Evidence on long-acting antipsychotics (LAIs) in unselected populations with severe mental illness is scant. In this mirror-image study, we compared multiple clinical outcomes 1 year before and after a first LAI prescription in adults with severe mental illness, describing clinical trajectories of LAI continuers and discontinuers. We compared LAI continuers and discontinuers through Mann–Whitney U test, Kaplan–Meier survival curves, regression for interval-censored data, and a maximum-likelihood mixed-model with individual random-effect and time as predictor. Of the 261 participants analyzed, 71.3% had schizophrenia-spectrum disorders, and 29.5% discontinued the LAI before 1 year. At baseline, LAI discontinuers had a shorter illness duration, lower attitude and adherence scores. The mirror-image analysis showed reduced hospital admissions only for LAI continuers. Over time, continuers spent less days hospitalized, but had more adverse events and more antipsychotics prescribed, with higher overall doses. In conclusion, this study shows that LAIs might be beneficial in unselected patient populations, provided that adherence is maintained. LAI continuers spent less time hospitalized, but received more antipsychotics and suffered from more cumulative adverse events over time. Therefore, the choice of initiating and maintaining a LAI should be carefully weighed on a case-by-case basis.

The text regards the way into which the Italian artist Francesco Somaini has confronted with the themes of sacred art, with particular attention to the works he realized at the end of the Sixties for a project about the relation to life, death and regeneration in his way of creating plastic forms for a "retablo" that he exhibited during a show in Palazzo Te in Mantova. That project has been object of a reprise in his thinking about his work in a letter to the Italian critic Enrico Crispolti, very close to the artist. The opportunity to exhibit all the scultpures that were part of that project, with the documents and pictures concerning that moment, permitted to pursue the reflections about the terms into which Somaini related to the themes of "sacred" as part of his inspiration and his way of creating in his sculpture of different moments, starting with the "Crosses" he realized at the end of the Fifties.

The other long text elaborated for the catalogue of the exhibition dedicated to the works of Dorazio between 1963 and 1968 is a lecture about the peintings that have been part of the exhibition. The works are divided in groups of them, selected with attention to the shapes, the combination of foms and the positions of them in the evolution of the painter, with indepths about most of the themes they suggest.

The essay is the main critical text in the big volume dedicated to the works of Piero Dorazio painted between 1963, when he left his former season of "textures" and starts a new way of creating forms with colours and stripes intertwined. This moment is very prcious for his position in the art world, concerning also the moment of great success and interest in the United States, wher the Italian artist teaches at the Penn State Unviersity in Philadelphia. In the essay his painting is confronted with the themes and the situations of an international horizon, concerning also his theoretical reading of a turning point in the Second World War art.

BACKGROUND: Vocal disorders are frequent in people with multiple sclerosis (MS). Cognitive impairment, fatigue, depression, and other clinical characteristics can be associated with treatment effectiveness in rehabilitation. Finding baseline characteristics that identify those who are responding to treatment can help the clinical decision-making process, which can then help improve the effectiveness of voice treatment. We developed a model to identify factors associated with treatment-related improvement on voice intensity in people with MS. METHODS: Data are from a randomized controlled trial of the effects of voice therapy. Forty-four people with MS were enrolled and randomized to receive Lee Silverman Voice Treatment LOUD, specifically addressing voice intensity, or conventional speech-therapy group. Voice intensity (dB) was measured during monologue before and after treatment and was used to differentiate those who responded (posttreatment voice intensity?> 60 dB) from those who did not. Possible associated factors were cognitive impairment, fatigue, depression, disability, and disease duration. Associations were assessed by univariate logistic regression and univariate and multivariate linear regressions. RESULTS: Mean ± SD monologue voice intensity is improved in the whole sample (before rehabilitation: 51.8 ± 4.2 dB; and after rehabilitation 57.0 ± 6.5 dB; P <.001), and 11 people with MS (27.5%) responded to treatment. Specificity of treatment was associated with the return to normal voice intensity (OR, 14.28; 95% CI, 12.17-309.56) and we found a linear association between voice improvement and the specificity of treatment (6.65 [SE = 1.54] dB; P <.05). Moreover, the analysis revealed a nonlinear association between improvement and fatigue, suggesting increased benefits for people with MS with moderate fatigue. Background: Recent guidelines suggested a wider use of long-acting injectable antipsychotics (LAI) than previously, but naturalistic data on the consequences of LAI use in terms of discontinuation rates and associated factors are still sparse, making it hard for clinicians to be informed on plausible treatment courses. Objective: Our objective was to assess, under real-world clinical circumstances, LAI discontinuation rates over a period of 12 months after a first prescription, reasons for discontinuation, and associated factors. Methods: The STAR Network ‘Depot Study’ was a naturalistic, multicentre, observational prospective study that enrolled subjects initiating a LAI without restrictions on diagnosis, clinical severity or setting. Participants from 32 Italian centres were assessed at baseline and at 6 and 12 months of follow-up. Psychopathology, drug attitude and treatment adherence were measured using the Brief Psychiatric Rating Scale, the Drug Attitude Inventory and the Kemp scale, respectively. Results: The study followed 394 participants for 12 months. The overall discontinuation rate at 12 months was 39.3% (95% confidence interval [CI] 34.4–44.3), with paliperidone LAI being the least discontinued LAI (33.9%; 95% CI 25.3–43.5) and olanzapine LAI the most discontinued (62.5%; 95% CI 35.4–84.8). The most frequent reason for discontinuation was onset of adverse events (32.9%; 95% CI 25.6–40.9) followed by participant refusal of the medication (20.6%; 95% CI 14.6–27.9). Medication adherence at baseline was negatively associated with discontinuation risk (hazard ratio [HR] 0.853; 95% CI 0.742–0.981; p = 0.026), whereas being prescribed olanzapine LAI was associated with increased discontinuation risk compared with being prescribed paliperidone LAI (HR 2.156; 95% CI 1.003–4.634; p = 0.049). Aims: As refugees and asylum seekers are at high risk of developing mental disorders, we assessed the effectiveness of Self-Help Plus (SH +), a psychological intervention developed by the World Health Organization, in reducing the risk of developing any mental disorders at 12-month follow-up in refugees and asylum seekers resettled in Western Europe. Methods: Refugees and asylum seekers with psychological distress (General Health Questionnaire-12 ≥ 3) but without a mental disorder according to the Mini International Neuropsychiatric Interview (M.I.N.I.) were randomised to either SH + or enhanced treatment as usual (ETAU). The frequency of mental disorders at 12 months was measured with the M.I.N.I., while secondary outcomes included self-identified problems, psychological symptoms and other outcomes. Results: Of 459 participants randomly assigned to SH + or ETAU, 246 accepted to be interviewed at 12 months. No difference in the frequency of any mental disorders was found (relative risk [RR] = 0.841; 95% confidence interval [CI] 0.389-1.819; p-value = 0.659). In the per protocol (PP) population, that is in participants attending at least three group-based sessions, SH + almost halved the frequency of mental disorders at 12 months compared to ETAU, however so few participants and events contributed to this analysis that it yielded a non-significant result (RR = 0.528; 95% CI 0.180-1.544; p-value = 0.230). SH + was associated with improvements at 12 months in psychological distress (p-value = 0.004), depressive symptoms (p-value = 0.011) and wellbeing (p-value = 0.001). Conclusions: The present study failed to show any long-term preventative effect of SH + in refugees and asylum seekers resettled in Western European countries. Analysis of the PP population and of secondary outcomes provided signals of a potential effect of SH + in the long-term, which would suggest the value of exploring the effects of booster sessions and strategies to increase SH + adherence.

Importance: The cost-effectiveness of the Self-Help Plus (SH+) program, a group-based, guided, self-help psychological intervention developed by the World Health Organization for people affected by adversity, is unclear. Objective: To investigate the cost-utility of providing the SH+ intervention combined with enhanced usual care vs enhanced usual care alone for Syrian refugees or asylum seekers hosted in Turkey. Design, Setting, and Participants: This economic evaluation was performed as a prespecified part of an assessor-blinded randomized clinical trial conducted between October 1, 2018, and November 30, 2019, with 6-month follow-up. A total of 627 adults with psychological distress but no diagnosed psychiatric disorder were randomly assigned to the intervention group or the enhanced usual care group. Interventions: The SH+ program was a 5-session (2 hours each), group-based, stress management course in which participants learned self-help skills for managing stress by listening to audio sessions. The SH+ sessions were facilitated by briefly trained, nonspecialist individuals, and an illustrated book was provided to group members. Th intervention group received the SH+ intervention plus enhanced usual care; the control group received only enhanced usual care from the local health care system. Enhanced usual care included access to free health care services provided by primary and secondary institutions plus details on nongovernmental organizations and freely available mental health services, social services, and community networks for people under temporary protection of Turkey and refugees. Main Outcomes and Measures: The primary outcome measure was incremental cost per quality-adjusted life-year (QALY) gained from the perspective of the Turkish health care system. BACKGROUNDS: Individuals with chronic medical conditions are considered highly exposed to COVID-19 pandemic stress, but emerging evidence is demonstrating that resilience is common even among them. We aimed at identifying sustained resilient outcomes and their predictors in chronically ill people during the first year of the pandemic. METHODS: This international 4-wave 1-year longitudinal online survey included items on socio-demographic characteristics, economic and living situation, lifestyle and habits, pandemic-related issues, and history of mental disorders. Adherence to and approval of imposed restrictions, trust in governments and in scientific community during the pandemic were also investigated. The following tools were administered: the Patient Health Questionnaire, the Generalized Anxiety Disorder scale, the PTSD Checklist DSM-5, the Oslo Social Support Scale, the Padua Inventory, and the Portrait Values Questionnaire. RESULTS: One thousand fifty-two individuals reporting a chronic condition out of 8011 total participants from 13 countries were included in the study, and 965 had data available for the final model. The estimated probability of being "sustained-resilient" was 34%. Older male individuals, participants employed before and during the pandemic or with perceived social support were more likely to belong to the sustained-resilience group. Loneliness, a previous mental disorder, high hedonism, fear of COVID-19 contamination, concern for the health of loved ones, and non-approving pandemic restrictions were predictors of not-resilient outcomes in our sample. CONCLUSIONS: We found similarities and differences from established predictors of resilience and identified some new ones specific to pandemics. Further investigation is warranted and could inform the design of resilience-building interventions in people with chronic diseases.

BACKGROUND AND AIMS: The coronavirus disease 2019 pandemic has challenged health services worldwide, with a worsening of healthcare workers' mental health within initial pandemic hotspots. In early 2022, the Omicron variant is spreading rapidly around the world. This study explores the effectiveness and cost-effectiveness of a stepped-care programme of scalable, internet-based psychological interventions for distressed health workers on self-reported anxiety and depression symptoms. METHODS: We present the study protocol for a multicentre (two sites), parallel-group (1:1 allocation ratio), analyst-blinded, superiority, randomised controlled trial. Healthcare workers with psychological distress will be allocated either to care as usual only or to care as usual plus a stepped-care programme that includes two scalable psychological interventions developed by the World Health Organization: A guided self-help stress management guide (Doing What Matters in Times of Stress) and a five-session cognitive behavioural intervention (Problem Management Plus). All participants will receive a single-session emotional support intervention, namely psychological first aid. We will include 212 participants. An intention-to-treat analysis using linear mixed models will be conducted to explore the programme's effect on anxiety and depression symptoms, as measured by the Patient Health Questionnaire - Anxiety and Depression Scale summary score at 21 weeks from baseline. Secondary outcomes include post-traumatic stress disorder symptoms, resilience, quality of life, cost impact and cost-effectiveness. CONCLUSIONS: This study is the first randomised trial that combines two World Health Organization psychological interventions tailored for health workers into one stepped-care programme. Background: Longitudinal studies examining mental health trajectories in refugees and asylum seekers are scarce. Objectives: To investigate trajectories of psychological symptoms and wellbeing in refugees and asylum seekers, and identify factors associated with these trajectories. Method: 912 asylum seekers and refugees from the control arm of three trials in Europe (n = 229), Turkey (n = 320), and Uganda (n = 363) were included. We described trajectories of psychological symptoms and wellbeing, and used trauma exposure, age, marital status, education, and individual trial as predictors. Then, we assessed the bidirectional interactions between wellbeing and psychological symptoms, and the effect of each predictor on each outcome controlling for baseline values. Results: Symptom improvement was identified in all trials, and for wellbeing in 64.7% of participants in Europe and Turkey, versus 31.5% in Uganda. In Europe and Turkey domestic violence predicted increased symptoms at post-intervention (ß = 1.36, 95% CI 0.17-2.56), whilst murder of family members at 6-month follow-up (ß = 1.23, 95% CI 0.27-2.19). Lower wellbeing was predicted by murder of family member (ß = -1.69, 95% CI -3.06 to -0.32), having been kidnapped (ß = -1.67, 95% CI -3.19 to -0.15), close to death (ß = -1.38, 95% CI -2.70 to -0.06), and being in the host country ≥2 years (ß = -1.60, 95% CI -3.05 to -0.14). In Uganda at post-intervention, having been kidnapped predicted increased symptoms (ß = 2.11, 95% CI 0.58-3.65), and lack of shelter (ß = -2.51, 95% CI -4.44 to -0.58) and domestic violence predicted lower wellbeing (ß = -1.36, 95% CI -2.67 to -0.05). Conclusion: Many participants adapt to adversity, but contextual factors play a critical role in determining mental health trajectories.

Background: Hypophonia is a prevailing problem in people with multiple sclerosis (PwMS). However, evidence supporting the effectiveness of voice rehabilitation is lacking. Objective: The aim of this study was to identify the most effective method to reduce hypophonia. Methods: In this randomized controlled trial, 44 PwMS were randomized to intensive and high-effort voice treatment groups, the LSVT-LOUD®, and conventional treatment group. Subjects received 16 treatments (4 sessions/week) lasting 45 minutes. The primary outcome was voice intensity (dB) in monologue, vocalization, and sentences while voice handicap index (VHI) measured voice self-perception. Outcomes were assessed by a blinded observer at baseline, post-treatment, and 15-month follow-up (FU). Results: Linear models revealed a significant post-intervention between-group mean difference in favor of LSVT-LOUD for monologue: +6.3 dB (95% CI: 2.5 to 10.1); vocalization: +7.4 dB (95% CI: 2.3 to 12.5); and sentences: +9.5 dB (95% CI: 4.7 to 14.3). However, 43.7% PwMS in the LSVT-LOUD and 10% in the conventional treatment group obtained a full recovery of voice intensity (>60 dB) post-treatment, Fisher’s test = 13.3, p < 0.01. However, these improvements were not maintained at FU. Between-group differences at VHI were −10.8 (95% CI: −21.2 to −0.4) and −11.3 (95% CI: −24.3 to −1.7) in favor of LSVT-LOUD at post and FU. Conclusion: LSVT-LOUD can be a valid treatment to increase voice intensity in PwMS. However, results suggest the need for FU interventions targeting maintenance.

La seconda via prende le mosse dalle strutture della coscienza ed è l’itinerario proposto in generale dalla fenomenologia. Si considera poi l’evento originario della trascendenza come un’anticipazione di senso, che giunge a espressione nel linguaggio analogico e simbolico., In the multiple historical forms of metaphysics, transcendence takes on different meanings. In particular, the distinction introduced by Heidegger regarding the difference of the transcendent from the contingent and from the immanent is analysed. The first way leads to a strong sense of transcendence, for which being cannot coincide with the totality of beings. The second way starts from the structures of consciousness and is the itinerary proposed in general by phenomenology. The original event of transcendence is then considered as an anticipation of meaning, which reaches expression in analogical and symbolic language.

Self-Help Plus was delivered in a group format by two facilitators over five sessions. The primary outcome measure was the presence of any mental disorder assessed by the Mini International Neuropsychiatric Interview at six-month follow-up. Secondary outcome measures were the presence of mental disorders at post-intervention, and psychological distress, symptoms of post-traumatic stress disorder and depression, personally identified psychological outcomes, functional impairment, subjective well-being, and quality of life at post-intervention and six-month follow-up. Between October 1, 2018 and November 30, 2019, 1,186 refugees were assessed for inclusion. Five hundred forty-four people were ineligible, and 642 participants were enrolled and randomly assigned to either Self-Help Plus (N=322) or ECAU (N=320). The essay addresses Piero Dorazio's tenure as instructor and Professor of Painting at the Univesrity of Pennsylvania, from 1960 to 1970. Among the process of reshaping of the the teaching of visual arts in the American university, the School of Fine Arts opted for a complex solution, choosing a polytechnic, Bauhaus-based program, while maintaining the categories and terminology of the Beaux-Arts model. Dorazio took this ambiguity further, developing connections between his Deaprtment and the contemporary art world, and promoting the Department's role as a main contributor in the creation of the ICA in Philadelphia. The essay traces the parable of this experience, analyzing its postioning among the debate on the teaching of art in the universities of the Ivy League.

Le "cartografie" sono le reazlizzazioni a rilievo in legno che Dorazio ha eseguito nei primi anni Cinquanta e che rappresentano il corpo portante delle sue prime esposizioni personali, realizzate negli Stati Uniti fra 1953 e 1954. Fanno parte di una sezione del suo lavoro in seguito ripresa in esame sporadicamente, ma costituisono un gruppo di lavori particolarmente significativi per cogliere la posizione di Dorazio in quel periodo, sia in relazione alle relazioni tra arte e ambiente, sia per la concezione originale di opere astratte di carattere lirico. Nell'intervento, fondato su una ripresa di documenti poco considerati, si avanza un confronto con le forme di rappresentazione ed elaborazione spaziale contemporanea.

Il contributo ricostruisce e ripercorre le sperimentazioni artistiche di Gianni Bertini al di là della pratica pittorica, svolte fra gli anni cinquanta e gli anni settanta. A partire dal primo happening veneziano del 1956. Bertini, pur restando essenzialmente un pittore, ha spesso creato ricercato momenti performativi e si è cimentato in molteplici sperimentazioni editoriali e poetiche, raggiungendo riconoscimenti internazionali e ribadendo il suo ruolo di outsider del mondo dell'arte.

The essay analyzes Gianni bertini's contribution to the definition and development of mec-art. Through a deep research into archive and photographic materials, the author reconstructed the peculiar techniques used by the artist, as well as several of his iconographic sources. After introducing mec-art and how its inception corresponded to Bertini's return to pop-like iconic materials, the essay addresses a few aspects of the artist's mec career: the relationship with serialization and the creation of variants; the imagery and the most recurring themes (considered in their historical development); the main exhibitions featuring Bertini's mec-art work; the continuous presence of mec-art in the following phases of the artist's career.

The article is essentially a reprise of the contributions that Enrico Crispolti gave through various essays on the subject of Fontana's sacred works of art. Crispolti is accredited as the most accuntable expert on Font ana's work and personality, having had the chance to cultivate personal relationships with artist, attested by a rich correspondence with him, and having been the editor of the Catalogue Raisonne of his works. Starting from the analysis of Fontana's first Via Crucis (1947), written when the work first reappeared, in 1988, through the last readings of Fontana's three Via Crucis, exhibited together in Milan in 2011, Crispolti's position is that Fontana wasn't really motivated by religious intentions. Moved by a strong attention to the relations between matter and the 'multi-dimensionality' of space attained by scientific processes, Fontana interpreted the way religious themes could connect the history of art with his new vision of an art of space-matter transcending the physical and figurative conventions. The only way, for Crispolti, to read Fontana's sacred works according to spiritual categories is their problematic declaration to go beyond the visible, though with firm roots in the 'here and now' of the vital process.

New York New York racconta la straordinaria avventura di un gruppo di importanti artisti italiani che, dai primi decenni del Novecento attraverso i loro viaggi e contatti con il mondo americano, hanno subìto il fascino di New York e hanno partecipato al fenomeno di graduale internazionalizzazione del mondo e del mercato dell’arte.

Aims. There is a need of more quantitative standardised data to compare local Mental Health Systems (MHSs) across international jurisdictions. Problems related to terminological variability and commensurability in the evaluation of services hamper like-with-like comparisons and hinder the development of work in this area. This study was aimed to provide standard assessment and comparison of MHS in selected local areas in Europe, contributing to a better understanding of MHS and related allocation of resources at local level and to lessen the scarcity in standard service comparison in Europe. This study is part of the Seventh Framework programme REFINEMENT (Research on Financing Systems’ Effect on the Quality of Mental Health Care in Europe) project. Methods. A total of eight study areas from European countries with different systems of care (Austria, England, Finland, France, Italy, Norway, Romania, Spain) were analysed using a standard open-access classification system (Description and Evaluation of Services for Long Term Care in Europe, DESDE-LTC). All publicly funded services universally accessible to adults (≥18 years) with a psychiatric disorder were coded. Care availability, diversity and capacity were compared across these eight local MHS. Results. The comparison of MHS revealed more community-oriented delivery systems in the areas of England (Hampshire) and Southern European countries (Verona – Italy and Girona – Spain). Community-oriented systems with a higher proportion of hospital care were identified in Austria (Industrieviertel) and Scandinavian countries (Sør-Trøndelag in Norway and Helsinki-Uusimaa in Finland), while Loiret (France) was considered as a predominantly hospital-based system. The MHS in Suceava (Romania) was still in transition to community care. Conclusions. There is a significant variation in care availability and capacity across MHS of local areas in Europe. Performative theater is one of the most important trends of our time. What, in brief, are its key features? It is in this area that we find works of great impact that deal thematically, not directly with Europe, but with the acquisitions whose layering and mingling has formed European culture. The works critically investigate their validity or distance or decay, raising the alarm over signals and the risks that their depreciation, degeneration or neglect may have in current growth of European consciousness. Nell’autunno 2013 la città di Milano ha ospitato alcune mostre dedicate all’arte americana del secondo dopoguerra. Contemporaneamente le Gallerie d’Italia presentavano 1963 e dintorni, un approfondimento monografico dedicato al 1963, anno che ha rappresentato uno snodo importante nelle vicende artistiche italiane, in dialogo con il panorama internazionale. A partire dai temi storico-critici utili a inquadrare quel periodo, con attenzione alla diffusione dell’arte americana e ai molti aspetti del confronto in atto nei primi anni Sessanta è stato realizzato un convegno, promosso e ospitato congiuntamente dal Museo del Novecento e da Gallerie d’Italia. Il tema della giornata di studi Arte italiana 1960-1964. Identità culturale, confronti internazionali, modelli americani, coordinata da Flavio Fergonzi e Francesco Tedeschi, è origine degli studi che sono pubblicati in questo volume. Accomunati dalla volontà di indagare il periodo con consapevolezza storica e attenzione filologica, superando approcci manualistici o sedimentati, i contributi qui raccolti sono suddivisi in due sezioni. La prima è dedicata a singole personalità, Lucio Fontana, Piero Manzoni, Giulio Paolini, Franco Angeli, Luciano Fabro, Cesare Vivaldi e l’esperienza internazionale di Costantino Nivola e Mirko Basaldella. La seconda sessione, invece, propone una pluralità di sguardi sul dibattito critico dell’inizio degli anni Sessanta, evidenziando le posizioni di differenti generazioni di critici, da Giulio Carlo Argan a Carla Lonzi, nonché il confronto fra la Pop Art e le sperimentazioni legate all’arte programmata, dialogo che vide il coinvolgimento diretto di molti artisti. L’intento di questa pubblicazione è fornire materiali di studio e stimoli per approfondire le ricerche sull’arte italiana degli anni Sessanta, per la sua specificità e in un aperto scambio con le esperienze internazionali.

Il contributo analizza l'operato di Costantino Nivola e Mirko Basaldella presso l'università di Harvard, in particolare come direttori del Design Workshop della Graduate School of Design. In particolare, sono messi in evidenza i legami con il Bauhaus e con la linea "politecnica" dell'insegnamento universitario, ma anche con i precedenti americani del "pUre Design".

Introduzione al percorso del volume e della mostra, a proposito del rapporto fra arte italiana e Stati Uniti d'America, dal Futurismo agli anni Sessanta, con riferimento ai temi di fondo di una relazione sviluppata attraverso contatti diretti, riflessioni critiche, viaggi e incontri.

Il contributo ripercorre le carriere di Mirko Basaldella, Costantino Nivola, Piero Dorazio e Angelo Savelli come docenti nelle università americane di Harvard e Pennsylvania, in un periodo che va dalla metà degli anni cinquanta all'inizio degli anni settanta. La produzione artistica dei quattro autori, anche in relazione ai lavori esposti, è letta in parallelo con la loro attività didattica.
