However, limited information is available regarding HuNoV and C. difficile coinfections. Our study was designed to evaluate the prevalence of HuNoV and C. difficile coinfections among adult patients in a hospital setting and disease symptomatology. Study design and setting: For a cross-sectional analysis, 384 fecal samples were tested for the presence of C. difficile toxins from patients (n=290), whom the provider suspected of C. difficile infections. Subsequent testing was then performed for HuNoV genogroups I and II. Multinomial logistic regression was performed to determine symptoms more frequently associated with coinfections. Results: The final cohort consisted of the following outcome groups: C. difficile (n=196), C. difficile + HuNoV coinfection (n=40), HuNoV only (n=12), and neither (n=136). Coinfected patients were more likely to develop nausea, gas, and abdominal pain and were more likely to seek treatment in the winter season compared with individuals not infected or infected with either pathogen alone. Conclusion: Our study revealed that patients with coinfection are more likely to experience certain gastrointestinal symptoms, in particular abdominal pain, suggesting an increased severity of disease symptomatology in coinfected patients. Titelblatt und Inhaltsverzeichnis Einleitung Materialien und Methoden Ergebnisse Diskussion Literaturverzeichnis Anhang, Zur Therapie der chronischen Hepatitis C wird derzeit pegyliertes Interferon (IFN)-a in Kombination mit dem Nukleosidanalog «Ribavirin» eingesetzt. Der Erfolg der Kombinationstherapie ist stark genotypabhängig. Derzeit sind etwa 40 % der Genotyp 1 und 70-80 % der Genotyp 2/3 Patienten in der Lage, das Virus nachhaltig zu eliminieren. In der vorliegenden Arbeit wurde der Mechanismus der Resistenz von HCV gegenüber der antiviralen IFN-Therapie untersucht. Als Grundlage zur molekularen Charakterisierung dienten Seren von 9 Patienten mit den HCV-Genotypen 1a/b und 3, die im Rahmen von klinischen Studien charakterisiert wurden. Der Fokus dieser Arbeit wurde auf das Nichtstrukturprotein 5A (NS5A) gelegt. Mit Hilfe von funktionalen Reporterassays wurde der Einfluss von insgesamt 14 verschiedenen NS5A- Proteinen untersucht. Hierbei wurden Isolate der HCV-Genotypen 1a, 1b und 3a von Patienten mit unterschiedlichem Therapieansprechen (non-response (NR), sustained virologic response (SVR) und breakthrough (BT)) analysiert. Es konnte gezeigt werden, dass NS5A IFN-Signaling funktional durch Inhibition des Transkriptionsfaktors p48 (IRF-9) inhibieren kann. Alle 14 NS5A-Proteine waren in der Lage IFN-stimulated response element (ISRE)-vermittelte Genexpression zu inhibieren. Dieser Effekt ist somit unabhängig vom Therapieansprechen des Patienten und vom viralen Genotyp. Co-Expression von p48 führte zu einer Wiederherstellung des IFN-Signalings in NS5A-transfizierten Zellen, nicht aber in Zellen, die mit dem IFN-Antagonisten M27 transfiziert wurden. Mutagenesestudien zeigten, dass die funktionale Domäne für diese Inhibition auf den N-terminalen 238 Aminosäuren von NS5A liegt und weder die putative ISDR, PKR oder V3-Region hierbei involviert sind. Die N-terminalen Deletionsmutanten NS5A-239-451 und 279-451 zeigten keinen Einfluss auf Typ-1-IFN vermitteltes Signaling. Ein Effekt auf IFN-Induktion wurde ebenfalls beobachtet, verhielt sich aber im Vergleich zu bekannten Antagonisten dieser Signalkaskade, wie z.B. dem Influenza A Protein «NS1» und dem Ebola Protein «VP35» sehr viel schlechter. Eine biologische Relevanz konnte mit Hilfe der IFN-sensitiven Rekombinante des Influenza A-Virus «delNS1» gezeigt werden. NS5A war hierbei in der Lage, Viruswachstum in IFN-kompetenten MDCK2-Zellen wiederherzustellen. Somit kann NS5A den IFN-Antagonisten NS1 funktional komplementieren. Insgesamt unterstützt diese Arbeit die Beobachtung, dass NS5A in der Lage ist, IFN-Signaling zu inhibieren. Es wurde erstmalig gezeigt, dass NS5A p48 (IRF 9) inhibieren kann und somit ISRE-abhängige Genexpression blockiert wird., Chronic hepatitis C is currently treated with pegylated interferon (IFN)-a in combination with the nucleoside analog «ribavirin». Success of the combination therapy strongly depends on the viral genotype. Today approximately 40 percent of the genotype 1 and 80 % of the genotype 3 patients show a sustained virus clearance. The mechanism of resistance of HCV against antiviral IFN-therapy is subject of this study. The basis of the molecular characterisation constitute sera of nine patients with genotypes 1a/b and 3a that have been characterised in clinical trials. Based on recent literature the emphasis of this work was put on the nonstructural protein 5A (NS5A). Using functional reporter-assays the influence of 14 different NS5A-proteins was analysed. Isolates of the genotypes 1a/b and 3a from patients with different therapy-responses (non- response (NR), sustained virologic response (SVR) und breakthrough (BT) were characterised. It was shown that NS5A functionally blocks IFN-signaling by inhibiting the transcription factor p48 (IRF-9). All 14 NS5A-proteins analysed were functionally capable of inhibiting IFN-stimulated response element (ISRE)-induced gene expression. Consequently this effect is independent of the therapy-response of the patient and independent of the viral genotype. Co- expression of p48 lead to a rescue of IFN-signaling in NS5A-transfected cells but not in cells transfected with the IFN-antagonist M27. Mutagenesis studies have shown that the functional domain for this inhibition resides on the n-terminal 238 amino acids of NS5A and neither the ISDR, the PKR nor the V3 region are involved in this process. The n-terminally deleted mutants NS5A-239-450 and NS5A-279-451 show no influence on type-1-IFN induced signaling. An effect on IFN-induction was also observed, although it did not compare to the known inhibitors of this pathway, such as the Influenza A protein «NS1» and the Ebola protein «VP35». A biological relevance was shown using the recombinant Influenza A virus «delNS1», which is IFN-sensitive. NS5A was capable of rescuing viral growth in IFN-competent MDCK2-cells. Hence NS5A was capable of complementing the IFN-antagonist NS1. This work adds to the observations that NS5A functionally inhibits IFN-signaling. It shows for the first time that NS5A can inhibit p48 (IRF 9) and blocks ISRE-dependent gene expression.
