Abstract

Plasma atrial natriuretic factor (ANF), plasma cyclic guanosine monophosphate (cGMP), plasma aldosterone, plasma-renin activity (PRA) and he. modynamic parameters were measured in heart-trasplant recipients and control patients chest pain syndrome) during right-sided heart catheterization under basal conditions and in response to an intravenous saline load. Bawl plasma ANF and cGMP were higher in heart-transplant recipients than in control patients, whereas PRA and plasma aldosterone did not differ. The high plasma ANF levels in heart-transplant recipients did not result from high atrial pressures but appeared to be related with elevated atrial dimensions and cyclosporine-induced renal failure. During volume expansion, plasma ANF increased in control patients and re. mained elevated during the postinfusion period. in heart-transplant recipients, the changes in plasma ANF were less marked despite identical increases of atrial pressures. The sluggish response of plasma ANF in this group was associated in the postinfusion period with a nonreturn of the hemodynamic Parameters to their basal values in contrast with what was observed in control patients. (Am J Cardiol 1990;66:477-M), Atrial natriuretic factor (ANF) is a hormone secreted by the atrial tissue of the heart in response to an increase in blood pressure. ANF regulates blood pressure, blood volume, and cardiac output, the amount of blood pumped by the heart per unit time. In addition, ANF increases the elimination of salt in the urine and decreases the workload of the heart. Previous studies have shown that blood levels of ANF are increased in heart transplant recipients, although the causes of this increase are not known. Factors that affect the release of ANF in heart transplant recipients were assessed under conditions in which blood volume was expanded, or increased. The blood levels of ANF, the nucleotide cyclic guanosine monophosphate (cGMP), and the hormones aldosterone and renin, and measurements of circulation were assessed in 13 heart transplant recipients. Before blood volume expansion, the blood levels of ANF and cGMP were higher in heart-transplant recipients than in patients without heart transplants. However, the blood levels of renin and aldosterone were similar among all patients. The increased levels of ANF in heart-transplant recipients were associated with increased dimensions or size of the atria and with kidney failure due to treatment with the drug cyclosporin. The levels of ANF increased in patients without heart transplants during blood volume expansion and remained elevated after volume expansion. Although ANF levels increased in heart-transplant recipients during volume expansion, the increase was less marked, despite similar changes in pressures within the right atria of the heart. After volume expansion, circulatory changes recovered to normal in patients without heart transplants, but not in heart-transplant recipients. Abstract

In cardiac allograft rejection, histopathologic changes suggesting that myocardial ischemia is a component of the rejection process have been documented. To further define the coronary vascular reactivity of human heart transplant, coronary sinus blood flow and coronary resistance were measured before and after intravenous dipyridamole within the first year after transplantation in 8 patients without rejection (group II) and in 5 patients with rejection (group III). All had normal coronary arteriograms. Results were compared to those of 8 control subjects (group I). After dipyridamole, coronary sinus blood flow was increased in groups I, II and III by 303, 212 (p [is than]0.01 vs group I) and 45%, respectively (p [is less than]0.001 vs groups I and II). Coronary resistance was reduced by 77, 73 (not significant vs goup I) and 36%, respectively (p [is less than]0.001 vs groups I and II). Concomitantly, coronary sinus blood oxygen content was increased by 172, 145 (not significant vs group I) and 78%, respectively (p [is less than]0.001 vs group I, not significant vs group II). Thus, the coronary flow reserve evaluated by the dipyridamole/basal coronary sinus blood flow ratio and the coronary resistance reserve evaluated by the basal/dipyridamole coronary resistance ratio were dramatically impaired in group III (1.56 [+ or -] 0.09 and 1.6 [+ or -] 0.30, respectively, p [is less than]0.001 vs groups I and II). In contrast, they were almost normal in group II (3.11 [+ or -] 0.42 vs. 4.03 [+ or -] 0.52 in group I, p [is less than ]0.02, and 3.83 [+ or -] 0.78 vs. 4.45 [+ or -] 0.81 in group I, difference not significant). Thus, the impairment of coronary reserve during heart rejection should be linked to abnormalities of the coronary microvasculature. This emphasizes the important involvement of the coronary circulation in the rejection process., Coronary circulation, which nourishes the heart muscle, is known to play an important role in the success of a heart transplant. For example, it is thought that the organ rejection process is related to myocardial ischemia (MI; heart attack caused by inadequate oxygen supply to the heart usually due to a blockage in a coronary artery). Changes in heart tissues which were influential in coronary blood flow and coronary resistance were examined in 19 subjects. Three groups were identified; group I consisted of eight normal individuals without symptoms of heart disease, group II was made up of eight patients who recently had a heart transplant and showed no histologic (cellular) symptoms of rejection, and group III was comprised of five recent heart transplant patients who were exhibiting histologic signs of rejection. Baseline measurements of all subjects were taken before and after the administration of dipyridamole, a vasodilator i.e., an agent which causes the relaxation of the blood vessels. All measurements were taken within one year of heart transplantation. Coronary blood flow and coronary resistance were recorded and quantitatively analyzed in all three groups. No significant differences were observed between the normal group and the asymptomatic transplant group. However, the transplant group which showed with histological irregularities had dramatically impaired capacities when blood flow ratios and coronary resistance ratios were calculated. The results of this study emphasized the importance of coronary circulation as a factor in rejection. Further research in this area is considered to be important because of the potential reversibility of these conditions via treatments which may aid in reducing heart transplant rejections.

17. CMR assessment of AMI patients treated by PCI shows delays in myocardial reperfusion despite initial achievement of TIMI 3 flow.

Abstract

Aims: Cardiovascular magnetic resonance imaging (CMR) perfusion studies performed early after successful angioplasty in AMI nearly always shows incomplete tissue reperfusion despite normal coronary blood flow by TIMI score. In contrast, when performed a few weeks or months after AMI, CMR first-pass perfusion (FP) is usually normal. This study was conducted to determine the kinetics of tissue reperfusion after reperfused AMI in humans., Methods and Results: We prospectively analyzed patients who presented with ST-segment elevation myocardial infarction between July 2002 and January 2005. Included patients had TIMI 3 flow after acute PCI and CMR FP imaging within four months post-MI. CMR FP was performed after injection of 20 cc of gadolinium chelate and imaging was performed with a Siemens Symphony 1.5 T. For FP analysis, each slice was divided into 8 segments which were subdivided into subepicardial and subendocardial layers. Normal perfusion was defined as less than 5% of segments involved by hypoperfusion. Patients were divided into groups according to the delay between MI/reperfusion and CMR. The reperfusion curve was established by plotting the percentage of patients with normal perfusion in each subgroup. A total of 184 CMR were analysed out of 146 patients included in the study. Probability of normal perfusion was 24%, 31%, 35%, 33% and 43% at respectively day 1, day 2, day 4, day 6 and day 8. This probability increased to 67% at week 2, and at week 3 reached a 100% probability (Spearman's rho -0.471, p = 0.001)., Conclusions: Despite complete restoration of epicardial flow, early myocardial reperfusion as assessed by CMR is very uncommon. Myocardial perfusion is progressively restored and capillary blood volume recovered normal values between weeks 2 and 4 after MI.
