Abstract

Multiple Sclerosis (MS) is a chronic autoimmune and inflammatory neurological disorder characterised by episodes of symptom exacerbation, known as relapses. In this study, we investigate the role of environmental factors in relapse occurrence among MS patients, using data from the H2020 BRAINTEASER project. We employed predictive models, including Random Forest (RF) and Logistic Regression (LR), with varying sets of input features to predict the occurrence of relapses based on clinical and pollutant data collected over a week. The RF yielded the best result, with an AUC-ROC score of 0.713. Environmental variables, such as precipitation, NO2, PM2.5, humidity, and temperature, were found to be relevant to the prediction.

Abstract

Applying Artificial Intelligence (AI) and Machine Learning (ML) in critical contexts, such as medicine, requires the implementation of safety measures to reduce risks of harm in case of prediction errors. Spotting ML failures is of paramount importance when ML predictions are used to drive clinical decisions. ML predictive reliability measures the degree of trust of a ML prediction on a new instance, thus allowing decision-makers to accept or reject it based on its reliability. To assess reliability, we propose a method that implements two principles. First, our approach evaluates whether an instance to be classified is coming from the same distribution of the training set. To do this, we leverage Autoencoders (AEs) ability to reconstruct the training set with low error. An instance is considered Out-of-Distribution (OOD) if the AE reconstructs it with a high error. Second, it is evaluated whether the ML classifier has good performances on samples similar to the newly classified instance by using a proxy model. We show that this approach is able to assess reliability both in a simulated scenario and on a model trained to predict disease progression of Multiple Sclerosis patients. We also developed a Python package, named relAI, to embed reliability measures into ML pipelines. We propose a simple approach that can be used in the deployment phase of any ML model to suggest whether to trust predictions or not. 9. Multimodal imaging assessment of neurodegeneration and neuroplasticity in multiple sclerosis

Abstract

Background: The advent of advanced magnetic resonance imaging (MRI) techniques has allowed to study multiple sclerosis (MS) evolution over time, providing qualitative information and quantitative measures related to tissue damage. However some relevant questions remain unanswered, mainly concerning a better definition of structural and functional features of disease progression as well as the identification of causal factors for disease evolution over time Aims to characterize different aspects of disease progression, by applying a multimodal imaging approach. Methods: Together with traditional measures of lesion volumes, such as T1-weighted and T2-weighted lesion loads, and of global and regional brain volumes, we applied: quantitative susceptibility mapping (QSM) to quantify iron; diffusion tensor imaging (DTI) to analyze microstructural tissue properties; functional MRI (fMRI) to analyze activity pattern in response to a motor task; resting state fMRI to analyze functional connectivity within regions of interest; optical coherence tomography (OCT) to evaluate the usefulness of an alternative imaging technique in the study of neuroaxonal damage. Results: We showed that disease progression is related to iron dyshomeostasis in deep grey matter structures. We confirmed the usefulness of a novel MRI marker such as atrophied T2-LV, reflecting both lesion accrual and simultaneously occurring irreversible tissue loss. Moreover, we confirmed the role of sNfL and OCT measures as markers reflective of neuroaxonal degeneration. Finally, we analyzed the effect of motor rehabilitation on clinical and MRI markers, supporting its usefulness as a non-pharmacological treatment able to impact on disability. Discussion/Conclusions: The findings here reported highlight that MS disease progression cannot be properly characterized using conventional radiological imaging, or even a single advanced neuroimaging modality. Only the combination of structural and functional MRI techniques investigating complementary aspects of MS-related pathogenetic mechanisms can lead to a better understanding of disease progression.

Abstract

Increased choroid plexus (CP) volume has been recently implicated as a potential predictor of worse multiple sclerosis (MS) outcomes. The biomarker signature of CP changes in MS are currently unknown. To determine the blood-based biomarker characteristics of the cross-sectional and longitudinal MRI-based CP changes in a heterogeneous group of people with MS (pwMS), a total of 202 pwMS (148 pwRRMS and 54 pwPMS) underwent MRI examination at baseline and at a 5-year follow-up. The CP was automatically segmented and subsequently refined manually in order to obtain a normalized CP volume. Serum samples were collected at both timepoints, and the concentration of 21 protein measures relevant to MS pathophysiology were determined using the Olink™ platform. Age-, sex-, and BMI-adjusted linear regression models explored the cross-sectional and longitudinal relationships between MRI CP outcomes and blood-based biomarkers. At baseline, there were no significant proteomic predictors of CP volume, while at follow-up, greater CP volume was significantly associated with higher neurofilament light chain levels, NfL (standardized β = 0.373, p = 0.001), and lower osteopontin levels (standardized β = −0.23, p = 0.02). Higher baseline GFAP and lower FLRT2 levels were associated with future 5-year CP % volume expansion (standardized β = 0.277, p = 0.004 and standardized β = −0.226, p = 0.014, respectively). The CP volume in pwMS is associated with inflammatory blood-based biomarkers of neuronal injury (neurofilament light chain; NfL) and glial activation such as GFAP, osteopontin, and FLRT2. Abstract

Optic neuritis (ON) is the most common cause of vision loss in young adults. It manifests as acute or subacute vision loss, often accompanied by retrobulbar discomfort or pain during eye movements. Typical ON is associated with Multiple Sclerosis (MS) and is generally mild and steroid-responsive. Atypical forms are characterized by unusual features, such as prominent optic disc edema, poor treatment response, and bilateral involvement, and they are often associated with autoantibodies against aquaporin-4 (AQP4) or Myelin Oligodendrocyte Glycoprotein (MOG). However, in some cases, AQP4 and MOG antibodies will return as negative, plunging the clinician into a diagnostic conundrum. AQP4- and MOG-seronegative ON warrants a broad differential diagnosis, including autoantibody-associated, granulomatous, and systemic disorders. These rare forms need to be identified promptly, as their management and prognosis are greatly different. The aim of this review is to describe the possible rarer etiologies of non-MS-related and AQP4- and MOG-IgG-seronegative inflammatory ON and discuss their diagnoses and treatments. Abstract

Parkinson's disease (PD) is the second most common neurological disease affecting the elderly population. Pharmacological and surgical interventions usually employed for PD treatment show transient effectiveness and are associated with the insurgence of side effects. Therefore, motor rehabilitation has been proposed as a promising supplement in the treatment of PD, reducing the global burden of the disease and improving patients quality of life. The present systematic review aimed to critically analyse the literature concerning MRI markers of brain functional and structural response to motor rehabilitation in PD. Fourteen out of 1313 studies were selected according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses criteria. Despite the limited number of retrieved studies coupled with their heterogeneity prevent ultimate conclusions from being drawn, motor rehabilitation seems to have beneficial effects on PD as measured both with clinical outcomes and MRI derived indices. Interestingly, consistent results seem to indicate that motor rehabilitation acts via a dual mechanism of strengthening cortico-subcortical pathways, restoring movements automaticity, or activating compensatory networks such as the fronto-parietal one. The employment of more advanced and quantitative MRI methods is warranted to establish and validate standardized metrics capable of reliably determining the changes induced by rehabilitative intervention.
