Recent advances in nanotechnology are expected to assist scientists in solving many key obstacles in biological complications. During the last decade, a great deal of research focused on the application of different types of nanoparticles in various areas of medicine, including imaging, diagnosis and therapy. Among various types of nanomaterials, inorganic ones have centered a lot of attention due to their unique physical and chemical characteristics, mechanical stability, decent surface area and size flexibility. Customizable particle size, a variety of morphologies and microporous structure of inorganic nanoparticles allow to deliver drug molecules and pass through biological barriers which make them promising candidates for various biomedical applications. Topical medications are applied through the skin or mucous membranes in order to achieve a local therapeutic effect. There is now a lot of research being done on inorganic nanoparticles and their possible applications in topical and transdermal drug delivery. With the influence of many factors such as the aging of the population, the younger smokers, and the serious air pollution, the incidence of chronic respiratory diseases is increasing year by year. In the treatment of respiratory diseases, clinical intervention is still mainly based on drug control of pulmonary symptoms. However, systemic drugs have disadvantages such as many adverse reactions and severe systemic side effects. In recent years, the research and development of local drug delivery systems for the respiratory tract has brought new changes to the treatment of respiratory diseases. Locally delivered drugs can directly act on the airways and have the characteristics of fast onset, good curative effect and small side effects. It is a simple, efficient and safe treatment method, which has a very significant effect, and has become a hot topic of current research and promotion. This paper briefly reviews the development track and latest research progress of respiratory local drug delivery systems at home and abroad, in order to provide reference for clinical workers in drug selection and application. Halobetasol propionate foam has been established as an efficacious and easy-to-use topical treatment for adults with plaque psoriasis. Its recent approval in the United States expanded its use for adolescents from ages 12 to 17 years old. We briefly summarize the chemistry of halobetasol and review clinical trials involving halobetasol propionate 0.05% foam to evaluate its efficacy and safety profile with a specific focus on adolescents with plaque psoriasis. Halobetasol propionate 0.05% foam is an effective and cosmetically elegant superpotent topical corticosteroid, with a tolerable safety profile in adolescents. The use of this foam offers another option to address patient-specific needs and preferences, adding to the toolbox of currently available treatments for adolescent psoriasis. [ABSTRACT FROM AUTHOR]

Scabies and hair lice are parasitic diseases that affect human skin and hair, respectively. The incidence and resistances of these infections are increasing. Tenutex® (disulfiram and benzyl benzoate emulsion) is an alternative to standard insecticides to avoid resistances. The aim of the work is to evaluate the transdermal absorption and the in vitro efficacy against scabies and hair lice after different exposition times. Dermatomed human skin was used to assess the dermal absorption using a validated High Performance Liquid Chromatography (HPLC) method. HEK001 keratinocytes were used to evaluate the cytotoxicity of benzyl benzoate. Only benzyl benzoate was able to cross the skin, but it did not show cytotoxicity at any of the tested concentrations. The product efficacy was tested on Psoroptes ovis after direct contact and after administration on sheep skin explants at different contact times. Permethrin/malathion-resistant strains of Pediculus humanis capitis adults and eggs were directly exposed to Tenutex, and the vitality and hatchability, respectively, were evaluated. The anti-scabies study demonstrated that exposure for 6 or 24 h completely eradicated the parasite. The pediculicidal activity of Tenutex exhibited superior efficacy than standard treatment on resistant lice. Itch (or pruritus) is an unpleasant sensation, inducing the desire to scratch. It is also a major and distressing symptom of many skin and systemic diseases. The involvement of histamine, which is a major itch mediator, has been extensively examined. Recent studies suggest that histamine-independent pathways may play roles in chronic itch. Therefore, antihistamines are not always effective in the treatment of patients with chronic itch. The development of biologics and κ-opioid receptor (KOR) agonists has contributed to advances in the treatment of itch; however, since biologics are expensive for patients to purchase, some patients may limit or discontinue their use of these agents. Furthermore, KOR agonists need to be prescribed with caution due to risks of side effects in the central nervous system. Janus kinase (JAK) inhibitors are sometimes associated with side effects, such as infection. In this review, we summarize antidepressants, antineuralgics, cyclosporine A, antibiotics, crotamiton, phosphodiesterase 4 inhibitor, botulinum toxin type A, herbal medicines, phototherapy, and acupuncture therapy as itch treatment options other than antihistamines, biologics, opioids, and JAK inhibitors; we also explain their underlying mechanisms of action. Objective: Topical corticosteroid irrigation plays critical role in the management of chronic rhinosinusitis (CRS). Yet, its efficacy can be highly variable. We sought to determine if personalized, 3-dimensional (3D)-printed nasal models can optimize head positioning and irrigation parameters, therefore improving patient outcomes., Study Design: Randomized, single-blinded clinical trial., Setting: Tertiary medical center from November 2021 to July 2023., Methods: Sixty-two patients with CRS were randomized into either control (CG), backfill (BG), or model (MG) groups; daily 2 mg mometasone irrigations were then performed for 2 months with either standard head-forward and natural side-tilt position (CG), a head tilt of 90° to the side with fluid entering the lower nostril (BG), or in an optimized position as determined by a patient-specific 3D printed irrigation model (MG), respectively., Results: A total of 36 patients completed the trial (CG: N = 14/23; BG N = 11/23, MG: N = 11/16). Significant posttreatment improvement in Lund-Mackay (LM) scoring was only observed in the MG (-3.73, 95% confidence interval = -5.71, -1.75; P  < .001). Patient-reported outcome measures (Nasal Obstruction Symptom Evaluation, Sinonasal Outcome Test-22, and Visual Analog Scale of nasal congestion) improved significantly among all groups. Optimal model penetration scores significantly correlated to posttreatment MG LM score (Spearman's r  = 0.65, P  < .05). Among all groups, patients with prior endoscopic sinus surgery (ESS) (n = 19) had objectively less opacification at baseline; however, experienced the same degree of opacification reduction and symptom reduction as those without prior ESS (n = 17)., Conclusion: The use of 3D printing to personalize head positioning may significantly improve objective corticosteroid irrigation outcomes. Meibomian gland (MG) dysfunction and glaucoma are very prevalent ocular conditions that significantly impact patients' quality of life. A growing number of clinical and experimental studies have reported an association between the use of topical eye drops to reduce intraocular pressure and the development or exacerbation of MG dysfunction. Studies investigating the impact of glaucoma eyedrops on MG diagnostic parameters have shown variable results, particularly regarding the morphology and function of MG and tear film instability. Herein, we corroborated the findings of greater changes in morphological and functional variables related to MG in patients treated with antiglaucoma eye drops through a systematic review and meta-analysis., (Copyright © 2024. ObjectiveAs monotherapy is insufficient for some patients, the existing fixed-dose combination (FDC) requires two or more daily administrations with declining adherence. The present study compared the efficacy and safety of netarsudil/latanoprost FDC with monotherapy of its individual components in patients with glaucoma.MethodsA systematic literature search was performed for studies comparing netarsudil/latanoprost fixed-dose combination (FDC) vs. monotherapy in patients with glaucoma. The primary endpoints included intraocular pressure (IOP), intraocular pressure reduction percentage (IOPR%) and adverse events (AEs).ResultsThree randomized controlled trial studies (RCTs) involving 1,692 patients (FDC: 556, netarsudil: 577, latanoprost: 559) were included in this meta-analysis. FDC was more effective than netarsudil, with significantly lower diurnal IOP over three time points (8:00 a.m., 10:00 a.m., 4:00 p.m.), mean diurnal IOP (MD = −2.36 [−3.08, −1.63], P < 0.00001) and higher IOPR% (MD = 9.60 [7.86, 11.33], P < 0.00001). When comparing FDC with latanoprost, both mean diurnal IOP (MD = −1.64 [−2.05, −1.23], P < 0.00001) and diurnal IOP across 3 time points were significantly lower with FDC than with latanoprost, while FDC induced significantly higher IOPR% (MD = 6.09 [4.40, 7.77], P < 0.00001). Incidence of total AEs was similar between netarsudil and FDC, but higher with FDC than with latanoprost.ConclusionNetarsudil/latanoprost FDC appears to be superior to netarsudil or latanoprost alone, with better ocular hypotensive effects. Stearyl alcohol is found in various cosmetics and topical medications and is regarded as safe. Allergic contact dermatitis is reported due to this chemical on rare occasions. We report seven cases, comprising three men and four women aged between 36 and 62 between the years 2013 to 2019, of allergic contact dermatitis due to the use of topical medication, where the patient showed a positive result to a patch test using stearyl alcohol. There were 10 topical medications that we considered to be the cause of this: three were from Oronine® H ointment, two from Eurax® cream, one from Eurax H cream, and four from topical antifungal medications. All these medications contained stearyl alcohol. Seven cases of patch tests with stearyl alcohol all showed positive results. Moreover, having done a patch test with cetyl alcohol, two out of three tests showed positive. When researching allergic contact dermatitis due to topical medications, it is important to test for allergy to stearyl alcohol as well as their main ingredients, because it is contained in numerous products and has the ability to cause allergic contact dermatitis. Purpose of Review: Traumatic neuromas in general, and trigeminal traumatic neuromas in particular, are relatively rare entities originating from a damage to a corresponding nerve or its branches. This manuscript is a comprehensive review of the literature on trigeminal traumatic neuromas based on an interesting and challenging case of bilateral intraoral lesions. Recent Findings: The diagnosis for this patient was bilateral trigeminal traumatic neuromas. It is possible that these patients have a genetic predisposition to the development of these lesions. It is a neuropathic pain condition and may mimic dental and other trigeminal pain entities. Topical treatment with lidocaine gel, utilizing a custom-made neurosensory stent, rendered the patient significant and sustained pain relief. Summary: Trigeminal traumatic neuromas present a diagnostic challenge even to a seasoned clinician, due to the complex clinical features that may mimic other entities. Topical medications such as local anesthetics may be a good viable alternative to systemic medications to manage the pain associated with the condition. Oral traumatic neuromas (TrNs) are relatively rare lesions and they originate from a damaged nerve. They present a diagnostic challenge, due to the complex clinical features that may mimic odontogenic, musculoskeletal, and other neuropathic pain conditions. We describe an interesting and challenging case of painful bilateral intraoral lesions in a 56-year-old South Indian female patient who presented with clinical features consistent with TrN lesions bilaterally, in relation to different branches of the trigeminal nerve. The patient had undergone numerous aggressive dental treatments and interventions over the past three decades, with little or no pain relief. Topical treatment with lidocaine gel utilizing a custom-made neurosensory stent rendered the patient significant and sustained pain relief.

Cataract surgery requires the use of multiple topical eye drops application in the pre and postoperative period. Certain strategies offer the patient and surgeon assured drug delivery without topical eye drop instillation avoiding the pitfalls of patient drug compliance, incorrect instillation, and topical drop-associated side effects. The aim of the current review is to evaluate the aspects of drop-free cataract surgery starting from intraoperative pupil dilation to intravitreal drug instillation.

Pharyngalgia is one of the most common complaints in both children and adults seeking medical advice. The causes of throat discomfort and pain vary and are explained by anatomo-physiological characteristics of pharynx. The lesion of pharyngeal mucosa and mucosal associated lymphoid tissue develops in 80% of acute respiratory disease cases. Detailed history taking and additional tests are required to confirm the etiology of pharyngalgia that is not related to respiratory infection and to provide appropriate treatment for the patient. Fixed topical combination with complex effectiveness established itself as a leading choice of inflammatory pharynx diseases treatment. Such treatment has the following advantages: the precise point of drug effect, minimal amount of side effects or its complete absence and risk reduction of general micro biome affection. The treatment should also be able to reach hard-to-reach areas and have good sensory properties. The purpose of topical treatment in patients with pharyngeal pathology is to relieve pain quickly, to reduce the inflammation and to prevent secondary infection of mucosa. The drug containing tyrothricin as antibiotic, benzocaine as antiseptic provides antibacterial and benzalkonium chloride as antiseptic, antiviral and analgetic effects. Not only do the early diagnosis and treatment meet the need of the patient to relieve the symptoms quickly but also have a positive impact on disease course and prevent the development of complications.

Purpose: To evaluate adherence to topical medication in patients with inflammatory eye disease.Methods: Questionnaire survey of patients attending inflammatory eye disease clinics. Treatment regimen was validated against hospital-generated clinic letters.Results: There were 86 patients (52 uveitis and 34 ocular surface disease) with 30% (26/86) failing to identify one or more of the medications they were using, and 28% (24/86) unable to offer the correct indication for their treatment. A total of 64% (55/86) failed to use their medication as advised (27% on a daily basis); the commonest reason being forgetfulness. In patients using multiple eye drops, 26% left insufficient time intervals between successive eye drops, and 58% (50/86) reported not being given any instruction on drop instillation.Conclusions: We highlight poor adherence to topical medication in patients with inflammatory eye disease. Treating progressive glaucoma can also be confounding for physicians, particularly when management relies on assumption of adherence. This study investigated associations between self-reported adherence and frequency of medication changes due to glaucoma progression.Methods: A total of 128 participants with primary open angle glaucoma were recruited from glaucoma clinics in Flinders Eye Center, South Australia, and completed confidential questionnaires. Information was obtained regarding beliefs about glaucoma and their treatment. Adherence was assessed using the four-item Morisky, Green and Levine Medication Adherence Questionnaire (MGL). Medical records were analyzed for the number of medication changes, due to glaucoma progression.Results: Adherence to topical glaucoma medication was categorized as ‘high’ in 41.4% (Morisky, Green and Levine (MGL). Data were analyzed for behaviors affecting adherence, history of adherence, and reasons for changed adherence. Chi-squared test demonstrated that there was no significant association noted between adherence and changes in medication regime (χ2 (2,128)=0.968, P=0.915); however, a significantly lower adherence was detected if participants had difficulties with their drop regime (χ2 (2,128)=7.24, P=0.027) or had help with drop insertion (χ2 (1,128)=9.77, P=0.008).Conclusion: This study revealed a higher rate of non-adherence than has previously been demonstrated in other studies. This may be attributed to the unique design of the confidential questionnaire and the independent and sympathetic questioning techniques used. Basal cell carcinoma (BCC) is the most common nonmelanoma skin cancer. It originates from undifferentiated cells in the basal cell layer of the epidermis or from the outer root sheath of the hair follicle. The most important factor in development of BCC is ultraviolet radiation. Surgery is considered the gold standard of treatment for BCC. However, nonsurgical options are available for individuals who are unsuitable for surgery. The purpose of this review is to summarize the efficacy and indications of alternative, nonsurgical treatments that can be used in the management of BCC. An extensive literature review was performed for the nonsurgical options for the treatment of BCC. Resources searched included PubMed and Google Scholars, limited to the years 1995 to 2020. Key words searched included BCC, destructive methods, photodynamic therapy (PDT), radiotherapy, topical medication, laser, hedgehog pathway inhibitors (HPIs). The most relevant results such as systematic reviews, randomized controlled trials, or comparative studies were selected to provide a summary for the most common nonsurgical methods used for treating BCC. Effective nonsurgical treatments for BCC include destructive methods (eg, curettage alone, cryosurgery, or electrodessication), PDT, topical medications, radiotherapy, laser, and HPIs. Nonsurgical therapeutic alternatives are safe and effective for the treatment of BCC. Factors such as tumor location, size, and histopathological subtype should be taken into consideration when selecting optimal treatment. AIM:To document the change in pattern of topical anti-glaucoma medicine by retrospective analysis in the outpatient of a local tertiary eye hospital(Second People's Hospital of Foshan)in China during 2009-2016. METHODS: The data of topical anti-glaucoma medications in outpatients in our hospital were collected from January 1st 2009 to December 31st 2016. These data were analyzed retrospectively. The main outcome measures were the percentages of prescriptions for all kinds of anti-glaucoma medications, the ratio of the mono-therapy and combination therapies, and the defined daily doses. RESULTS: The ratio of beta-adrenergic receptor blocker declined from 81.51% in 2009 to 69.37% in 2016. The use of cholinergic agent decreased from 12.04% in 2009 to 3.78% in 2016. The prostaglandin derivatives increased from 8.40% in 2009 to 31.40% in 2016. Alpha 2 adrenergic receptor agonists increased from 0 in 2009 to 29.44% in 2016. Local carbonic anhydrase inhibitors changed slightly. The proportion of mono-therapy was reduced from 65.83% in 2009 to 46.22% in 2016. The ration of two agents combination increased from 32.49% in 2009 to 38.77% in 2016. The three medications combination was increased from 1.68% in 2009 to 12.96% in 2016. The proportion of four medications combination increased from 0 in 2009 to 1.97% in 2016, while the combined use of five drugs was an isolated case. CONCLUSION: The proportion of topical anti-glaucoma agents has changed remarkably. The beta-adrenergic receptor blockers and cholinergic medications are decreasing, while the prostaglandin derivatives and alpha 2 adrenergic receptor agonists are increasing. The proportion of mono-therapy is declining, while the combined therapies are rising.

Purpose: To critically discuss the randomized clinical trials (RCTs) on glaucoma medical therapy for the management of pediatric glaucoma. Methods: RCTs on glaucoma drugs carried out on pediatric subjects with ocular hypertension and glaucoma were identified through systematic searches. The methods of the RCTs and the safety and the efficacy of the glaucoma drugs were reviewed and discussed. Results: We included five RCTs. One study compared dorzolamide with 0.5% timolol gel; one brinzolamide with 0.5% levobetaxolol; one 0.25% betaxolol, 0.25% timolol gel, and 0.5% timolol gel; one latanoprost with 0.5% timolol; and one travoprost with 0.5% timolol. The primary outcome was safety for two studies and efficacy for three studies. None of the RCTs was powered to detect statistically significant differences in intraocular pressure (IOP) between treatments. In total, 658 subjects received at least one dose of study medication. Beta-blockers were administered to 359 patients, carbonic anhydrase inhibitor (CAI) to 154, and prostaglandins to 145 patients. IOP-lowering efficacy ranged from 20 to 23% for CAI, from 9 to 36% for beta-blockers, and from 26 to 27% for prostaglandins. The percentage of responders was 50% for CAI, ranged from 38 to 74% for beta-blockers and from 60 to 83% for prostaglandins. Two patients receiving timolol experienced a systemic, drug-related serious adverse event (one patient bradycardia and one pneumonia). Systemic, nonserious drug-related events occurred in 15 patients randomized to beta-blockers and in 8 patients randomized to CAI. No adverse events occurred in children treated with prostaglandins. Conclusion: RCTs that are available on medical therapy for glaucoma are few and underpowered. The proportion of responders is lower in children; however, in subjects who are responders, the efficacy of glaucoma drugs seemed to be comparable to that in adults. As systemic adverse events have been reported, including serious events with timolol, a particular attempt to minimize the absorption of the drug (using the lowest dose and the gel formulation of beta-blockers or considering the lacrimal punctum occlusion) and a follow-up that is more frequent and more focused on safety should be considered in pediatric subjects who are on topical glaucoma medications. Introduction: Topical corticosteroids, available in an array of vehicles are used to control a variety of inflammatory skin diseases. Patients preferences for different vehicles may affect their willingness to use treatment. We assess corticosteroid vehicle preference and potential impact of topical characteristics on adherence and quality of life in patients with psoriasis. Methods: Subjects with psoriasis were recruited from Wake Forest University Dermatology Clinic. Subjects sampled desoximetasone 0.25% spray, betamethasone valerate 0.1% cream, triamcinolone acetonide 0.1% ointment, fluocinonide 0.05% gel, betamethasone valerate 0.1% lotion, clobetasol propionate 0.05% foam, and fluocinonide 0.05% solution in a predetermined randomized order. Subjects completed a Vehicle Preference Measure, Determinants of Adherence Measure, and a Determinants of Quality of Life Measure. Results: Patients preferences for the various products were highly variable. Regarding Determinants of Adherence, patients perception of absorption of the medication was ranked as 'quite important/extremely important' by 85% of total subjects. A majority of patients rated medication side effects as 'quite important/extremely important' when asked to consider topical characteristics effect on quality of life. Discussion: There was wide variation in patient preference for topical medication vehicles used for treating psoriasis. Several vehicle characteristics were considered important to adherence. Given the marked variation in vehicle preference, topical treatment should be individualized according to patients preferences. [ABSTRACT FROM AUTHOR]

Psoriasis is an inflammatory, autoimmune dermatosis with multifactorial etiology and chronic evolution, with major repercussions on the patient's quality of life (QoL). The main therapeutic purpose in psoriasis is to alleviate the symptomatology of the disease, to prevent recurrences and to increase the patient's QoL. The therapeutic management of psoriasis depends on the severity of the disease. If in moderate and severe forms, topical therapy is inefficient and not considered feasible, in mild forms of psoriasis, with few plaques, topical therapy is considered a first-line treatment. Topical treatments may be indicated during psoriasis outbreaks as new lesions show up. Conventionally, the topical therapy indicated for patients with psoriasis includes corticosteroids, vitamin D analogues (calcipotriol), retinoids, calcineurin inhibitors, anthralin and tar. Although the topical medication of this condition shows several advantages, mentioning the reduced risk of side effects and treatment compliance, however, the disadvantages of this therapy should also to be highlighted, the most important being the fluctuating bioavailability determined by the skin barrier function. According to public statistics, approximately 80% of patients diagnosed with psoriasis benefit from topical therapy, but the efficiency of conventional formulations is an intense topic studied by researchers in the medical-pharmaceutical field. The development of nano pharmaceutical formulations in close correlation with the influence of nanotechnology in pharmaceutical research opens new perspectives in optimizing topical medication of psoriasis. With the advanced research of the pathophysiology of psoriasis, topical nanoformulations are studied both as administration systems for conventional drug substances, but especially for the newest therapeutic agents included in gene medication.
