45. Staphylococcal Scalded Skin Syndrome in an Adult

Abstract

We report a case of a 77‐year‐old man with staphylococcal scalded skin syndrome (SSSS). The patient, who had no physical abnormalities other than herpes zoster, showed intact humoral and cellular immunity. A 3‐ and 1‐phage group strain of Staphylococcus aureuswas isolated from an unruptured pustule of the herpes zoster lesion, but not from the throat. Inoculation of the isolated bacterium into newborn mice produced a characteristic Nikolsky sign. Histological diagnosis of SSSS was established by findings of intraepithelial cleavage through the stratum granulosum and a lack of inflammatory cells in both dermis and epidermis. After a ten‐day course of oral cefaclor (250 mg every 8 hours) and topical triamcinolone acetonide containing fradiomycin and gramycidin J, the patient recovered completely. The patient left the hospital without any scars other than those of herpes zoster.

46. Inhibitory Activity of a Low‐molecular‐weight Proteinase Inhibitor (2,200 Mr.) against Various Proteinases

Abstract

A low‐molecular‐weight proteinase inhibitor which has a molecular mass of 2,200 daltons was recently isolated from human epidermis. The isolated fraction demonstrated one protein band; when it was preincubated with papain, no band corresponding to products from the inhibitor was demonstrated in sodium dodecyl sulfate (SDS) polyacrylamide gel electrophoresis (PAGE). The isolated fraction was found to be immunologically distinct from a2macroglobulin (α2‐M) by a double immunodiffusion test using anti‐α2‐M serum.

47. Comparison of the Nuclear Contour Index (NCI) and T cell Subsets of Erythematous, Plaque and Tumor Lesions in Patients with Mycosis Fungoides (MF)

Abstract

Using monoclonal T cell antibodies, we compared T cell subsets of skin infiltrates with the nuclear contour index (NCI) of pan T cells of skin infiltrates from erythematous, plaque and tumor lesions in the same patients with mycosis fungoides (MF). In all lesions, the majority of the skin‐infiltrate population were T cells; helper T cells were especially dominant in the plaque and tumor lesions. The NCI of the T cells in skin infiltrates increased in parallel with advancing stages of the lesions in the same patient. The NCI data for T cell and T cell subsets in different stages of lesions in the same patient were similar to data from different stages of lesions in different patients. Therefore, even in tumor stage patients, topical treatment should be selected in accordance with the cytological stage of their lesions.

Abstract

The extracellular keratinase from M. caniswas purified using ion exchange chromatography and gel filtration. The purified enzyme showed a single protein band on sodium dodecyl sulfate‐polyacrilamide gel electrophoresis. A molecular weight of approximately 45,000 was determined by SDS‐electrophoresis.

50. Partial Purification and Some Characteristics of Proteinase(s) Induced by Staphylococcal Exfoliative Toxin

Abstract

After 8 hrs incubation with epidermis of newborn mice and exfoliative toxin, a marked increase in caseinolytic activity was detected, which reached a maximum at 12 hrs. Casein‐hydrolyzing enzyme(s) induced by ET were partially purified by chromatography. A substantial increase in caseinolytic activity was detected in the fractions obtained from Sephadex G‐50, while practically no caseinolytic activity was observed when the extract obtained from the epidermis incubated without ET was applied. The caseinolytic activity appeared as a single peak eluted from DEAE‐Sepharose CL‐6B and Sephadex G‐75. However, on SDS‐PAGE, the partially purified fractions exhibited several protein bands. The molecular weights of these band were estimated as 78 KD, 68 KD, 45 KD, 14.5 KD and 8.8 KD. When the partially purified enzyme(s) was preincubated with EGTA or EDTA, substantial inhibition of the activity was observed; however, no recovery of the activity was detected after the addition of CaCl2. Treatment of the enzyme(s) with PMSF and NEM caused little inactivation of the activity. Enzyme activity retained about 57% of the initial activity following 3 min incubation at 60°C, but was completely inactivated after 4 min.
