35. What Sequences on High-Field MR Best Depict Temporal Resolution of Experimental ICH and Edema Formation in Mice?

Abstract

Background and Purpose. Pilot study to examine the use of T1-, T2-, and T2*-weighted images for evaluating hematoma size and extent of edema in mouse brain at high field. Methods. Following collagenase-induced intracerebral hemorrhage, nine mice were imaged at 4.7 T using T1-, T2-, and T2*-weighted images for hematoma and edema quantitation on days 1, 3, 10, and 21 after surgery. Values were compared with morphometric analysis of cryosections at the time of final MR imaging. Results. For hematoma quantitation, the Spearman correlation coefficient (r) between T1 signal change and histology was 0.70 (P < 0.04) compared with r = 0.61 (P < 0.09) for T2*. The extent of perihematomal edema formation on cryosections was well reflected on T2 with r = 0.73 (P < 0.03). Conclusions. Within the limits of our pilot study, MR imaging on 4.7 T appears to approximate the temporal changes in hematoma and edema sizes in murine ICH well, thus laying the groundwork for longitudinal studies on hematoma resorption and edema formation. Abstract

Zusammenfassung: Die klinische Relevanz von vaskulären Fehlbildungen des Hirnstamms soll mit den Befunden der bildgebenden Diagnostik verglichen werden. Acht Patienten mit vaskulären Fehlbildungen des Hirnstamms wurden magnetresonanztomographisch und teilweise angiographisch untersucht. Zusätzlich wurden klinischneurologische und elektrophysiologische Untersuchungen durchgeführt. In jeweils der Hälfte der Fälle wurden die vaskulären Veränderungen des Hirnstamms nach der MRT als venöse Dysplasie (DVA) und kapilläre Teleangiektasie eingestuft. Begleitende Kavernome wurden bei zwei Patienten gefunden, Übergangsformen einmal. Als Ursache klinischer Symptome wurden zweimal die Kavernome identifiziert, in einem Fall mit Trigeminusneuralgie die Drainagevene der DVA. Einmal bestand ein möglicher Zusammenhang zwischen einer kapillären Teleangiektasie und einer latenten Hemiparese. In den anderen Fällen bestand kein Zusammenhang. Die erste Methode zur Diagnostik der Gefäßveränderungen des Hirnstamms ist die MRT. Die differentialdiagnostische Abgrenzung zu pathologischen Hirnstammprozessen ist mittels Dünnschichtuntersuchung bei Kenntnis der Malformation fast immer möglich. Die klinische Relevanz von Kavernomen ist gesichert, von DVA und kapillärer Teleangiektasie nur in Ausnahmefällen. Es gibt Übergangsformen zwischen den venösen Veränderungen des Hirnstamms.

Abstract

Recombinant Desmoteplase was used for lysis of intracerebral hemorrhages in a well-established large-scale animal model. The fibrinolytic potential of the plasmonogen activator, as well as the effect on the delayed edema was evaluated which is most prominent three to five days after ictus. We observed a significant reduction in hematoma volume by DSPA-lysis (75%), comparable to rtPA-lysis. The delayed edema was not aggravated significantly by DSPA whereas an enlargement of perihematomal edema (413%) was observed in rtPA-treated pigs, comparable to the results of a previous study by Thiex et al. Histological examination of HE-stained brain slices show an increased inflammatory reaction of the perifocal area in tPA-treated animals but not in brains of DSPA-treated pigs. DSPA is a fibrinolytic that could potentially improve the treatment of spontaneous intracerebral hemorrhages if applied locally by minimally-invasive technique.
