Abstract

OBJECTIVES:To investigate the efficacy and safety of ixekizumab for up to 52 weeks in two phase 3 studies of patients with active radiographic axial spondyloarthritis (r-axSpA) who were biological disease-modifying antirheumatic drug (bDMARD)-naive (COAST-V) or tumour necrosis factor inhibitor (TNFi)-experienced (COAST-W). METHODS:Adults with active r-axSpA were randomised 1:1:1:1 (n=341) to 80 mg ixekizumab every 2 (IXE Q2W) or 4 weeks (IXE Q4W), placebo (PBO) or 40 mg adalimumab Q2W (ADA) in COAST-V and 1:1:1 (n=316) to IXE Q2W, IXE Q4W or PBO in COAST-W. At week 16, patients receiving ixekizumab continued their assigned treatment; patients receiving PBO or ADA were rerandomised 1:1 to IXE Q2W or IXE Q4W (PBO/IXE, ADA/IXE) through week 52. RESULTS:In COAST-V, Assessment of SpondyloArthritis international Society 40 (ASAS40) responses rates (intent-to-treat population, non-responder imputation) at weeks 16 and 52 were 48% and 53% (IXE Q4W); 52% and 51% (IXE Q2W); 36% and 51% (ADA/IXE); 19% and 47% (PBO/IXE). Corresponding ASAS40 response rates in COAST-W were 25% and 34% (IXE Q4W); 31% and 31% (IXE Q2W); 14% and 39% (PBO/IXE). Both ixekizumab regimens sustained improvements in disease activity, physical function, objective markers of inflammation, QoL, health status and overall function up to 52 weeks. Safety through 52 weeks of ixekizumab was consistent with safety through 16 weeks. CONCLUSION:The significant efficacy demonstrated with ixekizumab at week 16 was sustained for up to 52 weeks in bDMARD-naive and TNFi-experienced patients. bDMARD-naive patients initially treated with ADA demonstrated further numerical improvements after switching to ixekizumab. Abstract

Synovitis, acne, pustulosis, hyperostosis and osteitis (SAPHO) syndrome is a rare, underdiagnosed disease with a wide clinical spectrum. Sterile bone inflammation, predominantly of the anterior chest, and skin manifestations (palmoplantar pustulosis, psoriasis vulgaris and acne) are the key features of SAPHO, which shares certain similarities with SpA. SAPHO is closely related to paediatric chronic non-bacterial osteitis (CNO), a spectrum of autoinflammatory bone diseases. The aetiology of SAPHO is considered multifactorial based on a complex interplay of genetic, immune and infectious factors. Despite the increasing awareness of SAPHO/CNO, diagnostic delay is common, as validated classification and diagnostic criteria are lacking. Treatment of SAPHO represents a challenge and includes anti-inflammatory drugs, antibiotics, bisphosphonates, synthetic conventional DMARDs and off-label use of anti-cytokine biologics and Janus kinase inhibitors. This review summarizes the current diagnostic and practical treatment approach to SAPHO/CNO and highlights the ongoing research endeavours concerning the definition and validation of diagnostic criteria, core domains and treatment. Abstract

Purpose: The efficacy of medial pivot total knee arthroplasty (MP TKA) in treating valgus knees that may cause medial instability is unknown. The purpose of this study was to investigate the in vivo kinematics of MP TKA for the valgus knees and compare them to those for the varus knees. Methods: The kinematics of 19 valgus knees and 19 varus knees operated in the MP TKA were investigated under fluoroscopy during squatting using a two‐ to three‐dimensional registration technique. Accordingly, the valgus and varus knees were evaluated and compared in terms of knee flexion angle, anteroposterior translation for the medial and lateral low contact points, axial rotation and valgus–varus angle of the femoral component relative to the tibial component, as well as kinematic pathways. Results: The knee flexion angle was found to be identical in both knees. There was no anterior translation on the medial side of the valgus knees, and no difference was detected between the two knees. On the lateral side, posterior translation was observed in both knees, with no difference between the two. Femoral external rotation was observed in both knees, and no difference was detected between the two. There was no valgus–varus change in the valgus knees, nor was there a difference between the two knees. The valgus knees demonstrated MP motion, whereas the varus knees demonstrated MP motion and bicondylar rollback. Conclusion: The medial side of the valgus knees treated with MP TKA showed comparable stable kinematics to the varus knees. Abstract

We study the single transverse spin asymmetry for the inclusive pion production in the nucleon-nucleon collision, p^\uparrow p\to\pi X, based on the twist-3 mechanism in the collinear factorization. We derive the soft-fermion-pole (SFP) contribution to the twist-3 single-spin-dependent cross section associated with the twist-3 quark-gluon correlation functions in the polarized nucleon. We find that the SFP can give rise to a large effect to the asymmetry A_N owing to the large partonic hard cross sections with the large color factor, if the SFP function has a similar magnitude as the soft-gluon-pole (SGP) function, in spite of the absence of the "derivative term" for the SFP function unlike for the SGP function., Comment: 16 pages, 9 figures, some references added, slight modification in discussions

Abstract

Single spin asymmetry (SSA) is a twist-3 observable in the collinear factorization approach. We present a twist-3 single-spin-dependent cross section formula for the pion production in pp-collision, p^\uparrow p\to\pi X, relevant to RHIC experiment. In particular, we calculate the soft-fermion-pole (SFP) contribution to the cross section from the quark-gluon correlation functions.
