Abstract

This study presents a general theory of interacting elastic curves in two dimen-sions before moving into a more general model of interacting elastic rods in three dimensions. It begins by looking at methods of modelling a single continuous two dimensional curve, before making a first attempt at modelling interactions between such curves. Finding that a discrete model would be more appropriate, the theory of two dimensional discrete curves is introduced along with a definition of discrete curvature. This model is shown to provide a good approximation to the continuous case. With promising results in the case of a single curve we model the interac¬tion between several discrete curves with great success. The general equations are then applied to a variety of interacting curve models, providing physically realistic results. Continuous space curves, curve framing, and the general theory of elastic rods is introduced in order to build discrete analogies to each, before moving on to build a discrete model of an elastic curve deforming in three dimensions. This study provides the basis to support the eventual modelling of interacting elastic rods in three dimensions. Such a model can be applied to many bundle models, including but not limited to collagen type 1 fibrils.

Abstract

Low doses of mitotane were given orally to 36 patients with Cushing's disease, concurrently with or after pituitary cobalt irradiation. Clinical and biochemical remission occurred in 29. The response to treatment occurred early in 17 patients and late in 12. The different pattern of response to mitotane was not related to the dose given or to its serum level. Early biochemical indicators of adrenal suppression with mitotane were a sharp decrease in adrenal response to the infusion of ACTH and in plasma levels of dehydroepiandrosterone sulfate. Although mitotane was given together with pituitary irradiation, initial remission was due mainly to the adrenal effect of mitotane. Plasma ACTH levels were still elevated when cortisol had returned to normal. In seventeen of the 29 patients who responded to treatment drug therapy has been discontinued, and they remain in remission of Cushing's syndrome. Side-effects have been dose dependent, with anorexia, nausea, decreased memory, and gynecomastia in men being the commonest. [ABSTRACT FROM AUTHOR]

6. Transient thyrotoxicosis associated with acute hemorrhagic infarction of autonomously functioning thyroid nodules.

Abstract

Two patients with nontoxic autonomously functioning thyroid nodules had transient hyperthyroidism after acute hemorrhagic infarction. Previously solid "hot" nodules were converted to cystic "cold" nodules. Aspiration confirmed the hemorrhagic nature of the process. There was marked regression in nodule size, marked reduction of nodular autonomous secretory activity (indicated by a change from a blunted to a normal response to thyrotropin-releasing hormone), and spontaneous recovery of function in the previously suppressed extranodular thyroid tissue. Confusion of this entity with a nontoxic autonomously functioning thyroid nodule that has progressed to the toxic stage may be avoided if the loss of nodular function is noted on imaging. Abstract

Diabetes included several disorders associated with hyperglycemia. A difference in inheritance between the families of juvenile-onset- and maturity-onset-type diabetics, provides evidence for genetic heterogeneity. Heterogeneity of insulin responses to glucose was foung among nonobese patients with maturity-onset-type diabetes. Prospective studies in young patients have shown that glucose intolerance may not progress for as long as 22 years and that subnormal insulin responses to glucose have not decreased further, up to 12 years. However, patients who progressed to diabetes requiring insulin had insulin responses that were subnormal or below the control mean. None whose insulin responses exceeded this mean have decompensated. Thus, insulin response to glucose has prognostic implications. A tentative classification of diabetes in the young is proposed. There was a significant correlation between muscle capillary basement membrane width and known duration of carbohydrate intolerance.

Abstract

Reports of the prevalence of depression among alcoholics vary from 3% to 98%; the authors attribute this variation to the use of different diagnostic criteria. They used clinical diagnosis, the Hamilton Depression Rating Scale, the Zung Self-Rating Depression Scale, and the MMPI to diagnose 35 men recently withdrawn from alcohol. The percentages of depression diagnosed were 8.6%, 28%, 66%, and 43%, respectively. The authors point out that the Hamilton, Zung, and MMPI scales are not diagnostic of depression in themselves and that accurate diagnosis of depression in alcoholics will reduce inappropriate treatment of nondepressed alcoholics and ensure careful treatment of the truly depressed.
