Background: A diverse nursing workforce depends on whether higher education is accessible to historically disadvantaged students and whether such students are supported to achieve success. In South Africa, the Extended Curriculum Program (ECP) is a transition degree program created to widen access and success for underrepresented groups, including those aspiring to become nurses. This qualitative study explored the experience of nursing students in an ECP at a South African university. Method: Written reflections were collected from 16 nursing students in the ECP. Results: Four main themes emerged following thematic analysis: ECP selection process, emotional reaction to ECP placement, ECP as another experience, and acceptance and moving forward. Students reported an altered sense of belonging as the result of hostility from peers and faculty, making social and academic integration challenging. Conclusion: Students who gain access to nursing programs and the nursing workforce through transition degree programs may be at risk of low self-efficacy. [ J Nurs Educ . 2022;61(8):477–482.]

Stillbirth, the loss of a baby during pregnancy or childbirth, is one of the most devastating losses a parent can experience. The experience of stillbirth is associated with trauma and intense grief, but mothers’ belief systems can be protective against the impacts of grief. Women in Nigeria endure a high burden of stillbirth and the aim in this study was to describe the beliefs and strategies for coping with stillbirth. Twenty mothers bereaved by stillbirth in Nigeria were interviewed; seven of them also participated in a focus group. The findings of the study revealed that the experience of stillbirth was influenced by beliefs which originated from superstitions, religion, and social expectations. These beliefs played significant roles in how mothers coped with the loss, by providing them with a framework for sense-making and benefit-finding.

Background The World Health Organization recommends exclusive breastfeeding for the first 6 months of an infant’s life and continued breastfeeding for 2 years. The global rate of exclusive breastfeeding is low at 33%. Thus, it is important to identify philosophical and theory-based strategies that can promote exclusive breastfeeding. The aim of the study was to identify philosophical schools of thought and theories used in research on promoting the practice of exclusive breastfeeding. Methods A scoping review using Arksey and O'Malley's framework explored the phenomenon of exclusive breastfeeding practice promotion. Searches were conducted using CINAHL Plus full-text, PubMed, APA PsycInfo, and Academic Search Premier. Search terms included theory, philosophy, framework, model, exclusive breastfeeding, promotion, support, English, and publication between 2001—2022. Results The online search yielded 1,682 articles, however, only 44 met the inclusion criteria for the scoping review. The articles promoting exclusive breastfeeding used pragmatism (n = 1) or phenomenology (n = 2) philosophies and theories of self-efficacy (n = 10), theory of planned behaviour (n = 13), social cognitive theories (n = 18) and represented 16 countries. Theories of self-efficacy and planned behaviour were the most used theories. Conclusions This review suggests that theories and models are increasingly being used to promote exclusive breastfeeding. Orienting exclusive breastfeeding programmes within theoretical frameworks is a step in the right direction because theories can sensitize researchers and practitioners to contextually relevant factors and processes appropriate for effective exclusive breastfeeding strategies. Future research should examine the efficacy and effectiveness of theory-informed exclusive breastfeeding programmes over time. Such information is important for designing cost-effective EBF programmes.

The loss of a baby to stillbirth is a traumatic experience and can lead to secondary losses, such as the loss of social relationships. In Nigeria, stillbirths are a common public health problem. However, limited attention has been given to the social ramifications of stillbirths. This study describes the social networks of women who have experienced a stillbirth and the factors influencing their social networks. Interviews and social network diagrams were used to collect data from 20 women about their social networks before and after stillbirth. Findings suggest that the experience of shame, unmet expectation of support, and a lack of trust led to relationship changes after stillbirth. Most participants met bereavement needs with their existing social networks before stillbirth, but many participants also experienced relationship losses (even among family networks). Information from social network analysis can reveal the risks and strengths inherent in social networks, which can be helpful for the provision of tailored/personalized bereavement care.

Stillbirth commonly affects women in Nigeria, but their experiences of grief following stillbirth is under explored. This study aimed to describe Nigerian women’s experiences of grief after stillbirth. Face-to-face, semi-structured interviews were conducted with 20 women in Nigeria who experienced stillbirth. The results from the thematic analysis suggest that mothers had an unmet need to see their stillborn baby, and they experienced communication challenges such as being blindsided/misled about the baby during their interactions with health personnel. The participants experienced emotional and psychological reactions to grief that manifested in the form of emotional pain, sadness, blame and shock, but having a sense of gratitude helped them cope. The findings of this study highlight gaps in bereavement care and suggest the need for basic bereavement training for health personnel.

One impact of incurable HIV infection is the large number of orphans and vulnerable children (OVC) who are affected by HIV. The age-based criteria used to determine support eligibility for HIV orphans, however, exclude older orphans (≥18 years of age) from support. We conducted a literature survey in order to explore possible inclusion of older orphans (ages 18-24 years) in HIV orphanhood research. We found 17 studies conducted in eight countries that met the review inclusion criteria. Findings from the review revealed that older HIV orphans are underrepresented in the OVC literature. The emerging, but limited, evidence suggests that older orphans are at risk for poorer psychosocial and reproductive outcomes. We recommend increasing inclusion of older orphans in HIV orphan research because of their complex physical, reproductive, and psychosocial needs. This inclusion is necessary to allow their experiences and needs to become clearer.

Diarrhoea has been identified as the second leading cause of mortality among under-5s and also claims more life than HIV, measles and malaria combined together in the same category of population. This article is a combination of literature review and personal experience of lessons learnt from past diarrhoea outbreaks in Botswana that caused significant rate of mortality among under-5s. The paper used literature review to identify contributory factors to diarrhoea burden among under-5s in Botswana and applied a community health nursing framework (PRECEDE-PROCEED) to tackle the problems identified. The study revealed that Botswana mothers are lacking in knowledge related to exclusive breastfeeding, prevention and treatment of diarrhoea disease. The paper recommends that health-care workers in Botswana be sensitized on current diarrhoea management to tailor their health education methods appropriately.

Conceptual models assist in the better understanding of phenomena and also in the planning of effective responses. We applied the Dual Process Model (DPM) to examine how six later adolescents (ages 18-24 years) experienced the multiple stressors of bereavement, maturational tasks, and caregiving roles. Using a semi-structured interviewing approach, participants who were purposively recruited narrated how parental loss to HIV impacted their lives. In relation to the proposition of the DPM model, the study revealed that participants (a) anticipated grief before the death of their parents, (b) engaged in activities to return to normalcy after parental loss, and (c) neglected grief emotions in order to cope with the demands of caregiving. The application of a model to examine responses to bereavement can assist health care practitioners to appreciate and have a deeper understanding of how best to approach caring interventions for late adolescents with multiple stressors.
