Background: Chronic lateral epicondylitis (LE), also known as tennis elbow, affects 1–3% of the population, primarily those over 40 years old. Most cases resolve with conservative treatments, but some require more advanced interventions. Extracorporeal shockwave therapy (ESWT) has emerged as a non-surgical treatment option, utilizing either low- or high-energy levels to alleviate pain and improve function. Objective: This study aimed to compare the efficacy of low-energy versus high-energy ESWT in the treatment of chronic LE, focusing on pain relief and functional improvement. Methods: A retrospective observational study was conducted including patients treated for chronic LE between 2021 and 2024. Participants were divided into two groups: low-energy ESWT (0.10 mJ/mm
2 ) and high-energy ESWT (0.20 mJ/mm2 ). Both groups received 2400 pulses at a frequency of 6 Hz once a week for three weeks. Pain and functional outcomes were measured using a visual analog scale (VAS) and the Patient-Rated Tennis Elbow Evaluation Questionnaire (PRTEE) at the baseline, three months (T1), and six months (T2) post-treatment. Results: Forty-six patients participated, with 24 in the low-energy group and 22 in the high-energy group. Baseline demographics and clinical characteristics were similar across groups. At T1 and T2, the low-energy group showed significantly greater reductions in the VAS scores (T1: 4.45 ± 0.8 vs. 3.6 ± 1.7, p = 0.04; T2: 3.2 ± 1.2 vs. 2.1 ± 1.1, p = 0.004) and PRTEE scores (T1: 34.3 ± 6.9 vs. 26.8 ± 11.9, p = 0.03; T2: 25.3 ± 6 vs. 17.6 ± 9, p = 0.005). Significant treatment–time interactions were observed for both the VAS and PRTEE scores, indicating sustained improvements in the low-energy group. Conclusions: Low-energy ESWT was more effective than high-energy ESWT in treating chronic LE, providing greater and longer-lasting pain relief and functional improvement. These findings suggest that low-energy ESWT should be preferred in clinical practice for managing this condition. Future research should focus on larger sample sizes and randomized controlled trials to confirm these results and explore the underlying mechanisms of differential efficacy between energy levels. Different types of tissues respond differently to the action of oxidative stress. The visual system is very sensitive to oxidative action due to continuous exposure to light. In consideration of the growing interest of scientific studies towards various compounds endowed with antioxidant and anti-inflammatory properties, we performed a review of the literature focusing on the use of some antioxidant molecules for the treatment of conditions affecting the visual system. In this study, we focused on the ability of two antioxidant agents, the small molecule α- lipoic acid (ALA) and the enzyme superoxide dismutase (SOD), to influence the neurodegenerative physiological processes related to aging and oxidative stress affecting the ocular segment. The literature data report that ALA and SOD can protect against neurodegenerative effects both the optic nerve and retina and, if administered together, they are able to lower the levels of oxidative stress, thus preventing neurodegeneration and reducing the apoptotic process. Brain tumors account approximately for 20% of all childhood cancers and are characterized by a large diversity of morphological entities. The formation of abnormal, dysfunctional tumor vasculature and glioblastoma stem-like cells (GSCs) are believed to be the major components of the difficulty to treat these tumors effectively. Massive formation of blood vessels is one of the most important histological elements to determine the progression and histological grading of tumors. We hypothesized that an increased expression of TGF-β1 in tumor cells stimulates tumor neo-vascularization by mediating the secretion of relevant angiogenic factors via an autocrine mechanism. Expression of TGF-β in relation to VEGF and VEGF-receptors involved in angiogenesis and inflammation pathways was evaluated in pediatric patients with brain tumors and compared with normal tissues. Our results demonstrated that TGF-β1, VEGF-A and VEGF-RII were significantly related to the development and to the growth of glioblastoma. We can speculate that TGF-β1 and VEGF are involved in the cascade of the malignant progression of glioblastoma. These factors promote tumorigenesis and malignant progression of glioblastoma by a mechanism determining anti-apoptotic, angiogenetic and invasive behaviour of the tumor cells. Basing on our experimental data, we propose that VEGF may be the double promoter responsible not only for the tumor vessels, but also for the tumor stem cells [1]. The aim of this experimental thesis focuses on the study of eye diseases related to the involvement of growth factors, of inflammatory cytokines in chronic-inflammatory, and of tissue remodeling. Particular emphasis has been addressed to the physiopathology of the eye surface and the complex relationships existing between cells and mediators in the immune response. Furthermore, the research has been directed to molecular events that control these biological responses. The lost in regulation of the inflammatory processes corresponds to the loss of control of the production/action of cytokines followed by the onset of inflammatory or autoimmune pathologies. Immunohistochemical technique has been performed in order to evaluate the physiopathological behaviour of inflammatory cytokines and of some growth factors (TGF-b1, IL-1b, IL-6, TNF-a and VEGF) located in the ocular tissues. Samples were obtained from human patients affected by glaucoma, diabetic retinopathy and macular degeneration. Our main purpose was to assess the levels of expression and localization of pro-inflammatory cytokines by immunohistochemistry in order to identify their involvement in the pathology, assuming for them a role in the modulation of these factors as a potential therapeutic target. Our study has shown an imbalance between pro- and anti-inflammatory cytokine plays a key role in the pathogenesis of retinal diseases. This disproportion within a complex regulatory network is related to some immunological processes that promote autoimmunity, chronic inflammation and / or tissue destruction. We were able to confirm that the inflammatory process involves a complex cascade of molecular and cellular biological signals, altering the physiological responses of the ocular tissues. The transmission electron microscopy investigation (TEM) showed that at the site of the lesion cells the probable release of molecular signals produces changes in the affected area: dilation of blood vessels resulting in increased blood flow and vascular permeability, exudation of fluid containing proteins such as immunoglobulins and invasion by leukocytes. It is well known that chronic and prolonged inflammation is a fundamental feature in some diseases such as atherosclerosis, obesity, diabetes, rheumatoid arthritis, asthma and various types of cancer. These diseases are strictly connected with their ocular manifestations: diabetic retinopathy, scleritis, uveitis, dry eye syndrome and ocular tumours. It should be noticed that pharmacological and clinical advances occur in areas apparently distant from application in ophthalmology, but they should be potentially used also in the ocular field by a translational application. Since many ocular tissues have a immune-privileged system to protect the delicate visual apparatus, the control of the acute inflammation is regulated by the same pathways and mediators that manage inflammatory responses in other organs. These scientific informations not only provide further support to the chronic inflammation, that plays a significant role in the pathogenesis of diabetic retinopathy, but also suggest that the inflammatory process is active even before retinopathy (diabetic and/or proliferative) is clinically diagnosed. Therefore, the alteration of the profile of some cytokines might be useful both as a predictive marker as well as an important target for the development of new therapeutic treatments. A novel pharmacological approach to treat eye inflammation may be represented by progress in the cellular therapy. In recent years, research has been devoted to the development of cell therapy applied to eye diseases producing a remarkably increased risk of visual impairment. Recently, cellular therapy has evolved into two research pathways: one finalized to a remodeling of the structure and the relative functions of specific tissues using stem cells; the other one has been focused on the restore of the immunological homeostasis controlling the harmful effects caused by inflammatory diseases.

- Abstract
Background: Sepsis is a life-threatening organ dysfunction with high mortality and morbidity rate and with the disease progression many alterations are observed in different organs. The gastrointestinal tract is often damaged during sepsis and septic shock and main symptoms are related to increased permeability, bacterial translocation and malabsorption. These intestinal alterations can be both cause and effect of sepsis. Objective: The aim of this review is to analyze different pathways that lead to intestinal alteration in sepsis and to explore the most common methods for intestinal permeability measurement and, at the same time to evaluate if their use permit to identify patients at high risk of sepsis and eventually to estimate the prognosis. Material and Methods: The peer-reviewed articles analyzed were selected from PubMed databases using the keywords “sepsis” “gut alteration”, “bowel permeability”, “gut alteration”, “bacterial translocation”, “gut permeability tests”, “gut inflammation”. Among the 321 papers identified, 190 articles were selected, after title - abstract examination and removing the duplicates and studies on pediatric population,only 105 articles relating to sepsis and gut alterations were analyzed. Results: Integrity of the intestinal barrier plays a key role in the preventing of bacterial translocation and gut alteration related to sepsis. It is obvious that this dysfunction of the small intestine can have serious consequences and the early identification of patients at risk - to develop malabsorption or already malnourished - is very recommended to increase the survivor rate. Until now, in critical patients, the dosage of citrullinemia is easily applied test in clinical setting, in fact, it is relatively easy to administer and allows to accurately assess the functionality of enterocytes. Conclusion: The sepsis can have an important impact on the gastrointestinal function. In addition, the alteration of the permeability can become a source of systemic infection. At the moment, biological damage markers are not specific, but the dosage of LPS, citrulline, lactulose/mannitol test, FABP and fecal calprotectin are becoming an excellent alternative with high specificity and sensitivity.

In recent years atherosclerosis has become a serious health challenge given its causal association with cardiovascular disease and stroke, which predictably leads to most of the morbidity and mortality worldwide [1]. The mechanism by which in this pathology there is an extensive proliferation of vascular smooth muscle cells (VSMCs) and their migration to the intima remain poorly understood. Recently, low molecular weight heparin (LMWH) has been shown to inhibit the proliferation of VSMCs and, in particular, a kind of these heparins, parnaparin (PNP), acts on the development and clinical course of atherosclerosis in animal models [2].We hypothesized that PNP might modulate VSMCs migration and thereby decrease vascular remodelling in atherosclerosis. Vascular remodelling was evalu- ated in aortas of an animal model of atherosclerosis, (ApoE)-deficient mice (ApoE -/-) with normal or western diet. We treated animals with PNP to evaluate its pro- tective effect. Thereafter, we evaluated morphological changes and the expression of proteins linked to atherogenesis and atheroprogression in particular an enzyme involved in oxidative stress, iNOS, some inflammatory mediators, such as TNF-α, IL-1 and IL-6, and markers of VSMCs alterations (PAI-1 and TSP1).Our results demonstrated that PNP downregulates VSMCs proliferation and migration, mediated by PAI-1 and TSP1, and reduces inflammation and oxidative stress in vessels.These data suggested that LMWH, in particular PNP, could be a theoretically practical tool in the prevention of atherosclerotic vascular modification., Italian Journal of Anatomy and Embryology, Vol. 120, No. 1 (Supplement) 2015

Dupuytren’s contracture (DC) is a benign fibroproliferative disease causing fibrotic nodules and fascial cords with resultant debilitating flexion contracture deformities. The present study was designed to characterize pro-inflammatory cytokines and growth factors involved in the genesis, progression and recurrence of the disease to optimize therapeutic agents and strategies for controlling Dupuytren’s disease. The expression of pro-inflammatory cytokines and other growth factors was detected by immunohistochemistry and immunofluorescence in the fibrotic nodules and normal palmar fascia resected respectively from patients affected by Dupuytren’s contracture and Carpal Tunnel Syndrome (as negative controls). RT-PCR analysis was performed to quantify the expression of TGF-β1, IL-1β and VEGFa in the myofibroblasts and fibroblasts isolated from Dupuytren’s nodules. Histological analysis showed the high cellularity and rate of proliferation of Dupuytren’s tissue with the presence of myofibroblastic isotypes. Our data showed the strong expression of TGF-β1, IL-1β and VEGFa in Dupuytren’s fibromatosis nodules suggesting a direct role of these markers in the onset, progression and recurrence of the disease. Our observations suggest that TGF-β1, IL-1β and VEGFa may be considered potential therapeutic targets in the treatment of Dupuytren’s disease.
