Abstract Vestibular evoked myogenic potentials (VEMPs) are a tool to assess otolith function and a component of sensor specific vestibular diagnostics. The aim of the present study was to measure VEMP trough bone conducted (BC) stimulation using the B250 prototype and to report amplitudes, latencies and threshold levels for patients before resection of a sporadic unilateral vestibular schwannoma (VS) in order to assess function regarding to the reported nerve of origin. Twenty-seven participants (9 male/18 female) with a mean age of 55.9 years (SD: 10.8) were included for the analysis. In the side contralateral to the tumor, in 24 (89%) of the patients cVEMP could be measured, while oVEMP were recordable in 20 patients (74%). For patients with inferior vestibular nerve of origin (n = 11), cVEMP amplitudes of the affected side were significantly lower as compared to the non-affected side, while the force threshold level was increased. No statistically significant differences were observed for neither, oVEMP amplitudes nor threshold levels in the group with superior vestibular nerve of origin (n = 7). Across groups, p13 latency was significantly increased in the affected ear while all other VEMP latencies were not different between the ears. The B250 transducer was applicable to all participants of the clinical cohort. The sample size, however, was too low for a reliable statistical analysis and only allowed for exploratory analysis.

Abstract Objectives The study objective was to investigate the influence of electrical stimulus properties on cervical and ocular vestibular‐evoked myogenic potentials to electrical stimulation by cochlear implants (e‐cVEMPs, e‐oVEMPs). Methods E‐VEMPs were recorded in adult Nucleus cochlear implant (CI) patients using electric pulse trains (4 biphasic pulses at 1000 Hz burst rate). Ground path and stimulation electrodes were varied between monopolar stimulation at basal electrode contact E3 (MP1 + 2 E3), monopolar stimulation at apical electrode contact E20 (MP1 + 2 E20), and bipolar transmodiolar stimulation between E3 and E14 (BP E3‐E14). The electric pulse train was further varied to 2 pulses at 1000 Hz, 2 pulses at 500 Hz, and a single pulse, in patients with present e‐VEMP responses. VEMPs to bone‐conducted vibration (BCV) were recorded as reference in all participants. Results Measurements were conducted in 30 ears of 27 participants (mean age 49.3 years, SD 12.7 years). E‐VEMPs were present in 13 ears (43%). 5 of the 13 cases showed e‐VEMPs but no BCV evoked VEMPs. Response numbers increased with increasing stimulation levels. The highest response rate of 40% was obtained for MP1 + 2 E3 stimulation. Stimulus variation did not affect response numbers. E‐VEMP amplitudes were comparable to BCV‐stimulated VEMPs. Latencies were up to 3.1 ms shorter for electric stimulation. Some patients showed e‐VEMP thresholds close to or below the electric hearing threshold level. Conclusion The occurrence of e‐VEMPs is dependent on current path and stimulation level. Vestibular co‐stimulation by the CI is more likely in patients with high stimulation levels and for monopolar stimulation of basal electrode contacts. Level of Evidence 4.

Abstract Objective measurements could improve cochlear implant (CI) fitting, especially for CI users who have difficulty assessing their hearing impressions. In this study, we investigated the electrically evoked mismatch negativity (eMMN) brain potential as a mainly preattentive response to pitch and loudness changes. In an electrophysiological exploratory study with 21 CI users, pitch and loudness cues were presented in controlled oddball paradigms that directly electrically stimulated the CI via software. Out of them 17 valid data sets were analyzed. A pitch cue was produced by changing the stimulating CI electrodes (pairs of adjacent electrodes). A loudness cue originated from changing the stimulation amplitude on one CI electrode. MMN responses were measured unsing clinical electroencephalography recording according to a standard recording protocol. At the group level, significant eMMN responses were elicited for loudness cues and for pitch cues at basal electrode pairs but not at apical electrode pairs. The effect of deviance direction was not significant and no stimulus artifacts were observed. Recording an electrically evoked MMN in response to loudness changes in CI users is generally feasible, and is, therefore, promising to support CI fitting procedures in the future. Detection of pitch cues would require a greater electrode distance between selected electrodes for standard and deviant stimuli, especially in apical regions. A routine clinical setup can be used to measure eMMN.

BackgroundHearing-impaired listeners often have difficulty understanding complex sentences. It is not clear if perceptual or cognitive deficits have more impact on reduced language processing abilities, and how a hearing aid might compensate for that.MethodsIn a prospective study with 5 hearing aid users and 5 normal hearing, age-matched participants, processing of complex sentences was investigated. Audiometric and working memory tests were performed. Subject- and object-initial sentences from the Oldenburg Corpus of Linguistically and audiologically controlled Sentences (OLACS) were presented to the participants during recording of an electroencephalogram (EEG).ResultsThe perceptual difference between object and subject leading sentences does not lead to processing changes whereas the ambiguity in object leading sentences with feminine or neuter articles evokes a P600 potential. For hearing aid users, this P600 has a longer latency compared to normal hearing subjects.ConclusionThe EEG is a suitable method for investigating differences in complex speech processing for hearing aid users. Longer P600 latencies indicate higher cognitive effort for processing complex sentences in hearing aid users.

ObjectiveCochlear implants (CIs) can restore hearing not only in patients with profound hearing loss and deafness, but also in patients following tumour removal of intra-cochlear schwannomas. In such cases, design and placement differ from conventional electrode insertion, in which the cochlea remains filled with fluid. Despite these technical and surgical differences, previous studies have tended to show positive results in speech perception in tumour patients. The purpose of this study is to retrospectively evaluate the ability to predict speech recognition outcomes using individual electric field spreads and to investigate worldwide unique tumour cases.Study designIn a retrospective analysis in a university tertiary center electric field spreads were compared between two groups of electrode designs implanted between 2009 and 2020 i.e., between lateral wall electrodes and custom-made perimodiolar electrode carriers from the same company. The voltage gradients were analysed and grouped with speech recognition results.ResultsDifferences in electrical field spreads were found between lateral wall electrodes and the custom-made perimodiolar electrodes, whereas a significant influence of electric fields on scores in speech recognition cannot be demonstrated.ConclusionPrediction of speech recognition outcome based on electric field propagation results seems not feasible. Significant differences in field spread between electrode arrays can be clearly demonstrated. This observation and its relevance to hearing treatment and speech recognition should therefore be further investigated in upcoming studies.

Abstract For many cochlear implant (CI) users, frequency discrimination is still challenging. We studied the effect of frequency differences relative to the electrode frequency bands on pure tone discrimination. A single-center, prospective, controlled, psychoacoustic exploratory study was conducted in a tertiary university referral center. Thirty-four patients with Cochlear Ltd. and MED-EL CIs and 19 age-matched normal-hearing control subjects were included. Two sinusoidal tones were presented with varying frequency differences. The reference tone frequency was chosen according to the center frequency of basal or apical electrodes. Discrimination abilities were psychophysically measured in a three-interval, two-alternative, forced-choice procedure (3I-2AFC) for various CI electrodes. Hit rates were measured, particularly with respect to discrimination abilities at the corner frequency of the electrode frequency-bands. The mean rate of correct decision concerning pitch difference was about 60% for CI users and about 90% for the normal-hearing control group. In CI users, the difference limen was two semitones, while normal-hearing participants detected the difference of one semitone. No influence of the corner frequency of the CI electrodes was found. In CI users, pure tone discrimination seems to be independent of tone positions relative to the corner frequency of the electrode frequency-band. Differences of 2 semitones can be distinguished within one electrode.

Plain language summary Receptors for the hearing and balance systems are located in the inner ear. These are very delicate structures that convert sound and movement into nerve signals to control hearing and balance. Surgical damage to either system was widely believed to cause loss of function in the other. Here, we investigated the function of the balance receptors before and after severe damage to the part of the inner ear which is responsible for hearing (the cochlea) due to surgical removal of tumurs. We show that despite the close proximity of both types of receptors and the severe trauma to the cochlea, in most patients the balance system can still function normally. This observation may have important implications for how we treat patients with inner ear surgery and for the techniques we use to treat hearing and balance disorders.

Abstract Background A previously performed phase III trial on 112 subjects investigating prophylactic nimodipine treatment in vestibular schwannoma (VS) surgery showed no clear beneficial effects on preservation of facial and cochlear nerve functions, though it should be considered that protection of facial nerve function was the primary outcome. However, the risk for postoperative hearing loss was halved in the nimodipine group compared to the control group (OR 0.49; 95% CI 0.18–1.30; p = 0.15). Accordingly, this phase III extension trial investigates the efficacy and safety of prophylactic nimodipine for hearing preservation in VS surgery. Methods This is a randomized, multi-center, two-armed, open-label phase III trial with blinded expert review and two-stage with interim analysis. Three hundred thirty-six adults with the indication for microsurgical removal of VS (Koos I–IV) and serviceable preoperative hearing (Gardner-Robertson scale (GR) 1–3) are assigned to either the therapy (intravenous nimodipine 1–2 mg/h from the day before surgery until the fifth postoperative day and standard of care) or the control group (surgery only and standard of care). The primary endpoint of the trial is postoperative cochlear nerve function measured before discharge according to GR 1–3 versus GR 4–5 (binary). Hearing function will be determined by pre- and postoperative audiometry with speech discrimination, which will be evaluated by a blinded expert reviewer. Furthermore, patient-reported outcomes using standardized questionnaires will be analyzed. Discussion Prophylactic parenteral nimodipine treatment may have a positive effect on hearing preservation in VS surgery and would improve patient’s quality of life. Further secondary analyses are planned. Except for dose-depending hypotension, nimodipine is known as a safe drug. In the future, prophylactic nimodipine treatment may be recommended as a routine medication in VS surgery. VS can be considered as an ideal model for clinical evaluation of neuroprotection, since hearing outcome can be classified by well-recognized criteria. The beneficial effect of nimodipine may be transferable to other surgical procedures with nerves at risk and may have impact on basic research. BackgroundIntratympanic injections of glucocorticoids have become increasingly common in the treatment of idiopathic sudden sensorineural hearing loss (ISSHL). However, due to their fast elimination, sustained applications have been suggested for local drug delivery to the inner ear.Materials and methodsThe study is based on a retrospective chart review of patients treated for ISSHL at a single tertiary (university) referral center. We included patients who were treated with a solid, biodegradable, poly(D,L-lactic-co-glycolic acid) (PLGA)-based drug delivery system providing sustained delivery of dexamethasone extracochlear into the round window niche (n = 15) or intracochlear into scala tympani (n = 2) for tertiary therapy of ISSHL in patients without serviceable hearing after primary systemic and secondary intratympanic glucocorticoid therapy. We evaluated the feasibility and safety through clinical evaluation, histological examination, and functional tests [pure-tone threshold (PTA), word recognition scores (WRS)].ResultsWith adequate surgical preparation of the round window niche, implantation was feasible in all patients. Histologic examination of the material in the round window niche showed signs of resorption without relevant inflammation or foreign body reaction to the implant. In patients where the basal part of scala tympani was assessable during later cochlear implantation, no pathological findings were found. In the patients with extracochlear application, average preoperative PTA was 84.7 dB HL (SD: 20.0) and 76.7 dB HL (SD: 16.7) at follow-up (p = 0.08). The preoperative average maximum WRS was 14.6% (SD: 17.9) and 39.3% (SD: 30.7) at follow-up (p = 0.11). Six patients (40%), however, reached serviceable hearing. The two patients with intracochlear application did not improve.ConclusionThe extracochlear application of the controlled release system in the round window niche and – based on limited observations - intracochlear implantation into scala tympani appears feasible and safe. Due to the uncontrolled study design, conclusions about the efficacy of the treatment are limited. These observations, however, may encourage the initiation of prospective controlled studies using biodegradable controlled release implants as drug delivery systems for the treatment of inner ear diseases.

Abstract Evaluating the effectiveness of different bone conduction (BC) transducers with controlled coupling force to elicit cervical and ocular vestibular evoked myogenic potentials (cVEMPs, oVEMPs) in healthy subjects by comparing response rates, amplitudes, latencies, thresholds and asymmetry ratios. Prospective experimental study including healthy participants. VEMPs were measured to different stimulation modes; the BC transducer coupling force was controlled to 5.4 (± 0.5) Newton. cVEMPs: to bone conducted vibration (BCV) with the B81 transducer on the mastoid; oVEMPs: to BCV with the B81 on the mastoid, BCV with the B81 on the forehead, and BCV with the Mini-Shaker 4810 on the forehead. Air conducted sound (ACS) with insert earphones was used as reference. Data of 24 normal subjects (mean age 25.3 (± 3.0) years) were analyzed. ACS and BCV with the B81on the mastoid evoked cVEMPs in 100% of ears. The highest oVEMP response rates were obtained with the B81 on the mastoid (83–92%), the lowest with the B81 on the forehead (17–22%). The Mini-Shaker elicited lower response rates (65%) compared to results from the literature without coupling force control and compared to ACS (78–87%). Amplitudes were higher for BCV than ACS. ACS and BCV on the mastoid caused higher asymmetry compared to BCV forehead stimulation. The B81 was feasible to elicit VEMPs with mastoid placement and can be used as an approved medical device to measure BCV VEMPs in a clinical set-up. Normative asymmetry values have to be established due to higher variability for mastoid stimulation.

Abstract In vestibular schwannoma (VS) patients hearing outcome and surgery related risks can vary and depend on the nerve of origin. Preoperative origin differentiation between inferior or superior vestibular nerve may influence the decision on treatment, and the selection of optimal treatment and counselling modalities. A novel scoring system based on functional tests was designed to predict the nerve of origin for VS and was applied to a large number of consecutive patients. A prospective, double blind, cohort study including 93 patients with suspected unilateral VS was conducted at a tertiary referral centre. Preoperatively before tumor resection a functional test battery [video head-impulse test (vHIT) of all semicircular canals (SCC)], air-conducted cervical/ocular vestibular evoked myogenic potential tests (cVEMP, oVEMP), pure-tone audiometry, and speech discrimination was applied. Sensitivity and specificity of prediction of intraoperative finding by a preoperative score based on vHIT gain, cVEMP and oVEMP amplitudes and asymmetry ratios was calculated. For the prediction of inferior vestibular nerve origin, sensitivity was 73% and specificity was 80%. For the prediction of superior vestibular nerve origin, sensitivity was 60% and specificity was 90%. Based on the trade-off between sensitivity and specificity, optimized cut-off values of − 0.32 for cVEMP and − 0.11 for oVEMP asymmetry ratios and vHIT gain thresholds of 0.77 (anterior SCC), 0.84 (lateral SCC) and 0.80 (posterior SCC) were identified by receiver operator characteristic curves. The scoring system based on preoperative functional tests improves prediction of nerve of origin and can be applied in clinical routine.

BackgroundAs audition also seems to contribute to balance control, additionally to visual, proprioceptive, and vestibular information, we hypothesize that hearing rehabilitation with active middle ear and bone conduction implants can influence postural control.MethodsIn a prospective explorative study, the impact of hearing rehabilitation with active middle ear [Vibrant Soundbrige (VSB), MED-EL, Innsbruck, Austria] and bone conduction implants [Bonebridge (BB), MED-EL, Innsbruck, Austria] on postural control in adults was examined in three experiments. Vestibulospinal control was measured by cranio-corpography (CCG), trunk sway velocity (°/s) by the Standard Balance Deficit Test (SBDT), and postural stability with a force plate system, each time in best aided (BA) and unaided (UA) condition with frontal-noise presentation (Fastl noise, 65 dB SPL), followed by subjective evaluation, respectively.ResultsIn 26 subjects [age 55.0 ± 12.8 years; unilateral VSB/BB: n = 15; bilateral VSB/BB: n = 3, bimodal (VSB/BB + hearing aid): n = 8], CCG-analysis showed no difference between BA and UA conditions for the means of distance, angle of displacement, and angle of rotation, respectively. Trunk sway measurements revealed a relevant increase of sway in standing on foam (p = 0.01, r = 0.51) and a relevant sway reduction in walking (p = 0.026, r = 0.44, roll plane) in BA condition. Selective postural subsystem analysis revealed a relevant increase of the vestibular component in BA condition (p = 0.017, r = 0.47). As measured with the Interactive Balance System (IBS), 42% of the subjects improved stability (ST) in BA condition, 31% showed no difference, and 27% deteriorated, while no difference was seen in comparison of means. Subjectively, 4–7% of participants felt that noise improved their balance, 73–85% felt no difference, and 7–23% reported deterioration by noise. Furthermore, 46–50% reported a better task performance in BA condition; 35–46% felt no difference and 4–15% found the UA situation more helpful.ConclusionsSubjectively, approximately half of the participants reported a benefit in task performance in BA condition. Objectively, this could only be shown in one mobile SBDT-task. Subsystem analysis of trunk sway provided insights in multisensory reweighting mechanisms.

ObjectiveTo analyze intensity-latency functions of intraoperative auditory evoked brainstem responses (ABRs) to stimulation by the Vibrant Soundbridge (VSB) active middle ear implant with respect to coupling efficiency, VSB evoked ABR thresholds, and coupling modality [oval window (OW) placement vs. Incus placement and vs. round window (RW) placement].Study DesignExploratory study.SettingBi-centric study at tertiary referral centers.PatientsTwenty-four patients (10 female, 14 male, mean age: 58 years) who received a VSB.Outcome MeasuresWave-V intensity-latency functions of intraoperative VSB evoked ABRs using a modified audio processor programmed to preoperative bone conduction thresholds for stimulation. Threshold level correction to coupling efficiency and ABR thresholds. Individual plots and exponential function fits.ResultsAfter ABR threshold level correction, the latency functions could be aligned. A large variance of latencies was observed at individual threshold level. Wave-V latency was longest in the Incus placement subgroup (9.73 ms, SD: 1.04) as compared to OW placement subgroup (9.47 ms, SD: 1.05), with the shortest latency in the RW placement subgroup (8.99 ms, SD: 0.68). For increasing stimulation levels, the variance decreased with intensity-latency function slopes converging toward a steady-state (saturation) latency caused by saturation of audio processor (stimulation) gain. Latency saturation was reached at a stimulation level of 50 dB nHL for the OW placement subgroup, 35 dB nHL for the Incus placement subgroup, and 30 dB nHL for the RW placement subgroup. The latency and saturation results indicated decreased dynamic range for RW placement, i.e., reverse stimulation.ConclusionsVSB evoked ABR wave-V intensity-latency function slopes were similar to acoustic stimulation at high stimulation levels with a shift toward longer latencies caused by audio processor signal delay. Saturation of latencies occurred for higher stimulation levels due to saturation of audio processor gain. Thus, the analysis of VSB evoked intensity-latency functions appears to allow for the objective assessment of a patient's individual dynamic range. This can further improve diagnostics as well as intraoperative and postoperative quality control.

Daily-life conversation relies on speech perception in quiet and noise. Because of the COVID-19 pandemic, face masks have become mandatory in many situations. Acoustic attenuation of sound pressure by the mask tissue reduces speech perception ability, especially in noisy situations. Masks also can impede the process of speech comprehension by concealing the movements of the mouth, interfering with lip reading. In this prospective observational, cross-sectional study including 17 participants with normal hearing, we measured the influence of acoustic attenuation caused by medical face masks (mouth and nose protection) according to EN 14683 and of N95 masks according to EN 1149 (EN 14683) on the speech recognition threshold and listening effort in various types of background noise. Averaged over all noise signals, a surgical mask significantly reduced the speech perception threshold in noise was by 1.6 dB (95% confidence interval [CI], 1.0, 2.1) and an N95 mask reduced it significantly by 2.7 dB (95% CI, 2.2, 3.2). Use of a surgical mask did not significantly increase the 50% listening effort signal-to-noise ratio (increase of 0.58 dB; 95% CI, 0.4, 1.5), but use of an N95 mask did so significantly, by 2.2 dB (95% CI, 1.2, 3.1). In acoustic measures, mask tissue reduced amplitudes by up to 8 dB at frequencies above 1 kHz, whereas no reduction was observed below 1 kHz. We conclude that face masks reduce speech perception and increase listening effort in different noise signals. Together with additional interference because of impeded lip reading, the compound effect of face masks could have a relevant impact on daily life communication even in those with normal hearing.

Objective: To evaluate ocular and cervical vestibular evoked myogenic potentials (oVEMPs and cVEMPs) in patients with solely intracochlear localization of an intralabyrinthine schwannoma (ILS).Study Design: Retrospective analysis of a series of cases.Setting: Monocentric study at a tertiary referral center.Patients: Patients with intracochlear schwannoma (ICS) and VEMP measurements.Outcome Measures: Signed asymmetry ratio (AR) of cVEMPs and oVEMPs to air conducted sound with AR cut-offs considered to be asymmetrical when exceeding ±30% for cVEMPs and ±40% for oVEMPs with respect to the side affected by the tumor (reduced amplitudes on the affected side indicated by negative values, enhanced amplitudes by positive values); VEMP amplitudes and latencies; tumor localization in the cochlear turn and scala.Results: Nineteen patients with a solely intracochlear tumor (ICS patients) [10 males, 9 females, mean age 57.1 (SD: 13.4) years] were included in the study. On the affected side, cVEMPs were absent or reduced in 47% of the patients, normal in 32%, and enhanced in 21%. Ocular VEMPs on the affected side were absent or reduced in 53% of the patients, normal in 32% and enhanced in 15%. Latencies for cVEMPs and oVEMPs were not significantly different between the affected and non-affected side. In all patients with enhanced VEMPs, the tumor was located in the scala tympani and scala vestibuli.Conclusions: As a new and unexpected finding, VEMP amplitudes can be enhanced in patients with intracochlear schwannoma, mimicking the third window syndrome.

IntroductionTaste and smell are important for occupational performance and quality of life. Previous studies suggested that the function of these senses might be influenced by ambient pressure and noise. This knowledge would be helpful for divers, submarine crews, or mine workers. The present study aimed to investigate the effects of noise and hyperbaric pressure on olfactory and gustatory functions.MethodsThis prospective controlled study included 16 healthy male divers. Inside a hyperbaric chamber, participants performed olfactory and gustatory function tests at sea level pressure and at 2 bar pressure. The olfaction threshold, and the discrimination and identification of odorants were measured with validated ´Sniffin sticks´. Taste identification and the gustation threshold scores were examined with validated filter paper strips. Tests were performed under two conditions: noise reduction (silence) and white noise stimulation presented at 70 dB sound pressure level.ResultsThe results showed that normobaric and hyperbaric ambient pressures did not significantly affect olfactory or gustatory function. Moreover, noise had no relevant impact on taste or odor sensation. The odor identification score was not influenced in hyperbaric conditions, and the odor threshold score was not influenced by ambient noise or both barometric conditions. The only taste modality affected by hyperbaric conditions was the sensitivity to salty taste, but it was not significant.ConclusionWe concluded that hyperbaric and noisy environments have no influence on gustatory and olfactory function. From a practical point of view, the influence of pressure in moderate hyperbaric occupations should be negligible.

Speech perception in noise is challenging and is improved by binaural hearing. Since signal processing of assistive hearing devices often modifies or masks the peripheral binaural head-shadow or better-ear effects, central binaural processing should be measured separately. In a prospective study, 10 listeners with normal hearing were tested with the German matrix sentence test in a set-up with two loudspeakers located at opposite angles in the horizontal plane with respect to S0N0. The speech reception threshold (SRT) was investigated depending on the separation angle between speech and noise. The lowest (best) SRT was obtained for a separation of target and interfering source from S0N0 at an angle of about S±60°N∓60°. The derived normative curve was comparable to SRTs predicted by the binaural-speech-intelligibility-model. The systematic separation of signal and noise showed a significant improvement in speech intelligibility for normal-hearing people even for small separation angles. This experimental setting was verified. This study aimed to assess the effect of small sound source separation on binaural hearing and speech perception.

Multidimensional food perception is based mainly on gustatory and olfactory function. Recent research has demonstrated that hypobaric pressure impairs gustatory function and that background noise or distracting auditory stimulation impairs olfactory function. Using a hypobaric chamber, the odor identification, discrimination, and thresholds as well as taste identification and threshold scores were measured in 16 healthy male volunteers under normal and hypobaric (6380 ft) conditions using clinically validated tests. In both conditions, background noise was either canceled out or replaced by white noise presentation (70 dB sound pressure level). Olfactory sensitivity for n-butanol and gustatory sensitivity were impaired in a hypobaric atmosphere. White noise did not influence the odor test results. White noise stimulation impaired sensitivity for sour and sweet but not for bitter or salty tastants. We conclude that hypobaric or noisy environments could impair gustatory and olfactory sensitivity selectively for particular tastants and odorants.

Different types of bone conduction transducers with different physical and electro-acoustic properties are available for audiometric hearing threshold measurements. The reference equivalent threshold vibratory force levels (RETVFL) specified in ISO 389-3 are based on measurements conducted with the B71 and KH70 transducers but apply to all types of transducers available for bone conduction audiometry. The objective of this study was to compare bone conduction hearing thresholds measured by different transducers.In a prospective study the hearing thresholds were measured psychometrically between 125 Hz and 8000 Hz using the Radioear B71, B81 and Präcitronic KH70 transducers.Twenty-one normal hearing participants and fifteen hearing impaired participants.In both groups significant differences were found between the thresholds measured with the different transducers at the low frequencies 125 Hz and 250 Hz and the high frequencies 3000 Hz, 4000 Hz, 6000 Hz and 8000 Hz. In the normal hearing group, deviations from the reference threshold 0 dB HL towards lower thresholds were observed for the B71 and B81 at 125 Hz and at the high frequencies 3000 Hz, 4000 Hz, 6000 Hz and 8000 Hz.RETVFL-values should be reassessed and provided specifically for the different transducers.

Western music is based on intervals; thus, interval discrimination is important for distinguishing the character of melodies or tracking melodies in polyphonic music. In this study the encoding of intervals in simultaneously presented sound is studied.In an electrophysiological experiment in 15 normal-hearing non-musicians, major thirds or fifths were presented in a controlled oddball paradigm. Harmonic intervals were created by simultaneously presented sinusoidals with randomized root frequency. Mismatch negativity (MMN) responses were measured with an EEG recording. The discrimination index was calculated in a psychoacoustic experiment.A clear MMN response was found for the major third but not for the fifth. The neural generators were located within the auditory cortices. Psychoacoustically, no evidence was found that the subjects were able to detect the deviants.We conclude that pre-attentive discrimination of harmonic interval size is, in principle, possible in listeners without musical training although simultaneous presentation makes it harder to distinguish compared to non-overlapping intervals. Furthermore we see a difference in the response to infrequent dissonant stimuli in consonant standard stimuli compared to the opposite, rare consonant stimuli in dissonant standard stimuli.

This paper describes a logatome discrimination test for the assessment of speech perception in cochlear implant users (CI users), based on a multilingual speech database, the Oldenburg Logatome Corpus, which was originally recorded for the comparison of human and automated speech recognition. The logatome discrimination task is based on the presentation of 100 logatome pairs (i.e., nonsense syllables) with balanced representations of alternating “vowel-replacement” and “consonant-replacement” paradigms in order to assess phoneme confusions. Thirteen adult normal hearing listeners and eight adult CI users, including both good and poor performers, were included in the study and completed the test after their speech intelligibility abilities were evaluated with an established sentence test in noise. Furthermore, the discrimination abilities were measured electrophysiologically by recording the mismatch negativity (MMN) as a component of auditory event-related potentials. The results show a clear MMN response only for normal hearing listeners and CI users with good performance, correlating with their logatome discrimination abilities. Higher discrimination scores for vowel-replacement paradigms than for the consonant-replacement paradigms were found. We conclude that the logatome discrimination test is well suited to monitor the speech perception skills of CI users. Due to the large number of available spoken logatome items, the Oldenburg Logatome Corpus appears to provide a useful and powerful basis for further development of speech perception tests for CI users.

Patients suffering from conductive or mixed hearing loss may benefit from bone-conduction hearing systems (BAHS). The amount of amplification provided by the hearing system is selected based on the individual’s sensorineural frequency-specific threshold. With patients who are not able to provide thresholds behaviorally, such as young children, objective methods are required to estimate the unaided and aided hearing threshold and thus the success of the hearing system fitting. In a prospective study with ten adult Baha softband users, aided and unaided frequency-specific thresholds were estimated. Aided thresholds to tone bursts via Baha stimulation were obtained behaviorally and electrophysiologically using cortical auditory evoked potentials (CAEPs) and were compared to pure-tone thresholds using routine clinical audiometry. For all stimulation frequencies, the frequency-specific electrophysiological and behavioral hearing thresholds measured with Baha stimulation were highly correlated and not different. Increased thresholds were observed only with the 0.5 kHz Baha stimulation as compared to the pure-tone audiogram. Objective measurement of frequency-specific hearing thresholds with CAEPs is applicable to BAHS users.

The identification and discrimination of timbre are essential features of music perception. One dominating parameter within the multidimensional timbre space is the spectral shape of complex sounds. As hearing loss interferes with the perception and enjoyment of music, we approach the individual timbre discrimination skills in individuals with severe to profound hearing loss using a cochlear implant (CI) and normal hearing individuals using a bone-anchored hearing aid (Baha). With a recent developed behavioral test relying on synthetically sounds forming a spectral continuum, the timbre difference was changed adaptively to measure the individual just noticeable difference (JND) in a forced-choice paradigm. To explore the differences in timbre perception abilities caused by the hearing mode, the sound stimuli were varied in their fundamental frequency, thus generating different spectra which are not completely covered by a CI or Baha system. The resulting JNDs demonstrate differences in timbre perception between normal hearing individuals, Baha users, and CI users. Beside the physiological reasons, also technical limitations appear as the main contributing factors.
