IntroductionOlder adults racialized as Black experience higher rates of dementia than those racialized as White. Structural racism produces socioeconomic challenges, described by artist Marvin Gaye as "hang ups, let downs, bad breaks, setbacks" that likely contribute to dementia disparities. Robust dementia literature suggests socioeconomic factors may also be key resiliencies.MethodsWe linked state-level data reflecting the racialized landscape of economic opportunity across the 20th Century from the U.S. Census (1930-2010) with individual-level data on cognitive outcomes from the U.S. Health and Retirement Study participants racialized as Black. A purposive sample of participants born after the Brown v. Board ruling (born 1954-59) were selected who completed the modified Telephone Interview for Cognitive Status between 2010 and 2020 (N=1381). We tested associations of exposure to structural racism and resilience before birth, and during childhood, young-adulthood, and midlife with cognitive trajectories in mid-late life using mixed-effects regression models.ResultsOlder adults born in places with higher state-level structural socioeconomic racism experienced a more rapid cognitive decline in later life compared to those with lower levels of exposure. In addition, participants born in places with higher levels of state-level structural socioeconomic resilience experienced slower cognitive change over time than their counterparts.DiscussionThese findings reveal the impact of racist U.S. policies enacted in the past that influence cognitive health over time and dementia risk later in life.

Generally, organizational climate research does not focus on the work environment because the mindset and emotions of employees are often mistaken for organizational culture. Additionally, surveys to evaluate the organizational climate tend to be long, and therefore, organizational climate studies are conducted only once a year—that too if an organization is concerned about its employees. This research proposes a methodology to evaluate organizational climate; the methodology has the following characteristics: it is a short evaluation named “pulse”; it is oriented toward specific elements of culture that influence the organizational climate and its variability; and it considers organizational contexts. The study was conducted in three organizations encompassing three sectors (N=3,331 employees). The survey included three questions regarding employees’ feelings and climate perception at the individual, group, and organizational levels. Additionally, it had 56 questions related to the elements of organizational culture, grouped into six components after an exploratory analysis: Structure, Recognition, Leadership, Accountability, Work Team, and Ethics. The results showed significant differences between organizations based on the organizational climate perception, its strength, and the behavior of the variables associated with the organizational culture that impacts the climate. Additionally, cultural elements were reduced because of their relationship with the organizational climate. This research suggests that organizational climate studies should be conducted for specific organizational contexts. Additionally, it proposes a methodology to reduce the duration of organizational climate studies by focusing on specific cultural dimensions associated with the climate, which can be applied longitudinally throughout the year to monitor climate changes. The use of technologies is seen as a solution to provide high-quality education to the disadvantaged population in the Global South, based on the narrative of 'disruption and revolution designed to supplant 'broken' educational practices at the local level' (Gallagher 2019). Rural schools in Colombia are the most vulnerable to a highly unequal education system. In this context the 'Fundación para la Equidad Educativa' (FEE) has been carrying out the project 'LibreEscuela' in 9 rural schools in Colombia. The project involved the delivery of Raspberry Pi with pre-loaded Open Educational Resources (OER) through Kolibri and OER co-creation workshops. Revisiting the project retrospectively using minimal computing and 'rebusque' is an attempt to explore how rural teachers and FEE members negotiate and design digital educational projects in highly constrained contexts.

Introduction In primary care, the prevalence of neck pain has increased substantially. Evidence regarding treatment of chronic neck pain (CNP) is scarce, and its effectiveness is not entirely proven in different stages of chronicity, nor for different types of cervical disorders. The goal of this study is to evaluate the effectiveness of a complex intervention (COGMO intervention) compared with usual practice in primary care physiotherapy to improve neck pain intensity, severity and disability in patients with CNP.Methods and analysis Design: a pragmatic cluster-randomised clinical trial design with a 12-month follow-up. Setting: primary care. Participants: physiotherapists as randomisation unit, and patients as analysis unit. Inclusion criteria: individuals aged 18 to 65 years suffering from moderate to severe CNP. Sample size expected: 142 patients. Recruitment: patients referred from primary care physicians to physiotherapy. Intervention: pain neuroscience education (PNE), motivational interviewing (MI) and cognition targeted exercise therapy (CTE) compared with the standard treatment in primary care. Outcomes: the main variable is reduction in pain intensity; secondary variables include pain severity, conditioned pain modulation, temporal summation, neck disability, fear/avoidance behaviour, kinesiophobia, catastrophising, therapeutic alliance and quality of life. Sociodemographic information and adherence to the intervention will be recorded. Data collection: baseline, and follow-up at 3, 6 and 12 months. Analysis: it will follow intention-to-treat principles, and difference in percentage of subjects achieving success on the primary endpoint at 12 months. A model with multilevel analysis will be adjusted through logistic regression (being the dependent variable pain intensity, and the independent, the intervention).Ethics and dissemination Ethical approval has been awarded by the Regional Ethics Committee of Madrid (code: COGMO-AP) and the primary health care central commission of research (code: 20210011). The results of the study will be disseminated through international peer-reviewed journals, international conferences, press and social media.Trial registration number NCT05785455.

Introducción: la superación profesional en tecnologías para los docentes es un proceso de enseñanza aprendizaje que posee sustentos sólidos desde diversas corrientes, teorías y enfoques. Objetivo: analizar las teorías del aprendizaje que sirven como fundamentos para la estrategia de superación profesional en Tecnologías de la Información y la Comunicación para los docentes de Tecnología de la Salud. Métodos: se realizó una revisión bibliográfica en las bases de datos: Dialnet, SciELO, REDIB, Redalyc, Latindex y Google Scholar. Fueron seleccionados 26 entre documentos normativos, tesis de doctorado, libros, artículos científicos publicados en revistas nacionales e internacionales, con la utilización de los ordenadores lógicos boleanos: AND, OR, como estrategias de búsqueda para combinar los términos tecnologías, superación profesional, teorías de aprendizaje. La búsqueda abarcó el período desde 2016-2024, 3 anteceden a la fecha por su importancia. Desarrollo: la revisión documental permitió profundizar en los aportes de las teorías de aprendizaje que sirven de sustento a la superación profesional en tecnologías, lo que favoreció la determinación de las principales ideas, rasgos y características de dichas teorías que deben caracterizar las acciones de una estrategia de superación profesional en tecnologías para los docentes de las carreras tecnológicas de la salud. Conclusiones: la superación profesional en tecnologías constituye una necesidad de los docentes de las carreras de Tecnología de la Salud. Para desarrollarla con calidad se deben integrar y tomar como sustento los aspectos distintivos de cada una de las teorías de aprendizaje que sean aplicables a este proceso.

BackgroundTocilizumab (TCZ) is a monoclonal antibody that neutralizes interleukin (IL)-6 and is indicated for diseases characterized by markedly elevated inflammatory markers, such as rheumatoid arthritis (RA). The complement system has been implicated in the etiopathogenesis of RA.ObjectiveTo evaluate the effect of systemic IL-6 inhibition on complement pathways functional activity in RA patients treated with TCZ.DesingProspective non-interventional study.MethodsTwenty-seven RA patients included in the TOCRIVAR study who received TCZ (8mg/kg IV/q4w) were evaluated at baseline and at weeks 12, 24 and 52 of treatment. Disease activity, as assessed by composite indices, acute phase reactants, and new-generation functional assays of the three complement pathways, was evaluated at baseline and at each follow-up visit. Multivariable linear mixed models were used to determine changes in the complement system cascades over time.ResultsAfter adjustment for disease activity, basal levels of the classical and alternative pathways decreased significantly after TCZ treatment. The effect on the classical pathway remained significant after 52 weeks. The decrease in the alternative pathway was significant at weeks 12 and 24, but not at week 52 of TCZ treatment. TCZ had no effect on the lectin cascade throughout the follow-up.ConclusionTCZ reduces the activity of the classical and alternative pathways of the complement system in RA patients regardless of the improvement in disease activity. This finding may contribute to a better understanding of the mechanisms by which the IL-6 blockade reduces disease activity in RA patients.

BackgroundThe Andean blackberry (Rubus glaucus Benth), locally known as “Mora de Castilla,” is a high-altitude fruit rich in phytochemicals, mainly anthocyanins, with potent antioxidant properties. Although its chemical composition has been studied, the influence of altitude on its phytochemical profile and antioxidant capacity is still unclear. This research aimed to investigate the effects of development stages and altitude on the phytochemical profile and antioxidant activity of this fruit.MethodsAndean blackberry fruits were collected at different altitudes and development stages in the central Andean region of Ecuador. An hydroalcoholic extraction was used to quantify the compounds and determine the total antioxidant activity, while solid phase extraction (SPE) was performed to separate anthocyanins from other compounds. Ultra-high-performance liquid chromatography coupled to a triple quadrupole mass spectrometer (HPLC-DAD/ESI-MSn) was used to identify anthocyanin and non-anthocyanin phenols, while total antioxidant capacity, total polyphenols, total flavonoid content, and total anthocyanin content were quantified spectrophotometrically.ResultsFlavonols and anthocyanins constitute the majority of the flavonoids identified in the Andean blackberry extracts, both in terms of peak areas and number of identified compounds, followed by ellagic and gallic acid derivatives, as well as phenolic acids, mainly hydroxycinnamic acids. Quercetin was identified as the predominant flavonol in unripe berries, where anthocyanins were not significantly present. On the contrary, in ripe berries, cyanidin and its glycosides stood out as the main anthocyanins and predominant compounds. We observed that in the early stages of ripening, the total polyphenol content predominates in the berries and is mainly responsible for their antioxidant capacity. However, as the fruit ripens, the total anthocyanin content increases, becoming the most prominent bioactive compounds in fully ripe berries.ConclusionThe results suggest that higher altitude environmental conditions may improve the composition, concentration of phenolic compounds, and antioxidant activity of Andean blackberries. Overall, our findings highlight the high functional value of this fruit, supporting its health-protective effects when consumed regularly, either as fresh fruit or in nutraceutical form.

This study aims to explore the potential of combined CSP and PV systems in high latitude areas. Several performance metrics are evaluated and compared with standalone CSP or PV plants to identify the benefits and challenges of deploying such hybrid plants. Six different sites have been selected and the ASDELSOL hybrid power plant simulation tool, developed by the Thermodynamics and Renewable Energies Group at the University of Seville, has been used. This simulation tool allows performing dynamic performance simulations of hybrid solar plants under various operation strategies, along with conducting economic evaluations. We have analyzed a hybrid solar plant composed of 50 MW PTC and 75 MW PV, with 10 hours of TES and a 15 MW electric heater to transfer excess PV energy to the TES tanks in a 50 MW base load operation strategy, where the main objective of the PV plant is to cover the self-consumption of the PTC plant. Results obtained show that PV/PTC hybridization reduces LCOE and increases CF compared to standalone PV or PTC plants. These improvements are more pronounced in regions where the contribution of PTC plants is lower. In high latitude regions, an N-S orientation of PTC fields achieves higher production than an E-W orientation. However, a combination of both orientations can optimize their production and performance. Finally, it is concluded that it is necessary to carry out feasibility studies in high latitude locations before dismissing them outright, as it has been observed that, in certain regions, hybrid PV/PTC solar plants can offer an effective alternative.

Repensar la Educación, formada por un grupo de educadores dominicanos, comparte este documento buscando ofrecer una lectura y una perspectiva diferente, sobre las respuestas necesarias para garantizar la calidad educativa. El documento se estructura en cinco secciones comenzando con una justificación del mismo y concluyendo con sugerencias para convertir el repensar en acciones. En la segunda sección, se exploran los temas centrales de la educación sobre los que se invita a repensar, entre ellos: el estudiante como centro del proceso formativo, el docente como actor clave de la formación integral del estudiante, la escuela como un ecosistema del proceso formativo, la familia y la comunidad como pilares fundamentales, y los mecanismos estatales como entidades reguladoras, de apoyo y de dinamización de los cambios. En el documento, no solo se enfatiza el papel de las escuelas como instituciones clave, sino que también las concibe como ecosistemas sostenibles que fomentan el diálogo y empoderan a las comunidades. Se subraya la importancia de brindar a las escuelas el chance de realizar plenamente su potencial para alcanzar esta visión.

Abstract Honey produced by stingless bees (Tribe: Meliponini) is well‐known for their medicinal and antimicrobial properties from their diverse content of bioactive compounds. However, it has not been possible to fully characterize its active principles. The present systematic review is based on 117 full‐text articles. Our review highlights (1) the need to apply complementary methodologies and consolidate protocols to quantify antimicrobial activity (2) to characterize antimicrobial agents in honeys of Heterotrigona itama, Tetragonisca angustula, and Melipona beecheii, and; (3) to search for compounds against multidrug‐resistant and/or biofilm‐forming pathogens. Moreover, we performed a meta‐analysis of 29 articles with quantitative data using diffusion assays and minimum inhibitory concentration (MIC). Staphylococcus aureus and Escherichia coli were the most evaluated pathogens, evidencing 221 and 149 antimicrobial activity assays against honey samples of 36 and 24 stingless bee species, respectively. The highest pooled mean in diffusion assays against S. aureus showed a difference between methicillin‐resistant S. aureus (MRSA) and non‐MRSA strains {14.43 [95% confidence interval (CI): 12.16–16.71] mm vs. 11.55 (95% CI: 10.22–12.87) mm}, whereas E. coli showed pooled means of 9.09 (95% CI: 7.93–10.25) mm. Regarding MIC expressed as volume/volume percentage (%, v/v), the estimated inhibitory concentrations were 7.89 (95% CI: 3.94–11.83) mL/100 mL for MRSA and 5.60 (95% CI: 2.66–8.55) mL/100 mL considering all S. aureus strains. In contrast, the estimated MICs as weight/volume percentage (%, w/v) showed 15.00 [(95% CI: 12.84–17.16) g/100 mL for S. aureus and 16.17 (95% CI: 5.78–26.55) g/100 mL for E. coli. However, honey remains an unexplored source of antimicrobial molecules.

Protein structure determination has made progress with the aid of deep learning models, enabling the prediction of protein folding from protein sequences. However, obtaining accurate predictions becomes essential in certain cases where the protein structure remains undescribed. This is particularly challenging when dealing with rare, diverse structures and complex sample preparation. Different metrics assess prediction reliability and offer insights into result strength, providing a comprehensive understanding of protein structure by combining different models. In a previous study, two proteins named ARM58 and ARM56 were investigated. These proteins contain four domains of unknown function and are present in Leishmania spp. ARM refers to an antimony resistance marker. The study’s main objective is to assess the accuracy of the model’s predictions, thereby providing insights into the complexities and supporting metrics underlying these findings. The analysis also extends to the comparison of predictions obtained from other species and organisms. Notably, one of these proteins shares an ortholog with Trypanosoma cruzi and Trypanosoma brucei, leading further significance to our analysis. This attempt underscored the importance of evaluating the diverse outputs from deep learning models, facilitating comparisons across different organisms and proteins. This becomes particularly pertinent in cases where no previous structural information is available.

The study investigated the impact of nonsteroidal anti-inflammatory drugs (NSAIDs) on burn-induced coagulopathy in severely burned patients. Patients with a greater than 20% TBSA were identified in the TriNetX research network and categorized into receiving or not receiving NSAIDs in the first week after the burn. The statistical significance of the rate of burn-induced coagulopathy, mortality and sepsis in the week following injury was analysed. We observed 837 severely burned patients taking NSAIDS during the week following the burn and 1036 patients without. After matching for age, gender and race, the risk of burn-induced coagulopathy significantly decreased (p < 0.0001) in patients taking NSAIDs (17.7%) compared to those without (32.3%). Patients taking NSAIDs were also less likely to develop sepsis (p < 0.01) and thrombocytopenia (p < 0.001) or die the week following injury (p < 0.0001). In conclusion, the early protective effects of NSAIDs at reducing the risk of coagulopathy as well as sepsis and mortality occur during the acute phase of burns.
