Thiamine (vitamin B1) is an essential micronutrient in energy metabolism and cognitive and neurological health. Thiamine deficiency disorders (TDDs) have a range of clinical presentations that result in various morbidities and can be fatal if not promptly recognized and treated, especially in infants. To intervene, thiamine intakes by breastfeeding mothers and others at risk of thiamine deficiency should be increased to ensure adequate thiamine intake. Although thiamine fortification programs have a long history in high‐income countries, there are few mandatory fortification programs to address TDDs in low‐ and middle‐income countries (LMICs), particularly in the regions of greatest concern, South and Southeast Asia. This review highlights essential aspects for consideration in the development of a mandatory fortification program in LMICs, including an overview of the data required to model fortification dosing schemes, available thiamine fortificants, and potential fortification vehicles, as well as identifies current knowledge gaps., There are few mandatory fortification programs to address thiamine deficiency disorders in low‐ and middle‐income countries (LMICs), particularly in the regions of greatest concern, South and Southeast Asia. In 2014, there was an outbreak of beriberi on Kuria, a remote atoll in Kiribati, a small Pacific Island nation. A thiamine‐poor diet consisting mainly of rice, sugar, and small amounts of fortified flour was likely to blame. We aimed to design a food fortification strategy to improve thiamine intakes in Kuria. We surveyed all 104 households on Kuria with a pregnant woman or a child 0–59 months. Repeat 24‐h dietary recalls were collected from 90 men, 17 pregnant, 44 lactating, and 41 other women of reproductive age. The prevalence of inadequate thiamine intakes was >30% in all groups. The present study aimed to determine thiamine intake levels and the association between thiamine intake, diabetes, cardiovascular diseases and mental health. Participants were interviewed to obtain data on socio-demographic characteristics, lifestyle, current medications, medical and family history. The daily intake of thiamine was assessed by a 24-h recall. The mean age of the 34 700 study subjects was 42⋅9 years (sd 22⋅8, min–max: 1–80) and 19 342 (55⋅7 %) were women. The levels of thiamine intake were 1⋅126 mg (2016), 1⋅115 mg (2017) and 1⋅087 mg (2018) for women, which were equal to or only slightly above the recommended intake of 1⋅10 mg/d for women. The levels of thiamine intake from 2014–15 and 2016–18 significantly decreased. The estimated percentage of insufficient thiamine intake was 37⋅8 % (95 % CI 37⋅3, 38⋅4). Multivariable regression analysis adjusted for potential confounders showed that thiamine intake was critically associated with lower risks of hypertension, MI or angina, type 2 diabetes, depression and dyslipidemia. The daily thiamine intake from food can reversal the risks of hypertension (OR 0⋅95; 95 % CI 0⋅90, 0⋅99), MI or angina (OR 0⋅84; 95 % CI 0⋅74, 0⋅95), type 2 diabetes (OR 0⋅86; 95 % CI 0⋅81, 0⋅93), depression (OR 0⋅90; 95 % CI 0⋅83, 0⋅97) and dyslipidemia (OR 0⋅90; 95 % CI 0⋅86, 0⋅95), respectively. Further works are needed to identify the effects of thiamine and non-communicable diseases (NCDs) and mental health. A preventive thiamine supplementation strategy should be adopted to target NCDs and mental health and risk factors associated with thiamine deficiency. The optimisation of NCD control and mental health protection is also a vital integral part of Korea's public health system.

BACKGROUND Studies have shown that thiamine intake is associated with cervical cancer, but the relationship between thiamine and HPV infection remains unclear. In the present study, we used the National Health and Nutrition Examination Survey (NHANES) database to investigate whether HPV infection was associated with thiamine intake. MATERIAL AND METHODS A total of 13 471 women ages 18-59 years were selected from the NHANES database from 2003 to 2016. Using thiamine intake as the independent variable, HPV infection as the dependent variable, and sociodemographic data and other data as the covariates, we analyzed the relationship between thiamine and HPV infection by conducting a weighted logistic regression model in a cross-sectional research design. RESULTS The two-piecewise linear model indicated the inflection point of thiamine intake was 2.07 mg. On the left side of the inflection point, the difference in the thiamine intake of log2 conversion was related to the difference of 0.82 in HPV infection, which means that the increase of every 1 unit increase in thiamine intake is associated with the decrease of the HPV infection by 18%. On the right side of the inflection point, we did not observe a correlation between HPV infection and thiamine intake. CONCLUSIONS Thiamine intake is negatively correlated with HPV infection. Intake of an appropriate amount of thiamine can prevent HPV infection. The best preventive effect can be achieved when the intake is about 2 mg, and excessive intake will not increase the preventive effect.

Abstract. The thiamine status of South Korean people has not been recently reported. Therefore, the aim of this study was to determine thiamine intake and status of Korean adults based on a biochemical index. Three consecutive 24-h food recalls and morning first-urine samples were obtained from 143 healthy adults (65 men and 78 women), aged 20–64 years, living in Seoul metropolitan area, Korea. Daily dietary thiamine intake of men and women was 1.42 ± 0.37 mg and 1.18 ± 0.24 mg, respectively. Only 9.1% of the subjects consumed less total thiamine (dietary plus supplemental thiamine) than the estimated average requirement for Koreans. The top 10 major dietary thiamine sources were pork, polished rice, ramyeon (Korean instant noodles), baechukimchi (Chinese cabbage), mandarin oranges, chicken, cow’s milk, bread, beef, and potatoes. Those top 10 foods provided 57.85% of the subjects’ dietary thiamine intake and the top 30 food sources provided 77.23% of their dietary thiamine intake. Urinary thiamine excretions for men and women were 37.20 ± 26.54 and 39.09 ± 28.80 μg/g creatinine, respectively. The migraine headache is a disease related to the neurovascular system, which affects 10%-20% of people, worldwide. Recent evidences suggested a relation between thiamine status and migraine headaches. The current study was undertaken to assess dietary intake of the thiamine in migraine patients and to evaluate its association with the frequency of migraine attacks. In a case-control design, the current study was performed on 50 migraine patients and 50 healthy people, 20-60 years old in Isfahan, Iran, in 2017. Information about dietary intake was collected by Food frequency questionnaire and analyzed using the Nutritionist version 4 (N4) software (Tinuviel Software). Information about the history of disease was collected by demographic questionnaire. Analysis of covariance and independent t-test were used for data analysis and p value less than 0.05 was considered significant. Mean age, weight, height, and body mass index of participants were 35.1 ± 9.8 years, 65.3 ± 10.4 kg, 162.5 ± 8.4 cm, and 24.7 ± 4.0 kg/m2, respectively. Dietary intake of thiamine among the migraine patients was lower than that in the healthy participants (p < 0.001). Migraine patients with the high frequency attacks had significantly lower intake of thiamine compared with moderate frequency attacks group (p = 0.010), however, it was not significant after adjusting for energy intake (p = 0.410, p = 0.240). Dietary intake of thiamine in migraine patients was not significantly different in comparing with healthy subjects. In addition, no significant correlation between thiamine intake and the frequency of migraine attacks was observed.

The purpose of this study was to find the effect of endurance training and thiamine supplementation on anti-fatigue during the exercise. Each nine students from K Women's University went through three cross-over treatments: placebo treatment, training treatment and thiamine treatment. Training treatment was performed with bicycle ergometer exercise for four weeks (five days per week). Each exercise was performed for an hour with intensity set at 70% (50rpm) of maximal oxygen uptake. Thiamine treatment group was given 10mg of thiamine tetrahydrofurfuryl disulfide per one kilogram for four weeks. The bicycle ergometer exercise was performed at 70% of maximal oxygen uptake in exercise intensity which 60 minutes of exercise was performed at 50rpm . Lactate concentration was significantly decreased during 15 to 30 minutes of exercise for those with training treatment and 15 to 60 minutes of exercise for those with thiamine treatment compared to placebo treatment group. Ammonia concentration was significantly decreased during 15 to 60 minutes of exercise and 15 to 30 minutes of recovery for those with training and thiamine treatment compared to placebo treatment. Resting blood thiamine concentrations of placebo treatment were significantly lower than training treatment. 60 minutes after the exercise, plasma thiamine concentration was significantly increased in all treatment group. To sum up the previous, thiamine intake during exercise positively benefits carbohydrate metabolism in a way that will decrease lactate concentration, ammonia concentration, and anti- fatigue by reducing the RPE. Therefore, we can consider thiamine intake to be utilized as similar benefits as endurance training.

International audience; From September 2013 to July 2014, several gold miners working in the tropical forest consulted the Maripasoula Health Center in French Guiana for edema and findings consistent with right cardiac failure. Of the 42 cases of beriberi that were diagnosed, one patient died. The laboratory and clinical investigation demonstrated vitamin B1 deficiency in most of the patients tested. Furthermore, 30 of 42 patients responded favorably to 500 mg of intravenous or intramuscular thiamine supplementation. In addition, dietary investigation showed insufficient thiamine intake in these patients. We concluded that patients had acquired beriberi because of diet restrictions, hard labor, and infectious diseases, notably malaria. In 2016, cases were still being reported. We recommend screening for compatible symptoms in gold miners, thiamine supplementation, and nutritional intervention

Recent pet food recalls for insufficient dietary thiamine have highlighted the importance of adequate thiamine intake in dogs and cats, as thiamine is an essential dietary nutrient with a critical role in energy metabolism. Prolonged thiamine deficiency leads to clinical signs that can span several organ systems, and deficiency can be fatal if not reversed. In this review, the current knowledge of thiamine metabolism will be summarized. Dietary recommendations for dogs and cats will be discussed, and the risk factors and clinical signs associated with thiamine deficiency will be examined.

Animal species and muscle related differences in thiamine and riboflavin contents were studied in pork, chicken, veal and beef. Pork was the best thiamine source and there was no significant difference in thiamine content of longissimus dorsi and shoulder muscles. Also, no difference in thiamine content of chicken breast and thigh was found. In contrast, the riboflavin content significantly differed between the muscles within species (pork and chicken) examined. With the average daily lean meat consumption in Switzerland (105 g day −1 ), thiamine and riboflavin intakes were approximately 0.5 mg day −1 and 0.2mgday −1 , respectively. The recommended daily thiamine intake was met up to 25% for men and 29% for women. Pork itself contributed about 23% (men) and 27% (women). The recommendation for riboflavin intake was met up to 10% for men and 11% for women.

To investigate 24-h urinary thiamine as a potential biomarker for thiamine intake for use in validation studies to assess the validity of dietary intake data collected by self-reporting dietary methods.Seven male and six female healthy participants living for 30 days in a metabolic suite under strictly controlled conditions consuming their usual diet as assessed beforehand from four consecutive 7-day food diaries kept at home. During the 30-day study, all 24-h urine specimens were collected, validated for their completeness and analysed for thiamine.Thirty-day mean (+/-s.d.) calculated thiamine intake was 2.22+/-0.55 mg/day. Thirty-day mean (+/-s.d.) urinary excretion of thiamine was 526.5+/-193.0 microg/day (24.7+/-8.10% of intake). There was a highly significant correlation between individuals' 30-day means of thiamine intake and their mean excretion level (r=0.720; P=0.006), where 1 mg of thiamine intake predicted 268.2 microg of thiamine in urine. The correlations between intake and excretion remained significant when measurement from a single 24-h urine collection was used (r=0.56).Twenty-four-hour urinary thiamine can be used as a concentration biomarker for thiamine intake in dietary validation studies.

Erythrocyte transketolase activity as measured by a new fluorimetric method was used to define the tissue thiamine status of an age-stratified sample of 42 adults. Concurrent 7-day dietary assessments were made. 26% of subjects (6/21 men, 5/21 women) were biochemically abnormal. Subjects who were biochemically normal had mean daily thiamine intakes that were 41% (for men) and 36% (for women) greater than those for biochemically abnormal subjects. Differences in absolute thiamine intake (mg/day) were greater and more significant (p = 0.007 and 0.005) than the corresponding differences in thiamine intake expressed as mg/1000 kcal (p = 0.04). Absolute daily thiamine requirement (1.22 mg for men, 1.03 mg for women) was more useful in predicting biochemical thiamine status than thiamine intake expressed as mg/1000 kcal. The withdrawal of thiamine fortification of processed cereals would have serious implications for public health.

Various survey methods have been used in estimating the dietary intake of individuals, families and other groups. Every method has its advantages and disadvantages, and its practical, financial and technical difficulties. The choice is also limited by the quality of the population studied. The nutrient level is shown by many parameters: Cholesterol level, thiamine intake, uric acid level, ascorbic acid level in plasma, SO2-intake. Field studies were done with different groups of the population: pupils, elderly people living in homes, heavy workers, sedentary workers, clerks, policemen, sportsmen, students. SO2-intake is related with thiamin intake. It is very different from group to group (0,05--47,7 mg)/thiamin.

Summary The fraction of a one milligram dose of thiamine hydrochloride injected intramuscularly which was excreted in the urine in a four-hour period was determined in fifty consecutive, unselected, hospitalized patients, who had been grouped into one of four categories depending on their average thiamine intake as estimated from a detailed diet history. Since no correlation between the fraction of the thiamine excreted and the habitual thiamine intake of the patient was found, it is concluded that this procedure is valueless for diagnosing subclinical thiamine deficiency.

Conclusions1. No significant difference was observed in the length of time required by diabetic and non-diabetic rats on a thiamine-free diet to develop signs of marked deficiency (39 as compared with 41 to 45 days respectively). Following treatment with thiamine, the diabetic rats recovered as quickly as did the non-diabetic animals. 2. During the period of deprivation, the glycosuria of the diabetic rats diminished, presumably because of the lowered food intake. However, urinary sugar tended to reappear at the final stage of deficiency at the time of “polyneuritis”. Carbohydrate tolerance in both the diabetic and non-diabetic rats, as judged by glucose tolerance tests, showed little change until signs of marked deficiency appeared, when there was impairment of tolerance. 3. Following-deprivation, a thiamine intake 4 to 8 times the maintenance dosage appeared to improve carbohydrate tolerance.

• Ocular abnormalities and psychomotor difficulties were prominent in two unrelated children; in addition, the older child had respiratory irregularity during sleep. The pathologic findings included lesions of the optic nerve in the case with available material and established the diagnosis of Leigh's subacute necrotizing encephalopathy. This disorder is thought to result from inhibition of a thiamine-dependent enzymatic process and may be modified by greatly increased thiamine intake. Suspicion of the diagnosis in a child with ophthalmoplegia or other ocular abnormalities may lead to earlier recognition and more successful treatment of the disease. A boy with the intermediate variant of branched-chain ketoaciduria was studied. Treatment with an amino acid mixture was discontinued at the age of 7.5 years. Reintroduction of normal protein-containing foods precipitated the biochemical abnormalities, characteristic of MSUD, which were relieved by 10 mg thiamine/day. Adaptation to this regiment developed and thiamine intake was increased to 100 mg/day, later to 1 000 mg/day. The patient developed well and had no attacks of ketoacidosis. 1-14C-leucine degradation in intact fibroblasts was 15% of controls and did not increase upon addition of thiamine to the incubation medium.

These studies indicate that vitamin B complex restriction caused severe primary mental changes or aggravation of pre-existing psychotic trends among the psychotic subjects. The character of the mental changes appeared to depend on speed and severity of the deprivation. Patients whose B complex intake was so restricted that it contained 400 mcg. of thiamine and correspondingly small intake of other B complex factors showed diminution of psychomotor activity, dulling of affect, loss of interests and ambitions. In some of the severely depleted patients whose thiamine intake was restricted to 200 mcg. daily, serious emotional disturbances with loss of inhibitory control developed. In others, pre-existing affective trends such as hypomanic and mildly depressive features became markedly exaggerated. The formation of paranoid trends also was observed.
