Background Food anaphylaxis is commonly elicited by unintentional ingestion of foods containing the allergen above the tolerance threshold level of the individual. While labeling the 14 main allergens used as ingredients in food products is mandatory in the EU, there is no legal definition of declaring potential contaminants. Precautionary allergen labeling such as "may contain traces of" is often used. However, this is unsatisfactory for consumers as they get no information if the contamination is below their personal threshold. In discussions with the food industry and technologists, it was suggested to use a voluntary declaration indicating that all declared contaminants are below a threshold of 0.5 mg protein per 100 g of food. This concentration is known to be below the threshold of most patients, and it can be technically guaranteed in most food production. However, it was also important to assess that in case of accidental ingestion of contaminants below this threshold by highly allergic patients, no fatal anaphylactic reaction could occur. Therefore, we performed a systematic review to assess whether a fatal reaction to 5mg of protein or less has been reported, assuming that a maximum portion size of 1kg of a processed food exceeds any meal and thus gives a sufficient safety margin. Methods MEDLINE and EMBASE were searched until 24 January 2021 for provocation studies and case reports in which one of the 14 major food allergens was reported to elicit fatal or life-threatening anaphylactic reactions and assessed if these occurred below the ingestion of 5mg of protein. A Delphi process was performed to obtain an expert consensus on the results. Results In the 210 studies included, in our search, no reports of fatal anaphylactic reactions reported below 5 mg protein ingested were identified. However, in provocation studies and case reports, severe reactions below 5 mg were reported for the following allergens: eggs, fish, lupin, milk, nuts, peanuts, soy, and sesame seeds. Conclusion Based on the literature studied for this review, it can be stated that cross-contamination of the 14 major food allergens below 0.5 mg/100 g is likely not to endanger most food allergic patients when a standard portion of food is consumed. We propose to use the statement "this product contains the named allergens in the list of ingredients, it may contain traces of other contaminations (to be named, e.g. nut) at concentrations less than 0.5 mg per 100 g of this product" for a voluntary declaration on processed food packages. This level of avoidance of cross-contaminations can be achieved technically for most processed foods, and the statement would be a clear and helpful message to the consumers. However, it is clearly acknowledged that a voluntary declaration is only a first step to a legally binding solution. For this, further research on threshold levels is encouraged.

Aim: Although antithrombotic drugs have a routine place in the treatment of severe COVID-19 patients, it is not yet known whether the use of antithrombotic drugs for any reason before exposure to infection has an effect on the prognosis and clinical course of the disease. The aim of this study is to determine whether the use of antithrombotic drugs before exposure to SARS COV-2 has an effect on the clinical course and mortality of the disease.Material and Methods: All patients who were admitted to our hospital's COVID-19 outpatient clinic, diagnosed with COVID-19, confirmed by RT-qPCR test results, and diagnosed with COVID-19 pneumonia with a lung imaging were included in the study. Anamnesis information of patients with Covid-19 pneumonia, length of stay in hospital and intensive care unit were obtained and recorded in the hospital registry system. Clinical outcomes of all patients were recorded as survival or exitus.Results: The total number of patients with a positive PCR test result and viral pneumonia was 415. The mean age of the patients was 53 ±19 and 234 (56%) were male. While 168 (40%) of 415 patients were using at least one type of antithrombotic drug, 247 (60%) were not using any antithrombotic drug. While the mortality rate was 4.5% in 168 patients who used antithrombotic drugs before COVID-19, the mortality rate was 12.5% in 247 patients who did not use antithrombotic drugs (p= 0.008). While the rate of hospitalization in the intensive care unit was 10.1% in 168 patients who used antithrombotic drugs, the rate of intensive care was 13.7% in 247 patients who did not use antithrombotic drugs (p= 0.31).Conclusion: Antithrombotic therapy in early disease, especially in outpatients, may prevent coagulopathy and endothelial-associated disease, but administration of antithrombotic therapy too late during the thrombo-inflammatory phase may not provide the necessary protective effect. This hypothesis needs to be tested in appropriate prospective randomized trials., Amaç: Antitrombotik ilaçlar ağır COVID-19 hastalarının tedavisinde kendine rutin bir yer edinmiş olmasına rağmen, enfeksiyona maruz kalmadan önce herhangi bir sebeple antitrombotik ilaç kullanımının hastalığın prognozuna ve klinik seyrine bir etkisi olup olmadığı henüz bilinmemektedir. Bu çalışmanın amacı SARS COV-2’ye maruz kalmadan önce antitrombotik ilaç kullanımının hastalığın klinik seyri ve mortalitesi üzerine etkisi olup olmadığını belirlemektir.Gereç ve Yöntemler: Hastanemiz COVID-19 polikliniğine başvuran COVID-19 tanısı yapılan RT-qPCR test sonucu ile doğrulanan ve bir akciğer görüntülemesi ile COVID-19 pnömonisi tanısı alan tüm hastalar çalışmaya dahil edilmiştir. Covid-19 pnömonisi olan hastaların anamnez bilgileri, hastanede ve yoğun bakımda yatış süreleri hastane kayıt sisteminden elde edildi ve kayıt altına alındı. Tüm hastaların klinik sonlanımları; sağkalım veya exitus olarak sınıflandırılarak kayıt altına alındı.Bulgular: PCR testi pozitif olup aynı zamanda viral pnömonisi olan hasta sayısı toplam 415 idi. Hastaların yaş ortalaması 53±19 ve 234’ü (56%) erkek idi. 415 hastanın 168’i (40%) en az bir çeşit antitrombotik ilaç kullanmakta iken 247’si (60%) herhangi bir antitrombotik ilaç kullanmıyordu. COVID-19 öncesi antitrombotik ilaç kullanan 168 hastada mortalite oranı %4,5 iken, antitrombotik kullanmayan 247 hastada mortalite oranı %12,5 olarak bulundu (p=0.008). Antitrombotik ilaç kullanan 168 hastada yoğun bakıma yatış oranı %10,1 iken, antitrombotik kullanmayan 247 hastada yoğun bakıma yatış oranı %13,7 olarak (p=0.31).Sonuç: Erken hastalıkta, özellikle ayaktan hastalarda antitrombotik tedavi, koagülopatiyi ve endotelyal ilişkili hastalığı önleyebilir, ancak trombo-inflamatuar faz sırasında çok geç bir antitrombotik tedavi uygulaması gerekli koruyucu etkiyi sağlamayabilir. Bu hipotezin uygun prospektif randomize çalışmalarda test edilmesi gerekmektedir.

Noninvasive mechanical ventilation (NIMV) has a relevant role in the treatment of critically ill patients displaying severe dyspnea. Continuous positive airway pressure (CPAP), a method of NIMV, is also widely used in the management of acute heart failure, chronic obstructive pulmonary disease (COPD) exacerbation, and symptomatic sleep apnea. However, numerous traumatic complications of CPAP treatment in the face region, head, and thorax have been reported and may be related to the application of a continuous positive high pressure to the airway. Conversely, we have observed no complications due to CPAP-related increased intra-abdominal pressure. In this article, we describe a clinical case of a patient with an acute rectus sheath hematoma during CPAP treatment. This previously unreported complication demonstrates that CPAP should be carefully used in patients with exacerbated COPD with difficulty in expiration

Türkiye Florası içinde sucul bitkiler önemli bir yere sahiptir. Bir bölgedeki sucul bitkilerin tespit edilmesi biyoçeşitliliğin de yapısının belirlenmesine katkı sağlar. Bu çalışmada Kahramanmaraş’ta yapılan flora araştırmalarında tespit edilen sucul bitkiler belirlenerek tablo halinde verilmiştir. Kahramanmaraş florasında 30 familya ve 62 cinse ait 109 sucul bitki taksonu belirlenmiştir. Bölgede en fazla sucul bitki taksonu içeren familyalar sırasıyla; Poaceae (21), Asteraceae, Cyperaceae ve Polygonaceae (8), Onagraceae (6), Lamiaceae, Rosaceae ve Scrophulariaceae (5), Fabaceae, Gentianaceae, Plantaginaceae ve Tamaricaceae (4)’ dür., Aquatic plants important for Turkey flora. Identification of aquatic plants in a region which contributes to the determination of the structure of biodiversity. In this study, was determined aquatic plants in the Kahramanmaraş flora have been identified and are given in the table. As a result, it was determined that 109 aquatic plant taxa belonging to 30 families and 62 genera in the Kahramanmaraş flora. In terms of species number, the largest families are as following; Poaceae (21), Asteraceae, Cyperaceae ve Polygonaceae (8), Onagraceae (6), Lamiaceae, Rosaceae ve Scrophulariaceae (5), Fabaceae, Gentianaceae, Plantaginaceae ve Tamaricaceae (4).

Amaç: Bu çalışmada QTc intervalinin siroz hastalarında meydana gelen özofagus varis kanamasının mortalite ve morbidite üzerindeki etkisi araştırıldı. One of the main parts of the health system is medical residents. The residents who are new to the profession are under serious pressure due to inexperience and the difficulties related with jobs. However, it would not be correct to evaluate the physician as an employee only. Healthcare professionals are also a part of society. In this study, we aimed to determine the depression levels of medical residents. 141 medical residents were included in this descriptive cross-sectional study. Amaç:P2/MS sirozdaki karaciğer fibrozunu değerlendirmekkullanılan basit, pahalı olmayan, non-invaziv bir testtir. Hepatoselüler karsinomda artmış alfa-fetoprotein düzeyleri ise rekürrens ile ilişkilidir. Bu çalışmada hepatoselüler karsinom nüksünün erken öngörümünde P2/MS ve alfa-fetoprotein etkinliğini değerlendirmeyi ve bir cut-off değeri araştırmayı amaçladık. Gereç ve Yöntem:Hepatoselüler karsinoması olan 69 hasta retrospektif olarak rekürrens varlığı açısından tarandı. Hastalardaki rekürrens varlığı radyolojik olarak trifazik bilgisayarlı tomografi ve magnetik rezonans inceleme ile değerlendirildi. P2/MS formülü olarak trombosit sayısı (109/L)2/[monosit yüzdesi (%) x nötrofil yüzdesi (%)]. Bulgular:Toplam 42 hastada hepatoselüler karsinom rekürrensi belirlendi. Rekürrens öngörümü açısından P2/MS cut-offdeğeri ≤59.9 alındığında %100 spesifite ve %95.24 sensiviteye sahip olduğu bulundu. Alfa-fetoprotein için cut-off değeri >57.5 alındığındaysa sensivitesi %83.3 ve spesifitesi %92.6 saptandı. Sonuç:Bu bulgular ışığında P2/MS ve alfa-fetoproteinin hepatoselüler karsinom rekürrensini öngördürmede güvenilir belirteçler gibi değerlendirilebilmektedir. Elevated alpha-fetoprotein levels in hepatocellular carcinoma is related with recurrence. We aimed to examine the efficacy of P2/MS and alpha-fetoprotein in early prediction of hepatocellular carcinoma recurrence and to determine a cut-off value for P2/MS and alpha-fetoprotein. Methods: Sixty nine patients with hepatocellular carcinoma were retrospectively scanned for recurrence existence. The recurrence of hepatocellular carcinoma was diagnosed radiologically, via 3-phase tomography and magnetic resonance investigation. The formula of P2/MS was platelet count (109/L)2/[monocyte count (%) x neutrophil count (%)]. Results: Forty two patients had hepatocellular carcinoma recurrence. For the recurrence prediction, the cut-off value of P2/MS was ≤59.9 with 100% specificity and 95.24% sensitivity. The cut-off value of alpha-fetoprotein was found >57.5 with 83.3% sensitivity and 92.6% specificity. Conclusion: With these findings, P2/MS and alpha-fetoprotein seems as reliable markers for the prediction of hepatocellular carcinoma recurrence. We suggest that they can be used as non-invasive tools for the early estimation of hepatocellular carcinoma recurrence. Key Words: P2/MS, AFP, hepatocellular carcinoma, recurrence.

AMAÇ: İntihar, önlenebilir bir ölüm nedenidir. İntiharın önlenmesi için vaka verilerinin iyi analizi esastır. Bu çalışmanın amacı acil servise başvuran intihar olgularının klinik ve sosyodemografik özelliklerinin incelenmesidir.GEREÇ ve YÖNTEMLER: Bu çalışma geriye dönük olarak gerçekleştirilmiş tanımlayıcı bir çalışmadır. “Bir kamu hastanesi acil servisine 1 Ocak 2015 - 31 Ekim 2018 tarihleri arasında, intihar veya intihar girişimi nedeniyle başvuran hastaların dosyaları, intihar formları ve elektronik kayıtları analiz edilmiştir.BULGULAR: Uygun kayıtlara sahip 168 vakanın 92 si analiz edildi. İntihar vakalarının % 65.2'sini kadınlar oluştururken, % 69.6'sı 15-29 yaş aralığında ve % 40'ı öğrenci idi. En yaygın intihar yöntemi ilaç-toksik madde alımı (% 84.8) ve en yaygın intihar nedenleri aile sorunları (% 26.1) ve akıl hastalıkları (% 16.3) idi. Good analysis of case data is essential for the prevention of suicide. The aim of this study is to investigatethe clinical and socio-demographic characteristics of suicide cases admitted to emergency department.MATERIALS and METHODS: This is a descriptive study performed retrospectively. The files, suicide forms and electronic records of patients who admitted to a public hospital emergency department between 1 January 2015 and 31 October 2018 due to suicide or attempted suicide were analyzed.RESULTS: Ninety-two of the 168 cases with appropriate records were analyzed. Women represented 65.2% of the suicide cases, 69.6% were aged 15-29, and 40% were students. The most common method of suicide was drug-toxic substance use (84.8%), and the most common reasons for suicide were family problems (26.1%) and mental illnesses (16.3%). No previous history of attempted suicide was present in 89.1% of cases, while 13% had a history of medication use due to psychiatric disease within the previous six months.CONCLUSION: Detailed analysis of cases must constitute the first step in suicide prevention. The quality of data obtained from hospitals’ electronic databases and patient records therefore needs to be increased.
