Murine models have suggested that inflammation modulates red cell alloimmunisation. This study quantifies alloimmunisation risks during infectious episodes in humans. We performed a multicentre case-control study within a source population of patients receiving their first and subsequent red cell transfusions during an 8-year follow-up period. Patients developing a first transfusion-induced red cell alloantibody (N = 505) were each compared with two similarly exposed, but non-alloimmunised controls (N = 1010) during a 5-week 'alloimmunisation risk period' using multivariate logistic regression analysis. Transfusions during 'severe' bacterial (tissue-invasive) infections were associated with increased risks of alloantibody development [adjusted relative risk (RR) 1.34, 95% confidence interval (95% CI) 0.97-1.85], especially when these infections were accompanied with long-standing fever (RR 3.06, 95% CI 1.57-5.96). Disseminated viral disorders demonstrated a trend towards increased risks (RR 2.41, 95% CI 0.89-6.53), in apparent contrast to a possible protection associated with Gram-negative bacteraemia (RR 0.58, 95% CI 0.13-1.14). 'Simple' bacterial infections, Gram-positive bacteraemia, fungal infections, maximum C-reactive protein values and leucocytosis were not associated with red cell alloimmunisation. These findings are consistent with murine models. Confirmatory research is needed before patients likely to develop alloantibodies may be identified based on their infectious conditions at time of transfusion. 01 december 2016

Background and aim Being the primary organ that clears and processes ageing and damaged red cells, the spleen has been suggested to play a central role in red cell alloimmunization in mice experiments. We evaluated the effect of splenectomy on the occurrence of red cell alloimmunization in humans. Methods We performed a nested case-control study among a source population of 24,063 patients who received their first and subsequent red cell transfusion in one of six participating hospitals in the Netherlands between 2005 and 2013. Cases were defined as patients who developed a first transfusion-induced clinically relevant red cell alloantibody. Each case was matched to two non-alloimmunized control patients, based on the number of (lifetime) transfusions and the hospital. Using multivariate logistic regression analyses, we evaluated the association between (a history of) splenectomy and the risk for alloimmunization. Results Among a source population of 24,063 patients, 505 patients (2.1%) developed red cell alloantibodies and were matched to 1,010 controls. Splenectomy had been performed in one case and 19 controls. In 12 patients, splenic injury was caused by severe trauma or complicated abdominal surgery. No patient underwent a splenectomy in the context of an autoimmune disease. Using multivariate logistic regression analysis with conditioning on identified potential confounders, we found that splenectomized patients had a 20-fold reduced risk of alloimmunization as compared to patients lacking a history of splenectomy (adjusted RR 0.05, 95% confidence interval (CI) 0.01-0.55). With the assumption that the red cell exposure patterns of the 19 splenectomized controls represented the red cell exposure pattern of all splenectomized patients within the source population we estimated that the total number patients with alloimmunization after splenectomy was 24; yielding a RR of 0.04 which confirms our logistic regression based RR. Interpretation In conclusion, in accordance with the findings in mice, our findings suggest that splenectomized patients are strongly protected from primary red cell alloimmunization. Het jongste AWBZ-kind groeide op tot een volwassen discipline die snel, transparant en streng maar rechtvaardig oordeelt over de rechten op zorg van mensen met beperkingen. Dit boek doet hierover verslag. In het eerste deel komen de ontwikkelingen in de genoemde periode aan de orde. Conny Bellemakers beschrijft die in hoofdstuk 1 als patiënt, professional en beleidsmaker. Tineke van Ee doet dat in hoofdstuk 2 als manager van het Regionaal Indicatie Orgaan dat tot 1 januari 2005 in Friesland bestond. Nico Dam en Chantal IJland schetsen de vereenvoudiging van de indicatiestelling die anno 2004 volop aan de gang is (hoofdstuk 3). Dan volgen in deel 2 tal van goede voorbeelden die in de periode 1998 – 2005 tot stand zijn gekomen. Aan bod komen keuze cliënt ondersteunende informatie op een website (Marleen Vermeer, hoofdstuk 4) het gemeentelijke loket (Tom Kamsma, hoofdstuk 5), een gemeenschappelijk loket voor de AWBZ en de WVG (Hans Kraayeveld, hoofdstuk 6), één loket voor de geestelijke gezondheidszorg (Anja van den Bogaert, hoofdstuk 7), gemeenschappelijke zorgtoewijzing (Annemarie van Hout en Ria Tijmensen, hoofdstuk 8), een ziekenhuistransferpunt (Elly van der Veen, hoofdstuk 9), een ziekenhuistransferafdeling (André Vis, hoofdstuk 10) en goede zorgtoewijzing binnen een zorgketen (Rita van Gorcum, hoofdstuk 11). Voor een deel bestaan de goede voorbeelden in deel twee niet meer. Ze gingen ter ziele bij de aankondiging van de Wet Maatschappelijke Ondersteuning (WMO) en door concurrentie tussen zorgaanbieders. Met spijt doen de betreffende auteurs daarover verslag. Deel twee eindigt met een overzicht van 163 goede en slechte cliëntgebonden voorbeelden van indicatiestelling, die in een onderzoek werden opgetekend (Susan Jedeloo en Guus Schrijvers, hoofdstuk 12). Goede voorbeelden komen niet zo maar tot stand. Achter de schermen is daarvoor in de periode 1998 - 2005 veel werk verzet. Deel 3 doet daarover verslag. Er kwam een gemeenschappelijke ICF-taal tot stand (Sylvia Hoekstra, hoofdstuk 13), een gemeenschappelijk afwegingskader (Juul van Ogtrop, hoofdstuk 14), een samenhangende registratie (Joop Jaspers en Lisanne van de Weerdhof, hoofdstuk 15) en een scenario-aanpak om met verzamelde gegevens de toekomstige zorgvraag in kaart te brengen (Ying Li O en anderen, hoofdstuk 16). Wat zal de toekomst brengen voor cliënten die voor hun functioneren op zorg zijn aangewezen? Flip Buurmeier geeft hierop een antwoord. Eén en dezelfde, strenge maar rechtvaardige poortwachter gaat cliënten voor zowel de AWBZ, de WMO en andere voorzieningen indiceren (hoofdstuk 17). Voor de keuze van de onderwerpen en de daarbij uitgenodigde sprekers is de redactie verantwoordelijk. Alle auteurs hebben een grote expertise op het gebied van hun onderwerp: zij waren in de periode 1998 - 2005 allen professionele kartrekkers. Zij werken verspreid in Nederland. De redactie is hun zeer erkentelijk voor het schrijfwerk dat zij vaak in de avonden en weekeinden uitvoerden. Dit boek sluit de pioniersfase 1998 - 2005 van de indicatiestelling in Nederland af. Het kondigt ook een begin aan van een nieuwe decentrale periode van indicatiestelling. De redactie spreekt de hoop uit dat dit boek inspiratie oplevert om die eenheid van toegang tot AWBZ, WMO en andere voorzieningen voor mensen met beperkingen te behouden. Ons maakt het niet uit wie de indicatiestelling gaat uitvoeren en wie daarvoor eindverantwoordelijkheid draagt. Als die maar onafhankelijk, professioneel, integraal, doelmatig en snel blijft.

Interleukin-5 (IL-5) and granulocyte macrophage-colony stimulating factor (GM-CSF) are important cytokines for the proliferation, differentiation, and activation of myeloid lineages. The JAK/STAT pathway is one of the signaling pathways implicated in mediating biological responses induced by these cytokines. Previous studies have demonstrated that these cytokines predominantly activate an 80 kDa STAT5 isoform in mature granulocytes. To better understand the role of STAT proteins during growth and differentiation of granulocytes, we evaluated differentiation of human CD34+ hematopoietic stem cells ex vivo toward eosinophils and neutrophils. Bandshift experiments showed that in an early stage of both differentiation pathways (14 days), the 94 kDa STAT5B protein was activated by both IL-5 (eosinophil lineage) and GM-CSF (neutrophil lineage). However, during maturation of both lineages (days 21 and 28), increased expression of a functionally distinct 80 kDa STAT5 isoform was observed, resulting in heterodimer DNA-binding complexes containing both the 94 and 80 kDa STAT5 proteins. The finding that functionally distinct isoforms of STAT5 are activated during the early and late differentiation stages of granulocytes suggests that they might be involved in regulating different biological functions in these cells.

Previously, 158 nuclear families with probands suspected of having either Prader Willi (PWS) or Angelman syndrome (AS) were analyzed with polymorphic DNA markers from the 15q11-13 region. These cases have been re-evaluated with the probe PW71 (D15S63), which detects parent-of-origin-specific alleles after digestion with a methylation-sensitive restriction enzyme (HpaII). Application of PW71 to DNA samples isolated from leucocytes, confirmed the deletions and uniparental disomies detected earlier by marker analysis, and resolved 50% of the previously uninformative (n = 18) cases. PW71 and restriction fragment length polymorphism analysis indicated that, in all resolved cases, disomies of the 15q11-13 region were present. The use of PW71 increased the percentage of disomies detected in our PWS and AS patient groups. Almost 50% of our PWS patients and 17% of the AS patients showed a disomy of maternal or paternal origin, respectively. DNA of first trimester chorionic villi and of fibroblast cultures was not suitable for analysis with PW71 because of different methylation patterns. The application of PW71 is recommended for the diagnosis of the PWS and AS, with respect to DNA samples from blood.
