1. Enhanced Multi-Model Deep Learning for Rapid and Precise Diagnosis of Pulmonary Diseases Using Chest X-Ray Imaging

Abstract

Background: The global burden of respiratory diseases such as influenza, tuberculosis, and viral pneumonia necessitates rapid, accurate diagnostic tools to improve healthcare responses. Current methods, including RT-PCR and chest radiography, face limitations in accuracy, speed, accessibility, and cost-effectiveness, especially in resource-constrained settings, often delaying treatment and increasing transmission. Methods: This study introduces an Enhanced Multi-Model Deep Learning (EMDL) approach to address these challenges. EMDL integrates an ensemble of five pre-trained deep learning models (VGG-16, VGG-19, ResNet, AlexNet, and GoogleNet) with advanced image preprocessing (histogram equalization and contrast enhancement) and a novel multi-stage feature selection and optimization pipeline (PCA, SelectKBest, Binary Particle Swarm Optimization (BPSO), and Binary Grey Wolf Optimization (BGWO)). Results: Evaluated on two independent chest X-ray datasets, EMDL achieved high accuracy in the multiclass classification of influenza, pneumonia, and tuberculosis. The combined image enhancement and feature optimization strategies significantly improved diagnostic precision and model robustness. Conclusions: The EMDL framework provides a scalable and efficient solution for accurate and accessible pulmonary disease diagnosis, potentially improving treatment efficacy and patient outcomes, particularly in resource-limited settings.

Abstract

For highly conserved mammalian protein, chicken is a suitable immune host to generate antibodies. Monoclonal antibodies have been successfully targeted with immunity checkpoint proteins as a means of cancer treatment; this treatment enhances tumor-specific immunity responses through immunoregulation. Studies have identified the importance of B7-H4 in immunoregulation and its use as a potential target for cancer treatment. High levels of B7-H4 expression are found in tumor tissues and are associated with adverse clinical and pathological characteristics. Using the phage display technique, this study isolated specific single-chain antibody fragments (scFvs) against B7-H4 from chickens. Our experiment proved that B7-H4 clearly induced the inhibition of T-cell activation. Therefore, use of anti-B7-H4 scFvs can effectively block the exhaustion of immunity cells and also stimulate and activate T-cells in peripheral blood mononuclear cells. Sequence analysis revealed that two isolated scFv S2 and S4 have the same VH complementarity-determining regions (CDRs) sequence. Molecule docking was employed to simulate the complex structures of scFv with B7-H4 to analyze the interaction. Our findings revealed that both scFvs employed CDR-H1 and CDR-H3 as main driving forces and had strong binding effects with the B7-H4. The affinity of scFv S2 was better because the CDR-L2 loop of the scFv S2 had three more hydrogen bond interactions with B7-H4. The results of this experiment suggest the usefulness of B7-H4 as a target for immunity checkpoints; the isolated B7-H4-specific chicken antibodies have the potential for use in future cancer immunotherapy applications.

Abstract

Apoptosis has been considered to be involved in schizophrenia. Water channels are modulated just before apoptosis. In the aquaporin family, aquaporin 4 (AQP-4) is most highly expressed in the brain and is supposed to play an important role in a neuronal environment. In this clinical study, we investigated the relationship between the AQP-4 polymorphism and drug response in schizophrenia under the control of the MAOA (monoamine oxidase A) promoter gene. We recruited 91 patients with schizophrenia, and they were randomized to receive olanzapine (n = 44), risperidone (n = 23), or paliperidone (n = 24). Genotyping of AQP-4 and MAOA polymorphisms was done in all patients. Patients with the AQP-4 non-C polymorphism needed a higher dosage of olanzapine for treatment (z = 4.163, P = 0.041), and patients with a short form of the MAOA polymorphism needed a higher dosage of risperidone for treatment (z = 5.124, P = 0.024). Patients who smoked cigarettes needed a higher dosage of olanzapine for treatment (z = 4.905, P = 0.027), but cigarette smoking did not affect the dosage of paliperidone. The AQP-4 polymorphism may have an effect in influencing the dosage of olanzapine. However, the roles of AQP-4 polymorphisms in the blood-brain barrier and different neuroprotective effects need further exploration in future studies.

5. Refractory and Complicated Neuroleptic Malignant Syndrome Cured by ECT and Followed by Brain Atrophy: Case Report.

Abstract

Introduction: Neuroleptic malignant syndrome (NMS) is an idiosyncratic and rare but serious adverse effect that has been reported with the use of all antipsychotics. The mortality rate may be as high as 50% if failure of one organ is present. Presentation of case: Herein, we report a rare case of paliperidone-induced 92-day NMS in a 60-year-old female with silent lacunar infarctions at the lentiform nucleuses. The patient suffered from episodes of nosocomial pneumonia and urosepsis. Then, her chronic subclinical hyperthyroidism transformed to a thyroid storm. Her dependence on nasogastric and foley tubes was refractory to hydration, carbimazole, antibiotics, and the combined antiparkinsonian therapy of high-dose bromocriptine and amatadine. After 6 sessions of electroconvulsive therapy (ECT), the patient returned to normal consciousness, mobility of the 4 limbs, and normal speech function. Discussion: This case serves as a reminder that it is never too late to use ECT to resolve NMS, in spite of a complicated and prolonged medical illness, and that NMS may incur brain atrophy as a sequela.
