ContextAlendronate sodium reduces fracture risk in postmenopausal women who have vertebral fractures, but its effects on fracture risk have not been studied for women without vertebral fractures.ObjectiveTo test the hypothesis that 4 years of alendronate would decrease the risk of clinical and vertebral fractures in women who have low bone mineral density (BMD) but no vertebral fractures.DesignRandomized, blinded, placebo-controlled trial.SettingEleven community-based clinical research centers.SubjectsWomen aged 54 to 81 years with a femoral neck BMD of 0.68 g/cm2 or less (Hologic Inc, Waltham, Mass) but no vertebral fracture; 4432 were randomized to alendronate or placebo and 4272 (96%) completed outcome measurements at the final visit (an average of 4.2 years later).InterventionAll participants reporting calcium intakes of 1000 mg/d or less received a supplement containing 500 mg of calcium and 250 IU of cholecalciferol. Subjects were randomly assigned to either placebo or 5 mg/d of alendronate sodium for 2 years followed by 10 mg/d for the remainder of the trial.Main outcome measuresClinical fractures confirmed by x-ray reports, new vertebral deformities detected by morphometric measurements on radiographs, and BMD measured by dual x-ray absorptiometry.ResultsAlendronate increased BMD at all sites studied (P2.5 SDs below the normal young adult mean; RH, 0.64; 95% CI, 0.50-0.82; treatment-control difference, 6.5%; number needed to treat [NNT], 15), but there was no significant reduction among those with higher BMD (RH, 1.08; 95% CI, 0.87-1.35). Alendronate decreased the risk of radiographic vertebral fractures by 44% overall (relative risk, 0.56; 95% CI, 0.39-0.80; treatment-control difference, 1.7%; NNT, 60). Alendronate did not increase the risk of gastrointestinal or other adverse effects.ConclusionsIn women with low BMD but without vertebral fractures, 4 years of alendronate safely increased BMD and decreased the risk of first vertebral deformity. Alendronate significantly reduced the risk of clinical fractures among women with osteoporosis but not among women with higher BMD.

O modelo de vigilância epidemiológica da sífilis é baseado na notificação compulsória dos serviços de saúde pública e privado por todo o território nacional, sendo essas informações registradas no Sistema de Informação e Agravos de Notificação (SINAN). Desta maneira, o objetivo deste estudo pautou-se em descrever os coeficientes epidemiológicos e estatísticos dos casos notificados de sífilis materna no município de Palmas, Tocantins, durante os anos de 2017 a 2021. Como metodologia, realizou-se um estudo transversal com análise de série temporal referente ao período de 2017 a 2021, utilizando dados do Sinan. Os resultados demonstraram que, durante o período do estudo (2017 a 2021) foram notificados 749 casos de Sífilis materna. A classe modal dessa variável corresponde a faixa etária de 20 a 29 anos, seguida pela faixa de 15 a 19 anos, com predominância da região Sul de Palmas-TO. A raça que mais prevaleceu foi a parda 493 (65,8%), seguida da amarela 101 (13,5%), com ensino médio completo n=209 (27,9%), sob a forma latente com n=490 (65,4%), acompanhado da forma primária n=140 (18,7%). O esquema de tratamento mais utilizado foi Penicilina G benzantina 7.200.000 UI, com n=593 (79,2%) e Penicilina G benzantina 2.400.000 UI, com n= 18 (2,4%). O teste treponêmico, reativo foi o mais prevalente, com n=536 (71,6%). Embora o diagnóstico e o tratamento da sífilis sejam ofertados a todas as gestantes de maneira gratuita, disponibilizado integralmente através do Sistema Único de Saúde (SUS), esse agravo ainda é um grande desafio para a redução dos casos. The syphilis epidemiological surveillance model is based on the compulsory notification of public and private health services throughout the national territory, and this information is registered in the Information System and Notifiable Diseases (SINAN). Thus, the objective of this study was to describe the epidemiological and statistical coefficients of reported cases of maternal syphilis in the municipality of Palmas, Tocantins, during the years 2017 to 2021. As a methodology, a cross-sectional study was carried out with analysis of time series for the period from 2017 to 2021, using data from Sinan. The results showed that during the study period (2017 to 2021) 749 cases of maternal syphilis were reported. The modal class of this variable corresponds to the age group from 20 to 29 years old, followed by the group from 15 to 19 years old, with a predominance of the South region of Palmas-TO. The most prevalent race was brown 493 (65.8%), followed by yellow 101 (13.5%), with complete high school n=209 (27.9%), in the latent form with n=490 (65.4%), followed by the primary form n=140 (18.7%). The most used treatment regimen was Penicillin G benzanthine 7,200,000 IU, with n=593 (79.2%) and Penicillin G benzanthine 2,400,000 IU, with n= 18 (2.4%). The reactive treponemal test was the most prevalent, with n=536 (71.6%). Although the diagnosis and treatment of syphilis is offered to all pregnant women free of charge, fully available through the Unified Health System (SUS), this condition is still a major challenge for reducing cases. El modelo de vigilancia epidemiológica de sífilis se basa en la notificación obligatoria a los servicios de salud públicos y privados en todo el territorio nacional, y esta información se registra en el Sistema de Información y Enfermedades de Notificación Obligatoria (SINAN). Así, el objetivo de este estudio fue describir los coeficientes epidemiológicos y estadísticos de los casos notificados de sífilis materna en el municipio de Palmas, Tocantins, durante los años 2017 a 2021. Como metodología se realizó un estudio transversal con análisis de series temporales para el período de 2017 a 2021, utilizando datos de Sinan. Los resultados mostraron que durante el período de estudio (2017 a 2021) se notificaron 749 casos de sífilis materna. Aunque el diagnóstico y tratamiento de la sífilis se ofrece a todas las mujeres embarazadas de forma gratuita, totalmente disponible a través del Sistema Único de Salud (SUS), esa condición sigue siendo un gran desafío para la reducción de casos.

15. Tratando o câncer de mama em tempos de COVID-19: uma revisão integrativa

Objective: To describe the most relevant findings in the literature on the treatment of breast cancer in times of the COVID-19 pandemic. Methodology: It is an integrative review on the way in which breast cancer treatment was being provided in the pandemic period, carried out between October and November 2021, in the following databases: Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS), PEDRo, Scopus (Elsevier Science), Medical Literature Analysis and Retrieval System Online (MEDLINE/PubMed) e Scientific Eletronic Library Online (SciELO). Results: 19 publications were analyzed that discussed ways of monitoring breast cancer patients during the pandemic, their limitations and impacts caused. The main results found point to the use of tele-services, an increase in the number of cancellations of exams and consultations, in addition to a drop in the practice of physical activity and the emergence of emotions that can generate psychological pathologies in the COVID-19 scenario. Closing remarks: The studies found reveal the fragility of health systems and the lack of public health policies aimed at women undergoing breast cancer treatment, in times of public health emergencies. This fact negatively impacts the quality of life and prognosis of these patients. Objetivo: Describir los hallazgos más relevantes en la literatura sobre el tratamiento del cáncer de mama en tiempos de la pandemia de COVID-19. Metodología: Se trata de una revisión integradora sobre la forma en que se estaba brindando el tratamiento del cáncer de mama en tiempos de pandemia, realizada entre octubre y noviembre de 2021, en las siguientes bases de datos: Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS), PEDRo, Scopus (Elsevier Science), Medical Literature Analysis and Retrieval System Online (MEDLINE/PubMed) e Scientific Eletronic Library Online (SciELO). Resultados: Se analizaron 19 publicaciones que discutieron formas de acompañamiento de pacientes con cáncer de mama durante la pandemia, sus limitaciones e impactos causados. Los principales resultados encontrados apuntan al uso de teleservicios, un aumento en el número de cancelaciones de exámenes y consultas, además de una caída en la práctica de actividad física y la aparición de emociones que pueden generar patologías psicológicas en la pandemia de COVID-19. 19 escenario. Accidents by freshwater stingrays are common in northern Brazil, there is no specific therapy for high morbidity and local tissue destruction. The irradiation of venoms and toxins by ionizing radiation has been used to produce appropriate immunogens for the production of antisera. We planned to study the efficacy of stinging mucus irradiation in the production of antisera, with serum neutralization assays of edematogenic activity and quantification of cytokines performed in animal models of immunization with native and irradiated mucus of Paratrygon aiereba, a large freshwater stingray. Antiserum potency and its cross-reactivity with mucus from other freshwater stingrays were detected by ELISA. Immunization models demonstrated the ability to stimulate a strong humoral response with elevated levels of serum IgG detectable by ELISA, and both native and irradiated mucus were immunogenic and capable of recognizing mucus proteins from other freshwater neotropical stingrays. Mucus P. aiereba causes cellular and humoral adaptive immune responses in cells of immunized mice producing antibodies and cytokines such as TNF-α, IL-6 and IL-17. Rabbit antisera immunized with mucus from P. aiereba irradiated at 2 kGy showed a significant reduction of mucus-induced edematogenic activity in mice. Our data suggest that the use of antisera against freshwater stingray mucus show the possibility of specific therapy for these accidents.

The physiotherapist minimizes the impacts of functionality throughout the treatment of breast cancer. It is believed that many women are unaware of the performance of this professional, causing misunderstandings about their role in the care process. This study investigated the perception of women with breast cancer about the importance of the physiotherapist's performance in comprehensive care during treatment. A cross-sectional and qualitative study was carried out by means of a focus group. The following categories were identified: Comprehensive care and Vision of the role of the physiotherapist. These were examined through Bardin's content analysis. The results revealed that women know and recognize the importance of the role of this professional. The highlight on the physio therapeutic approach in breast cancer was concluded, at all levels of care, becoming essential in the care process. It is necessary to advance the discussion of the physiotherapist's role, from the patients' perspective, aiming at improving health planning and management and decision making during prevention, diagnosis, maintenance and rehabilitation, reaffirming the role of this professional in the process care. El fisioterapeuta minimiza los impactos de la funcionalidad a lo largo del tratamiento del cáncer de mama. Se cree que muchas mujeres desconocen el papel de este profesional, provocando malentendidos sobre su papel en el proceso de atención. Este estudio investigó la percepción de mujeres con cáncer de mama sobre la importancia del papel del fisioterapeuta en la atención integral durante el tratamiento. Se realizó un estudio transversal y cualitativo mediante un grupo focal. Se identificaron las siguientes categorías: Atención integral y Visión del rol del fisioterapeuta. Estos fueron examinados utilizando el análisis de contenido de Bardin. Los resultados revelaron que las mujeres conocen y reconocen la importancia del papel de este profesional. Se concluyó el énfasis en el abordaje fisioterapéutico en el cáncer de mama, en todos los niveles de atención, haciéndolo fundamental en el proceso de atención. Es necesario avanzar en la discusión sobre el papel del fisioterapeuta, en la perspectiva de los pacientes, con el objetivo de mejorar la planificación y gestión de la salud y la toma de decisiones durante la prevención, diagnóstico, mantenimiento y rehabilitación, reafirmando el papel de este profesional en el proceso de cuidado. O fisioterapeuta minimiza os impactos da funcionalidade durante todo o tratamento do câncer de mama. Acredita-se que muitas mulheres desconhecem a atuação desse profissional, ocasionando incompreensões sobre o seu papel no processo de cuidado. Esse estudo investigou a percepção das mulheres com câncer de mama sobre a importância da atuação do fisioterapeuta no cuidado integral durante o tratamento. Realizou-se um estudo transversal e qualitativo, por meio de grupo focal. Identificaram-se as seguintes categorias: Cuidado integral e Visão do papel do fisioterapeuta. Estas foram examinadas por meio da análise de conteúdo de Bardin. Os resultados revelaram que as mulheres conhecem e reconhecem a importância do papel desse profissional. Concluiu-se o destaque sobre a abordagem fisioterapêutica no câncer de mama, em todos os níveis de atenção, tornando-se fundamental no processo de cuidado. Faz-se necessário avançar a discussão do papel do fisioterapeuta, sob a perspectiva das pacientes, objetivando a melhora do planejamento e gerenciamento em saúde e da tomada de decisões durante a prevenção, diagnóstico, manutenção e reabilitação, reafirmando o papel deste profissional, no processo do cuidado.

O presente estudo tem por objetivo abordar o perfil epidemiológico de casos de Dengue no município de Palmas- Tocantins no período de janeiro de 2015 a setembro de 2020. Trata-se de um estudo retrospectivo, transversal e descritivo. Farão parte desse estudo todas as fichas de notificações do SINAN, de indivíduos de todas as faixas etárias que tenham sido diagnosticados com dengue, no município entre o período referido. Com relação as notificação de Dengue no sistema do SINAN, em Palmas-Tocantins , no período de 2015 a 2020, observou-se uma número total de 14.533 casos confirmados de dengue, sendo 2019 o ano com maior número de registros, sendo 1.743 casos confirmados. No estudo verificou-se uma significante prevalência nas notificações de casos de dengue na zona urbana em detrimento da periurbana e rural. Houve ainda uma ligeira prevalência de registros no gênero feminino, 51,7 % em relação ao masculino. Observou-se também uma acentuada queda nos números de casos suspeitos e confirmados para dengue no ano de 2020 em relação aos demais anos. Conclui-se que existe uma vigente necessidade de aprimorar o conhecimento dos profissionais de saúde no tocante a um adequado registro dos parâmetros da ficha de notificação do SINAN, visto que a dengue segue sendo a principal arbovirose a acometer cidades endêmicas. Ademais, a ausência de dados ou a subnotificação prejudica o levantamento de informações realistas, comprometendo a abordagem e desenvolvimento de políticas públicas no combate à Dengue. The present study aims to address the epidemiological profile of dengue cases in the municipality of Palmas – Tocantins from January 2015 to September 2020. This is a retrospective, cross-sectional and descriptive study. All the SINAN notification forms, from individuals of all age groups who have been diagnosed with dengue in the municipality between the referred period will be part of this study. Regarding dengue notifications in the SINAN system, in Palmas – Tocantins, in the period from 2015 to 2020, there was a total number of 14.533 confirmed cases of dengue, with 2019 being the year with the highest number of registrations, with 1.743 confirmed cases. In the study, there was a significant prevalence in the notifications of dengue cases in urban areas rather than peri-urban and rural areas. There was also a slight prevalence of registries in females, 51,7% in comparison to the male gender. It was also observed a sharp drop in the numbers of suspected and confirmed cases of dengue in 2020 compared to the other years. It was concluded that there is a current need to improve the knowledge of health professionals regarding an adequate registration of the parameters of the SINAN notification form, since dengue continue to be the main arbovirus disease to affect endemic cities. In addition, the absence of data or underreporting impairs the gathering of realistic information, compromising the approach and development of public policies to fight dengue. El presente estudio tiene como objetivo abordar el perfil epidemiológico de los casos de Dengue en la ciudad de Palmas-Tocantins desde enero de 2015 hasta septiembre de 2020. Se trata de un estudio retrospectivo, transversal y descriptivo. Harán parte de este estudio todos los formularios de notificación SINAN de personas de todos los grupos de edad que hayan sido diagnosticadas con dengue en el municipio entre el período dicho. En cuanto a las notificaciones de Dengue en el sistema SINAN, en Palmas-Tocantins, en el período de 2015 a 2020, hubo un total de 14.533 casos confirmados de dengue, siendo el 2019 el año con mayor número de registros, con 1.743 casos confirmados. En el estudio, hubo una prevalencia significativa en las notificaciones de casos de dengue en áreas urbanas en comparación a las periurbanas y rurales. También hubo un ligero predominio de registros en el sexo femenino, 51,7% con relación al sexo masculino. Se observó una fuerte caída en el número de casos sospechosos y confirmados de dengue en 2020 en comparación con otros años. Se concluye que existe la necesidad actual de mejorar el conocimiento de los profesionales de la salud en cuanto a un registro adecuado de los parámetros del formulario de notificación del SINAN, ya que el dengue continúa siendo la principal enfermedad por arbovirus que afecta a las ciudades endémicas. Además, la ausencia de datos o el subregistro perjudica la recolección de informaciónes realistas, comprometiendo el abordaje y desarrollo de políticas públicas para combatir el Dengue.

Objetivo: Descrever os achados mais relevantes da literatura sobre o tratamento de câncer de mama em tempos de pandemia da COVID-19. Metodologia: Trata-se de uma revisão integrativa sobre a forma como estava sendo fornecido o tratamento para o câncer de mama em tempos de pandemia, realizada entre outubro e novembro de 2021, nas seguintes bases de dados: Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS), PEDRo, Scopus (Elsevier Science), Medical Literature Analysis and Retrieval System Online (MEDLINE/PubMed) e Scientific Eletronic Library Online (SciELO). Resultados: Foram analisadas 19 publicações que discorreram sobre as formas de acompanhamento das pacientes com câncer de mama durante a pandemia, suas limitações e impactos causados. Os principais resultados encontrados apontam o uso de teleatendimentos, aumento no número de cancelamentos de exames e consultas, além de queda na prática de atividade física e o afloramento de emoções que podem gerar patologias psicológicas no cenário da COVID-19. Considerações finais: Os estudos encontrados revelam a fragilidade dos sistemas de saúde e a falta de políticas públicas de saúde voltadas para mulheres em tratamento para o câncer de mama, em tempos de emergências de saúde pública. Objective: Face-to-face experiential work with emotions is effective, but it remains unclear what the feasibility of teletherapeutic work with emotions is. The current Covid-19 worldwide situation represents an opportunity for psychotherapists to test new ways of working with clients, including from an emotion-focused and emotionally focused perspective. Methods: The present brief report describes a clinical experience based on 6 weeks, so far, of telepsychotherapeutic work focused on emotions via online tools, during the Covid-19 partial confinement in Switzerland. Results: We found a certain feasibility of remote work with emotions in psychotherapy, for individual clients, and for couples. Specific challenges represent relationship and contextual factors (clear communication, using tone of voice, regular check ins, setting up a clear frame, technical aspects of communication, the Self of the therapist), accessing and deepening emotions and managing dysregulated experience and interpersonal escalation. Discussion: Based on this initial clinical experience, we propose that integrative experiential practice partially delivered via a computer screen be studied in controlled settings by psychotherapy research in the future. A epidemiologia dos agravos por peixes peçonhentos é pouco relatada, apesar da elevada morbidade e relevância para a saúde. Na região central do Brasil, as bacias do Tocantins e do rio Araguaia são muito utilizadas para atividades recreativas, especialmente entre junho e agosto. Apesar da dificuldade da identificação do animal causador do acidente por parte do acidentado, a maioria dos trabalhos cita que o gênero Potamotrygon é o mais relacionado a estes agravos. Por outro lado, arraias do gênero Paratrygon embora apresentem um ferrão menor que arraias de outros gêneros, causam bastante acidentes por se camuflarem com facilidade devido a sua cor clara, se confundindo com o fundo dos rios e lagos. O objetivo foi identificar atividades biológicas do muco da arraia Paratrygon aiereba. Os desenhos experimentais utilizando a secreção epidérmica foram a atividade edemaciante, por meio de Pletismografia, a dinâmica da alteração no sistema microcirculatório usando a microscopia intravital, a atividade nociceptiva através de estímulos químicos, o dano celular e atividade necrosante. Analisando o comportamento da atividade edemaciante em relação ao tempo, nota-se que o efeito do edema é mais acentuado nas primeiras horas do acidente, entretanto averígua-se que após 4h do acidente esta ação tende a diminuir. A fast paced industrial and port development has occurred at Suape Estuary, Northeast Brazil, but no information about hydrocarbon concentrations in water is available to this area. Considering that, the contamination level of Suape was determined by UV-Fluorescence in terms of dissolved and/or dispersed petroleum hydrocarbons (DDPHs), during wet and dry seasons. DDPHs ranged between 0.05 and 4.59 μg L(-1) Carmopolis oil equivalents and 0.01-1.39 μg L(-1) chrysene equivalents, indicating DDPHs close to a baseline contamination level. Some relatively high concentrations (>1 μg L(-1)) were probably associated with shipyard operations (hull paintings and ship docking), pollutants remobilization by dredging operations, occasional industrial discharges and oil derivatives released by vessels. DDPHs concentrations were lower in the wet season suggesting that the increased dilution rates caused by rainfall dominated upon the wet deposition of atmospheric combustion-derived PAHs process. Results in this study may be used as baseline to further studies in this area.

OBJETIVO: O objetivo desse trabalho é avaliar por meio do OCT Stratus a relação da espessura macular com a espessura de camadas de fibras nervosas. MÉTODOS: Neste trabalho foram analisados 106 olhos do serviço de oftalmologia da Faculdade de Medicina do ABC. Estudou-se 55 olhos que apresentavam valores de CFNR dentro ou acima dos valores normais em todos o quatro quadrantes e 51 olhos apresentando valores em apenas um ou mais quadrantes que indique suspeita ou fora do limite de normalidade dos valores de CFNR, comparando com sua espessura macular. RESULTADOS: Este estudo mostrou uma diminuição da espessura macular nos pacientes que apresentavam suspeitas ou diminuição da espessura de CFNR. CONCLUSÃO: O trabalho demonstrou que a espessura macular pode ser importante para avaliar uma possível perda de camada de fibras nervosas. OBJECTIVE: The objective of this work is to evaluate by means of the OCT Stratus the relation of the macular thickness with the thickness of nervous fiber layers. METHODS: In this work 106 eyes of the service of ophthalmology of the College of Medicine of the ABC had been analyzed. The study included 55 eyes that inside presented values of CFNR or above of the normal values in all the four quadrants and 51 eyes presenting values in only one or more quadrants that suspicion indicates or is of the limit of normality of the values of CFNR, comparing with macular thickness. RESULTS: This study it showed a reduction of the macular thickness in the patients who presented suspicion or reduction of the thickness of CFNR. CONCLUSION: The work demonstrated that the macular thickness can be a weapon important to evaluate a possible loss of nervous fiber layer.

Previously, spectra of certain weighted composition operators W ѱ, φ on H2 were determined under one of two hypotheses: either φ converges under iteration to the Denjoy-Wolff point uniformly on all of 𝔻 rather than simply on compact subsets, or φ is “essentially linear fractional.” We show that if φ is a quadratic self-map of 𝔻 of parabolic type, then the spectrum of Wѱ, φ can be found when these maps exhibit both of the aforementioned properties, and we determine which symbols do so.

CNPq O Estuário de Suape está localizado no litoral sul de Pernambuco, Brasil, cerca de 40 km da capital Recife. Este estuário está presenciando um rápido desenvolvimento industrial e portuário nas últimas décadas, com a implantação do Complexo Industrial Portuário de Suape (CIPS). Várias empresas instaladas na região operam cargas com derivados de petróleo, representando uma potencial fonte de hidrocarbonetos para o estuário. Amostras de água e sedimento foram coletadas com o objetivo principal de avaliar a presença de hidrocarbonetos no Estuário de Suape, identificando suas fontes e concentrações, em duas estações do ano (chuvosa e seca). As concentrações de hidrocarbonetos de petróleo dissolvidos e/ou dispersos na água foram determinadas usando a técnica de UV-Fluorescência. Os resultados variaram entre 0,05 e 4,59 μg L-1 equivalentes em óleo Carmópolis e entre 0,01 e 1,39 μg L-1 equivalentes em criseno, sugerindo que a região ainda pode ser classificada como não poluída. Algumas concentrações relativamente altas (>1 μg L-1) estão possivelmente associadas a resíduos originados nas atividades portuárias e do estaleiro, bem como a eventuais descargas industriais e lançamentos de resíduos de óleo por embarcações de pesca ou lazer. As concentrações de hidrocarbonetos de petróleo na água foram significativamente menores durante o período chuvoso, provavelmente devido a maior diluição dos contaminantes causada pelo aumento da precipitação e do fluxo de água doce no Rio Massangana. The complete nucleotide (nt) sequence of a 5.1-kb EcoRI DNA restriction fragment carrying the replication region of the Streptococcus faecalis plasmid pAM beta 1 has been determined. Of the seven major open reading frames (ORF A-G) identified within this fragment, two (C and E) were shown to be encoding by in vitro transcription/translation assays. Evidence was obtained that synthesis of the polypeptide (Mr 57,380) encoded by the largest ORF (E) was essential for replication. Deletion analysis indicated that the minimum unit of DNA required for replication resided on a 2.59-kb AccI-HpaI subfragment. ORF C resided outside of this fragment and encompassed an extensive region of directly repeated nt sequence. The encoded polypeptide (Mr 30,471) was therefore composed of large tracts of reiterated amino acid sequence (11 x VDP and 35 x TEP tripeptides) which probably caused the observed anomalous electrophoretic mobility of the synthesised protein (equivalent to 61 kDa). Deletion of a 416-bp segment of DNA between unique KpnI and StyI sites caused an increase in copy number, which correlated with the in vitro production of higher levels of ORF E polypeptide. Although homology was detected between the sequenced DNA, and the replicon of a closely related streptococcal plasmid (pSM19035), none was evident to any other characterised Gram+ plasmid.

Among gay and bisexual men, body dissatisfaction can manifest itself in the form of a desire for increased muscularity. Possibly in response to homophobia, the socio-sexual culture of gay and bisexual men privileges muscularity and may help to perpetuate a sense of body dissatisfaction. Little is known about factors associated with a drive for muscularity among gay and bisexual men. This study recruited participants (n = 400) at Toronto's 2008 lesbian, gay, bisexual and transgender festival to provide data used to examine the relationship between a drive for muscularity and demographics (age, race, education, HIV status), psychological factors (depression, disordered eating, internalised homophobia, substance use), body mass, a history of childhood sexual abuse and sexual risk. Multivariate analyses revealed that a drive for muscularity was associated with age, disordered eating, depression, sexual risk and internalised homonegativity. These findings can be used to advance the health and wellbeing of gay and bisexual men, particularly interventions designed to mitigate the effects of internalised homonegativity and policies aimed at reducing homophobia. - Abstract
Objectives: To determine the efficacy of alendronate treatment on risk of vertebral fracture in a subgroup of women from the Fracture Intervention Trial who had bone mineral density T scores between -1.6 and -2.5 at the femoral neck and to describe how soon after initiation of therapy alendronate becomes effective and whether it is consistent in women with and without existing radiographic vertebral fracture.Patients and Methods: From May 1992 to March 1997, postmenopausal women aged 55 to 80 years were randomized to receive alendronate at 5 mg/d for 2 years and 10 mg/d thereafter or placebo for up to 4.5 years (mean, 3.8 years) in a controlled, double-blind, multicenter study.Results: A total of 3737 postmenopausal women were included in the study, 1878 in the alendronate group and 1859 in the placebo group. Risk of vertebral fracture was significantly reduced by alendronate compared with placebo for clinical (relative risk [RR], 0.40; 95% confidence interval [CI], 0.19-0.76; P=.005) and radiographic (RR, 0.57; 95% CI, 0.41-0.81; P=-.002) fracture. The reductions in vertebral fracture risk were consistent in women with and without an existing radiographic vertebral fracture for clinical (RR, 0.34; 95% CI, 0.12-0.84; and RR, 0.46; 95% CI, 0.16-1.17; respectively) and radiographic (RR, 0.53; 95% CI, 0.34-0.82; and RR, 0.64; 95% CI, 0.38-1.10; respectively) fractures. In both groups, the effect of alendronate on clinical vertebral fracture was noted soon after therapy was initiated. The absolute risk of vertebral fracture was low in women without a baseline radiographic fracture.Conclusions: In women with low bone mass who do not meet the bone mineral density criterion for osteoporosis, alendronate is effective in reducing the risk of vertebral fractures. The absolute benefit of this therapy in women with a T score between -1.6 and -2.5 is greater in women with an existing vertebral fracture and/or with other risk factors. Dosing convenience is a key element in the effective management of any chronic disease, and is particularly important in the long-term management of osteoporosis. Less frequent dosing with any medication may enhance compliance, thereby maximizing the effectiveness of therapy. Animal data support the rationale that once-weekly dosing with alendronate 70 mg (7 times the daily oral treatment dose) could provide similar efficacy to daily dosing with alendronate 10 mg due to its long duration of effect in bone. In addition, dog studies suggest that the potential for esophageal irritation, observed with daily oral bisphosphonates, may be substantially reduced with once-weekly dosing. This dosing regimen would provide patients with increased convenience and would be likely to enhance patient compliance. We compared the efficacy and safety of treatment with oral once-weekly alendronate 70 mg (N=519), twice-weekly alendronate 35 mg (N=369), and daily alendronate 10 mg (N=370) in a one-year, double-blind, multicenter study of postmenopausal women (ages 42 to 95) with osteoporosis (bone mineral density [BMD] of either lumbar spine or femoral neck at least 2.5 SDs below peak premenopausal mean, or prior vertebral or hip fracture). The primary efficacy endpoint was the comparability of increases in lumbar spine BMD, using strict pre-defined equivalence criteria. Secondary endpoints included changes in BMD at the hip and total body and rate of bone turnover, as assessed by biochemical markers. Both of the new regimens fully satisfied the equivalence criteria relative to daily therapy. Mean increases in lumbar spine BMD at 12 months were: 5.1% (95% CI 4.8, 5.4) in the 70 mg once-weekly group, 5.2% (4.9, 5.6) in the 35 mg twice-weekly group, and 5.4% (5.0, 5.8) in the 10 mg daily treatment group. Increases in BMD at the total hip, femoral neck, trochanter, and total body were similar for the three dosing regimens. All three treatment groups similarly reduced biochemical markers of bone resorption (urinary N-telopeptides of type I collagen) and bone formation (serum bone-specific alkaline phosphatase) into the middle of the premenopausal reference range. All treatment regimens were well tolerated with a similar incidence of upper GI adverse experiences. There were fewer serious upper GI adverse experiences and a trend toward a lower incidence of esophageal events in the once-weekly dosing group compared to the daily dosing group. These data are consistent with preclinical animal models, and suggest that once-weekly dosing has the potential for improved upper GI tolerability. Clinical fractures, captured as adverse experiences, were similar among the groups. We conclude that the alendronate 70 mg once-weekly dosing regimen will provide patients with a more convenient, therapeutically equivalent alternative to daily dosing, and may enhance compliance and long-term persistence with therapy.
