Cone beam computed tomography (CBCT) is frequently used in dental diagnosis and treatment. Comparative studies of the effects of CBCT on implant navigation, however, are still limited. The objective of this study was to evaluate whether the computed tomography images of the four commercial brands will affect the accuracy of the new version of IRIS implant navigation system.In the first part, the accuracy of the IRIS implant navigation system was evaluated by a precision confirmation jig whose position is confirmed. In the second part, the IRIS implant navigation system was used in conjunction with 4 brands of CBCT scans analyzed by its effect on accuracy.The results showed that the mean deviation of the new version of IRIS-100 system accuracy was less than 1 mm. Among the four groups, the overall average deviation caused by CBCT images showed that the 3D eXam group had the smallest error of approximately 0.94 ± 0.12 mm and the AZ 3000 CT group had the largest error of approximately 1.34 ± 0.10 mm.Based on the study, the accuracy of the IRIS implant navigation system will vary with the CBCT image resolution and the status of the CBCT machine.

Manoalide provides preferential antiproliferation of oral cancer but is non-cytotoxic to normal cells by modulating reactive oxygen species (ROS) and apoptosis. Although ROS interplays with endoplasmic reticulum (ER) stress and apoptosis, the influence of ER stress on manoalide-triggered apoptosis has not been reported. The role of ER stress in manoalide-induced preferential antiproliferation and apoptosis was assessed in this study. Manoalide induces a higher ER expansion and aggresome accumulation of oral cancer than normal cells. Generally, manoalide differentially influences higher mRNA and protein expressions of ER-stress-associated genes (PERK, IRE1α, ATF6, and BIP) in oral cancer cells than in normal cells. Subsequently, the contribution of ER stress on manoalide-treated oral cancer cells was further examined. ER stress inducer, thapsigargin, enhances the manoalide-induced antiproliferation, caspase 3/7 activation, and autophagy of oral cancer cells rather than normal cells. Moreover, N-acetylcysteine, an ROS inhibitor, reverses the responses of ER stress, aggresome formation, and the antiproliferation of oral cancer cells. Consequently, the preferential ER stress of manoalide-treated oral cancer cells is crucial for its antiproliferative effect.

BACKGROUND/PURPOSE: The optimal strategy for scanning removable partial dentures remains unknown. This study investigated scanning strategies for patients with a maxillary Kennedy Class I arch as well as the measurement deviations of three scanning strategies. MATERIALS AND METHODS: A standard maxilla model was positioned with a holder in a dental chair to simulate a natural patient position and posture. Standard Tessellation Language files for reference models were formatted with a desktop scanner, and model operation files were obtained with a TRIOS 3 Pod intraoral scanner and superimposed using Exocad computer-aided design software. The three scanning strategies evaluated in this study (Strategy M, T-R, and R-T) were used for nine scans each, and the resulting data were recorded. The deviation of the three strategies was statistically analyzed through one-way ANOVA and Tukey post hoc testing. RESULTS: The trueness of Strategy M, T-R, and R-T was 52.6 ± 31.0, 54.9 ± 27.6, and 50.1 ± 22.3 μm, respectively. No statistically significant differences in trueness were detected among the three groups (P > 0.05). However, Strategy T-R had the evenest distribution of all measuring points. The deviations of the measurements obtained by three scanning strategies were mostly between 30 and 70 μm. The precision of the three strategies was similar as well. CONCLUSION: Trueness did not differ significantly among the three strategies. However, Strategy T-R is recommended for use with a TRIOS 3 Pod scanner because of its reduction of the seesaw effect and high stabilization of the RPD framework.

Oral cancer is one of the highest-incidence malignancies worldwide, with the occurrence of oral squamous cell carcinoma (OSCC) being the most frequently diagnosed form. A barrier for oral cancer management may arise from tumor cells that possess properties of cancer stemness, which has been recognized as a crucial factor in tumor recurrence and metastasis. As such, understanding the molecular mechanisms underlying these tumor cells may provide insights for improving cancer treatment. MRE11 is the core protein of the RAD50/MRE11/NBS1 complex with a primary role in DNA damage repair, and it has been diversely associated with tumor development including OSCC. In this study, we aimed to investigate the engagement of CD44, a cancer stemness marker functioning in the control of cell growth and motility, in OSCC malignancy under the influence of MRE11. We found that overexpression of MRE11 enhanced CD44 expression and tumorsphere formation in OSCC cells, whereas knockdown of MRE11 reduced these phenomena. In addition, the MRE11-promoted tumorsphere formation or cell migration ability was compromised in OSCC cells carrying siRNA that targets CD44, as was the MRE11-promoted AKT phosphorylation. These were further supported by analyzing clinical samples, where higher CD44 expression was associated with lymph node metastasis. Additionally, a positive correlation between the expression of MRE11 and CD44, or that of CD44 and phosphorylated AKT, was observed in OSCC tumor tissues. Finally, the expression of CD44 was found to be higher in the metastatic lung nodules from mice receiving tail vein-injection with MRE11-overexpressing OSCC cells compared with control mice, and a positive correlation between CD44 and phosphorylated AKT was also observed in these metastatic lung nodules. Altogether, our current study revealed a previously unidentified mechanism linking CD44 and AKT in MRE11-promoted OSCC malignancy, which may shed light to the development of novel therapeutic strategies in consideration of this new pathway in OSCC.

Discovering drug candidates for the modulation of metastasis is of great importance in inhibiting oral cancer malignancy. Although most pomegranate extract applications aim at the antiproliferation of cancer cells, its antimetastatic effects remain unclear, especially for oral cancer cells. The aim of this study is to evaluate the change of two main metastasis characters, migration and invasion of oral cancer cells. Further, we want to explore the molecular mechanisms of action of pomegranate extract (POMx) at low cytotoxic concentration. We found that POMx ranged from 0 to 50 μg/mL showing low cytotoxicity to oral cancer cells. In the case of oral cancer HSC-3 and Ca9-22 cells, POMx inhibits wound healing migration, transwell migration, and matrix gel invasion. Mechanistically, POMx downregulates matrix metalloproteinase (MMP)-2 and MMP-9 activities and expressions as well as epithelial-mesenchymal transition (EMT) signaling. POMx upregulates extracellular signal-regulated kinases 1/2 (ERK1/2), but not c-Jun N-terminal kinase (JNK) and p38 expression. Addition of ERK1/2 inhibitor (PD98059) significantly recovered the POMx-suppressed transwell migration and MMP-2/-9 activities in HSC-3 cells. Taken together, these findings suggest to further test low cytotoxic concentrations of POMx as a potential antimetastatic therapy against oral cancer cells.

The computer-aided design/computer-aided manufacturing (CAD/CAM) fabrication technique has become one of the hottest topics in the dental field. This technology can be applied to fixed partial dentures, removable dentures, and implant prostheses. This study aimed to evaluate the feasibility of NaCaPO4-blended zirconia as a new CAD/CAM material. Eleven different proportional samples of zirconia and NaCaPO4 (xZyN) were prepared and characterized by X-ray diffractometry (XRD) and Vickers microhardness, and the milling property of these new samples was tested via a digital optical microscope. After calcination at 950 °C for 4 h, XRD results showed that the intensity of tetragonal ZrO2 gradually decreased with an increase in the content of NaCaPO4. Furthermore, with the increase in NaCaPO4 content, the sintering became more obvious, which improved the densification of the sintered body and reduced its porosity. Specimens went through milling by a computer numerical control (CNC) machine, and the marginal integrity revealed that being sintered at 1350 °C was better than being sintered at 950 °C. Moreover, 7Z3N showed better marginal fit than that of 6Z4N among thirty-six samples when sintered at 1350 °C (p &lt, 0.05). The milling test results revealed that 7Z3N could be a new CAD/CAM material for dental restoration use in the future.

Betel quid (BQ), a group I human carcinogen, strongly contributes to an increased risk of oral potentially malignant disorders (OPMD) and cancers of the oral cavity and pharynx. This study was conducted to discover whether monoamine oxidase (MAO) and catechol-O-methyltransferase (COMT) variants play a potential role in the risk assessment of oral cavity and pharynx cancers and OPMD, particularly among BQ users. We applied a case–control study to confirm the polymorphism of MAO and COMT using single-nucleotide polymorphisms. We used qRT-PCR, Western blotting, and immunohistochemistry (IHC) to determine MAO and COMT expression. Carriers of the MAOA rs6323 G-allele, MAOB rs6324 G-allele, and COMT rs4633 C/C-genotype had a prominently increased risk of oral cavity and pharynx cancers (AOR = 56.99, p &lt, 0.001). Compared to adjacent noncancerous tissues, a significant downregulation of MAO and COMT expression was exhibited in cancerous tissues (p &lt, 0.01). Furthermore, in different cell models, MAO and COMT expression was significantly downregulated with an increased dose of arecoline (p &lt, 0.01). In personalized preventive medicine for oral and pharyngeal cancers, our findings are the first to demonstrate the potential role of lower MAO and COMT expression levels, with the risk polymorphisms utilized as clinical biomarkers.

OBJECTIVES To determine the minimum thickness required for a monolithic hybrid ceramic crown on different substrates (soft vs stiff) used in posterior dentition for bruxism. METHODS 80 polymer-infiltrated ceramic networks Vita Enamic (PICN VE) disc specimens with four different occlusal thicknesses (0.8, 1.2, 1.6 and 2.0 mm), were produced using a computer-aided design/manufacturing system, and cemented on a stiff (zirconia) or soft (polyamide) substrate of 4-mm thickness. The ten specimens, in soft or stiff groups, were subjected to compressive loading by a MTS machine until fracture or maximum load (4500 N) was reached. The unbroken specimens were examined using optical coherence tomography. Eight axisymmetric finite element models and eight 3D models comprising the four different occlusal thicknesses and two substrates under different vertical loads and sliding movements were constructed. Background/purpose Among the ceramic materials used for all-ceramic crowns, zirconia has high biocompatibility and favorable mechanical properties, but its main drawbacks include low translucency and stress-induced phase transformation. To stabilize high-strength tetragonal zirconia polycrystal (TZP), 3–5 mol% yttria is usually added to prepare yttria-stabilized TZP (Y-TZP). In this study, the optical properties of three commercial Y-TZP ceramics were compared with those of the clinically available glass-ceramic material of lithium disilicate, and the relationship between translucency and crystal properties was analyzed in vitro. Materials and methods Twelve 5-mm-thick standardized disks were prepared from three Y-TZP ceramics and one lithium disilicate block. Absolute translucency was measured using a spectrophotometer with an integrating sphere. X-ray diffraction was used to quantify the main structural parameters (i.e., preferred plane, quantitative phase, and grain size) of Y-TZP crystals. Results The product-dominated phase of Y-TZP exhibited a tetragonal lattice pattern, and the preferred planes had minor variations. The diffraction patterns of the three Y-TZP ceramics demonstrated minor effects on translucency, without significant differences (p > 0.05). The grain size of 54–70 nm was negatively related to translucency in Y-TZP. Lithium disilicate specimens had significantly higher translucency than the three Y-TZP specimens (p Conclusion Grain size reduction played an essential role in developing highly translucent Y-TZP ceramics. The three Y-TZP ceramics were essentially opaque but exhibited poorer translucency than lithium disilicate in terms of esthetics.

Withaferin A (WFA), a Withania somnifera-derived triterpenoid, is an anticancer natural product. The anticancer effect of nonionizing radiation such as ultraviolet-C (UVC) as well as the combined treatment of UVC and WFA is rarely investigated. Low dose UVC and/or WFA treatments (12 J/m2 and/or 1 &mu, M) were chosen to evaluate antioral cancer cell line effects by examining cytotoxicity, cell cycle disruption, apoptosis induction, and DNA damage. For two cancer cell lines (Ca9-22 and HSC-3), single treatment (UVC or WFA) showed about 80% viability, while a combined treatment of UVC/WFA showed about 40% viability. In contrast, there was noncytotoxicity to normal oral cell lines (HGF-1). Compared to single treatment and control, low dose UVC/WFA shows high inductions of apoptosis in terms of flow cytometric detections for subG1, annexin V, pancaspase changes as well as Western blotting for detecting cleaved poly (ADP-ribose) polymerase (c-PARP) and caspase 3 (c-Cas 3) and luciferase assay for detecting Cas 3/7 activity. Low dose UVC/WFA also showed high inductions of oxidative stress and DNA damage in terms of flow cytometric detections of reactive oxygen species (ROS), mitochondrial superoxide (MitoSOX) generation, and membrane potential (MitoMP) destruction, &gamma, H2AX and 8-oxo-2&rsquo, deoxyguanosine (8-oxodG) types of DNA damages. For comparison, low dose UVC/WFA show rare inductions of annexin V, Cas 3/7 activity, ROS, MitoSOX, and MitoMP changes to normal oral HGF-1 cells. Therefore, low dose UVC/WFA provides a novel selectively killing mechanism to oral cancer cells, suggesting that WFA is a UVC sensitizer to inhibit the proliferation of oral cancer cells.

No study has investigated the effect of learning curves on the accuracy of dental implant navigation systems. This study evaluated the accuracy of the dental implant navigation system and established the learning curve according to operation site and operating time. Each dental model was used for drilling 3 missing tooth positions, and a patient tracking module was created. The same dentist performed the drilling test for 5 sets of dental models. CT back scanning was performed on the dental models. Customized implants based on the drilled holes were inserted. The relative error between the preoperative planning and actual implant was calculated. Using the dental navigation system could help dentists position implants more accurately. This in vitro study investigated the accuracy and wear conditions of three drill sleeve distances (0.01, 0.02, and 0.03 mm) for 3D-guided stents in simulated clinical dental implant surgery. Fifteen sets of upper and lower partially edentulous epoxy tooling board models with four edentulous first molar sites were prepared in a Nissin Simple Manikin II and set on a dental chair. Sixty computer-aided design and computer-aided manufacturing (CAD/CAM) designed and guided stents with three drill sleeve distances were prepared in this study. The maximum height (Sz) of the wear roughness of drills, maximum deviation, and drilling time were observed. The highest maximum horizontal deviations were observed at the upper first molar (0.48 ± 0.12 mm, p &lt, 0.001). The highest maximum vertical and angular deviations were observed at the lower left first molar (1.08 ± 0.35 mm and 5.61° ± 1.21°, respectively, p &lt, 0.001). Only angular deviation significantly differed among the three drill sleeve distances (p = 0.046), the 0.03 mm distance exhibited the maximum angular deviation (3.92° ± 1.87°). The bigger drill sleeve distance (0.03 mm) was associated with more wear roughness (8.70 ± 2.29 µm) of the drills. Guided stents with varying drill-sleeve distances (0.01, 0.02, and 0.03 mm) exhibited no significant difference in preparation drilling time and abrasive wear. In practice, the optimal drill sleeve distance for single-type CAD/CAM-guided stents of dental implantation was 0.01 mm.

Cleidocranial dysplasia (CCD) is an autosomal-dominant malformation syndrome affecting bones and teeth. The most common skeletal and dental abnormalities in affected individuals are hypoplastic/aplastic clavicles, open fontanelles, short stature, retention of primary teeth, delayed eruption of permanent teeth, supernumerary teeth, and multiple impacted teeth. Treatment of CCD requires a multidisciplinary approach that may include dental corrections, orthognathic surgery and cranioplasty along with management of any complications of CCD. Early diagnosis of this condition enables application of the treatment strategy that provides the best quality of life to such patients. Notably, Runx2 gene mutations have been identified in CCD patients. Therefore, further elucidation of the molecular mechanism of supernumerary teeth formation related to Runx2 mutations may improve understanding of dental development in CCD. To investigate the milling properties of different yttria-tetragonal zirconia polycrystalline (Y-TZP) block materials by applying a dental computer numerical control (CNC) milling center. Low temperature sintering zirconia block denoted by KMUZ (experimental) with two commercial zirconia blocks for T block made in Taiwan and a G block made in Germany were compared for the milling properties. Seventy-two specimens were milled using the same CNC milling center, and properties were evaluated by measuring the weight loss (g), milling time (s), margin integrity (%) and broken diameter (μm). The densification and biocompatibility of sintered 3.0 mol% yttria-tetragonal zirconia polycrystal (3Y-TZP) ceramics, with X wt% Fe 2 O 3 and 5.0 wt% mica powders (denoted by 3Y-TZP: X-5.0 wt% mica) have been studied. When the pellets of 3Y-TZP: X-5.0 wt% mica were sintered at 1300 °C for 1 h, the relative shrinkage increases from 19.20–19.43% with the X increased from 0.3 to 1.0. The relative shrinkage of pellets containing 1.0 wt% Fe 2 O 3 ( X =1.0) increased from 19.43–19.59% when sintering temperatures were raised from 1300 °C to 1450 °C. X-ray diffraction results show that the pellets of 3Y-TZP: X-5.0 wt% mica sintered at 1400 °C for 1 h only contained single phase of tetragonal ZrO 2 (t-ZrO 2 ). When the sintering temperature was higher than 1400 °C, the Vickers microhardness was greatest in the pellets with X =0.5. Within pellets with the same Fe 2 O 3 content, the dominant wavelength ( λ d ) was only slightly different for pellets sintered at 1300 °C and those sintered at 1450 °C. The results of the materials were evaluated in vitro cytotoxicity tests reveals that the powders and sintered pellets are safe materials. The oral mucosa irritation tests did not find erythema or histopathological change including normal epithelium, and was free from leucocyte infiltration, vascular congestion and oedema.

Adjusting implant abutment for crown delivery is a common practice during implant installation. The purpose of this study was to compare the fracture resistance and stress distribution of zirconia specimens on four occlusal surface areas of implant abutment. Four implant abutment designs [occlusal surface area (SA) SA100, SA75, SA50, and SA25] with 15 zirconia prostheses over the molar area per group were prepared for cyclic loading with 5 Hz, 300 N in a servo-hydraulic testing machine until fracture or automatic stoppage after 30,000 counts. The minimum occlusal thickness of all specimens was 0.5 mm. Four finite element models were simulated under vertical or oblique 10-degree loading to analyze the stress distribution and peak value of zirconia specimens. Data were statistically analyzed, and fracture patterns were observed under a scanning electron microscope. Cyclic loading tests revealed that specimen breakage had moderately strong correlation with the abutment occlusal area (r = 0.475). Specimen breakage differed significantly among the four groups (P = 0.001). The lowest von Mises stress value was measured for prosthesis with a smallest abutment occlusal surface area (SA25) and the thickest zirconia crown. Thicker zirconia specimens (SA25) had higher fracture resistance and lowest stress values under 300 N loading.

The aim of this study was to compare the accuracy of implant placement by using the conventional freehand method, the surgical guide alone, the dental navigation system alone, and the dental navigation system with a surgical guide. The participants were aged 20 years or older and were requiring dental implant surgery according to an assessment made by a dentist between July 2014 and December 2017. A total of 128 dental implants were inserted, 32 dental implants in each group, and participants with similar or identical age (i.e., 20&ndash, 50 years or 50 years or above) and missing tooth locations were paired for comparison. Accuracy was measured by overlaying the real position in the postoperative Cone Beam Computerized Tomography (CBCT) on the virtual presurgical placement of the implant in a CBCT image. Using the dental navigation system with a surgical guide could help dentists to position implants more accurately. Total, longitudinal, and angular error deviation were significantly different (p &lt, 0.0001). The same level of accuracy could be obtained for the different jaws and tooth positions. The one-way analysis of variance (ANOVA) showed that the total, longitudinal, and angular errors differed significantly (p &lt, 0.0001). A comparison of the four dental implant surgical methods indicated that the combination of a dental implant navigation system and a surgical guide kit achieved the highest accuracy in terms of the different tooth positions and jaws.

This study aimed to conduct a stress analysis of four types of cervical finish lines in posterior all-ceramic crowns on the primary roots of molar teeth. Four different types of finish lines (shoulder 0.5 mm, feather-edged, chamfer 0.6 mm, and mini chamfer 0.4 mm) and two all-ceramic crown materials (zirconia and lithium disilicate) were used to construct eight finite element primary tooth models with full-coverage crowns. A load of 200 N was applied at two different loading angles (0&deg, and 15&deg, ) so as to mimic children&rsquo, s masticatory force and occlusal tendency. The maximum stress distribution from the three-dimensional finite element models was determined, and the main effect of each factor (loading type, material, and finish line types) was evaluated in terms of the stress values for all of the models. The results indicated that the loading type (90.25%) was the main factor influencing the maximum stress value of the primary root, and that the feather-edged margin showed the highest stress value (p = 0.002). In conclusion, shoulder and chamfer types of finish lines with a 0.4&ndash, 0.6 mm thickness are recommended for deciduous tooth preparation, according to the biomechanical analysis.

The aim of this study is to determine the minimum required thickness of a monolithic zirconia crown in the mandibular posterior area for patients with bruxism. Forty-nine full zirconia crowns, with seven different occlusal thicknesses of 0.4, 0.5, 0.6, 0.7, 0.8, 0.9, and 1.0 mm, were made by using a computer-aided design/computer-aided manufacturing system (CAD/CAM). Seven crowns in each group were subjected to cyclic loading at 800 N and 5 Hz in a servohydraulic testing machine until fracture or completion of 100,000 cycles. Seven finite element models comprising seven different occlusal thicknesses of 0.4, 0.5, 0.6, 0.7, 0.8, 0.9, and 1.0 mm were simulated using three different loads of vertical 800 N, oblique 10 degrees 800 N, and vertical 800 N + x N torque (x = 10, 50, and 100). The results of cyclic loading tests showed that the fracture resistance of the crown was positively associated with thickness. Specimen breakage differed significantly according to the different thicknesses of the prostheses (p &lt, 0.01). Lowest von Mises stress values were determined for prostheses with a minimal thickness of 1.0 mm in different loading directions and with different forces. Zirconia specimens of 1.0 mm thickness had the lowest stress values and high fracture resistance and under 800 N of loading.

This clinical report illustrates a modern method for oral rehabilitation of severe edentulous atrophic ridge. Shallow vestibule and small denture-bearing area of the maxilla provide insufficient retention, and only the symphyseal region of the mandible has obvious alveolar ridge. The surgery of deepening vestibule of the maxilla ridge was done by using tentative miniscrew combined with old denture and soft liner. The osseointegrated dental implants were installed over symphyseal region of the mandible to act as retentive devices for prostheses. In addition, functional reline with rebase material to build buccal-tongue contact over the retromolar area after denture delivery obtained high satisfaction from the patient. Many clinical conditions cannot be managed solely with implants or preprosthetic surgery alone, whereas a combination of the 2 can achieve successful outcome.

The purpose of this study is to analyze biomechanical interactions in the alveolar bone surrounding implants with smaller-diameter abutments by changing position of the fixture-abutment interface, loading direction, and thickness of cortical bone using the finite element method.Twenty different finite element models including four types of cortical bone thickness (0.5, 1, 1.5, and 2 mm) and five implant positions relative to bone crest (subcrestal 1, implant shoulder 1 mm below bone crest; subcrestal 0.5, implant shoulder 0.5 mm below bone crest; at crestal implant shoulder even with bone crest; supracrestal 0.5, implant shoulder 0.5 mm above bone crest; and supracrestal 1, implant shoulder 1 mm above bone crest) were analyzed. All models were simulated under two different loading angles (0 and 45 degrees) relative to the long axis of the implant, respectively. The three factors of implant position, loading type, and thickness of cortical bone were computed for all models.The results revealed that loading type and implant position were the main factors affecting the stress distribution in bone. The stress values of implants in the supracrestal 1 position were higher than all other implant positions. Additionally, compared with models under axial load, the stress values of models under off-axis load increased significantly.Both loading type and implant position were crucial for stress distribution in bone. The supracrestal 1 implant position may not be ideal to avoid overloading the alveolar bone surrounding implants.

Oral appliances are a range of devices that are designed to alter upper airway patency. The efficiency of these appliances for treatment of obstructive sleep apnea (OSA) has been rarely discussed. This case report describes two designs of mandibular repositioning appliances used to treat OSA. A 36-year-old man was diagnosed with mild OSA by his physician. A single-piece, soft oral appliance and a two-piece, acrylic appliance were used in this patient. Polysomnography was performed three times (baseline, and with the single-piece, and two-piece appliances). The apnea/hypopnea index was improved significantly with both appliances, but to a greater degree with the single-piece soft appliance. Our study concluded that oral appliances may assist in the management of OSA.

Objective To compare the results of IVF and intracytoplasmic sperm injection (ICSI) in extremely low oocyte retrieval cycles. Design Case–control study. Setting An IVF laboratory in a university hospital in Taiwan. Patient(s) Between January 2003 and December 2007, infertile couples with one, two, or three oocytes per retrieval were divided into two groups: 56 IVF cycles based on 28 demographic characteristics from the ICSI group. Intervention(s) None. Main Outcome Measure(s) Fertilization rate, cycle cancellation rate, clinical pregnancy rate, and implantation rate per transfer. Result(s) The normal fertilization rate was significantly higher after ICSI than after standard IVF (86.4% vs. 69.3%). The cycle cancellation and complete fertilization failure rates were comparable between the two groups. The clinical pregnancy rate per transfer (38.5% vs. 21.6%), implantation rate (23.5% vs. 11.1%), and live birth rate per transfer (30.8% vs. 13.7%) had a favorable trend, but no statistically significant differences, in the ICSI group compared with the IVF group. Conclusion(s) A higher normal fertilization rate can be achieved by ICSI in comparison with conventional IVF in extremely low oocyte retrieval cycles.

Clinically, many implant cases with different angulation over the lower posterior area have been found. The purpose of this study was to analyze the bony stress with different implant tilting during normal masticatory load using the finite element method (FEM), with the hope of discovering a desirable installation of implant. A three-dimensional finite element method was employed to analyze the bony stress generated by different angulation designs (15 degrees) of implant bodies. Eight solid models of the mandibular first and second molars were built up and then transferred to a mesh model in FEM (ANSYS) to perform a stress analysis. A simulated load (400 N) was applied to the splinted crowns with vertical and horizontal forces. The loading sites were on the central fossa of the splinted crowns. For stress distribution, some designs will be better than a parallel installation. The results suggested that not all implant bodies tilting with the splinted crowns lead to stress concentration.

Conical crown-retained dentures (CCRD) show a higher survival rate and greater patient satisfaction than transitional removable partial dentures during long-term follow-up. However, unsustainable denture retention force on supporting abutments after initial delivery and loss retention are frequently seen in long-term follow-up of clinical cases. The main causes are insufficient information concerning denture retention designs and the retention-tolerance of the supporting abutments. Monitoring by dental technicians of the quality of dental prostheses is critical. This case report describes an optimal method for CCRD construction that determines and distributes an optimal denture retention force on the supporting abutments to allow the patient to easily remove the denture while ensuring that the CCRD remains in place during physiologic activities. Oral rehabilitation with CCRD should consider the condition of the abutment periodontal support, the interarch occlusal relationship, supplemental fatigue of the terminal abutment, and patient's estimated bite force. The effects of friction on the abutment's inner crown were based on an optimal a angle. The dental laboratory used these measurements to fabricate a CCRD using a Koni-Meter to adjust the retention of the inner crown. This method protects the abutments and reduces the wear between the inner and outer crowns. The CCRD achieved good esthetic results and physiologic functions. Periodic long-term follow-up of the patient and CCRD after initial placement is recommended.

Objective To compare the differences between the right and left ovarian responses in patients undergoing standard in vitro fertilization (IVF) treatment. Design Prospective, observational study. Setting A university hospital in Taiwan. Patient(s) Seven hundred eighty-nine consecutive patients undergoing IVF or intracytoplasmic sperm injection. Intervention(s) None. Main Outcome Measure(s) The number of oocytes retrieved, fertilization rates, number of grade 1 zygotes and number of embryos produced, embryo arrest-free rate, and mean number of top-quality embryos on the day of transfer. Result(s) The number of follicles ≥16 mm in diameter on day 2.5 versus 2.2 of human chorionic gonadotropin (hCG) administration, the number of follicles ≥10 mm in diameter on hCG day 6.5 versus 6.1, the number of oocytes (3.6 vs. 3.0), the percentage fertilization (79.9 vs. 74.6%), and the mean number of mature oocytes and grade 1 zygotes (2.3 vs. 1.8 and 1.79 vs. 0.93, respectively), and the mean available number of top-quality embryos on transfer day 1.39 versus 0.73 were all statistically significantly greater in the right compared with the left ovary; however, the pregnancy and implantation rates were similar. Conclusion(s) There are statistically significant differences between the right and left ovarian responses in IVF patients with healthy ovaries, and the right ovarian responses are superior to the left ovarian responses.
