Background: Jaundice is a common and benign problem in neonatal period. Several therapeutic procedures for decreasing of serum bilirubin level has been recommended. phototherapy is most common them. Our goal Form this study is the evaluation of serum therapy effects in decreasing of serum bilirubin concentration in icteric infants that are treated with phototherapy. Methods: This is a prospective clinical trial in Najmeih Hospital in 2002. In this study 80 term icteric infants with bilirubin level greater than 17 mg/dl were randomized in two groups, both groups underwent phototherapy and in the case group intravenous fluid supplementation was added. There were no significant differences in the mean gestational age, birth weight, hemoglobin, and also in total serum bilirubin level at admission in the two groups. Results: There were no significant differences in the mean rate of of serum bilirubin level decline during first 24 and 48 hours of hospitalization and also the time of bilirubin decreasing to less than 15 mg/dl and the length of hospitalization in two groups. Conclution: Our study showed intravenous fluid supplementation could be limited to special cases of neonatal icter such as moderate to severe dehydration.

This cross-sectional descriptive study was undertaken to know the rate of intestinal parasites infestations in our school children population. A sum of 1155 fecal samples were analyzed from an equal number of children whose age were between 6-11 years old. The percentage of infestations were: Giardia lamblia (14.11%), hymenolepis nana (1.21%), ascaris lumbricoides (0.08%) and enterobius vermicularis (0.08%). We found no differences with regard to sex, age, father and mother levels of education, family size, body mass index (BMI) and the erythrocyte sedimentation rate (ER) between the stools positive and the stools negative cases. Nearly all cases of giardiasis (99.4%) were symptomatic. Eosinophilia was seen in (5.5%) of the infested children with giardiasis and in (0.5%) of the non-infested children, the difference was statistically non-significant (P=0.056). All cases of giardiasis (N=163) were treated with metronidazole 15 mg/kg/day in 3 divided doses for 7 days. Metronidazole was effective in (92.2%) of the treated children.

Since pebrine disease, as the most important and dangerous disease in silkworms, spreads horizontally through the spores and vertically through the eggs, combating the disease and eliminating it completely from livestock production has been associated with numerous problems. This project aimed to identify the molecular cause of pebrine disease in silkworms using a sensitive, specific, and accurate method. To this purpose, a 136 bp fragment was selected based on the Nosema bombycis partial SSU rDNA sequence, and a pair of primers was designed. Afterward, using the conventional polymerase chain reaction (PCR) method, the target fragment was amplified and sequenced. After that, to determine the detection sensitivity, using the Real-Time PCR method, 5-fold serial dilutions of N. bombycis DNA were prepared, and the last dilution that produced a fluorescent signal was considered the minimum detection limit. All tests were performed in duplicates. Based on the results of the sensitivity test, the standard curve including Ct values and DNA concentration was used for analysis. Moreover, 80 unknown samples examined by light microscope were evaluated using conventional PCR and Real-Time PCR. Both PCR results showed no amplification for the negative control samples. The findings demonstrated that the lowest detection limit for N. bombycis was less than 6 pg of DNA, while, this amount was 8 ng for conventional PCR. Out of 80 samples examined, 55, 60, and 62 samples were positive for light microscope, conventional PCR, and Real-Time PCR methods, respectively. The findings suggested that the Real-Time PCR method had a higher ability to detect the causative agent of pebrine disease than the conventional PCR method, and both methods were superior to light microscopy. Therefore, due to the fewer steps and higher accuracy of Real-Time PCR, it can be introduced as a suitable method for diagnosing pebrine disease. Objective Febrile seizure is the most common convulsive disorder in childhood. The role of iron in metabolism of neurotransmitters and carrying oxygen to the brain suggests the possibility of a relationship between iron deficiency anemia and febrile seizures. The aim of this study was to investigate the relationship between iron deficiency anemia and febrile seizures. Materials & Methods This case - control study was performed on 132 cases and 88 controls, aged 9 months to 5 years, from July 2007 to June 2009 in Baqyiatallah Hospital. Patients were selected using simple random sampling. The case group included children with first febrile seizure (core temperature over 38.5°C during seizure) without a central nervous system infection or an acute brain insult. The control group included children suffering from a febrile illness without seizure. Iron deficiency anemia was defined with one of these laboratory indexes: 1) Hemoglobin (Hb) <10.5mg/dl 2) Plasma ferritin <12ng/dl 3) Mean corpuscular volume (MCV) <70 fl. The data collected from patients were analyzed with SPSS.13 software. Results Low plasma ferritin was found in 35 cases (26.5%) compared to 26 controls (29.5%), low Hb level was found in 4 cases (3%) compared to 6 controls (6.8%) and low MCV was found in 5 cases (3.8%) compared to 6 controls (6.8%). There was no significant difference in plasma ferritin , Hb level and MCV indices between the two group. Conclusion Considering the above-mentioned results, there is no relationship between iron deficiency anemia and febrile seizures. [ABSTRACT FROM AUTHOR]

Objectives: Recent randomized controlled trials (RCTs) have studied the potential effect of the topical use of sesame oil (SO), obtained from the sesame plant seeds (Sesamum indicum L., Pedaliaceae family), in preventing or alleviating the symptoms of infusion-related phlebitis (IRP); nevertheless, their data are inconsistent. Thus, this review sought to qualitatively and quantitatively synthesize data from all available RCTs concerning the effect of the topical administration of SO on managing IRP., Methods: The online databases were searched up to July 13, 2024. Studies were eligible if they compared administering standard care plus topical SO to applying an alternative modality and/or standard care. The Cochrane risk-of-bias tool and GRADE framework were employed to appraise the quality of the evidence., Results: Eight studies of 755 records in the initial search met the inclusion criteria, which investigated inpatients and/or outpatients with chemotherapy-induced phlebitis (n = 6) and amiodarone-induced phlebitis (n = 2). According to the quantitative analysis, adults who had received coadministration of standard care and topical SO on the infusion site were significantly less affected by IRP than those who had received a control condition (effect sizes= 5, risk ratio= 0.54; 95 % confidence interval[0.32, 0.92]; P = 0.025). Also, based on the qualitative syntheses, SO can potentially prevent the formation of advanced stages of IRP, delay the appearance of IRP symptoms, and reduce IRP-induced pain severity., Conclusion: Topical SO had a favorable effect on caring for adults with IRP. However, uncertainty remains because the evidence quality was moderate, some RCTs needed better methodological rigor, and most required to address the safety of the intervention or independent verification of SO used in terms of purity and potency. Background: 18-Fluoro-deoxyglucose positron emission tomography/computed tomography (FDG-PET/CT) is a valuable imaging tool widely used in the management of cancer patients. Deep learning models excel at segmenting highly metabolic tumors but face challenges in regions with complex anatomy and normal cell uptake, such as the gastro-intestinal tract. Despite these challenges, it remains important to achieve accurate segmentation of gastro-intestinal tumors., Methods: Here, we present an international multicenter comparative study between a novel organ-focused approach and a whole-body training method to evaluate the effectiveness of training data homogeneity in accurately identifying gastro-intestinal tumors. In the organ-focused method, the training data is limited to cases with intestinal tumors which makes the network trained with more homogeneous data and with stronger presence of intestinal tumor signals. The whole body approach extracts the intestinal tumors from the results of a model trained on the whole-body scans. Both approaches were trained using diffuse large B cell (DLBCL) patients from a large multi-center clinical trial (NCT01287741)., Results: We report an improved mean(±std) Dice score of 0.78(±0.21) for the organ-based approach on the hold-out set, compared to 0.63(±0.30) for the whole-body approach, with the p-value of less than 0.0001. At the lesion level, the proposed organ-based approach also shows increased precision, recall, and F1-score. An independent trial was used to evaluate the generalizability of the proposed method to non-Hodgkin's lymphoma (NHL) patients with follicular lymphoma (FL)., Conclusion: Given the variability in structure and metabolism across tissues in the body, our quantitative findings suggest organ-focused training enhances intestinal tumor segmentation by leveraging tissue homogeneity in the training data, contrasting with the whole-body training approach, which, by its very nature, is a more heterogeneous data set., Competing Interests: Declarations. This study aimed to review the efficacy of commonly used supplements on the health status and performance of football players. We searched several databases for relevant publications published in English up to June 2024 using keywords such as nutritional supplementation, performance, football, and football players. The available literature indicated that l-arginine supplements may improve cardiovascular function, ventilation, the serum level of lactate, and maximal oxygen consumption (VO 2 Max) of football players. Branched-chain amino acids (BCAAs) may improve multiple-choice reaction time. Creatine supplements improve jumping, sprinting, change of direction speed, and reduce fatigue. Among the vitamins, vitamin D improves the serum level of injury-related hormones such as cortisol and testosterone. B vitamins reduce the level of blood lactate in the recovery periods. Vitamin E and vitamin C may improve performance by decreasing oxidative stress and increasing the antioxidant capacity and oxygen delivery to exercising skeletal muscles. Iron supplementation improves endurance capacity. Background: Non-communicable diseases in adulthood are reported to be strongly associated with adolescent obesity. The present study aimed to assess the effect of a comprehensive lifestyle modification intervention on the anthropometric indices, dietary intake, and physical activity of adolescent boys with overweight., Methods: This pragmatic trial was conducted on 126 adolescent boys with overweight. A comprehensive school-based intervention was carried out at two levels for four months: at the school level based on the Ottawa Charter framework and at the personal level including individualized diet and physical activity., Results: The percentage of mean difference of BMI and BF loss in the intervention group were 1.20% (P < 0.05) and 6.41% (P < 0.01), respectively. The intervention group had a lower intake of calorie (2350 ± 861 vs. 2634 ± 917 kcal/d), carbohydrate (255.68 ± 41 vs. 286.97 + 47 g/d), and fat (112.67 ± 78 vs. 217.72 ± 86 g/d) after the intervention compared to the control group (All P < 0.05). The intervention resulted in a significant increase in physical activity parameters including distance (3501 ± 287 vs. 614 ± 56 m/d), duration (203 ± 35 vs. 72 ± 10 min/d), and calorie expenditure (359.24 ± 62.4 vs. 46.74 ± 7.5 kcal/d) in the intervention group compared to the control group (P < 0.01)., Conclusion: A comprehensive lifestyle modification intervention which covers both school and individual levels may positively influence BMI, body fat, calorie intake, and distance and duration of physical activity in adolescents. Thus, adopting multifaceted strategies using the Ottawa Charter framework may be considered as an effective approach to managing obesity in adolescents., Competing Interests: Declarations. Background: Despite advancements in breast cancer (BC) diagnosis and treatment, it continues to be a serious health concern among women due to its high incidence rate. Thus, prevention strategies in BC are essential. Some nutrients such as vitamin D may play a preventive role against BC through different genes which have a vital role in several pathways. These pathways include autophagy, tumorigenesis, apoptosis, immunity, and genome stability. This study aimed to review the role of vitamin D in BC via the network of vitamin D-regulated pathways., Methods: This systematic review was conducted following PRISMA guidelines. PubMed, ScienceDirect, and Scopus were searched using a combination of MeSH terms and keywords related to molecular and cellular mechanisms of the effects of vitamin D on breast cancer. A total of 200 articles were initially found, from which 14 relevant studies were selected based on specific inclusion and exclusion criteria., Results: Experimental studies have shown possible anti-carcinogenic effects of vitamin D-related genes due to their participation in regulating autophagy, tumorigenesis, apoptosis, immunity, and genome stability in normal and malignant breast cells. Moreover, vitamin D deficiency has the potential to create a supportive environment that promotes proangiogenic processes, tumor cell dissemination, metastasis, and establishment at secondary sites., Conclusion: Vitamin D may have systematic roles against BC in humans through various interactions with different genes, which have roles in different and important pathways as underlying mechanisms in the pathophysiology of BC. More broadly, research is also needed to determine the exact protective effect of vitamin D on BC risk.

Background: To evaluate the safety and efficacy of 2.5 and 1.25 mg nebulized salbutamol on Transient Tachypnea of the Newborn (TTN) compared with placebo. Methods: We conducted a triple-blind, phase II/III parallel randomized controlled trial in two university-affiliated hospitals with neonatal intensive care units. Newborns with a confirmed diagnosis of TTN, with gestational age >35 weeks and gestational weight >2 kg were included. Cases of asphyxia, meconium aspiration syndrome, and persistent pulmonary hypertension were excluded. Ninety eligible patients were randomly allocated in three intervention groups (2.5 mg salbutamol, 1.25 mg salbutamol, and placebo), and a single-dose nebulized product was prescribed 6 hours after the birth. Safety outcomes included postintervention tachycardia, hyperglycemia, hypokalemia, and changes in blood pressure. To evaluate the efficacy, the duration of postintervention tachypnea, TTN clinical score, and clinical and paraclinical respiratory indices were assessed. Parents, Outcome assessors, and data analyzer were blind to the intervention. Results: There was no adverse reaction, including tachycardia, hypokalemia, and jitteriness. Both groups of salbutamol recipients showed significant improvement regarding respiratory rate, TTN clinical score, and oxygenation indices compared with the placebo ( p -values <0.001). Nonstatistically significant higher hospital stay was observed in the placebo group. Single 2.5 mg salbutamol nebulization showed a little better outcome than the dose of 1.25 mg, although we could not find statistical superiority. Conclusion: The newly applied single high dose of 2.5 mg nebulized salbutamol is safe in treating TTN and leads to notable faster improvement of respiratory status without any considerable adverse reaction. Background: The association of hypertension (HTN) and different types of dietary amino acids is not yet clear. The aim of the present study was to investigate the association of branch chain amino acids (BCAAs) and the prevention of HTN., Methods: This cross-sectional study was conducted on 4184 people aged 35 to 70 using data from the Sabzevar Persian cohort study in Sabzevar, Iran. Data on dietary intake of BCAAs including leucine, isoleucine, and valine were obtained using a validated Food Frequency Questionnaire (FFQ). Multivariable logistic regression analysis assessed the link between HTN and BCAAs., Results: The results showed that participants with HTN had a higher total protein and BCAAs intake than participants with normal BP (P < 0.01). A marginally significant association was found between the risk of HTN with the total intake of BCAAs (OR = 1.018, CI95%: 1.001-1.035, P = 0.04), leucine (OR = 1.040, CI95%:1.002-1.080, P = 0.03), isoleucine (OR = 1.068, CI95%:1.001-1.140, P = 0.04), and valine (OR = 1.060, CI95%:1.003-1.121, P = 0.04). However, the association disappeared after adjusting the total protein and calorie intake., Conclusions: The results indicated that the dietary intake of BCAAs may be associated with the risk of HTN. Background and Aim: Colorectal cancer (CRC) is one of the most prevalent cancers around the world. The link between nutrients and the likelihood of developing CRC remains uncertain. The primary objective of the present study was to investigate the potential connection between dietary intake/dietary supplements and the occurrence of CRC through a literature review., Methods: A comprehensive online search was conducted in PubMed, Scopus, Web of Science, and the Cochrane Library from January 1990 to March 2023 using appropriate keywords. A systematic search was conducted for clinical trials and cohort studies in order to determine the relationship between dietary components/supplements and CRC., Results: The intake of long-chain n-3 polyunsaturated fatty acids (n-3 LCPUFAs), consisting of eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA), has the potential to decrease the likelihood of developing CRC (eight studies found positive effects and four studies found no association). Some other dietary components such as probiotics, prebiotics, and synbiotics may contribute to suppressing CRC development (three studies found positive effects, whereas three studies did not find any association). There is inconclusive evidence that supplementation with certain micronutrients including vitamin D (one trial found positive effects and another trial reported no association), folate, zinc, and selenium may reduce the risk of CRC., Conclusion: Some dietary supplements such as n-3 LCPUFAs and probiotics have the potential to reduce the risk of developing CRC. Previous studies reported that iron may have an indispensable role in the risk of hypertension (HTN). However, the result of the studies on the relationship between iron and risk of HTN is inconsistent. This study aimed to assess the association between the association of dietary iron intake and HTN in the Iranian population. This case-control study was conducted on 4184 people aged 35 to 70, including 1239 people with HTN and 2945 people with normal blood pressure (BP) in Sabzevar, Iran. Dietary intake was assessed using a food frequency questionnaire (FFQ). The Nutritionist IV software was used in terms of the assessment of dietary intake of iron. An inverse association was found between iron intake and HTN (OR = 0.97, CI 95%: 0.94-0.99, P = 0.04). The association remained significant after adjustment for age, gender, smoking, drinking alcohol, calorie intake, and BMI (OR = 0.94, CI 95%: 0.89-0.98, P = 0.01). As a conclusion, iron intake was inversely associated with HTN. Further longitudinal studies on the effect of iron intake on BP are required to confirm this finding., (© 2024. Background: Pediatric coronavirus disease 2019 infection usually presents with respiratory and gastrointestinal symptoms. In this report we present fulminant meningitis as the main presentation of coronavirus disease 2019 without major signs and symptoms of other organs' involvement in 3 infants., Cases: The first case was a 4 months Iranian male infant with fulminant meningitis as the main presentation of coronavirus disease 2019 without other organ involvement. He was treated as suspected bacterial meningitis but CSF PCR and CSF culture were negative for common meningeal pathogens. On 3rd day, his coronavirus disease 2019 PCR test became positive, while it was negative on 1st day. The second case was a 13 months Iranian male infant with fever, irritability, and photophobia for 24 h before poorly controlled status epilepticus. CSF coronavirus disease 2019 PCR became positive while CSF PCR and CSF culture were negative for other common meningeal pathogens. Seizures were controlled with multiple anti-seizure medications. The third case was a 14 months Iranian female infant with fever and seizure1 hour before admission, leading to poorly controlled status epilepticus despite anti-epileptic therapy 10 h after admission. CSF coronavirus disease 2019 PCR became positive while CSF PCR and CSF culture were negative for other common meningeal pathogens. He was controlled with multiple anti-seizure medications., Conclusion: Meningitis of coronavirus disease 2019 should be considered in severely ill pediatric cases with poorly controlled seizures and RBC in CSF smear. Background: Patients undergoing hemodialysis (HD) frequently experience the chronic kidney disease-associated pruritus (CKD-aP)., Objective: The aim of this study was to investigate the effectiveness of omega-3 supplementation in the management of CKD-aP in patients undergoing hemodialysis., Methods: In this triple blind, randomized clinical trial, the effect of the omega-3 supplement on uremic CKD-aP was assessed in 112 chronic hemodialysis patients at Caspian Hemodialysis Center in Rasht, Iran. Patients were randomly divided into the intervention group receiving omega-3 supplements (3 g/day) and the control group receiving placebo containing MCT oil for 2 months., Results: Omega-3 supplementation had no effect on CKD-aP. The results did not change after adjusting for age and sex, additional adjustments for weight, height, physical activity, smoking, and alcohol use, additional adjustments for underlying diseases and weight, height, physical activity, smoking, and drinking alcohol, and further adjustments for underlying diseases and biochemical indices., Discussion: Omega-3 supplementation for 2 months had no effect on CKD-aP in patients with CKD. Background: Diabetic foot ulcer (DFU) is a prevalent complication of diabetes that can result in severe consequences. The cost of treating DFUs is high, and there is a lack of new therapies available in developing countries. This has created a demand for complementary and alternative treatments. The objective of this study was to assess the impact of aloe vera gel on the healing process of diabetic foot ulcers., Materials and Methods: The study was a double-blind, randomized, controlled clinical trial. The study samples were 66 patients with diabetic foot ulcers who were randomly assigned to two groups (intervention and control). All ulcers in both groups were washed with normal saline and dressed in sterile gauze. The ulcers in the intervention group were covered with a thin layer of Aloe Vera gel before the dressing. The Bates-Jensen Wound Assessment Tool (BWAT) was used on three occasions, including before the intervention period and at the end of each week. Data were analyzed using SPSS 16., Results: After three weeks, there was a notable contrast in the recovery pattern of the two groups. The patients who had aloe vera gel added to their dressing showed a more substantial decrease in the mean scores of their BWAT., Conclusion: These findings are promising and suggest that Aloe vera may be a safe and effective adjunctive treatment for diabetic foot ulcers. Background: "Partnership Care Model (PCM)", which is the first partnership conceptual framework founded on the Iranian culture to control chronic diseases, has been recently used in different fields of nursing research with no levels of valid evidence to support its application. Therefore, this systematic review and meta-analysis sought to clarify the impacts of interventions developed based on PCM on quality of life (QoL), sleep quality, anxiety, and depression among adults and children with chronic diseases., Methods: International data sources (e.g., PubMed, Web of Science, Scopus) and national databases (e.g., SID, MagIran, IranDoc, IRCT) were searched from 2001 to September 23, 2023, to find Randomized Controlled Trials (RCTs) on PCM-driven interventions for the experimental groups versus no intervention or standard care groups. The studies' methodological quality and evidence quality were rated utilizing the Cochrane risk of bias instruction and the Grading of Recommendations Assessment, Development, and Evaluation (GRADE). Data were pooled by a random-effects approach employing STATA (vers. 11.2)., Result: Eighteen RCTs, reported in 22 publications, were qualified. The PCM compared to the standard care significantly improved the QoL among both adults (10 effect sizes [ESs], mean difference [MD]: 3.17, P < 0.001) and children (4 ESs, MD: 4.45, P < 0.001). Likewise, the intervention enhanced adults' sleep quality (3 ESs, MD: 7.15, P < 0.001). The anxiety of adults and children was also significantly lower in the PCM group (4 ESs, MD: -4.52, P = 0.001; 2 ESs, MD: -4.04, P < 0.001, respectively). However, regarding depression, a significant effect of PCM was found only among children (3 ESs, MD: -7.99, P = 0.011). The methodological quality of the studies and the evidence quality were undesirable., Conclusion: The PCM had a promising influence on the caring of adults and children suffering from chronic diseases. We present a novel approach for volume exploration that is versatile yet effective in isolating semantic structures in both noisy and clean data. Specifically, we describe a hierarchical active contours approach based on Bhattacharyya gradient flow which is easier to control, robust to noise, and can incorporate various types of statistical information to drive an edge-agnostic exploration process. To facilitate a time-bound user-driven volume exploration process that is applicable to a wide variety of data sources, we present an efficient multi-GPU implementation that (1) is approximately 400 times faster than a single thread CPU implementation, (2) allows hierarchical exploration of 2D and 3D images, (3) supports customization through multidimensional attribute spaces, and (4) is applicable to a variety of data sources and semantic structures. The exploration system follows a 2-step process. It first applies active contours to isolate semantically meaningful subsets of the volume. It then applies transfer functions to the isolated regions locally to produce clear and clutter-free visualizations. We show the effectiveness of our approach in isolating and visualizing structures-of-interest without needing any specialized segmentation methods on a variety of data sources, including 3D optical microscopy, multi-channel optical volumes, abdominal and chest CT, micro-CT, MRI, simulation, and synthetic data. We also gathered feedback from a medical trainee regarding the usefulness of our approach and discussion on potential applications in clinical workflows.

Purpose: This study examined the relationship between patient safety competence and safe care from the viewpoints of nurses working in psychiatry wards., Design and Methods: The present descriptive correctional study was conducted in two psychiatry hospitals in Iran in 2020. All the nurses were selected as the study participants using the census sampling method (N = 209)., Findings: Nurses' patient safety competency was at a low level (2.54 ± 0.52), but nurses' safe care was at a moderate level (242.08 ± 61.32).
