Abstract

Graphical models serve as effective tools for visualizing conditional dependencies between variables. However, as the number of variables grows, interpretation becomes increasingly difficult, and estimation uncertainty increases due to the large number of parameters relative to the number of observations. To address these challenges, we introduce the Clusterpath estimator of the Gaussian Graphical Model (CGGM) that encourages variable clustering in the graphical model in a data-driven way. Through the use of a clusterpath penalty, we group variables together, which in turn results in a block-structured precision matrix whose block structure remains preserved in the covariance matrix. We present a computationally efficient implementation of the CGGM estimator by using a cyclic block coordinate descent algorithm. In simulations, we show that CGGM not only matches, but oftentimes outperforms other state-of-the-art methods for variable clustering in graphical models. Abstract

IntroductionGlobal multidrug-resistant tuberculosis (MDR-TB) treatment success rates remain suboptimal. Highly active WHO group A drugs moxifloxacin and levofloxacin show intraindividual and interindividual pharmacokinetic variability which can cause low drug exposure. Therefore, therapeutic drug monitoring (TDM) of fluoroquinolones is recommended to personalise the drug dosage, aiming to prevent the development of drug resistance and optimise treatment. However, TDM is considered laborious and expensive, and the clinical benefit in MDR-TB has not been extensively studied. This observational multicentre study aims to determine the feasibility of centralised TDM and to investigate the impact of fluoroquinolone TDM on sputum conversion rates in patients with MDR-TB compared with historical controls.Methods and analysisPatients aged 18 years or older with sputum smear and culture-positive pulmonary MDR-TB will be eligible for inclusion. Patients receiving TDM using a limited sampling strategy (t=0 and t=5 hours) will be matched to historical controls without TDM in a 1:2 ratio. Sample analysis and dosing advice will be performed in a centralised laboratory. Centralised TDM will be considered feasible if >80% of the dosing recommendations are returned within 7 days after sampling and 100% within 14 days. The number of patients who are sputum smear and culture-negative after 2 months of treatment will be determined in the prospective TDM group and will be compared with the control group without TDM to determine the impact of TDM.Ethics and disseminationEthical clearance was obtained by the ethical review committees of the 10 participating hospitals according to local procedures or is pending (online supplementary file 1). Patients will be included after obtaining written informed consent. We aim to publish the study results in a peer-reviewed journal.Trial registration numberClinicalTrials.gov Registry (NCT03409315).

Abstract

Post‐transplant anaemia and reduced muscle mass and strength are highly prevalent in kidney transplant recipients (KTRs). Decreased haemoglobin levels, a marker of anaemia, could adversely affect muscle mass and strength through multiple mechanisms, among others, through diminished tissue oxygenation. We aimed to investigate the association between haemoglobin levels with muscle mass and strength in KTRs. We included stable KTRs from the TransplantLines Biobank and Cohort study with a functional graft ≥1 year post‐transplantation. Muscle mass was assessed using 24 h urinary creatinine excretion rate (CER) and bioelectrical impedance analysis (BIA). Muscle strength was assessed with a handgrip strength test using a dynamometer and, in a subgroup (n= 290), with the five‐times sit‐to‐stand (FTSTS) test. We used multivariable linear and logistic regression analyses to investigate the associations of haemoglobin levels with muscle mass and strength. In 871 included KTRs [median age 58 (interquartile range (IQR), 48–66)] years; 60% men; eGFR 51 ± 18 mL/min/1.73 m2) who were 3.5 (1.0–10.2) years post‐transplantation, the mean serum haemoglobin level was 13.9 ± 1.8 g/dL in men and 12.8 ± 1.5 g/dL in women. Lower haemoglobin levels were independently associated with a lower CER (std. β = 0.07, P= 0.01), BIA‐derived skeletal muscle mass (std. β = 0.22, P< 0.001), handgrip strength (std. β = 0.15, P< 0.001), and worse FTSTS test scores (std. β = −0.17, P= 0.02). KTRs in the lowest age‐specific and sex‐specific quartile of haemoglobin levels had an increased risk of being in the worst age‐specific and sex‐specific quartile of CER (fully adjusted OR, 2.09; 95% CI 1.15–3.77; P= 0.02), handgrip strength (fully adjusted OR, 3.30; 95% CI 1.95–5.59; P< 0.001), and FTSTS test score (fully adjusted OR, 7.21; 95% CI 2.59–20.05; P< 0.001). Low haemoglobin levels are strongly associated with decreased muscle mass and strength in KTRs. Future investigation will need to investigate whether maintaining higher haemoglobin levels may improve muscle mass and strength in KTRs.

Abstract

Background: Measurement of rifampin levels is not part of routine practice. However, low levels are associated with failure of tuberculosis treatment. The clinical relevance of serum levels in daily practice is unclear. The objective was to evaluate rifampin serum concentrations and factors associated with insufficient concentrations. Methods: Patients with at least one rifampin concentration drawn 3 hours after intake (C-3) between 2005 and 2014 were included. Data on demographic and clinical characteristics were collected, including side effects and dose adjustments. Two different criteria were used to define adequate concentrations (criterion 1: C(3 )and C-6 >= 3 mg/l; criterion 2: C-3 or C-6 >= 5 mg/l). Results: Of 63 patients, 66% and 76% had a sufficient level according to criterion 1 or 2, respectively. C(3 )exceeded C-6 in most patients, while a late maximum was significantly associated with diabetes mellitus (p = 0.003). A dose adjustment was made in 19% of cases, more frequently in patients with insufficient levels (p = 0.02) or with 2 side effects (p = 0.03). Conclusion: Rifampin levels varied but were mostly adequate and a single measurement at 3 hours after intake provided the required information in most cases, indicating that full AUC(0.24) measurements could be limited to specific situations.

Abstract

A new guideline: 'Intoxication: initial approach in the hospital' will be published this year. This guideline sets out the latest insights on gastrointestinal decontamination in intoxication; the advice is summarized in a flowchart.- The advice is to generally administer activated charcoal, unless there are indications that the toxin will not bind to activated charcoal or that the amount of toxin that the patient has ingested is too great; in these cases gastric lavage can be considered.- Activated charcoal can be administered up to 2 hours after the ingestion of a toxic substance, unless there are contra-indications. Multiple-dose activated charcoal in combination with a laxative can be administered in cases of overdose with toxins that use the enterohepatic circulation (such as theophylline, carbamazepine, quinine, dapsone and phenobarbital).- Gastric lavage should be limited to extremely serious intoxication, when the substance has been ingested less than 1-2 hours previously.- Whole-bowel irrigation should not be performed routinely but should be limited to ingestion of toxins with sustained release or enteric coating, or for toxins that do not bind to activated charcoal.

Abstract

Women with cystic fibrosis (CF) now regularly survive into their reproductive years in good health and wish to have a baby. Many pregnancies have been reported in the literature and it is clear that whilst the outcome for the baby is generally good and some mothers do very well, others find either their CF complicates the pregnancy or is adversely affected by the pregnancy. For some, pregnancy may only become possible after transplantation. Optimal treatment of all aspects of CF needs to be maintained from the preconceptual period until after the baby is born. This supplement offers consensus guidelines based on review of the literature and experience of paediatricians, adult and transplant physicians, and nurses, physiotherapists, dietitians, pharmacists and psychologists experienced in CF and anaesthetist and obstetricians with experience of CF pregnancy.
