Abstract

A study was conducted to analyze changes in the levels of novel luminal cholecystokinin-releasing factor (LCRF) in the small intestinal lumen before and after bile-pancreatic juice diversions in rats. A specific radioimmunoassay was utilized to carry out the analysis. Anti-LCRF antiserum was raised in rabbits as a sensitive radioimmunoassay was established.The locationalization of LCRF was then analyzed by immunohistochemistry.

Abstract

Plasma cholecystokinin (CCK) responses after ingestion of a test meal in patients with mild chronic pancreatitis having abdominal pain were studied with a radioimmunoassay using the CCK specific antiserum (OAL-656) produced by a novel immunization procedure. Mean concentration of the fasting plasma CCK determined using CCK-8 as a standard was 31.5 ± 5.8 pg/ml in six patients who had mild impaired exocrine function with pain, and was significantly higher than 10 healthy subjects (9.8 ± 1.8 pg/ml). In those patients, the ingestion of a liquid test meal led to a peak of 75.1 ± 25.4 pg/ml at 30 min, and the 120-min integrated CCK response (5427 ± 1217.3 pg · min/ml) was significantly higher than in healthy subjects (1538 ± 110.1 pg · min/ml). Abstract

Postprandial responses of plasma cholecystokinin (CCK) in patients with severe chronic pancreatitis (n = 7) were studied. Plasma CCK level rose from 11.2 ± 1.8 pg/ml at the basal level to a maximum of 23.3 ± 3.0 pg/ml at 10 min after the ingestion of a liquid meal in healthy subjects (n = 6). However, such significant plasma CCK response to the meal was not observed in patients with chronic pancreatitis in whom CCK levels rose from a basal level of 9.7 ± 0.91 pg/ml to a peak of 13.8 ± 1.6 pg/ml at 60 min. It is suggested that the low response of CCK after the meal might reflect impaired function of the enteropancreatic axis to intraluminal stimuli in patients with severe chronic pancreatitis. Abstract

For exploration on the elimination of cholecystokinin from the circulation, the disappearance half-time of cholecystokinin-octapeptide was estimated with cholecystokinin specific radioimmunoassay in normal subjects and patients with chronic renal failure and with hepatic cirrhosis. With a brief infusion of 30.4 ng/kg of cholecystokinin-octapeptide for 2 min, plasma cholecystokinin level rose from 16.1 ± 3.6 pg/ml (mean ± SE) to 216.5 ± 6.1 pg/ml at 3 min after starting infusion, and decreased rapidly in a single exponential fashion for 10 min in hepatic cirrhosis. The disappearance half-time of cholecystokinin-octapeptide in patients with hepatic cirrhosis was 2.45 ± 0.07 min, and it was significantly longer than that in normal subjects (1.30 ± 0.07) or patients with chronic renal failure (1.70 ± 0.11). Abstract

A highly sensitive and precise radioimmunoassay system for plasma cholecystokinin (CCK) was developed with the anti-CCK-8 specific antiserum which raised against N-terminal amino acids residue of sulfated CCK-8 and reacted with CCK-8, CCK-33, and CCK-39 but not with gastrin and its related peptides. Mean concentration of the fasting plasma CCK determined with this method using CCK-8 as standard was 12.9 ± 5.9 pg/ml in normal subjects (n = 26), and in patients with hepatic cirrhosis it was significantly higher (36.7 ± 16.9 pg/ml, n = 9, p < 0.01) than in normal subjects. In six young healthy volunteers, intraduodenal infusion of fat caused a significant increase (p < 0.05) of plasma CCK from a basal level of 8.0 pg/ml to a peak of 43.0 ± 12.0 pg/ml at 20 min after starting of infusion. In the same subjects, a significant increase of plasma CCK was also observed by amino acids infusion, but no elevation of plasma CCK level was found during intraduodenal acidification. Abstract

A protein with pancreastatin-tike immunoreactivity has been isolated and purified from liver metastasis of a patient with insulinoma. NH2-terminal residue analysis, in conjunction with the use of antibodies that are specific for the C-terminal amide peptide of porcine pancreastatin, identified this protein as a 186-amino-acid protein corresponding to human chromogranin A-1 16-301 (the fragment corresponding to the positions from 116 to 301 of human chromogranin A). Digestion of this protein with trypsin yielded a 48-amino-acid peptide with the retention of full pancreastatin activity. Serum from patient with insulinoma contains a peptide specie(s) that comigrates with the 48-amino-acid pancreastatin, suggesting that this peptide might be a physiologically important circulation form of pancreastatin in humans. A sensitive radioimmunoassay was established using antibody developed against a synthetic 29-amino-acid peptide amide of pancreastatin. Immunocytochemical staining revealed that a major population of human pancreatic islet cells were immunoreactive to the antiserum but with varying intensity of staining.
