We herein report a case of heterotopic gastric mucosa in the hilar bile duct. An asymptomatic 58-year-old male was noted to have mild liver dysfunction in March 2009 during the follow-up for angina pectoris. Abdominal-enhanced CT revealed wall thickening from the upper common hepatic bile duct to the left hepatic bile duct. Endoscopic retrograde cholangiopancreatography (ERCP) showed stenosis at the junction of the left hepatic bile duct. Although the patient's serum tumor markers were all within the normal ranges, the possibility of malignant disease of the biliary tree could not be ruled out. Left hepatectomy with the caudate lobe and resection of the extrahepatic bile duct were performed. Histopathologically, the resected specimen showed a polypoid lesion measuring 2 × 2 cm in size that projected into the lumen of the left hepatic bile duct. Microscopic examination revealed this polypoid lesion to be composed of mucous glands resembling gastric fundic glands, with parietal and chief cells. We also review eight other reports of heterotopic gastric mucosa in the biliary tree previously published in the English literature.

肺原発淡明細胞腺癌の報告は極めてまれである．今回，上縦隔までリンパ節転移を伴った淡明細胞腺癌の1例を胸腔鏡補助下に完全切除したので報告する．症例は69歳，女性．CTにて右肺S8に23mm大の結節を認め，PET-CTで結節，肺門および縦隔リンパ節に集積を指摘された．術前診断はcT1bN2M0 Stage III Aの右下葉肺癌で，胸腔鏡補助下右下葉切除＋ND2aをおこなった．組織学的所見では，病変の90％以上を淡明細胞が占め，上縦隔リンパ節を含めた19/23のリンパ節に転移を認めた．免疫組織学的にThyroid transcription factor-1（TTF-1）陽性であり，画像検査においても他の部位に腫瘍はなく，肺原発淡明細胞腺癌と診断した．術後病理診断はpT1aN2M0 Stage III Aであったため，術後はシスプラチン（CDDP）とビノレルビン（VNR）施行後，テガフール・ウラシル（UFT）内服に変更し補助化学療法を継続している．

われわれは，乳房温存療法困難症例11例に対し，遊離真皮脂肪弁移植による一期的乳房再建を施行し，術後乳房照射を行った．全症例の術後経過は良好で，移植弁の感染，壊死をきたすことなく順調に軽快した．術後移植弁のMRI検査では，移植弁の周囲にのみ血流の再開が確認されたが，中心部には血流は認めず，移植弁のサイズは採取時より縮小していた．Core needle biopsyでは，移植弁は脂肪組織の組織構築を保ちつつ脂肪細胞は脱落し，組織学的には循環障害による組織凝固壊死像に相当した．乳房の整容性については，日本乳癌学会沢井班による基準では平均10.0点であった．術後の移植弁については多くの症例で硬く触知されたが，乳房最下垂点，乳頭の位置など，左右のバランスにおいては全例良好であった．遊離真皮脂肪弁移植は，乳房再建術としては筋皮弁やインプラントに比べ簡便で低侵襲であり，体位変換の必要もなく，形成外科医でなくとも可能な術式である．

症例は68歳の女性で，66歳時，左肝管内に限局する乳頭状隆起性病変を認め，術中の胆道鏡では，右肝管，総胆管内には病変を認めなかったため，左葉切除を施行した．病理組織学的には，胆管被覆上皮の乳頭状増殖を示すものの浸潤性増殖像は認めず，胆管乳頭腫症と診断された．退院後半年に1度，CTでフォローしていたところ，2年後のCTで下部胆管に淡い造影効果を示す結節陰影を認めた．Endoscopic Retrograde Cholangiograpy（ERC），Magnetic Resonance Cholangiopancreatography（MRCP）で下部胆管内に12 mm大の不整形の隆起性病変を認めたため，下部胆管に発生した胆管癌を疑い，膵頭十二指腸切除を施行．病理の結果，病変は前回と同様に乳頭状増殖を示し胆管乳頭腫症と診断された．胆管乳頭腫症は胆道の良性疾患として分類されているが，悪性転化の報告があり，癌に準じた根治的切除が必要と考えられているが，本症例のように根治的切除がなされた後でも異時性に発生することもあり，注意深い観察が必要と考えられる．

von Recklinghausen病に合併した空腸悪性神経鞘腫穿孔のまれな1例を経験した. 39歳の女性, 腹痛にて来院. 胸部X線写真にて横隔膜下にfree air像, 腹部CT検査にて骨盤腔に内部壊死とair像を伴う12cm大の腫瘍を認め, 腫瘍穿孔あるいは腸管穿孔による腹膜炎の診断のもと緊急手術を行った. Treitz靭帯から30cm肛側の空腸より壁外性に突出した腫瘍の穿孔および同部空腸内腔と腫瘍壊死腔間の交通を認めた. 病理では紡錘形細胞の不規則な束状配列, palisading pattern, 核異型増加を認め, 免疫組織学的にはS-100蛋白陽性, actin陰性であり本腫瘍を悪性神経鞘腫と診断した. わが国におけるvon Recklinghausen病に合併した小腸悪性神経鞘腫の報告は本例が13例目であり, そのうち腫瘍穿孔例は本例が2例目であった.小腸内腔と腫瘍壊死腔間の交通は穿孔したこの2例のみに認められ, この交通が腫瘍穿孔の一因として考えられた.

われわれは,肝細胞癌破裂部の肝部分切除術施行後に腹膜播種をきたし,外科的に切除した症例を経験したので報告する.症例は56歳男性,慢性B型肝炎にて経過観察中,平成6年2月,肝細胞癌破裂(S2)による出血性ショックをきたし緊急手術(肝部分切除術)を施行した.外来にて経過観察していたが,平成8年3月,腹部に腫瘤を触知し,腹部CT,血管造影にて肝S4,大網に腫瘤を認めた.残肝再発,腹膜播種と診断し,手術を施行,摘出した大網腫瘤は転移性肝細胞癌であった.患者は,播種巣切除後12カ月(肝細胞癌破裂後39カ月)で癌死した.肝細胞癌破裂症例では,腹膜播種のリスクが高く,播種巣の外科的切除により生存期間の延長が得られる場合もあるため,破裂後の厳重な経過観察と腹膜播種の早期発見が重要であると考えられた.

III b型肝損傷を主とする多発交通外傷の5歳男児の1例を経験した.われわれはこれまでIII b型肝損傷に対し肝切除術を主に行い, 7例中6例を救命した.本例は5歳の小児であり,損傷肝部の形態もIII b型にしては比較的保たれていたため,肝の温存と手術時間短縮を目的に肝縫合術を選択した.しかし,術後損傷部の肝仮性動脈瘤から腹腔内へ出血をきたし, transcatheter arterial embolization (TAE)にて止血し救命できた.肝縫合術は損傷肝の温存や手術時間の短縮になるが,遺残膿瘍や後出血を起こすことがあり慎重な手術操作が必要である.小児肝外傷部からの出血に対するTAEの報告は,本例のような術後の肝仮性大動脈瘤破裂からの出血も含めて極めて稀である.しかし本例に示す如く,循環動態が比較的安定していれば,小児の肝仮性動脈瘤破裂に対してTAEは極めて有効な止血手段と考えられる.

われわれはS状結腸癌切除後9年8カ月を経過して肝転移を認めた1例を経験し,これを切除しえたので報告する. 症例は50歳の男性で,健診の腹部エコー上,肝S3に径40mm大のSOLを指摘され,精査加療目的で紹介となった. 9年8カ月前当科においてS状結腸癌の診断でS状結腸切除を施行していた.胸腹部CT,上部,下部内視鏡検査では他に原発巣となる所見はなく, 1999年7月26日肝左葉外側区域切除を施行した.切離面に組織学的に癌の露出は認めなかった.組織標本において高分化型腺癌を認め,組織像は9年8カ月前に切除したS状結腸癌に類似していたこと,他に原発巣を認めなかったことよりS状結腸癌の肝転移と診断した. 本症例のように大腸癌は術後数年経過後に再発が確認される症例もあり,大腸癌治癒切除後のfollow upは長期にわたり厳重に施行することが大切である.

Our uncommon experience with a case of accessory breast cancer is described. A 67-year-old woman was seen at the hospital because of a left axillary tumor. An excisional biopsy of the tumor revealed medullary carcinoma with mammary gland, without connection to the proper mammary gland. There was no other primary lesion in general exploration and clinical findings also offered a suspicion of accessory breast cancer. A wide local resection with lymph nodes dissection level III was carried out. She is doing well without any evidence of recurrence, as of 10 months after the operation.

Angiomyolipomas are benign mesenchymal tumors mostly arising from the kidney. Angiomyolipoma of the colon is extremely rare. Here we report the findings of a 51-year-old man who presented with a submucosal tumor covered with normal mucosa and hemorrhage in the descending colon. He underwent a partial resection of the descending colon. A histopathological examination showed that the tumor of 5.7 cm in diameter included smooth muscle (spindle cell type), mature adipose tissue, and vessels, and therefore a diagnosis of angiomyolipoma was made. A submucosal type of angiomyolipoma of the colon is extremely rare. If a renal angiomyolipoma is larger than 4 cm, there is a risk of rupture. When colonoscopy shows a submucosal tumor of the colon with hemorrhage, angiomyolipoma should be considered. If an angiomyolipoma of the colon is large, surgical resection should be considered as a treatment option due to the risk of hemorrhage.

We investigated a newly synthesized conjugate of ursodeoxycholic acid with para-aminobenzoic acid (PABA) to determine its suitability to evaluate enteric bacteria. This compound, PABA-UDCA, is deconjugated by cholylglycine hydrolase to release free PABA whereas it is completely resistant to deconjugation by pancreatic and intestinal mucosal enzymes. In bacteriological experiments, almost all of the microorganisms which split glycocholic acid deconjugated this compound. In animal experiments, urinary excretions of PABA were measured for 6 hours following oral administration of 10 mg PABA-UDCA. Ten control rats excreted 338.5 +/- 43.8 micrograms (mean +/- SD) of PABA; 10 rats with intestinal bacterial overgrowth due to enteric stagnant loops excreted more (673.6 +/- 222.1 micrograms; p less than 0.01), whereas 10 rats in each of 8 groups with intestinal antisepsis by oral administration of various antibiotics excreted significantly less (p less than 0.001) (ampicillin + doxycycline + fradiomycin: 18.3 +/- 16.7, polymixin B + tinidazole: 14.0 +/- 8.0, polymixin B: 224.9 +/- 74.3, tinidazole: 42.7 +/- 27.3, kanamycin: 50.3 +/- 18.2, clindamycin: 57.4 +/- 23.3, vancomycin: 70.4 +/- 27.0, and paromomycin: 160.4 +/- 51.9 micrograms). These observations indicate that this compound is likely to offer a simple and rapid method for evaluation of intestinal microorganisms without the use of radioisotopes or expensive, special apparatus.

The partial purification and characterization of a hepatocyte proliferation stimulatory factor (HPSF) isolated from the liver of D-galactosamine-treated rats are described. The HPSF was a heat-labile, acid-stable and trypsin-sensitive protein. The partially purified HPSF stimulated DNA synthesis and increased the labeling index of parenchymal hepatocytes at 5 micrograms/ml and maximally at 50 micrograms/ml. The effect of HPSF in stimulating DNA synthesis was synergistic with that of insulin plus epidermal growth factor (EGF). The HPSF was scarcely detected in normal rat liver. The results obtained indicate that this HPSF is distinct from insulin, multiplication-stimulating activity (MSA), EGF and other hepatocyte growth factors previously reported, and suggest a plausible role for HPSF in the regeneration of liver tissue following hepatotoxic damage.
