin and to the nature of their assignment. Originality/value: Given the existence of many MNCs in New Zealand, it is surprising that the issue of CCT in this context has received little research attention. The current study endeavors to address this gap. This paper hopes that the findings may also be useful for consultants and human resource managers in MNCs who are involved in preparing expatriates for foreign assignments in Australasia, especially New Zealand.

Objective: To evaluate the impact of introducing newly-designed warning labels in relation to the available conventional cigarette packs. Method: The quasi-experimental study was conducted at four co-educational academic institutions selected using cluster random sampling technique in Gulshan-e-Iqbal town of Karachi from September 2016 to October 2017. The respondents, aged 16-24 years, were exposed with an experimental condition using five graphic cards; each comprising of two separate labels; label 1 being a cigarette pack currently rotated, and label 2 being the newly designed graphic and text warning label. Each participant responded over efficacy measures for each label separately. The difference in the efficacy scores between the two labels was assessed using repeated measure ANOVA with Bonferroni correction in analysis. Results: Of the 388 students, 256(66%) were males and 132(34%) were females. The overall mean age was 18±2.76 years. Also, 188(48.5%) subjects were studying in higher secondary, 179(46.1%) undergraduate and 21(5.4%) postgraduate levels. Of them, 104(26.8%) were ever-smokers and 284(73.2%) reported to be never-smokers. In the newly-designed warning label elements of ‘poverty’ and ‘not to start smoking’ in the current cigarette pack were significant (p=0.006). Non-smoking males perceived newly-designed label element ‘dead person’ as fear-arousing (p=0.045), while non-smoking females considered ‘look after family’ as efficacious (p=0.024). Conclusion: There was some evidence of variation in efficacy scores after stratifying on gender and smoking status. A more holistic approach is needed in this regard. Purpose: This qualitative study is grounded on in-depth interviews with 30 Western women self-initiated expatriate (SIEs) currently living and working in the UAE. When selecting the interviewees, the author used purposeful sampling to ensure a diverse sample of interviewees with respect to nationality, age, gender and occupation. Design/methodology/approach: The present study, drawing on boundary theory, aims to investigate the work–life balance (WLB) of Western women SIEs regarding how these women construct and manage the borders between the non-work and work lives in the United Arab Emirates (UAE). Findings: The results demonstrate that women SIEs experience conflicts and enrichment during overseas employment. Both directions – the impact flowing from working life to personal life and vice versa – were significant. Different career and life phases appeared to be crucial to these experiences. The study also found that some women SIEs in the UAE experience high pressure in the WLB approaches, which are primarily impacted by the specific work–life environment in the UAE. Mostly, work–life boundaries are culturally and socially induced. Hence, many women SIEs encounter disparities between the robust work–life separations in the home country compared to the host country; women SIEs, therefore, need to relax the boundaries to adapt to the competitive work–life environment in the UAE. Originality/value: The present study contributes to research on work–life boundary management approaches in local settings, such as UAE, by analyzing cross-cultural and individual dimensions. Moreover, although women are still a minority among SIEs, the number of women is increasing. As prior studies have mainly focused on male SIEs, more research is required focusing specifically on women with overseas jobs. The present study endeavors to fill this research gap. Background: Pakistan holds rank 8 th among the world countries in which high prevalence of tuberculosis (TB) is present. There is evidenced that up to 30% of close contacts of TB patients become infected and at least half of them progress to active disease. The aim of this study was to screen active contacts of sputum positive pulmonary TB (SS +ve PTB) among household contacts (HHCs) of TB patients in Karachi, Pakistan. Materials and Methods: Cross-sectional study, which was conducted at chest clinics of Karachi. Total 750 HHCs were screened out of which 135 diagnosed smear positive TB patients was registered as index cases (ICs) from July 2012 till March 2013. Logistic regression analysis was carried out to estimate the odds ratio (OR) of different associated factors. Results: Prevalence among males (6.1%) compared with female (5.6%). The crude OR for age less than 15 years was 0.28 (95% confidence interval (CI) 0.14-0.55). Eighty-eight HHCs (11.73%) were found to have SS +ve TB. Disease frequency was significantly higher in HHCs who were smokers (adjusted OR 36.41; 95% CI 12.07, 109.79), had age less than 12 years (adjusted OR 3.63; 95% CI 1.68, 7.86), and who lived in houses less than 80 square yards (adjusted OR 3.77; 95% CI 1.27, 6.05). Conclusion: The IC with positive sputum smear constitutes great risk for TB infection and disease in HHC. There is needed for contact tracing strategy in the high-risk population.
