1. Ultra-narrow linewidth laser across the C-band using polarization-controlled dual-cavity feedback

Abstract

A standard method to reduce the linewidth of semiconductor lasers involves the use of external optical feedback (EOF). However, feedback powers less than 1 % usually trigger coherence collapse (CC), leading to chaotic laser dynamics and linewidth broadening. This paper explores a method to mitigate CC through precise tuning of the feedback polarization depending on the feedback power. We report a semiconductor laser with a sub-kHz linewidth, achieved via EOF. The laser features a U-shaped cavity with two sampled grating distributed Bragg reflectors (SG-DBRs), enabling broad tunability across a 42 nm wavelength range (1513-1555 nm). By injecting optical feedback into both sides of the laser cavity via an external fiber-based cavity, we reduce the linewidth by more than three orders of magnitude, from MHz to sub-kHz across the laser's tuning range. Our approach achieves significant linewidth reduction while maintaining coherence at high feedback levels, marking an improvement over prior studies where CC limited performance. These results pave the way for ultra-narrow linewidth diode lasers with wide tunability, which would benefit fields like coherent optical communications and spectroscopy.

BackgroundDigital imaging of dermatological patients is a novel approach to remote assessment and has recently become more relevant since telehealth and remote decentralized clinical trials are gaining ground. ObjectiveWe aimed to investigate whether photographs taken by a smartphone are of adequate quality to allow severity assessments to be made and to explore the usefulness of an established atopic dermatitis severity assessment instrument on photograph evaluation. MethodsDuring scheduled visits in a previously published study, the investigating doctor evaluated the severity of atopic dermatitis using the Scoring AD (SCORAD) index and took photographs of the most representative lesions (target lesions) with both a smartphone and a digital single-lens reflex camera (DSLR). The photographs were then assessed by 5 dermatologists using the intensity items of the SCORAD (iSCORAD), which consists of erythema, oedema/papulation, excoriations, lichenification, oozing/crusts, and dryness (scale 0-3, maximum score 18). The mean iSCORAD of the photographs was calculated and compared with in-person assessments using Pearson correlation and Bland-Altman plots. Intraclass correlation coefficients were used for interrater reliability. ResultsA total of 942 photographs from 95 patients were assessed. Abstract

BackgroundRecruitment for clinical trials continues to be a challenge, as patient recruitment is the single biggest cause of trial delays. Around 80% of trials fail to meet the initial enrollment target and timeline, and these delays can result in lost revenue of as much as US $8 million per day for drug developing companies. ObjectiveThis study aimed to conduct a systematic review and meta-analysis examining the effectiveness of online recruitment of participants for clinical trials compared with traditional in-clinic/offline recruitment methods. MethodsData on recruitment rates (the average number of patients enrolled in the study per month and per day of active recruitment) and conversion rates (the percentage of participants screened who proceed to enroll into the clinical trial), as well as study characteristics and patient demographics were collected from the included studies. Differences in online and offline recruitment rates and conversion rates were examined using random effects models. Abstract

Far-UVC light in the wavelength range of 200-230 nm has attracted renewed interest because of its safety for human exposure and effectiveness in inactivating pathogens. Here we present a compact solid-state far-UVC laser source based on second-harmonic generation (SHG) using a low-cost commercially-available blue laser diode pump. Leveraging the high intensity of light in a nanophotonic waveguide and heterogeneous integration, our approach achieves Cherenkov phase-matching across a bonded interface consisting of a silicon nitride (SiN) waveguide and a beta barium borate (BBO) nonlinear crystal. Through systematic investigations of waveguide dimensions and pump power, we analyze the dependencies of Cherenkov emission angle, conversion efficiency, and output power. Experimental results confirm the feasibility of generating far-UVC, paving the way for mass production in a compact form factor. This solid-state far-UVC laser source shows significant potential for applications in human-safe disinfection, non-line-of-sight free-space communication, and deep-UV Raman spectroscopy.

29. Predicting the Long-Term Outcomes of Biologics in Psoriasis Patients Using Machine Learning

Abstract

Background. Real-world data show that approximately 50% of psoriasis patients treated with a biologic agent will discontinue the drug because of loss of efficacy. History of previous therapy with another biologic, female sex and obesity were identified as predictors of drug discontinuations, but their individual predictive value is low. Objectives. To determine whether machine learning algorithms can produce models that can accurately predict outcomes of biologic therapy in psoriasis on individual patient level. Results. All tested machine learning algorithms could accurately predict the risk of drug discontinuation and its cause (e.g. lack of efficacy vs adverse event). The learned generalized linear model achieved diagnostic accuracy of 82%, requiring under 2 seconds per patient using the psoriasis patients dataset. Input optimization analysis established a profile of a patient who has best chances of long-term treatment success: biologic-naive patient under 49 years, early-onset plaque psoriasis without psoriatic arthritis, weight < 100 kg, and moderate-to-severe psoriasis activity (DLQI $\geq$ 16; PASI $\geq$ 10). Moreover, a different generalized linear model is used to predict the length of treatment for each patient with mean absolute error (MAE) of 4.5 months. However Pearson Correlation Coefficient indicates 0.935 linear dependencies between the actual treatment lengths and predicted ones. Conclusions. Machine learning algorithms predict the risk of drug discontinuation and treatment duration with accuracy exceeding 80%, based on a small set of predictive variables. This approach can be used as a decision-making tool, communicating expected outcomes to the patient, and development of evidence-based guidelines.

35. The prevalence of atopic dermatitis in adults: systematic review on population studies

Abstract

Atopic dermatitis (AD) is a common multifactorial skin disease occurring primarily in young children. AD has increased in prevalence over the past decades, but little knowledge exists on the prevalence of AD in adults. Herein, published estimates of the point-prevalence and one-year prevalence of AD in adults are reviewed in the context of various study characteristics such as the age and gender distribution of the populations, sampling methods, study design, and geographical area of origin. In total, 14 different population studies reporting the prevalence of AD in adults in 17 countries were identified. There was a substantial between-country variation in both the point-prevalence (1.6 to 11.5%) and one-year prevalence (2.2 to 17.6%) of AD with heterogeneity explained partly by gender, age, geography, study design, and diagnostic criteria.

Abstract

This article discusses a study that aimed to identify potential biomarkers for chronic spontaneous urticaria (CSU), a skin disease characterized by severe itching. The researchers collected blood samples from 118 CSU patients and analyzed the plasma proteomics to identify associations with key disease characteristics. They found that certain plasma proteins were significantly associated with clinical traits such as sex, positive basophil histamine release assay (BHRA), angioedema, and concomitant chronic inducible urticaria (CIndU).
