Abstract

Scopo del lavoro Re-TUR is strongly advocated for T1G3, because of the high incidence residual disease and mainly the risk of substaging. Its real clinical value remains to be determined and the clinical factors that may influence the decision. Some authors suggest that when muscle is present in the specimen, re-TUR may be avoided. To evaluate if the presence of muscle or not at the first TUR in T1G3 bladder cancer makes a difference in recurrence, progression and cancer specific survival after re-TUR. Materiali e metodi In a large retrospective cohort of 2530 primary T1G3 initially treated with BCG, 953 (37.7%) had a re-TUR. According to the presence or not of muscle in the specimen of primary TUR, patients were divided in 4 groups: group 1 (no muscle, no re-TUR), group 2 (no muscle, re-TUR), group 3 (muscle, no re-TUR) and group 4 (muscle, re-TUR). Clinical outcomes were compared across the 4 groups. Risultati Table 1 shows the distribution of prognostic factors and clinical outcomes across the 4 groups. Large tumours and multifocal tumours were more likely to have received a re-TUR independent of whether or not the primary specimen contained muscle. Re-TUR had a significant impact on progression, cancer specific (CSS) and overall survival (OS) only when muscle was not present in the primary specimen. When adjusting for the most important prognostic factors including age, tumour size and the presence of CIS, re-TUR in the absence of muscle maintained a positive impact on time to progression (HR 0.44, p=0.048), CSS (HR 0.31; p=0.067), and OS (HR 0.50; p=0.060) and showed a borderline effect on time to first recurrence (HR 0.69; p=0.100). Re-TUR in presence of muscle in the primary specimen did not improve the outcome for any of the endpoints after adjusting for prognostic factors. Discussione Even if re TUR is recommended in high risk tumors, it can delay the intravescial therapy. When muscle is present in TUR specimen the risk of substaging or residual tumor should be balanced with the need of keep on with adequate treatments. Conclusioni Our retrospective analysis shows that re-TUR may not be mandatory in T1G3 patients when muscle is present in the specimen of the primary TUR.

Abstract

The lipase-catalyzed synthesis of thiamphenicol derivatives has been studied through complementary acylation and hydrolytic approaches, finding Candida antarctica lipase B as the most efficient biocatalyst for the selective modification of both thiamphenicol and thiamphenicol diacetate, respectively. The best results have been obtained using acylation reactions with different vinyl esters of variable length, yielding the corresponding 3'-monoesters with excellent yields and in short reaction times. The conditions have been analyzed in terms of substrate concentration, enzyme loading and type of acyl donor. The reuse of the enzyme for five-times without significant loss of the activity has also been demonstrated. Alternatively, the hydrolytic approach has allowed the preparation of some 1'-monoesters in good yields, although the reactivity and selectivity levels were lower than the ones achieved for the complementary acetylation reaction. A síntese de derivados do tianfenicol, catalisada por lipases, foi investigada através da realização das reações de acilação e de hidrólise, como abordagens complementares, sendo a lipase Candida antarctica B o biocatalisador mais eficiente para as modificações seletivas de ambos, o tianfenicol e o correspondente diacetato do tianfenicol, respectivamente. Os melhores resultados foram obtidos nas reações de acilação com diferentes ésteres vinílicos, contendo cadeias carbônicas de comprimento variável, levando aos correspondentes 3'-monoésteres com excelentes rendimentos e em tempos reacionais curtos. As condições reacionais foram analisadas em termos de concentração de substrato, carga de enzima e tipo de doador de acila. Foi demonstrado o reuso da enzima por cinco vezes sem perda significativa de atividade. Alternativamente, a abordagem hidrolítica propiciou a preparação de alguns 1'-monoésteres em bons rendimentos, ainda que os níveis de reatividade e seletividade foram mais baixos que os obtidos na reação complementar de acetilação.

Abstract

The cytotoxic activity at 50 μg/ml of extracts obtained from eleven fungal strains associated to Eudistoma vannamei, an endemic ascidian from Northeast Brazil, against two cell lines, i.e., the HCT-8 (colon cancer) and the MDA-MB-435 (melanoma) cell lines, was investigated. The most promising extract (EV10) was obtained from a fungus identified as Aspergillus sp. by molecular analysis and was selected for bioassay-guided isolation of its active principals. Large-scale fermentation of EV10 in potato-dextrose broth followed by chromatographic purification of the active extract from the liquid medium allowed the isolation of the isocoumarins mellein, cis-4-hydroxymellein, and trans-4-hydroxymellein, besides penicillic acid. All isolated compounds were tested for their cytotoxicity against the tumor cell lines MDA-MB-435 and HCT-8 and revealed penicillic acid as the only cytotoxic compound (cell growth inhibitions >95%).
