2. Factors associated with laryngeal injury after intubation in children:a systematic review

Abstract

Purpose: The purpose of this study is to evaluate all potential factors associated with laryngeal injury after endotracheal intubation in the pediatric population. Methods: A systematic literature search was conducted in Medline, Embase, Cochrane, web of science and Google scholar up to 20th of March 2023. We included all unique articles focusing on factors possibly associated with intubation-injury in pediatric patients. Two independent reviewers determined which articles were relevant by coming to a consensus, quality of evidence was rated using GRADE criteria. All articles were critically appraised according to the PRISMA guidelines. The articles were categorized in four outcome measures: post-extubation stridor, post-extubation upper airway obstruction (UAO) necessitating treatment, laryngeal injury found at laryngoscopy and a diagnosed laryngotracheal stenosis (LTS). Results: A total of 24 articles with a total of 15.520 patients were included. The incidence of post-extubation stridor varied between 1.0 and 30.3%, of post-extubation UAO necessitating treatment between 1.2 and 39.6%, of laryngeal injury found at laryngoscopy between 34.9 to 97.0% and of a diagnosed LTS between 0 and 11.1%. Although the literature is limited and quality of evidence very low, the level of sedation and gastro-esophageal reflux are the only confirmed associated factors with post-extubation laryngeal injury. The relation with age, weight, gender, duration of intubation, multiple intubations, traumatic intubation, tube size, absence of air leak and infection remain unresolved. The remaining factors are not associated with intubation injury. Conclusion: We clarify the role of the potential factors associated with laryngeal injury after endotracheal intubation in the pediatric population.

Abstract

The Netherlands, renowned for its flat landscape and strategic location at the confluence of major rivers, combined with fertile soil and a mild climate, boasts optimal natural conditions for agricultural production. However, the success of the Dutch agricultural sector and its developments has significant implications for biodiversity, as excessive nitrogen levels contribute to a decline in plant and animal species, ultimately disrupting the ecosystem balance. Currently, functions are separated by human engineered boundaries with little to no interaction with each other. Agriculture is hyper focused on the productive part of the system, lacking equilibrium ripping the balanced system apart. Ideally, water, agriculture and soil live in perfect harmony and so envisioned for the year 2070 is a dynamic river landscape in the Dutch Delta region; a landscape where there is room for the water to flow as it pleases. In this vision, a river’s natural flow is embraced; free flowing water with little constraints. Instead of seeing this freedom as a problem that must be controlled, this project values the river’s morphological changes and water fluctuations through time. This vision and strategy not only makes room for the river, but creates a new dynamic system in which water, biodiversity and agriculture works together. Adjustments are mainly made to the land-uses to correspond to the shifts of the river, using a catalogue with several interventions, divided into practical, spatial and facilitating tools that support the previous categories. A green network of ‘in between’ wetlands is situated within the river basin. This network serves as vital ecosystems that enhance the resilience of the region. These green marshes overflow with biodiversity, serving as havens for formerly threatened animal and plant species alike to thrive and flourish. 5. Local Language a Medium of Instruction: Challenges and Way Forward

Abstract

The paper reports on a participatory action research study conducted in six rural primary schools in Uganda in 2013 to establish why children taught in the local language had difficulties in reading and writing. Findings through interviews, focus group discussions, reviews of exercise books and lesson observations indicated that though it was easier for pupils to learn the concepts in the local language; challenges ranging from poor translation, inadequate teachers' language proficiency, lack of instructional materials, high pupils' enrolment, lack of administrative support and teacher-centred approach of teaching, affected pupils' learning to read and write. Participants recommended adopting the child-centred pedagogy, incorporating instructional materials, conducting continuous assessment and recording pupils' competencies attained in reading and writing. Teachers need to engage more in Participatory action research in order to reflect on their practices and pupils' learning, and collaboratively decide what works best and what needs improvement in their classrooms.

6. Formative Assessment: The Role of Participatory Action Research in Blending Policy and Practice in Uganda

Abstract

This paper documents teachers' assessment practices and pupils' learning to read and write in large classes. To gain insights into the assessment practices and pupils' learning, the principles of Vygotsky's zone of proximal development and participatory action research (PAR) design were adopted. Collaboratively, teachers reflected on their assessment practices, developed, implemented and monitored the assessment practices. Through observation, in-depth interview and review of exercise books, data were obtained. We argue that teachers' involvement in PAR has contributed to changing assessment practices in large classes and in improving pupils' competencies in reading and writing. On the other hand to promote better assessment practices, we advocate for reduced number of pupils in a class and teachers' workload to enable them offer more support to the pupils during assessment of reading and writing.

15. 'Soft' Pedagogy? The Invention of a 'Feminine' Pedagogy as a Cause of Educational Crises

Abstract

Every now and then the feminisation of the teaching force is put forth as a barrier to reduce educational equality between the sexes. The feminisation of education is supposed to have a negative impact on boys' achievement, causing educational as well as behavioural problems. It is not just boys who allegedly suffer; over the years, the feminisation of education has been held responsible for countless other education-related problems and social crises. In the latest parliamentary and public discussions in the Netherlands, the failure of educational reforms in the second half of the twenty-first century is attributed to the feminisation of education. This time it is not too many female teachers, but a so-called "female pedagogy" that is to blame. It is assumed that women teachers have a different ("soft") approach to methodology, didactics and pedagogy than their male counterparts. We have sought historical evidence for this assumption by examining the rise and development of pedagogy within the teaching profession, using a content analysis of the professional association journal for the (teaching of) modern languages in secondary schooling. 26. Safe Schools and Sexual Harassment: The Relationship between School Climate and Coping with Unwanted Sexual Behaviour

Abstract

Objective: To explore the impact of the school climate on adolescents' reporting of sexual harassment. Design: A quantitative survey among students in their 4th year of secondary education. Setting: Questionnaires were completed in a class setting. Method: An a-select sampling strategy was used to select 2808 students in 22 schools. Results: Findings indicate that a safe school climate is not enough for students to speak freely or report unwanted sexual experiences. Of more importance is an atmosphere where, generally speaking, sexuality is a topic for (an open) discussion. Conclusion: Good sex education classes (as perceived and evaluated by the students themselves) provide students with the communicative tools necessary to talk about their experiences of unwanted sexual behaviour.
