Dissertação para obtenção do grau de Mestre no Instituto Universitário Egas Moniz Introdução: O cancro da mama avançado (ABC) constitui a principal causa de morte por cancro no sexo feminino, sendo importante compreender o resultado das terapêuticas existentes em contexto real. O Registo Oncológico Nacional (RON) constitui uma fonte de dados muito completa para a realização deste tipo de estudos. Objetivos: O objetivo global do estudo é avaliar, de forma retrospetiva e com dados da vida real, a efetividade terapêutica dos inibidores da aromatase (IA) e do fulvestrant no tratamento do ABC no Instituto Português de Oncologia de Lisboa Francisco Gentil. Métodos: Recorreu-se a um estudo de coorte retrospetivo. Os casos foram identificados através das bases de dados da Farmácia Hospitalar e do RON. Incluíram-se mulheres ≥ 18 anos, com ABC RH+ HER2–, que realizaram terapêutica com IA e/ou fulvestrant entre 31/05/2017 e 31/03/2019. Os doentes foram seguidos até à data de morte ou de cut-off (31/03/2021). O indicador de resultados primário foi o tempo até à falha terapêutica (TTF) e o secundário foi a sobrevivência global (OS). O gap de eficácia-efetividade foi avaliado através da comparação com os ensaios clínicos. Resultados: Obteve-se uma coorte com 192 doentes, que integraram a coorte do fármaco a que foram expostas. Letrozol: TTF mediano de 11,18 meses (IC95% 8.65 – 13,72) e OS mediana de 30,82 meses (IC95% 20,25 – 41,39). Exemestano: TTF mediano de 5,99 meses (IC95% 4,14 – 7,83) e OS mediana de 22,14 meses (IC95% 9,70 – 34,58). Fulvestrant: TTF mediano de 5,60 meses (IC95% 4,51 – 6,61) e OS mediana de 21,61 meses (IC95% 16,53 –26,69). Anastrozol: não foi possível analisar. Conclusão: Tendo em conta as limitações encontradas para o TTF, considerou-se a OS como o indicador mais relevante para este estudo. Assim sendo, os dados obtidos sugerem que a efetividade em contexto real é inferior à eficácia reportada nos ensaios clínicos. Introduction: Advanced breast cancer (ABC) is the leading cause of mortality by cancer among women, therefore it is important to understand the outcome of the approved therapies in real-world. The National Cancer Registry database is a very important source for carrying out this type of studies. Aims: The main objective is to evaluate, retrospectively and with real world data, the effectiveness of aromatase inhibitors (AI) and fulvestrant in the treatment of ABC in Portuguese Institute of Oncology of Lisbon. Methods: A retrospective cohort study was conducted. Cases of interest were identified through the Hospital Pharmacy database and the National Cancer Registry database. Females aged ≥ 18 years, with ABC RH+ HER 2– and exposed to AI and/or fulvestrant, between 31/05/2017 and 31/03/2019, were included. Patients were followed-up until death or cut-off date (31/03/2021). Primary outcome was time to treatment failure (TTF). Secondary outcome was overall survival (OS). The efficacy-effectiveness gap was evaluated by comparing results with clinical trial data. Results: The sample included 192 patients, who were included in one of the 4 cohorts according to the drug that they were exposed to. Letrozole cohort: median TTF was 11.18 months (95%CI 8.65 – 13.72) and median OS was 30.82 months (95%CI 20.25 – 41.39). Exemestane cohort: median TTF was 5.99 months (95%CI 4.14 – 7.83) and median OS was 22.14 months (95%CI 9.70 – 34.58). Fulvestrant cohort: median TTF was 5.60 months (95%CI 4.51 – 6.61) and median OS was 21.61 months (95%CI 16.53 – 26.69). It was not possible to perform the analysis for the anastrozole cohort. Conclusion: Considering the limitations found for the TTF, we considered OS to be the most relevant outcome of therapeutic effectiveness for this study. Thus, our data suggest lower effectiveness in a real-life context than the efficacy reported in the clinical trial.
