Abstract

Abstract Purpose A multi-centre study to assess the value of combined surgical resection and radiotherapy for the treatment of desmoid tumours. Patients and methods One hundred and ten patients from several European countries qualified for this study. Pathology slides of all patients were reviewed by an independent pathologist. Sixty-eight patients received post-operative radiotherapy and 42 surgery only. Median follow-up was 6 years (1 to 44). The progression-free survival time (PFS) and prognostic factors were analysed. Results The combined treatment with radiotherapy showed a significantly longer progression-free survival than surgical resection alone (p smaller than 0.001). Extremities could be preserved in all patients treated with combined surgery and radiotherapy for tumours located in the limb, whereas amputation was necessary for 23% of patients treated with surgery alone. A comparison of PFS for tumour locations proved the abdominal wall to be a positive prognostic factor and a localization in the extremities to be a negative prognostic factor. Additional irradiation, a fraction size larger than or equal to 2 Gy and a total dose larger than 50 Gy to the tumour were found to be positive prognostic factors with a significantly lower risk for a recurrence in the univariate analysis. This analysis revealed radiotherapy at recurrence as a significantly worse prognostic factor compared with adjuvant radiotherapy. The addition of radiotherapy to the treatment concept was a positive prognostic factor in the multivariate analysis. Conclusion Postoperative radiotherapy significantly improved the PFS compared to surgery alone. Therefore it should always be considered after a non-radical tumour resection and should be given preferably in an adjuvant setting. It is effective in limb preservation and for preserving the function of joints in situations where surgery alone would result in deficits, which is especially important in young patients.

7. Synovial sarcoma in children and adolescents: a report from the Kiel Pediatric Tumor Registry

Abstract

Synovial membranes are found near joints, tendons, and ligaments in areas where movement might create friction between tissues. Because of their close anatomical association, it is widely believed that synovial sarcomas arise from these tissues, but conclusive proof of this is still lacking. While there have been many scientific reports on the characteristics of synovial sarcomas, few of these reports have specifically dealt with the characteristics of these cancers in children. The authors review the cases of 35 patients under 18 years of age who developed synovial sarcomas. These childhood cases represent 71 percent of the total number of synovial sarcomas; synovial sarcomas represent 5.8 percent of all soft-tissue sarcomas recorded in this particular tumor registry. While it was possible to determine the histological structure of these cancers and the identity of the individual cell types by conventional light microscopy, the authors found that the use of specific antibodies to different cellular constituents, particularly the proteins of the cytoskeleton, made the task easier. In the course of using antibodies for specific cellular components, the researchers also observed some anomalous expression of antigens. Ten of 11 specimens examined revealed the presence of neuron-specific enolase, which, as its name implies, is generally considered to be found only in neurons. Likewise, 5 of 13 specimens examined were found to contain S-100, another protein considered to be specific for neural tissue. A 7-years survival rate of 72 percent was observed among the patients in the present study. Because these patients have not been followed-up for an extended period, the actual survival rate may turn out to be less than this. 8. Steady-state behavior of large water distribution systems: Algebraic multigrid method for the fast solution of the linear step

Abstract

The Newton-based global gradient algorithm (GGA) (also known as the Todini and Pilati method) is a widely used method for computing the steady-state solution of the hydraulic variables within a water distribution system (WDS). The Newton-based computation involves solving a linear system of equations arising from the Jacobian of the WDS equations. This step is the most computationally expensive process within the GGA, particularly for large networks involving up to O(105) variables. An increasingly popular solver for large linear systems of the M-matrix class is the algebraic multigrid (AMG) method, a hierarchical-based method that uses a sequence of smaller dimensional systems to approximate the original system. This paper studies the application of AMG to the steady-state solution of WDSs through its incorporation as the linear solver within the GGA. The form of the Jacobian within the GGA is proved to be an M-matrix (under specific criteria on the pipe resistance functions), and thus able to be solved using AMG. A new interpretation of the Jacobian from the GGA is derived, enabling physically based interpretations of the AMG's automatically created hierarchy. Finally, extensive numerical studies are undertaken where it is seen that AMG outperforms the sparse Cholesky method with node reordering (the solver used in EPANET2), incomplete LU factorization (ILU), and PARDISO, which are standard iterative and direct sparse linear solvers.
