© 2022 The AuthorsObjective and aims: Acute pancreatitis in inflammatory bowel disease occurs mainly as an extraintestinal manifestation or a side effect of medications. We aimed to investigate the prognostic factors and severity indicators of acute pancreatitis and the treatment of patients with both diseases. Design: We performed a matched case-control registry analysis of a multicentre, prospective, international acute pancreatitis registry. Patients with both diseases were matched to patients with acute pancreatitis only in a 1:3 ratio by age and gender. Subgroup analyses were also carried out based on disease type, activity, and treatment of inflammatory bowel disease. Results: No difference in prognostic factors (laboratory parameters, bedside index of severity in acute pancreatitis, imaging results) and outcomes of acute pancreatitis (length of hospitalization, severity, and local or systemic complications) were detected between groups. Significantly lower analgesic use was observed in the inflammatory bowel disease population. Antibiotic use during acute pancreatitis was significantly more common in the immunosuppressed group than in the non-immunosuppressed group (p = 0.017). However, none of the prognostic parameters or the severity indicators showed a significant difference between any subgroup of patients with inflammatory bowel disease. Conclusion: No significant differences in the prognosis and severity of acute pancreatitis could be detected between patients with both diseases and with pancreatitis only. The need for different acute pancreatitis management is not justified in the coexistence of inflammatory bowel disease, and antibiotic overuse should be avoided.

Az endoszkópos retrográd cholangiopancreatographia (ERCP) a több mint 50 éves története során diagnosztikus módszerből szinte kizárólag terápiás beavatkozássá vált. Köszönhető ez a kevésbé invazív vizsgálati módszerek kialakulásának, illetve annak a felismerésnek is, hogy számos szövődmény kialakulásának veszélyét hordozza magában. Ezek ismerete manapság alapvető fontosságú. Az eltelt évtizedek során a szövődmények hajlamosító tényezőiről, illetve a megelőzés lehetőségeiről jelentős ismeretanyag gyűlt össze, melyhez magyar szakemberek komoly eredményekkel járultak hozzá. Jelen közleményünkben az ERCP szövődményeit, azok definícióját, súlyossági osztályozásukat, kockázati tényezőiket, a profilaktikus lehetőségeket és az endoszkópos ellátás módjait foglaljuk össze. The European Society of Gastrointestinal Endoscopy and United European Gastroenterology have defined performance measures for upper and lower gastrointestinal, pancreaticobiliary, and small-bowel endoscopy. Quality indicators to guide endoscopists in the growing field of advanced endoscopy are also underway. We propose that equal attention is given to developing the entire advanced endoscopy team and not the individual endoscopist alone.We suggest that the practice of teams intending to deliver high quality advanced endoscopy is underpinned by six crucial principles concerning: selection, acceptance, complications, reconnaissance, envelopment, and documentation (SACRED).

Összefoglaló. Bevezetés: A musculus cricopharyngeus flexibilis endoszkópos myotomiája a tüneteket okozó Zenker-diverticulum hatékony, biztonságos kezelési módszere. Célkitűzés és módszer: Retrospektíven elemeztük a hagyományos flexibilis endoszkópos myotomia eljárással szerzett tapasztalatainkat. Eredmények és megbeszélés: 2012. szeptember és 2020. február között 38 betegnél történt flexibilis endoszkópos myotomia tüneteket okozó Zenker-diverticulum miatt; összesen 47 myotomiát végeztünk. Betegeink többsége férfi (23/38), az átlagéletkor 71,5 év (40–88) volt. A diverticulumok átlagos mérete 3,94 (2–10) cm volt. A legtöbb esetben divertikuloszkópot alkalmaztunk, szabad kezes vágási technikára 8 esetben volt szükség. Betegeink tüneteit a kezelés előtt és azt követően a DRC- (dysphagia, regurgitatio, komplikációk) skála szerint értékeltük. A jelentősebb szövődmények előfordulása 4,2% (2/47) volt, a beavatkozással összefüggésbe hozható mortalitás nem fordult elő. Pneumomediastinum 1 betegnél alakult ki, melyet konzervatívan sikeresen kezeltünk. Beavatkozás közbeni vérzés 8 esetben (17%) jelentkezett, ezeket a beavatkozás során sikeresen megállítottuk. 1 esetben korai, kiújuló masszív vérzés miatt sürgős műtétre volt szükség. Mind a 38 beteget követtük, átlagosan 34,7 hónapig. A klinikai sikeresség 1,5 hónapnál 91,9% volt (34/37). 3 betegnek maradtak tünetei, kettőjüknél remyotomia történt, egyikük később műtétre szorult, 1 beteg pedig percutan endoszkópos gastrostomián esett át 18 hónap után. Introduction: In the treatment of symptomatic Zenker’s diverticulum, the flexible endoscopic myotomy of the cricopharyngeal muscle is considered to be a safe and effective technique. Objective and method: We retrospectively analyzed our experiences with conventional flexible endoscopic myotomy. Results and discussion: 38 patients with symptomatic Zenker’s diverticulum were treated with flexible endoscopic myotomy and 47 myotomies were performed from September 2012 until February 2020. Most of our patients were male (23/38), with an average age of 71.5 (40–88) years. The mean size of diverticula was 3.94 (2–10) cm. In most cases, we used diverticuloscope, while free-hand technique was needed in 8 cases. We assessed our patients’ symptoms by applying DRC (dysphagia, regurgitation, complication) score before the treatment and during follow-up. The overall rate of significant complications was 4.2% (2/47), and there was no procedure-related mortality. We observed pneumomediastinum in one patient that was treated conservatively. Intraprocedural bleeding occurred in several (8/47) cases, in all of them the bleeding was successfully stopped during intervention. In one of them, early recurrent massive bleeding required urgent surgery. All 38 patients were followed (mean 34.7 months). Clinical success at 1.5 months was 91.9% among endoscopically treated patients (34/37). 3 patients remained symptomatic, 2 of them were treated with re-myotomy, 1 of them needed surgery later on, another patient underwent percutan endoscopic gastrostomy at 18 months. Item does not contain fulltext BACKGROUND: Etrolizumab is a gut-targeted anti-β7 integrin monoclonal antibody. In a previous phase 2 induction study, etrolizumab significantly improved clinical remission versus placebo in patients with moderately to severely active ulcerative colitis. We aimed to compare the safety and efficacy of etrolizumab with infliximab in patients with moderately to severely active ulcerative colitis. METHODS: We conducted a randomised, double-blind, double-dummy, parallel-group, phase 3 study (GARDENIA) across 114 treatment centres worldwide. We included adults (age 18-80 years) with moderately to severely active ulcerative colitis (Mayo Clinic total score [MCS] of 6-12 with an endoscopic subscore of ≥2, a rectal bleeding subscore of ≥1, and a stool frequency subscore of ≥1) who were naive to tumour necrosis factor inhibitors. Patients were required to have had an established diagnosis of ulcerative colitis for at least 3 months, corroborated by both clinical and endoscopic evidence, and evidence of disease extending at least 20 cm from the anal verge. Participants were randomly assigned (1:1) to receive subcutaneous etrolizumab 105 mg once every 4 weeks or intravenous infliximab 5 mg/kg at 0, 2, and 6 weeks and every 8 weeks thereafter for 52 weeks. Randomisation was stratified by baseline concomitant treatment with corticosteroids, concomitant treatment with immunosuppressants, and baseline disease activity. All participants and study site personnel were masked to treatment assignment. The primary endpoint was the proportion of patients who had both clinical response at week 10 (MCS ≥3-point decrease and ≥30% reduction from baseline, plus ≥1-point decrease in rectal bleeding subscore or absolute rectal bleeding score of 0 or 1) and clinical remission at week 54 (MCS ≤2, with individual subscores ≤1); efficacy was analysed using a modified intention-to-treat population (all randomised patients who received at least one dose of study drug). GARDENIA was designed to show superiority of etrolizumab over infliximab for the primary endpoint. This trial is registered with ClinicalTrials.gov, NCT02136069, and is now closed to recruitment. FINDINGS: Between Dec 24, 2014, and June 23, 2020, 730 patients were screened for eligibility and 397 were enrolled and randomly assigned to etrolizumab (n=199) or infliximab (n=198). 95 (48%) patients in the etrolizumab group and 103 (52%) in the infliximab group completed the study through week 54. At week 54, 37 (18·6%) of 199 patients in the etrolizumab group and 39 (19·7%) of 198 in the infliximab group met the primary endpoint (adjusted treatment difference -0·9% [95% CI -8·7 to 6·8]; p=0·81). The number of patients reporting one or more adverse events was similar between treatment groups (154 [77%] of 199 in the etrolizumab group and 151 [76%] of 198 in the infliximab group); the most common adverse event in both groups was ulcerative colitis (55 [28%] patients in the etrolizumab group and 43 [22%] in the infliximab group). More patients in the etrolizumab group reported serious adverse events (including serious infections) than did those in the infliximab group (32 [16%] vs 20 [10%]); the most common serious adverse event was ulcerative colitis (12 [6%] and 11 [6%]). There was one death during follow-up, in the infliximab group due to a pulmonary embolism, which was not considered to be related to study treatment. INTERPRETATION: To our knowledge, this trial is the first phase 3 maintenance study in moderately to severely active ulcerative colitis to use infliximab as an active comparator. Although the study did not show statistical superiority for the primary endpoint, etrolizumab performed similarly to infliximab from a clinical viewpoint. FUNDING: F Hoffmann-La Roche.

Bevezetés: Az endoszkópos retrográd cholangiopancreatographia (ERCP) magas minőségű elvégzéséhez nélkülözhetetlen a megfelelő elméleti és gyakorlati oktatás. Hazánkban a Magyar Gasztroenterológiai Társaság Endoszkópos Szekciója célul tűzte ki ennek megszervezését, ehhez azonban a hazai viszonyok felmérése elengedhetetlen. Módszerek: A Nemzeti Egészségbiztosítási Alapkezelő által rendelkezésünkre bocsátott adatokat elemeztük. Felmértük az ERCP-centrumok számát, földrajzi elhelyezkedését és az elvégzett ERCP-vizsgálatok számát, kiszámoltuk az egy orvosra jutó vizsgálatok számát, illetve a centrumokban a 30 napon belül megismételt, illetve más centrumba irányított vizsgálati számot is. Eredmények: Negyvennégy intézményben történik ERCP (az összes endoszkópos centrum 36%-a). Centrumonként átlagosan évi 231 ERCP történik. Csak 16 intézmény (36%) végez átlagot meghaladó számú vizsgálatot, ez az éves vizsgálatok 76%-a. Harminc intézmény végez heti legalább 2 ERCP-t, ők végezték az utolsó 3 év ERCP-inek 90%-át, ahol összesen 72 orvos végez vizsgálatot (a legjellemzőbb a szolgáltatónkénti 1-3 orvos). Tizenhárom vizsgálóhelyen (31%) csak egy orvos végez ERCP-t. Tizenhárom helyen az orvosonkénti ERCP-szám nem éri el az évi 52-t, ezekből 5 helyen (38%) csak 1 orvos vizsgál. A helyben ismételt ERCP aránya 11 helyen, míg a 30 napon belül továbbküldött sikertelen ERCP-k aránya 4 intézményben haladta meg a 10%-ot. Ezen intézményekben az egy főre jutó vizsgálatszám nem érte el a kívánatos évi 52 ERCP-számot. Ugyanakkor 8 intézményben történik nagyobb számban más helyen sikertelen ERCP utáni, második vizsgálat. Megbeszélés: Közleményünk az első felmérés hazánkban, amely az ERCP működésére és minőségére vonatkozik. Felmérésünk alapján feltárhatók azon hiányosságok, ahol a kívánatos és nemzetközileg elfogadott minőségmutatók eléréséhez fejlesztésre van szükség. Következtetések: Központosításra és az ERCP-licenc bevezetésére van szükség.

Endoszkópos retrográd cholangiopancreatographia (ERCP) során gyakran kerül sor a Vater-papilla endo­szkópos átmetszésére, a sphincterotomiára, rendszerint epeúti kő eltávolítása vagy sztent behelyezése előtt. A papillotomia a záróizom szabályozó és barrier funkciójának végleges elvesztésével jár. A jól ismert rövid távú szövődmények mellett a hosszú távon tapasztalható hátrányos hatások is figyelmet érdemelnek. A visszatérő epeúti kövek nagyarányú előfordulása mellett ilyen az epehólyag-gyulladás gyakori újrafellépése, ha az epehólyagot röviddel a papillotomia után nem távolítják el. Ezen hosszú távú szövődmények egyetlen rizikófaktora a pneumobilia megléte. Ez azonban csak a betegek kisebb részét érinti, mivel jelenleg ezekben az esetekben az epeúti kőeltávolítás után a köves epehólyag korai sebészi eltávolítása a kezelési stratégia. Azokban az esetekben, ahol az endoszkópos papillotomia elkerülhető (pl. 8 mm-nél kisebb epeúti kövek eltávolítása esetén), ott érdemes a sphincter funkciójának megőrzése, hogy a kései negatív következmények (visszatérő cholecystitis bent maradt epehólyag esetén, epeúti kőrecidíva, összes adverz esemény) esélyét csökkentsük. A teljesen fedetlen vagy részben fedett öntáguló epeúti fémsztentek, vagy egyszeres műanyag sztent behelyezése előtt a rutinszerű papillotomia kerülendő, kivéve a posztoperatív epecsorgás esetét, ahol a papillotomiát követő műanyag sztent behelyezése kevesebb poszt-ERCP pancreatitisszel jár, mint a papillotomia nélküli.

Összefoglaló. A tápcsatorna endoszkópiája a gasztrointesztinális rendszer betegségeinek legfontosabb vizsgáló eljárása, a diagnosztikán túl egyre szélesedő körben terápiás beavatkozási lehetőséget is magában foglal. A jó minőségben végzett endoszkópia javítja az esetlegesen fennálló betegség kimenetelét és növeli a betegelégedettséget. A nemzetközi emésztőszervi endoszkópos társaságok elsőrendű fontosságúnak határozták meg az endoszkópia minőségének javítását célzó törekvéseket. Az endoszkópos tevékenységek minőségének ellenőrzésében és javításában a meghatározott indikátorok folyamatos követése fontos feladat, ezek révén biztosítható betegeink számára a legmagasabb szintű ellátás. A bizonyítékon alapuló minőségi indikátorok lehetővé teszik az egyes endoszkóposok és az endoszkópos vizsgálóhelyek összehasonlítását és az általuk nyújtott szolgáltatás értékelését. A fenntartónak betegbiztonsági és költséghatékonysági szempontból is fontos ismerni, hogy melyik szolgáltató tudja teljesíteni a minimálisan elvárt és fejlődési célként kitűzött teljesítménymutatókat, melyik ellátónál milyen tényezőket szükséges megváltoztatni, javítani. A szerzők az európai útmutatókat alapul véve a felső tápcsatorna endoszkópiája, az alsó tápcsatorna endoszkópia, a pancreatobiliaris endoszkópia, a vékonybél endoszkópia, a kapszula endoszkópia és az endoszkópos szolgáltatás területén határozták meg a klinikailag releváns teljesítménymutatók auditálható magyar rendszerét. Az egységesség, a jobb digitalizálhatóság és könnyebb auditálhatóság céljából a felső, az alsó, a pancreatobiliaris, a vékonybél és a kapszula endoszkópia vonatkozásában a magyar ajánlásban a minőségi alterületeket egységesen (1–8) számozták, minden szám ugyanazon szempontrendszert jelenti. Az endoszkópos szolgáltatás komplex minőségi mutatóinál szigorúan ragaszkodtak az európai irányelvhez, itt 9 különböző alterületbe foglalták össze a 30 minőségi mutatót. A szűrő kolonoszkópia kiemelt jelentősége miatt a jelen minőségi endoszkópos útmutatóba foglalták bele eltérő szerkezettel a szűrő kolonoszkópos program során alkalmazott minőségi mutatókat, illetve a bélelőkészítést érintő néhány alapvetést is. Summary. Nowadays, endoscopy is the cornerstone in the diagnosis and therapy of gastrointestinal diseases. Good quality endoscopy can improve outcome of the disease and patients experience. International endoscopy societies prioritized efforts improving quality of endoscopy. The highest level of patient care can be provided through continuous assessment and improvement of relevant quality indicators. The comparison of these evidence based performance measures between endoscopists and endoscopy providers allow the objective evaluation of the service. Furthermore, from the point of view of patient safety and cost effectiveness the health care provider should know the minimum standards and target goals, as well, to make grounded decisions about fields of necessary changes and improvements. The authors based on European guidelines worked out this comprehensive auditable Hungarian system of performance measures in the fields of upper endoscopy, lower endoscopy, pancreatobiliary endoscopy, capsule endoscopy, enteroscopy and general endoscopy service. Due to commonality all domains were counted similarly (1–8) in different endoscopic procedures. The general endoscopy service is an exception with 9 domains and 30 quality parameters. The outstanding importance of colorectal cancer screening required involving this topic into this guideline with separate structure, as well as the basics of bowel preparation.

Introduction: COVID–19 significantly affects endoscopic labs’ workflow. Endoscopic examinations are considered high-risk for virus transmission. Objectives: To determine impact of COVID–19 pandemic on Hungarian endoscopic labs’ workflow and on infection risk of endoscopic staff. Method: A nation-wide, cross-sectional online questionnaire was sent to heads of endoscopic labs in Hungary. The average number (with 95% confidence intervals) of upper and lower gastrointestinal endoscopies performed in 2020 was compared to that in 2019. The number of SARS-CoV-2-infected endoscopic staff members and the source of infection was also investigated. Results: Completion rate was 30% (33/111). Neither the number of upper (1.593 [743–1.514] vs. 1.129 [1.020–2.166], p = 0.053), nor that of lower gastrointestinal endoscopies (1.181 [823–1.538] vs. 871 [591–1.150], p = 0.072) decreased in 2020, but both upper and lower gastrointestinal endoscopies’ number decreased by 80% during peak phases. Separate examination room was available in 12% of institutes. Appropriate quality personal protective equipment (PPE) was available during the first and second peak phase in 70% and 82%, respectively. Infection risk stratification by questionnaire and PCR testing was routinely performed in 85% and 42%, respectively. Employee number decreased by 33% and 26% for physicians, and by 19% and 21% for assistants during peak phases, mainly due to age restrictions and COVID care assignments. 32% of assistants and 41% of physicians were infected (associated with inappropriate PPE use in 16% and 18%, respectively). Conclusion: Peak phases’ restrictions increase endoscopic workload afterwards. Despite PPE availability, 15% of employees’ COVID infection resulted from inappropriate PPE use in pre-vaccination era.Bevezetés: A COVID–19-pandémia jelentősen befolyásolta az endoszkópos laboratóriumok működését. Az endoszkópos vizsgálatok nagy vírusátviteli rizikójúnak számítanak. Célkitűzés: A koronavírus-járványnak a magyarországi endoszkópos laborok működésére kifejtett hatásának és az endoszkópos személyzet SARS-CoV-2-fertőzésben való érintettségének felmérése a 2020-as évben. Módszerek: Országos szintű, keresztmetszeti, kérdőíves tanulmányunk során magyarországi endoszkópos laboratóriumok vezetőit kerestük meg online formában. Vizsgáltuk a 2020-ban elvégzett felső és alsó tápcsatornai endoszkópiák számát a 2019-es referenciaévhez viszonyítva (átlag [95%-os konfidenciaintervallum]), továbbá a SARS-CoV-2-fertőzéssel érintett dolgozók számát és fertőződésük potenciális forrását. Eredmények: A megkeresett intézmények 30%-ából érkeztek válaszok (33/111). A referenciaévhez képest sem a felső (1593 [743–1514] vs. 1129 [1020–2166], p = 0,053), sem az alsó tápcsatornai endoszkópos vizsgálatok száma (1181 [823–1538] vs. 871 [591–1150], p = 0,072) nem csökkent, bár a vírushullámoknak megfelelően mindkét vizsgálattípus esetén 80%-ot meghaladó vizsgálatszám-csökkenést tapasztaltunk. Dedikáltan fertőző betegek vizsgálatára kialakított helyiség az intézetek 12%-ában állt rendelkezésre. A védőfelszerelések az első és a második hullám alatt 70%-ban, illetve 82%-ban voltak mennyiségileg és minőségileg megfelelőek. A vizsgálat előtt fertőzöttségirizikó-stratifikáció az intézetek 85%-ában, míg PCR-vizsgálat csupán 42%-ukban történt minden esetben. Az első és második járványhullám alatt a dolgozói létszám az orvosoknál 33%-kal, illetve 26%-kal, az asszisztenseknél 19%-kal, illetve 21%-kal csökkent, elsősorban életkori korlátozás, COVID-ellátásba való áthelyezés miatt. Az asszisztensek 32%-a, az orvosok 41%-a esett át COVID-fertőzésen, 16%-ban, illetve 18%-ban nem megfelelő védőfelszerelés-használattal összefüggésben. Következtetés: A járványhullámok alatti restrikciók feloldását követően a vizsgálati terhelés emelkedik. A SARS-CoV-2-vakcinák elérhetősége előtt az endoszkópos laborok dolgozóit érintő COVID-fertőzések 15%-a nem megfelelő védőfelszerelés-használattal volt összefüggésbe hozható. A gasztrointesztinális betegségek adekvát diagnózisához és kezeléséhez nélkülözhetetlen a közös nyelv és a standardizált leletek használata. A Magyar Gasztroenterológiai Társaság Endoszkópos Szekciójának vezetősége összegyűjtötte azokat a kulcsszavakat és klasszifikációkat, amelyek használata szükséges a mindennapi endoszkópos leletkészítésben.

Objective The efficacy of argon plasma coagulation (APC) on gastric antral vascular ectasia (GAVE) may be impaired over time and depends greatly on the application settings. Endoscopic band ligation (EBL) may be an alternative, but study on its efficacy is limited. This study aimed to evaluate and compare the clinical efficacy of APC and EBL in treating GAVE. Methods Changes in the need for blood transfusion, number of treatment sessions and hospitalizations were retrospectively assessed in 63 transfusion-dependent patients with GAVE (mean age: 67.1 y, 54.0% female) treated with either APC or EBL (45 and 18 patients, respectively) in four tertiary endoscopic centers. Results Both methods substantially increased hemoglobin levels and decreased patients' need for a transfusion (22.0 ± 4.0 g/L and -5.62 ± 2.30 units of packed red blood cells [RBC] with APC, and 27.4 ± 6.1 g/L and -4.79 ± 2.46 units of packed RBC with EBL), without a significant statistical difference between the methods. However, fewer EBL sessions were required both for the cessation of need for a transfusion compared with those for the resolution of GAVE lesions (0.90 ± 0.10 vs 1.69 ± 0.31, P = 0.028). Conclusions Both APC and EBL are effective in GAVE treatment. EBL may be superior in terms of number of treatment sessions, but not in its influence on hemoglobin level and need for transfusion. Further prospective studies with large, homogeneous sample size and standardized APC settings are needed.

Absztrakt: A gastrointestinalis vérzés gyakori előfordulása, súlyossága miatt kiemelkedő népegészségügyi jelentőséggel bír. Az antikoagulált és/vagy rendszeres thrombocytaaggregáció-gátló készítményeket, nemszteroid gyulladásgátlókat szedő, idősödő populáció arányának növekedésével az emésztőszervi vérzés a jövőben várhatóan számban és súlyosságban is egyre növekvő terhet ró az egészségügyi ellátórendszerre. A gastrointestinalis vérzés ellátásában a sürgősségi szakember mellett a gasztroenterológus-endoszkóposnak van alapvető szerepe. A társszakmák (radiológus, invazív radiológus, intenzív terapeuta, sebész) képviselői sok esetben nem mellőzhetők a döntéshozatalban, a betegek ellátásában. Az érintett szakterületek képviselői szükségét érezték egy átfogó, multidiszciplináris, közösen elfogadott szemléletet tükröző, gyakorlati szemléletű hazai iránymutatás összeállításának az érvényes nemzetközi irányelvek alapján. A szerzők részletesen tárgyalják a gastrointestinalis vérzés ellátásának feltételeit, a kezdeti betegészlelést, a rizikóbecslést, a laborvizsgálatokat, a betegek resuscitatióját, valamint a vérzések diagnosztikájának és terápiájának lépéseit a vérzésforrás lokalizációja szerint külön csoportosítva. A szerzők iránymutatást adnak az elsődleges ellátás sikertelensége, illetve az ismételten fellépő vérzés eseteiben, végül a sebészet mai helyét is definiálják a gastrointestinalis vérzésekben. Summary Background Etrolizumab is a gut-targeted, anti-β7 integrin, monoclonal antibody. In an earlier phase 2 induction study, etrolizumab significantly improved clinical remission compared with placebo in patients with moderately to severely active ulcerative colitis. We aimed to evaluate the efficacy and safety of etrolizumab in patients with moderately to severely active ulcerative colitis who had been previously treated with anti-tumour necrosis factor (TNF) agents. Methods HICKORY was a multicentre, phase 3, double-blind, placebo-controlled study in adult (18–80 years) patients with moderately to severely active ulcerative colitis (Mayo Clinic total score [MCS] of 6–12 with an endoscopic subscore of ≥2, a rectal bleeding subscore of ≥1, and a stool frequency subscore of ≥1) previously treated with TNF inhibitors. Patients were recruited from 184 treatment centres across 24 countries in North America, South America, Europe, Asia, Oceania, and the Middle East. Patients needed to have an established diagnosis of ulcerative colitis for at least 3 months, corroborated by both clinical and endoscopic evidence, and evidence of disease extending at least 20 cm from the anal verge. In cohort 1, patients received open-label etrolizumab 105 mg every 4 weeks for a 14-week induction period. In cohort 2, patients were randomly assigned (4:1) to receive subcutaneous etrolizumab 105 mg or placebo every 4 weeks for the 14-week induction phase. Patients in either cohort achieving clinical response to etrolizumab induction were eligible for the maintenance phase, in which they were randomly assigned (1:1) to receive subcutaneous etrolizumab 105 mg or placebo every 4 weeks through to week 66. Randomisation was stratified by baseline concomitant treatment with corticosteroids, concomitant treatment with immunosuppressants (induction randomisation only), baseline disease activity, week 14 MCS remission status (maintenance randomisation only), and induction cohort (maintenance randomisation only). All patients and study site personnel were masked to treatment assignment. Primary endpoints were remission (Mayo Clinic total score [MCS] ≤2, with individual subscores of ≤1 and a rectal bleeding subscore of 0) at week 14, and remission at week 66 among patients with a clinical response (MCS with ≥3-point decrease and ≥30% reduction from baseline, plus ≥1 point decrease in rectal bleeding subscore or absolute rectal bleeding score of 0 or 1) at week 14. Efficacy was analysed using a modified intent-to-treat population. Safety analyses included all patients who received at least one dose of study drug during the induction phase. This study is registered at ClinicalTrials.gov , NCT02100696 . Findings HICKORY was conducted from May 21, 2014, to April 16, 2020, during which time 1081 patients were screened, and 609 deemed eligible for inclusion. 130 patients were included in cohort 1. In cohort 2,479 patients were randomly assigned to the induction phase (etrolizumab n=384, placebo n=95). 232 patients were randomly assigned to the maintenance phase (etrolizumab to etrolizumab n=117, etrolizumab to placebo n=115). At week 14, 71 (18·5%) of 384 patients in the etrolizumab group and six (6·3%) of 95 patients in the placebo group achieved the primary induction endpoint of remission (p=0·0033). No significant difference between etrolizumab and placebo was observed for the primary maintenance endpoint of remission at week 66 among patients with a clinical response at week 14 (27 [24·1%] of 112 vs 23 [20·2%] of 114; p=0·50). Four patients in the etrolizumab group reported treatment-related adverse events leading to treatment discontinuation. The proportion of patients reporting at least adverse event was similar between treatment groups for induction (etrolizumab 253 [66%] of 384; placebo 63 [66%] of 95) and maintenance (etrolizumab to etrolizumab 98 [88%] of 112; etrolizumab to placebo 97 [85%] of 114). The most common adverse event in both groups was ulcerative colitis flare. Most adverse events were mild or moderate. During induction, the most common serious adverse event was ulcerative colitis flare (etrolizumab ten [3%] of 384; placebo: two [2%] of 95). During maintenance, the most common serious adverse event in the etrolizumab to etrolizumab group was appendicitis (two [2%] of 112) and the most common serious adverse events in the etrolizumab to placebo group were ulcerative colitis flare (two [2%] of 114) and anaemia (two [2%] of 114). Interpretation HICKORY demonstrated that a significantly higher proportion of patients with moderately to severely active ulcerative colitis who had been previously treated with anti-TNF agent were able to achieve remission at week 14 when treated with etrolizumab compared with placebo; however, there was no significant difference between groups in remission at week 66 among patients with a clinical response at week 14. Bevezetés: A COVID–19-pandémia jelentősen befolyásolta az endoszkópos laboratóriumok működését. Az endoszkópos vizsgálatok nagy vírusátviteli rizikójúnak számítanak. Célkitűzés: A koronavírus-járványnak a magyarországi endoszkópos laborok működésére kifejtett hatásának és az endoszkópos személyzet SARS-CoV-2-fertőzésben való érintettségének felmérése a 2020-as évben. Módszerek: Országos szintű, keresztmetszeti, kérdőíves tanulmányunk során magyarországi endoszkópos laboratóriumok vezetőit kerestük meg online formában. Vizsgáltuk a 2020-ban elvégzett felső és alsó tápcsatornai endoszkópiák számát a 2019-es referenciaévhez viszonyítva (átlag [95%-os konfidenciaintervallum]), továbbá a SARS-CoV-2-fertőzéssel érintett dolgozók számát és fertőződésük potenciális forrását. Eredmények: A megkeresett intézmények 30%-ából érkeztek válaszok (33/111). A referenciaévhez képest sem a felső (1593 [743–1514] vs. 1129 [1020–2166], p = 0,053), sem az alsó tápcsatornai endoszkópos vizsgálatok száma (1181 [823–1538] vs. 871 [591–1150], p = 0,072) nem csökkent, bár a vírushullámoknak megfelelően mindkét vizsgálattípus esetén 80%-ot meghaladó vizsgálatszám-csökkenést tapasztaltunk. Dedikáltan fertőző betegek vizsgálatára kialakított helyiség az intézetek 12%-ában állt rendelkezésre. A védőfelszerelések az első és a második hullám alatt 70%-ban, illetve 82%-ban voltak mennyiségileg és minőségileg megfelelőek. A vizsgálat előtt fertőzöttségirizikó-stratifikáció az intézetek 85%-ában, míg PCR-vizsgálat csupán 42%-ukban történt minden esetben. Az első és második járványhullám alatt a dolgozói létszám az orvosoknál 33%-kal, illetve 26%-kal, az asszisztenseknél 19%-kal, illetve 21%-kal csökkent, elsősorban életkori korlátozás, COVID-ellátásba való áthelyezés miatt. Az asszisztensek 32%-a, az orvosok 41%-a esett át COVID-fertőzésen, 16%-ban, illetve 18%-ban nem megfelelő védőfelszerelés-használattal összefüggésben. Következtetés: A járványhullámok alatti restrikciók feloldását követően a vizsgálati terhelés emelkedik. A SARS-CoV-2-vakcinák elérhetősége előtt az endoszkópos laborok dolgozóit érintő COVID-fertőzések 15%-a nem megfelelő védőfelszerelés-használattal volt összefüggésbe hozható. Összefoglaló. Bevezetés: A musculus cricopharyngeus flexibilis endoszkópos myotomiája a tüneteket okozó Zenker-diverticulum hatékony, biztonságos kezelési módszere. Célkitűzés és módszer: Retrospektíven elemeztük a hagyományos flexibilis endoszkópos myotomia eljárással szerzett tapasztalatainkat. Eredmények és megbeszélés: 2012. szeptember és 2020. február között 38 betegnél történt flexibilis endoszkópos myotomia tüneteket okozó Zenker-diverticulum miatt; összesen 47 myotomiát végeztünk. Betegeink többsége férfi (23/38), az átlagéletkor 71,5 év (40-88) volt. A diverticulumok átlagos mérete 3,94 (2-10) cm volt. A legtöbb esetben divertikuloszkópot alkalmaztunk, szabad kezes vágási technikára 8 esetben volt szükség. Betegeink tüneteit a kezelés előtt és azt követően a DRC- (dysphagia, regurgitatio, komplikációk) skála szerint értékeltük. A jelentősebb szövődmények előfordulása 4,2% (2/47) volt, a beavatkozással összefüggésbe hozható mortalitás nem fordult elő. Pneumomediastinum 1 betegnél alakult ki, melyet konzervatívan sikeresen kezeltünk. Beavatkozás közbeni vérzés 8 esetben (17%) jelentkezett, ezeket a beavatkozás során sikeresen megállítottuk. 1 esetben korai, kiújuló masszív vérzés miatt sürgős műtétre volt szükség. Mind a 38 beteget követtük, átlagosan 34,7 hónapig. A klinikai sikeresség 1,5 hónapnál 91,9% volt (34/37). 3 betegnek maradtak tünetei, kettőjüknél remyotomia történt, egyikük később műtétre szorult, 1 beteg pedig percutan endoszkópos gastrostomián esett át 18 hónap után. A hosszú távú követés során a teljes sikeresség (DRC2) 78,4% volt (29/37), míg lényeges klinikai javulást (DRC: 0/1/2) a betegek 89,2%-ában (33/37) sikerült elérnünk. Következtetés: Eredményeink megerősítik, hogy a hagyományos flexibilis endoszkópos myotomia biztonságos, hatékony a tüneteket okozó Zenker-diverticulum kezelésében. Orv Hetil. 2022; 163(17): 677-687.In the treatment of symptomatic Zenker's diverticulum, the flexible endoscopic myotomy of the cricopharyngeal muscle is considered to be a safe and effective technique.We retrospectively analyzed our experiences with conventional flexible endoscopic myotomy.38 patients with symptomatic Zenker's diverticulum were treated with flexible endoscopic myotomy and 47 myotomies were performed from September 2012 until February 2020. Most of our patients were male (23/38), with an average age of 71.5 (40-88) years. The mean size of diverticula was 3.94 (2-10) cm. In most cases, we used diverticuloscope, while free-hand technique was needed in 8 cases. We assessed our patients' symptoms by applying DRC (dysphagia, regurgitation, complication) score before the treatment and during follow-up. The overall rate of significant complications was 4.2% (2/47), and there was no procedure-related mortality. We observed pneumomediastinum in one patient that was treated conservatively. Intraprocedural bleeding occurred in several (8/47) cases, in all of them the bleeding was successfully stopped during intervention. In one of them, early recurrent massive bleeding required urgent surgery. All 38 patients were followed (mean 34.7 months). Clinical success at 1.5 months was 91.9% among endoscopically treated patients (34/37). 3 patients remained symptomatic, 2 of them were treated with re-myotomy, 1 of them needed surgery later on, another patient underwent percutan endoscopic gastrostomy at 18 months. Over long-term period, complete success (DRC2) was 78.4% (29/37), while clinical success (DRC: 0/1/2) reached in 89.2% (33/37).Our experiences confirmed that conventional method of flexible endoscopic myotomy is safe and effective for the treatment of Zenker's diverticulum symptoms. During the more than 50-year history of endoscopic retrograde cholangiopancreatography (ERCP), it has become an almost exclusively therapeutic procedure from a diagnostic method. This was the result of the evolution of far less invasive diagnostic procedures and the identification of its complications. Nowadays, being aware of these complications is fundamental. Remarkable knowledge has been gathered over the past decades about the risk factors, preventive methods and endoscopic treatments. A significant number of relevant publications from Hungarian authors can be found in this field. In our article, we summarize the complications of ERCP, their definitions, severity classifications, risk factors, prophylactic methods and endoscopic treatments.Az endoszkópos retrográd cholangiopancreatographia (ERCP) a több mint 50 éves története során diagnosztikus módszerből szinte kizárólag terápiás beavatkozássá vált. Köszönhető ez a kevésbé invazív vizsgálati módszerek kialakulásának, illetve annak a felismerésnek is, hogy számos szövődmény kialakulásának veszélyét hordozza magában. Ezek ismerete manapság alapvető fontosságú. Az eltelt évtizedek során a szövődmények hajlamosító tényezőiről, illetve a megelőzés lehetőségeiről jelentős ismeretanyag gyűlt össze, melyhez magyar szakemberek komoly eredményekkel járultak hozzá. Jelen közleményünkben az ERCP szövődményeit, azok definícióját, súlyossági osztályozásukat, kockázati tényezőiket, a profilaktikus lehetőségeket és az endoszkópos ellátás módjait foglaljuk össze. This review provides an overview about the role of endoscopy in the care of patients suffering from pancreatic cancer. In the field of diagnostics the role of endoscopic ultrasound is highlighted in both solid and cystous pancreatic tumors. The decreasing diagnostic relevance of endoscopic retrograde cholangiopancreatography (ERCP) is also discussed. The issue of preoperative biliary drainage in case of obstruction is negotiated in detail, while palliative settings are appointed thoroughly. Besides conventional enteral stenting in case of gastric outlet syndrome caused by local spreading of pancreatic tumor, some new innovative endoscopic solutions are summarized. Several endoscopic ultrasound-guided antitumor interventions that are mainly in clinical trial phase are referred in the article. The diagnostics and treatment of pancreatic neuroendocrine tumors are discussed separately due to their different biological behavior. The review emphasizes the need for multidisciplinary approach of the patients suffering from malignant pancreatic tumors.

Background Endoscopic retrograde cholangiopancreatography (ERCP) is an important therapeutic modality in acute biliary pancreatitis (ABP) cases with cholangitis or ongoing common bile duct obstruction. Theoretically, inflammation of the surrounding tissues would result in a more difficult procedure. No previous studies examined this hypothesis. Objectives ABP and acute cholangitis (AC) without ABP cases were compared to assess difficulty of ERCP. Methods The rate of successful biliary access, advanced cannulation method, adverse events, cannulation and fluoroscopy time were compared in 240 ABP cases and 250 AC cases without ABP. Previous papillotomy, altered gastroduodenal anatomy, and cases with biliary stricture were excluded. Results Significantly more pancreatic guidewire manipulation (adjusted odds ratio (aOR) 1.921 [1.241–2.974]) and prophylactic pancreatic stent use (aOR 4.687 [2.415–9.098]) were seen in the ABP than in AC group. Average cannulation time in the ABP patients (248 vs. 185 s; p = 0.043) were longer than in AC cases. No difference was found between biliary cannulation and adverse events rates. Conclusion ERCP in ABP cases seem to be more challenging than in AC. Difficult biliary access is more frequent in the ABP cases which warrants the involvement of an experienced endoscopist.

Gastrointestinal bleeding has a profound impact on public health due to its high prevalence and severity. With the elderly population taking more anticoagulants/antiaggregants/non-steroid anti-inflammatory drugs, the digestive bleeding will certainly raise more and more challenges in quantity as well as in severity for the public healthcare system. The emergency medicine specialists and gastroenterologists have a central role in the management of patients presenting with gastrointestinal bleeding. In certain cases, radiologists, invasive radiologists, intensive care specialists and surgeons should also be involved in the decision making process and management of patients. Therefore, Hungarian experts felt the need to elaborate a comprehensive, multidisciplinary, practical local guideline reflecting the frequently arisen aspects based on current international guidelines. This guideline proposal covers topics of basic requirements, initial assessment of patients, risk evaluation, laboratory tests, hemodynamic resuscitation in the case of gastrointestinal bleeding followed by its consecutive steps of diagnosis and therapy sorted by location of the source of the hemorrhage. The authors give practical instructions for unsuccessful hemostasis or rebleeding. Finally, the role of surgery is also summarized in the management of gastrointestinal bleeding. Magnetic resonance imaging (MRI) may be used instead of CT in patients with contraindications to contrast-enhanced CT, and after the 4th week from onset when invasive intervention is considered because the contents (liquid vs. solid) of pancreatic collections are better characterized by MRI and evaluation of pancreatic duct integrity is possible. The European Society of Gastrointestinal Endoscopy and United European Gastroenterology present a short list of key performance measures for endoscopic ultrasound (EUS) and endoscopic retrograde cholangiopancreatography (ERCP). We recommend that endoscopy services across Europe adopt the following seven key and one minor performance measures for EUS and ERCP, for measurement and evaluation in daily practice at center and endoscopist level: 1 Adequate antibiotic prophylaxis before ERCP (key performance measure, at least 90 %); 2 Antibiotic prophylaxis before EUS-guided puncture of cystic lesions (key performance measure, at least 95 %); 3 Bile duct cannulation rate (key performance measure, at least 90 %); 4 Tissue sampling during EUS (key performance measure, at least 85 %); 5 Appropriate stent placement in patients with biliary obstruction below the hilum (key performance measure, at least 95 %); 6 Bile duct stone extraction (key performance measure, at least 90 %); 7 Post-ERCP pancreatitis (key performance measure, less than 10 %). 8 Adequate documentation of EUS landmarks (minor performance measure, at least 90 %).This present list of quality performance measures for ERCP and EUS recommended by ESGE should not be considered to be exhaustive: it might be extended in future to address further clinical and scientific issues.

Background The aim of the study was to evaluate the current opinion and clinical decision-making process of international pancreatologists, and to systematically identify key study questions regarding the diagnosis and treatment of chronic pancreatitis (CP) for future research. Methods An online survey, including questions regarding the diagnosis and treatment of CP and several controversial clinical case vignettes, was send by e-mail to members of various international pancreatic associations: IHPBA, APA, EPC, ESGE and DPSG. Results A total of 288 pancreatologists, 56% surgeons and 44% gastroenterologists, from at least 47 countries, participated in the survey. About half (48%) of the specialists used a classification tool for the diagnosis of CP, including the Mayo Clinic (28%), Mannheim (25%), or Buchler (25%) tools. Overall, CT was the preferred imaging modality for evaluation of an enlarged pancreatic head (59%), pseudocyst (55%), calcifications (75%), and peripancreatic fat infiltration (68%). MRI was preferred for assessment of main pancreatic duct (MPD) abnormalities (60%). Total pancreatectomy with auto-islet transplantation was the preferred treatment in patients with parenchymal calcifications without MPD abnormalities and in patients with refractory pain despite maximal medical, endoscopic, and surgical treatment. In patients with an enlarged pancreatic head, 58% preferred initial surgery (PPPD) versus 42% initial endoscopy. In patients with a dilated MPD and intraductal stones 56% preferred initial endoscopic ± ESWL treatment and 29% preferred initial surgical treatment. Conclusion Worldwide, clinical decision-making in CP is largely based on local expertise, beliefs and disbeliefs. Further development of evidence-based guidelines based on well designed (randomized) studies is strongly encouraged.

The quality of endoscopic examinations substantially determines their value. In developed countries, Continuous Quality Management is used to improve it permanently. In Hungary there is no example for measuring quality in the field of gastrointestinal endoscopy. Aim: The measurement and improvement of quality of endoscopy applying completeness index (cecum intubation rate) during colonoscopy. Patients and methods: The authors defined base values retrospectively from 841 colonoscopy reports, performed in the last quarter of the year, before starting the project. The next two years (3160 colonoscopy in 2009 and 3167 in 2010) every three months they calculated the cecum intubation rate for each endoscopist. Results: The cecum intubation rate was 81.6% in the base period. When the authors excluded examinations with poor preparations and those with a previously unknown stenosis that prevented the total colonoscopy, the adjusted cecal intubation rate was 90.9%. In the next 2 years, the cecum intubation rate was 84.2% and 85.7% (p = 0.0394), while adjusted cecum intubation rate proved to be 92.3% and 92.6% (p = 0.381 NS) for the whole endoscopy unit. Of the 14 endoscopists only 6 reached an adjusted cecum intubation rate of 90%, but in the second year of the project 10 of them reached this rate and only one endoscopist remained below 87%. The endoscopists performing more than 100 colonoscopies per year had better adjusted cecum intubation rate (base 91.2%; 92.7% and 93.1% during the 2 project years) compared to those with less than 100 colonoscopies per year (base, 86.7%; project period, 85.5 and 89%). Conclusions: The evaluation and publicity of the cecal intubation rate resulted in an improvement of the quality of colonoscopy. To date, most cases of autoimmune pancreatitis (AIP) have been reported from Japan. The aim of the present study was to assess the clinical features and management of AIP cases in Hungary.The demographics, clinical presentation, laboratory and imaging findings, extrapancreatic involvement, treatment response and recurrence were evaluated in the first 17 patients diagnosed with AIP in Hungary.The mean age at presentation was 42.7 years (range: 16-74); 47% of the patients were women. New-onset mild abdominal pain (76%), weight loss (41%) and jaundice (41%) were the most common symptoms, with inflammatory bowel disease being the most frequent (36%) extrapancreatic manifestation. Diffuse pancreatic swelling was seen in 7 patients (41%) and a focal pancreatic mass in 8 (47%). Endoscopic retrograde cholangiopancreatography revealed pancreatic duct strictures in all study patients. The serum IgG4 level at presentation was elevated in 62% of the 8 patients in whom it was measured. All the percutaneous core biopsies (5 patients) and surgical specimens (2 patients), and 2 of the 4 biopsies of the papilla of Vater revealed the typical characteristic findings of AIP: a diffuse lymphoplasmacytic infiltration, marked interstitial fibrosis and obliterative phlebitis. Immunostaining indicated IgG4-positive plasma cells in 62% of the 8 patients in whom it was performed. Granulocytic epithelial lesions (GEL) were present in 3 patients. The patients without GELs were older (mean age 59 years), while those with GEL were younger (mean age 34 years), and 2 of 3 were female and had ulcerative colitis. A complete response to steroid treatment was achieved in all 15 patients. Because of the suspicion of a pancreatic tumor, 2 patients with focal AIP underwent partial pancreatectomy. One patient relapsed, but responded to azathioprine.This first Hungarian series has confirmed several previously reported findings on AIP. AIP with GEL was relatively frequent among our patients: these patients tended to be younger than in earlier studies and displayed a female preponderance with a high coincidence of ulcerative colitis. Performance of a percutaneous biopsy is strongly recommended. The response to immunosuppressive therapy was excellent. and IAP.

Neuroendocrine tumors of the digestive system can cause very diverse clinical symptoms. Due to the secretion of biogenic amines, peptides and hormones secreted by the tumor cells, various paraneoplastic syndromes can evolve, on the other hand, the growth and spreading of hormonally inactive tumors can result in different local symptoms. Patients can be symptom-free for a long time or aspecific, often periodical symptoms can prevent recognition or lead to misdiagnosis for years. The symptomatology of hormonally active tumors, derived mainly from the pancreas is very characteristic. Carcinoid syndrome can be seen in 10-18% of patients with neuroendocrine tumors. In this review, the critical appreciation of laboratory and imaging modalities is discussed. Among the major new developments in this field, the introduction of serum chromogranin A assay and new small bowel examination methods should be mentioned. Capsule endoscopy and balloon enteroscopy can provide possibility of much more earlier diagnosis, as previously. Absztrakt Háttér: A nyelőcsőszűkületet okozó daganat kiindulási helye általában maga a nyelőcső, de okozhat stenosist a cardia, a tüdő, a mediastinum tumora és ritka esetekben mammatumor metastasisa is. Az ilyen esetek nagy része inoperábilis, általában a betegség előrehaladott stádiuma, illetve a komorbiditás és az életkor miatt. A betegek többsége ezért csak palliatív terápiában részesülhet. A legáltalánosabb és az életminőséget leginkább befolyásoló tünet a dysphagia, melynek megoldására több lehetőség is kínálkozik. Dolgozatunkban az öntáguló fémsztentekkel szerzett tapasztalatainkat foglaljuk össze. Betegek és módszerek: 2003–2008 között tumoros nyelészavar/-képtelenség miatt osztályunkon összesen 64 betegnél (15 nő / 49 férfi) végeztünk 68 endoscopos sztentbehelyezést. Radiológiai kivizsgálást követően a tumor pozíciója alapján distalisan nyitódó antireflux-billentyűvel ellátott vagy a nélküli fedett sztenteket alkalmaztunk, egy esetben proximalisan nyitódó fedett sztent került behelyezésre a tumor magas pozíciója miatt. Célunk a per os táplálhatóság biztosítása, illetve az esetleges fistulák fedése volt. Erdemények: A nyelés minden esetben jelentősen javult. A dysphagia score a sztentbehelyezés előtt átlagosan 3,2 volt, míg a sztent behelyezése után 1,7-re csökkent. Fontos adat, hogy a fistulák zárása minden esetben eredményes volt. A sztentek behelyezése kapcsán technikai problémák alacsony arányban (2/68 – 2,94%) adódtak. A beavatkozásokat követően egy esetben észleltünk 7 napon belül migratiót. Egy betegnél a beavatkozást követően 2 nappal nem fatális myocardialis infarctus alakult ki. Késői szövődményként tumorbenövés, illetve túlnövés öt alkalommal, vérzéses szövődmény összesen négy alkalommal jelentkezett. Fistulaképződést három esetben észleltünk. Hét napon túli sztentmigratio két esetben következett be. Resztentelésre összesen négy esetben, falatelakadás miatt ismételt endoscopos beavatkozásra három esetben került sor. Következtetések: A dysphagia-értékek javulása bizonyítja, hogy a fémsztentek hatékonyan javítják a betegek nyelési funkcióját. Tapasztalataink alapján a sztentfüggő szövődmények összaránya alacsony. Véleményünk szerint az öntáguló fémsztentekkel végzett nyelőcső-intubatio biztonságosan és eredményesen alkalmazható a malignus oesophagus obstructio palliatív terápiájában.

This Guideline is an official statement of the European Society of Gastrointestinal Endoscopy (ESGE). It provides practical advice on how to achieve successful cannulation and sphincterotomy at minimum risk to the patient. The Grading of Recommendations Assessment, Development, and Evaluation (GRADE) system was adopted to define the strength of recommendations and the quality of evidence. Main recommendations 1 ESGE suggests that difficult biliary cannulation is defined by the presence of one or more of the following: more than 5 contacts with the papilla whilst attempting to cannulate; more than 5 minutes spent attempting to cannulate following visualization of the papilla; more than one unintended pancreatic duct cannulation or opacification (low quality evidence, weak recommendation). 2 ESGE recommends the guidewire-assisted technique for primary biliary cannulation, since it reduces the risk of post-ERCP pancreatitis (moderate quality evidence, strong recommendation). 3 ESGE recommends using pancreatic guidewire (PGW)-assisted biliary cannulation in patients where biliary cannulation is difficult and repeated unintentional access to the main pancreatic duct occurs (moderate quality evidence, strong recommendation). ESGE recommends attempting prophylactic pancreatic stenting in all patients with PGW-assisted attempts at biliary cannulation (moderate quality evidence, strong recommendation). 4 ESGE recommends needle-knife fistulotomy as the preferred technique for precutting (moderate quality evidence, strong recommendation). ESGE suggests that precutting should be used only by endoscopists who achieve selective biliary cannulation in more than 80% of cases using standard cannulation techniques (low quality evidence, weak recommendation). When access to the pancreatic duct is easy to obtain, ESGE suggests placement of a pancreatic stent prior to precutting (moderate quality evidence, weak recommendation). 5 ESGE recommends that in patients with a small papilla that is difficult to cannulate, transpancreatic biliary sphincterotomy should be considered if unintentional insertion of a guidewire into the pancreatic duct occurs (moderate quality evidence, strong recommendation). In patients who have had transpancreatic sphincterotomy, ESGE suggests prophylactic pancreatic stenting (moderate quality evidence, strong recommendation). 6 ESGE recommends that mixed current is used for sphincterotomy rather than pure cut current alone, as there is a decreased risk of mild bleeding with the former (moderate quality evidence, strong recommendation). 7 ESGE suggests endoscopic papillary balloon dilation (EPBD) as an alternative to endoscopic sphincterotomy (EST) for extracting CBD stones

Background: Crohn's disease of the esophagus is rare, but if present, it may cause stricture of the involved parts. Even though the primary therapeutic intervention is endoscopic balloon dilation, some patients with refractory strictures require surgical resection. However radial electroincision of refractory esophagogastric anastomotic stricture after surgery is mentioned, Crohn's disease related needle-knife stricturoplasty is not known in the literature. Case report: A 64 years old female patient was diagnosed with buccal and esophageal manifestation of Crohn's disease 5 years ago. No other part of the gastrointestinal tract was involved. Balloon dilation was performed in 2010 and 2011. She was admitted to our gastroenterology department in May 2014 with relapsing dysphagy. The former buccal ulcers scarred, while in the esophagus a ring-like stricture was revealed starting at 20 cm from incisors. The next 10 cm was narrowed by pseudopolyps without active ulceration. We used needle knife to perfom three radial incisions at the level of the ring. One incision was clipped thereafter due to its deepness. The patient was discharged some days later. After 10 months of follow-up the patient was symptom-free, the gastroscopy revealed a tight ring with easy passage of the scope. Conclusion: In case of ring-like fibrotic stricture of the esophagus caused by Crohn's disease beside balloon dilation, the needle knife incision can be useful method to reveal the symptoms for a long-term duration.
