Abstract

Objectives. Pituicytomas are rare, solid, well-circumscribed, low grade (grade I), non-neuroendocrine, and noninfiltrative tumors of the neurohypophysis or infundibulum, which appear in the sellar/suprasellar regions. Herein, we present a case with Cushing’s disease (CD) caused by an ACTH-secreting pituitary adenoma in association with an infundibular pituicytoma.

Abstract

Aim: Gestational diabetes mellitus (GDM) is associated with an increased risk of fetal and maternal complications, such astype 2 DM (diabetes mellitus) and cardiovascular disease (CVD). This study aimed to predict the potential for future vascular complications in mothers with GDM by evaluating oxidative stress, endothelial NO synthase (eNOS) expression, and vascular smooth muscle cell (VSMC) proliferation in the umbilical vessels of mothers with GDM. Material and Methods: Subjects were divided into two groups: the normoglycemic control (NGC) group (n = 10) and the GDM group (n = 12). Expression of eNOS and production of reactive oxygen species (ROS) in human umbilical vein endothelial cells (HUVECs) were determined. The mitochondrial mass of HUVECs was evaluated by spectrofluorometry. VSMC proliferation was ascertained in vitro with an EdU cell proliferation assay. Advanced glycation end products (AGEs) accumulation was measured by ELISA and assessed by immunohistochemical staining. Results: VSMC proliferation, eNOS expression, and ROS production in HUVECs were significantly increased, and greater immunohistochemical staining to AGEs was observed in endothelium in GDM. Conclusion: Increased oxidative stress, which elevates eNOS expression and VSMC proliferation in the umbilical vessels of mothers with GDM, may be a sign that mothers have a high potential for developing diabetes or cardiovascular disease in the future. Abstract

Objective: Diabetes Mellitus constitutes a significant health concern given its incidence and associated complications. Furthermore, obesity, being a chronic disease, represents a risk factor for hypertension, cardiovascular diseases, and diabetes. In light of these considerations, our study aims to perform a public screening for conditions that are closely related, including hypertension, obesity, diabetes, and foot health. Methods: The study was conducted over a three-day period from September 12 to September 15, 2017, between the hours of 09:00- 17:00. The sample comprised of volunteers who passed by the established stands and were not diagnosed with diabetes or hypertension. Blood glucose levels (BG) were measured using a glucometer, while weight and height were measured using a scale. Waist circumference was measured using a tape measure, and foot examinations were conducted using a podoscope. In addition, a questionnaire was administered to gather sociodemographic information and health-related data from participants. Measurements were taken of random capillary BG, blood pressure (BP), height, weight, and waist circumference. A general foot examination was performed, and any pressing foot issues were evaluated with a podoscope. Results: In our study, the mean BG level of 428 participants was 126 g/dl, with 7% of them recording BG levels above 200 g/dl. During foot examinations, 29% of participants reported experiencing pain, while 26% had issues with false nail clipping. Pes planus was detected in 14% of individuals during podoscopic examination, and it was observed that those who used sports shoes and those who had previous foot examinations had fewer foot health problems. Additionally, individuals with high BMI values exhibited a higher prevalence of claudication findings, ingrown nails, and dryness and cracks in the skin. Participants with higher systolic BP were found to have more instances of ingrown nails and other nail problems, including interdigital maceration. Conclusion: The selection of appropriate footwear and regular foot examinations are crucial in the prevention of foot health problems. Abstract

Aim: Primary hyperparathyroidism is the most common cause of hypercalcemia.Primaryhyperparathyroidism shows a marked increase in postmenopausal women.In this study, we aimedto evaluate the estrogen receptors alpha (ERα) expression in parathyroid adenomas and theassociation of preoperative serum calcium and parathyroid hormone (PTH) levels with adenomaweight of patients diagnosed with primary hyperparathyroidism.Material and Methods: Total of 50 patients were included in the study, after excluding the patientsdiagnosed with tertiary hyperparathyroidism, double adenoma, parathyroid hyperplasia andparathyroid carcinoma. The diagnosis of parathyroid adenoma was confirmed by pathologist. Aftervalidation of parathyroid adenoma, ERα expression was evaluated with immunohistochemicalmethod. Pathological features of the parathyroid adenomas were recorded.Results: The patients included in the study are mostly women and are in the postmenopausalperiod. Parathyroid adenoma hasn’t shown ERα expression in immunohistochemical staining.Male patients had higher mean pre-operative calcium and vitamin D levels than women (p=0.026,p=0.009 respectively). There was no correlation between menopause and pre-postoperativecalcium, vitamin D and PTH. Correlation was found between adenoma diameter and adenomaweight with pre-operative serum calcium and PTH levels.Conclusion: In our study, statistically significant positive correlation was detected between thediameter of parathyroid adenoma and preoperative serum calcium together with PTH levels. Thisfinding suggests the preoperative clinical features of the patients may be useful in predicting theadenoma diameter. However, it is important to determine the cut-off values of serum calcium andPTH level which might have a significant association with the adenoma diameter., Amaç: Primer hiperparatiroidizm hiperkalseminin en yaygın nedenidir. Primer hiperparatiroidizm postmenopozal kadınlarda belirgin bir artış gösterir. Bu çalışmada primer hiperparatiroidi tanısı alan hastaların paratiroid adenomlarında östrojen reseptör alfa (ERα) ekspresyonu ile preoperatif serum kalsiyum ve paratiroid hormon (PTH) düzeylerinin adenom ağırlığı arasındaki ilişkiyi değerlendirmeyi amaçladık.Gereç ve Yöntemler: Tersiyer hiperparatiroidizm, çift adenom, paratiroid hiperplazi ve paratiroid karsinomu tanısı alan hastalar hariç tutulduktan sonra toplam 50 hasta çalışmaya dâhil edildi. Paratiroid adenom tanısı bir patolog tarafından doğrulandı. Paratiroid adenomunun doğrulanmasından sonra immünohistokimyasal yöntemle ERα ekspresyonu değerlendirildi. Paratiroid adenomlarının patolojik özellikleri kaydedildi.Bulgular: Çalışmaya dâhil edilen hastalar çoğunlukla kadın ve menopoz sonrası dönemdedirler. Hiçbir paratiroid adenomu immünohistokimyasal boyamada ERα ekspresyonu göstermemiştir. Erkek hastaların ameliyat öncesi ortalama kalsiyum ve D vitamini düzeyleri kadınlara göre daha yüksekti (sırasıyla p = 0.026, p = 0.009). Menopoz ile ameliyat öncesi kalsiyum, D vitamini ve PTH arasında korelasyon yoktu. Ameliyat öncesi serum kalsiyum ve PTH düzeyleri ile adenom çapı ve adenom ağırlığı arasında güçlü bir korelasyon bulundu.Sonuç: Çalışmamızda PTH düzeyleri ile birlikte paratiroid adenom çapı ve preoperatif serum kalsiyum arasında istatistiksel olarak anlamlı pozitif güçlü korelasyon saptandı. Bu bulgu hastaların ameliyat öncesi klinik özelliklerinin adenom çapının öngörülmesinde yararlı olabileceğini düşündürmektedir. Bununla birlikte, adenom çapı ile anlamlı bir ilişkisi olabilecek serum kalsiyum ve PTH seviyesinin cut-off değerlerini belirlemek önemlidir.

18. Assessment of Mean Platelet Volume in Acromegaly And Its Relation With The Treatment

Abstract

Amaç: Ortalama trombosit volümü (MPV) ateroskleroz ve trombotik olaylar için bağımsız bir risk faktörüdür. Çalışmamızda akromegalik hastalarda MPV düzeyini ve tedavi ile ilişkisini değerlendirmeyi amaçladık.Materyal ve Metod: Çalışmamızda akromegali hastalarında tedavi öncesi, tedavi sonrası ve remisyondan 2 yıl sonraki MPV düzeylerini değerlendirdik. Ayrıca sonuçları kontrol grubu ile karşılaştırdık. Kliniğimizde takipli 80 akromegali hastası yaş ve cinsiyet açısından benzer kontrol grubu ile retrospektif olarak kıyaslandı. Preoperatif ve postoperatif MPV değerleri ile medikal tedavi alan hastalarda hastalık kontrolünden iki yıl sonraki MPV değerleri karşılaştırıldı.Bulgular: Diyabetik ve non-diyabetik akromegali hastalarında MPV düzeyi kontrol grubuna göre anlamlı oranda yüksekti (p=0,02). Postoperatif kür sağlananlarda postoperatif MPV preoperatif MPV’ye göre anlamlı oranda düşüktü (p=0,001). Cerrahi tedavi başarısızlığından sonra medikal tedavi ile hastalık kontrolü sağlananlarda MPV düzeyi cerrahi ile kür sağlananlarla ameliyattan iki yıl sonra bile benzerdi (p=0,001).Sonuç: Bulgularımız, MPV'nin akromegali hastalarında ateroskleroz ve inflamasyon için yararlı bir belirteç olabileceğini düşündürmektedir.Anahtar kelimeler: Akromegali, Ateroskleroz, İnflamasyon, Ortalama trombosit volümü, Background: Mean platelet volume (MPV) is an independent risk factor of atherosclerosis and thrombotic events. We aimed this study evaluates the levels of MPV in acromegalic patients and its relationship with treatment.Materials and Methods: In this study, we examined the levels of MPV in acromegalic patients before and after treatment and also at two years of remission. The results were also compared with a control group. A total of 80 patients treated at our clinic with the diagnosis of acromegaly age- and gender-matched nonacromegalic controls were reviewed retrospectively. Preoperative and postoperative MPV values as well as MPV values two years after controlling disease in patients with medical treatment were compared.Results: MPV values were significantly higher in both the diabetic and non-diabetic acromegaly patients compared with the control subjects (p=0,02). Postoperative MPV values were significantly reduced compared to preoperative values in patients cured after surgery (p=0,001). Patients who achieved disease control with medical therapy after failure of surgical intervention demonstrated MPV values similar to those reported in patients cured with, even after two years from surgery (p=0,001).Conclusions: Our findings suggested that MPV would be a beneficial marker for atherosclerosis and inflammation in patients with acromegaly.

19. Identification Of Key Genes In Papillary Thyroid Cancer By Transcriptome Analysis

Abstract

Papillary thyroid cancer (PTC) is the most common type of thyroid malignancies. PTC has good prognosis, but it can dedifferentiate into aggressive forms. In this study, we aimed to identify differentially expressed genes (DEGs) between PTC samples and normal controls. We used gene expression microarrays to identify DEGs between 20 PTC samples and 10 normal controls. We performed enrichment analysis to discover biological processes and signalling pathways associated with PTC and construct protein-protein interaction (PPI) networks to find out key genes for the disease. We identified 1554 up-regulated and 912 down-regulated DEGs in PTC samples compared to normal controls. The coagulation system was the most significant pathway and SERPINA1 was the most up-regulated gene of this pathway. CCND1, PGR, CEBPA, CDKN1A, SPDEF, PLAU and MDM2 were key nodes in PPI networks. Causal network analysis revealed that SFN, which was one of the up-regulated DEGs found in our study, was the most causative upstream regulator for PTC. In conclusion, deregulation of SERPINA1, CCND1, PGR, CEBPA, CDKN1A, SPDEF, PLAU and MDM2 genes and coagulation system pathway may contribute to PTC development. SFN may be an important gene in diagnosis, prognosis and novel anticancer drug approaches for PTC. Further experiments are required to confirm the functions of identified DEGs in our study.

26. Proteomic analysis of thyroid tissue reveals enhanced catabolic activity in Graves' disease compared to toxic multinodular goitre.

Abstract

Graves' disease (GD) and toxic multinodular goitre (TMNG) are the most common thyroid diseases which mainly lead to thyrotoxicosis, however, the underlying mechanism of distinct clinical presentations remains unclear. Protein extracts from the thyroid tissue specimens of the patients with GD and TMNG were subjected to Difference Gel Electrophoresis (DIGE). Differentially regulated protein spots were determined by image analysis, and the spots displaying statistically significant differences were identified by Matrix-Assisted Laser Desorption Ionization Time of Flight Mass Spectrometer (MALDI-TOF) followed by MASCOT search. Western blot analysis was used to verify changes occurring at the protein levels. The identified proteins were classified based on their functions in metabolic pathways using bioinformatics algorithms. Fifteen proteins showed significant alterations in abundance between the two disease groups. Bioinformatic analysis revealed the differentially regulated proteins were particularly related to catabolism, oxidative stress and especially energy utilization pathways, including glycolysis, proteolysis, ketone body catabolism and other energy metabolism-related pathways. Significance of the study Previously, GD has been the subject of many studies that performed the proteomics approaches in the orbital tissue samples or tear. This is one of the very few studies that investigate the changes in the proteome of thyroid tissue in GD. We demonstrated mainly the upregulation of catabolic activity-related proteins in patients with GD compared to TMNG. Although it remains to be elucidated, some of these proteins can be used as markers for GD or have a role in the pathogenesis of the disease. Abstract

Aim: In this study, we aimed to evaluate the levels of brown adipose tissue markers uncoupling protein-1 (UCP-1), irisin, bone morphogenetic protein-7 (BMP-7) and PR-Domain Zinc Finger Protein-16 (PRDM-16) in Cushing's disease (CH) with hypercortisolism. Methods: The study was conducted prospectively with 48 CH patients and 40 healthy volunteers between 2018 and 2019. Two cc of peripheral blood taken from the participants was centrifuged and stored at -80° C degrees. Cushing's syndrome was excluded by performing 1 mg dexamethasone suppression test in the control group. Blood samples were analyzed by Enzyme-linked Immunosorbent Assay method. Results: The patient group included 11 males (22.9%), 37 female (77.1%); 9 male (22.5%) and 31 females (77.5%) in the control group. Body mass index was 31.29±5.76 kg/m² in the patient group and 33.42±3.11 kg/m² in the control group. PRDM-16, Irisin, BMP-7, UCP-1 levels were not significantly different between the two groups. While there was a positive correlation between serum cortisol and irisin, a negative correlation was observed between urinary free cortisol and UCP-1. Conclusion: These data suggest that long-term exposure to high doses of glucocorticoids in CH patients causes loss of adipose tissue functionality and development of resistance in vivo. Abstract

To compare the serum levels of adiponectin and resistin between patients with polycystic ovary syndrome (PCOS) and healthy control subjects matched for age and body mass index (BMI), and to assess possible correlations of adiponectin and resistin to the hormonal and metabolic parameters of the syndrome. Materials and methods: Fifty five patients with PCOS diagnosis composed of 25 obese, 13 overweight, 17 normal weight subjects, and 49 healthy women matched for age and BMI were included in this study. Hormone and lipid profile, adiponectin, and resistin were measured in all cases. Results: Serum adiponectin levels were similar in the lean PCOS patients and the healthy control group. In obese patients and the control groups, serum adiponectin levels were significantly lower than those in the lean control group. Serum adiponectin levels were similar in the obese PCOS and the obese control groups. Serum resistin levels were similar in both patient and control groups. There was a correlation between serum adiponectin levels and fasting insulin, HOMA-IR, triglyceride, and HDL-C in the PCOS group. A significant correlation in the positive direction was observed among serum resistin levels and BMI in the patient group. Conclusion: Our study showed that serum adiponectin levels were lower in obese women with PCOS than in the normal weight control group. There was a negative correlation between serum adiponectin levels with insulin resistance. We suggest that resistin is not associated with PCOS pathogenesis but it may be an adipocytokine that is affected by BMI.
