Cancer therapy-related thrombopenia, which is called "medicinal poison purpura" in traditional Chinese medicine, is a common hematologic adverse reaction during oncology treatment that is difficult to treat due to the differences in oncology treatments and the complexity of the pathogenesis, resulting in various degrees of thrombocytopenia. Based on the theory that " spleen controlling blood", this paper believes that "medicinal poison purpura" is mainly caused by direct damage to the blood and qi by medicinal poison, leading to qi and blood deficiency; it also attacks the spleen and stomach, resulting in the deficiency of spleen qi and no source of qi and blood production. Due to the spleen deficiency, there is no essence to nourish kidney and bone marrow; their function of generating blood decreases, eventually it becomes " medicinal poison purpura". The theory of "regulating balance and flat regulation" is an important academic idea of our team in the treatment of malignant hematological tumors. In this paper, we have systematically elaborated on the etiology, pathogenesis, and therapeutic principles of the treatment of cancer therapy-related thrombopenia with spleen deficiency pattern through the collation of relevant literature. We believe that the prescription formulated according to the method of invigorating spleen and benefiting qi and controlling blood for the treatment of cancer therapy-associated thrombocytopenia with spleen deficiency pattern is in line with the principle of correspondence between prescription and syndrome, and correspondence between drugs and syndrome in traditional Chinese medicine, which is theoretically feasible and has a high clinical application value. Objective To explore the etiology, clinical features, treatment and prognosis of multiple primary malignant neoplasms. Methods The medical records of 120 patients with multiple primary malignant neoplasms in Dongzhimen Hospital Affiliated to Beijing University of Chinese Medicine were collected and analyzed retrospectively. Results (1) Similar sex ratio; (2) The age of onset was 26-89 years old, with a median age of 58 years; (3) There were 116 cases of double primary malignant tumors, 2 cases of three primary malignant tumors, 1 case of four primary malignant tumors and 1 case of five primary malignant tumors; (4) There were 25 cases of simultaneous multiple primary malignant neoplasms, and 95 cases of metachronous multiple primary malignant neoplasms. The interval between onset was 6-444 months, and the median interval was 32 months; (5) The first malignant tumor was mostly in digestive system, the second primary malignant tumor was mostly in blood system; (6) The 3- and 5-year survival rates were 48.3% and 37.5%, respectively. Conclusion Through clinical analysis, we could improve the understanding of multiple primary malignant neoplasms, guide the early clinical detection, diagnosis and treatment, so as to prolong the patient's life and improve the prognosis.

In view of the differences in the knowledge systems between Chinese and Western medicine and the concept of maintenance treatment of malignant tumors, this paper reviews the concept of maintenance treatment of malignant tumors, the characteristics of maintenance treatment in traditional Chinese medicine (TCM), and the existing problems and coping strategies of TCM maintenance treatment. Firstly, the definition, ways of thinking and treatment methods of TCM maintenance treatment are clarified. It is emphasized that the maintenance treatment in TCM should be guided by the core theory of "Tao conforms to the nature", and the concepts of people-orientedness, commitment to the golden mean, adherence to internal energy, restoration and protection of the original qi, regulation of the emotions and correspondence between human beings and nature throughout the whole process of maintenance treatment. In addition, patients with malignant tumors should be placed in the best state of mind and body by adhering to the highest principle of "nourishing the heart". Such concepts and theories should be applied with flexibility as pattern differentiation and treatment, holistic concept, balanced state, consolidating the basis and clearing the source, targeted meridian tropism and preventive treatment. It is proposed that the maintenance treatment of malignant tumors in TCM is a multi-dimensional system that integrates maintenance treatment of diseases, of tumor-related symptoms, and of tumor-related complications. The goal of the treatment should be control of patterns (symptoms) and improvement in the quality of life of patients as well as longer progression-free survival (FPS) and overall survival (OS), so that more patients with malignant tumors can benefit from maintenance treatment. Objective To investigate the clinical efficacy of Zaizao Shengxue ( Blood-engendering Renewal) Capsule (ZZSXC) in the treatment of myelodysplastic syndrome. Methods A total of 139 patients who met the diagnostic criteria of Chinese and Western medicine for myelodysplastic syndrome were enrolled according to the principles of randomized multicenter controlled trials. The 103 patients in the treatment group were given 5 ZZSXCs each time, thrice daily. The 36 patients in the control group were given 8 Fufang Zaofan (Compound Zaofan) Pills (FFZFP) each time, thrice daily. The two groups were treated for 3 months. Then their hematological improvements in HI-E, HI-P and HI-N and peripheral blood parameters (HGB, RBC, MCV, PLT, WBC and NEUT) as well as subgroup analysis, TCM pattern and physical sign scores, bleeding, infection, clinical efficacy and safety were evaluated. Results In both the treatment group and the control group, the overall percentages of improvement in such hematological parameters as HI-EE, HI-P and HI-N were 51. 6% and 38. 5%, 44. 8% and 40. 0%, and 51. 0% and 68. 2% respectively without statistically significant difference between the two groups (P >0.05). The diferences in RBC and HGB of all the patients and the subgroup of low-risk patients in the treatment group before and after treatment were statistically significant (P <0. 05) . The diferences in WBC of the subgroup of high-risk patients in the treatment group before and after treatment were statistically significant (P <0. 05) . The diferences in NEUT of all the patients and the subgroup of low-risk patients in the control group before and after treatment were statistically significant (P <0. 05). MCV was decreased of the subgroup of low-risk patients in the treatment group after treatment compared with that before treatment with statistical significance ( P < 0. 05 ) . The percentages of patients that enjoyed marked curative effects and curative effects in terms of TCM patterns in the treatment group and the control group were 29. 9% and 30. 3%, and 62. 1% and 57. 6%, respectively, with no statistically significant diference between the two groups ( P > 0. 05 ) . There was no statistically significant diference in curative effects for individual symptoms and signs between the two groups (P >0. 05) . There was no statistically significant dUference in hemostasis, incidence of infection and safety parameters between the two groups (P > 0. 05 ) . Conclusion Both ZZSXC and FFZFP can improve morbid hematopoiesis and hematological parameters in patients with MDS, and remarkably improve their TCM patterns and individual symptoms and signs with good clinical drug safety. Objectives: The main objective of this study was to investigate the variations of β-endorphin (β-EP), vasoactive intestinal peptide (VIP), serotonin (5-HT) and norepinephrine (NE) of immune thrombocytopenia (ITP) mice as well as the regulatory mechanism of prednison., Materials and Methods: Sixty BALB/c mice were randomly divided into control group, model group and prednison intervention group. ITP mice model was duplicated by injecting with glycoprotein-antiplatelet serum (GP-APS) except in control group. After ITP disease model was successful established, prednison was used in prednison intervention group. The β-EP, VIP, 5-HT and NE contents of ITP mice were detected by enzyme linked immunosorbent assay (ELISA)., Results: Compared with the values in control group, the detection values of VIP and 5-HT in model group declined, while the detection values of β-EP and NE increased. Compared with prednison intervention group, the detection values of VIP and 5-HT in model group increased, while the detection values of β-EP and NE showed no significant change., Conclusion: In this study, the β-EP, VIP, 5-HT and NE contents in ITP mice injected with GP-APS were changed by prednison. It shows that prednison as the first-line therapy for ITP with effective hemostasis function is likely to increasing the contents of VIP and 5-HT. These results suggest the therapeutic value of prednison for the treatment of ITP.
