Abstract

Background: There is insufficient evidence to guide ventilatory targets in acute brain injury (ABI). Recent studies have shown associations between mechanical power (MP) and mortality in critical care populations. We aimed to describe MP in ventilated patients with ABI, and evaluate associations between MP and clinical outcomes. Methods: In this preplanned, secondary analysis of a prospective, multi-center, observational cohort study (ENIO, NCT03400904), we included adult patients with ABI (Glasgow Coma Scale ≤ 12 before intubation) who required mechanical ventilation (MV) ≥ 24 h. Using multivariable log binomial regressions, we separately assessed associations between MP on hospital day (HD)1, HD3, HD7 and clinical outcomes: hospital mortality, need for reintubation, tracheostomy placement, and development of acute respiratory distress syndrome (ARDS). Results: We included 1217 patients (mean age 51.2 years [SD 18.1], 66% male, mean body mass index [BMI] 26.3 [SD 5.18]) hospitalized at 62 intensive care units in 18 countries. Hospital mortality was 11% (n = 139), 44% (n = 536) were extubated by HD7 of which 20% (107/536) required reintubation, 28% (n = 340) underwent tracheostomy placement, and 9% (n = 114) developed ARDS. The median MP on HD1, HD3, and HD7 was 11.9 J/min [IQR 9.2–15.1], 13 J/min [IQR 10–17], and 14 J/min [IQR 11–20], respectively. MP was overall higher in patients with ARDS, especially those with higher ARDS severity. After controlling for same-day pressure of arterial oxygen/fraction of inspired oxygen (P/F ratio), BMI, and neurological severity, MP at HD1, HD3, and HD7 was independently associated with hospital mortality, reintubation and tracheostomy placement. The adjusted relative risk (aRR) was greater at higher MP, and strongest for: mortality on HD1 (compared to the HD1 median MP 11.9 J/min, aRR at 17 J/min was 1.22, 95% CI 1.14–1.30) and HD3 (1.38, 95% CI 1.23–1.53), reintubation on HD1 (1.64; 95% CI 1.57–1.72), and tracheostomy on HD7 (1

5. Benchmarking and Performance Measurement.

Abstract

This paper defines benchmarking and its relationship to quality management, describes a project which applied the technique in a library context, and explores the relationship between performance measurement and benchmarking. Numerous benchmarking methods contain similar elements: deciding what to benchmark; identifying partners; gathering information; analyzing what has been collected; and implementing improvements. The Library of the Royal Military College of Science (RMCS) at Cranfield University (Shrivenham, Swindon, England) initiated a Total Quality Management (TQM) program to help attain high quality service for customers, control costs, and use optimum pricing to satisfy the interests of sponsors. The study surveyed and conducted site visits to other university libraries which acted as benchmarking partners. The benchmarking exercise measured the following areas: availability of up-to-date stock; unit costs; staff development, ability, and approachability; user experience, education, and feedback; innovation; and learning environment. Knowledge gained from the project and pitfalls that can adversely affect the success of benchmarking activities are discussed. As a result of the benchmarking exercise, the RMCS Library initiated a number of specific service improvements, and gained confidence, reassurance, tangible evidence about the relative quality of its service, and a strong sense of the common issues and concerns within the "industry" of information provision. 11. Value, Impact, and the Transcendent Library: Progress and Pressures in Performance Measurement and Evaluation

Abstract

Libraries are under pressure to prove their worth and may not have achieved this fully successfully. There is a resultant growing requirement for value and impact measurement in academic and research libraries. This essay reviews the natural history of library performance measurement and suggests that proof of worth will be measured by the higher-order beneficial effects that libraries deliver. Evaluation within current frames of reference will therefore be insufficient. Compelling proof will reside in transcendent contribution beyond immediate temporal, spatial, and influential boundaries of libraries. Answers to value contribution will not arise from goals but from values, as concepts of value depend on values systems. A reflection on the meaning of value for libraries is presented. A new, higher-order framework for evaluation and performance measurement based on a values scorecard is suggested. The concept of the transcendent library is offered as a route to further progress.

27. Benchmarking of Online Information Literacy Tutorials to Identify Lessons Learnt and Best Practice

Abstract

This paper reports the process and findings of a benchmarking exercise conducted on 10 online information literacy tutorials from around the English speaking World. The benchmarking exercise comprised site visits and the completion of an online survey. The aim of the exercise was to establish best practice and to gather any lessons that could be learnt, with the overall purpose of determining ways that our information literacy tutorial could provide the best quality and performance through the identification of user requirements. The method of selecting the tutorials to benchmark against and the areas to include in the review is described. A summary of results from each of the benchmarked areas is included for: pedagogical issues; design, development and user feedback; content; interactivity; and look and feel.

Abstract

This paper discusses the use of the LibQUAL+ library survey instrument and its ability to define an agenda for further improvements in a setting where quality maturity is high and library performance tends to be generally acknowledged as good. The results from the survey run in 2003 and 2005 are presented, and how these results have been used to form a change agenda discussed. In particular the indications of the effect of improvement projects across the years are described. The revelation of differing perceptions of library staff and users to the service is described, and the approaches taken to close this gap through more effective internal marketing and communication are discussed. Comparisons of the LibQUAL+ results with an internal exit questionnaire are also presented. Conclusions about the benefit of the use of the LibQUAL+ survey instrument within a local context of quality are drawn.

29. LibQUAL+ in the UK and Ireland: three years findings and experience

Abstract

Presents an overview of the SCONUL LibQUAL+ participation, reports some of the overall results of the 2003-2005 SCONUL cohorts, and describes some feedback from participants and the lessons learnt from the process. The overall results for SCONUL in 2005 show that performance is a little above the minimum in Affect of Service and some problems of performance below the minimum in Information Control and Library as Place. There is a marked difference between the results for 2003 and the subsequent years, and one without obvious immediate explanation. Library as Place is the dimension which suggests more research is needed to understand the variation across the three years, and the particularly good results in the 2003 cohort. Consistently across all years the ability of libraries to deliver printed materials that staff and students require for their work, and the print or electronic periodical collection provision has scored below users' minimum expectations in the UK. For all major dimensions there was a decrease in perceptions in 2004. Library as Place is the only dimension to see a steady increase in users' expectations over the three years, Information Control expectations have remained fairly stable and Affect of Service has seen a decline in users' expectations. Abstract

Purpose: To provide a critique, some sources of data, and a broader conception for informing development of e-measures frameworks for use in academic library services. Approach: Suggests that the broader context for e-measurement is investigated and provides a critique of current thinking. Provides four additional sources or routes for improved measures, including scholarly communication methods, information literacy, developments in measuring library and e-service quality, and the critical success factors of serials staff. Develops a proposed framework for e-measures based on the balanced scorecard approach. Findings: Provides specific suggestions arising from the four sources for relevant e-measures, and provides a framework based on the balanced scorecard which incorporates these and other suggestions for data collection under the following perspectives: financial, customers, process and projects, staff development, and organisational learning and development. Value: The paper will be valuable to library directors and managers and library researchers interested in the field of performance measurement and evaluation of e-resources. It provides some original thinking about the problem and suggests some innovative techniques and approaches to addressing the need to develop effective and useful performance measurement frameworks.

Abstract

Purpose – The purpose of this paper is to define a framework and categorization of the types of evidence required to prove the value and impact of libraries. It questions the limitations of current measurement for library value, and hence for contributing to academic and research library planning and advocacy. The paper describes and draws on some of the recent progress in value and impact measurement over the last five years. Scenario planning exercises conducted by both ARL and SCONUL are used to analyse the likely future value proposition emerging for libraries. A values-based value scorecard is proposed, which would demonstrate the transcendent value of academic and research libraries now and in the future. Design/methodology/approach – The paper examines research literature from library and general management texts to propose a framework for measuring the value and impact of libraries. Findings – The paper concludes that a value scorecard can be used alongside the balanced scorecard to add a more value-oriented picture of library strengths and contribution. The potential benefit of the proposed value scorecard is to gather evidence which will assist both strategic planning and decision making in relation to key areas for future investment. Originality/value – The proposal defines a new framework aimed at measuring the full value of academic research libraries, considering both tangible and intangible assets. The proposal has been developed by exploring measurement gaps in the library management field and exploring potential options from this area and general management literature. The article discusses a study on the chemical description and in vitro fermentation of Brassica napus straw (BNS) treated with the Trametes versicolor aerobic fungus. It mentions that the straw was either not treated or treated with two strains of the said fungus wherein glucose was provided to urge their growth. It also notes that treatments under the fungi have decreased neutral-detergent fibre and increased protein and lignin degradation products' concentration in the straw.

41. LibQUAL+®: The SCONUL experience.

Academic libraries in Great Britain quickly recognized that a higher level of designed survey would provide more useful data to create agendas for action, and to identify the priorities of users, in addition to indications of satisfaction with existing services. From the outset, Great Britain and Irish engagement with LibQUAL+ has been through SCONUL with the Working Group on Performance Improvement undertaking the leadership and coordinating role. Since 2003 Great Britain LibQUAL+ return has continued to rise, with an increase in responses of 45%. SCONUL's Working Group on Performance Improvement is encouraging and organizing a 2006 consortium of SCONUL members for the LibQUAL+ survey.

Abstract

LibQUAL+® is a library customer satisfaction survey developed by the Association of Research Libraries (ARL) in association with Texas A&M University (Association of Research Libraries, 2011). The survey consists of 22 questions on library services across three dimensions: Affect of Service, containing questions relating to the library staff; Information Control, covering the library resources and access to them; and Library as Place, assessing the physical environment. One of the key strengths of LibQUAL+® is its use of gap theory to evaluate customer expectations as well as perceptions. For each of the 22 questions respondents are asked their minimum and desired expectations along with their current perceived level of service on a nine-point scoring scale. The standardised survey enables participating institutions to benchmark their scores against one another, and against an aggregated score for their consortium.

Abstract

Purpose: This paper reports the progress and development of the Society of College, National and University Libraries (SCONUL) LibQUAL+® consortium in the UK and Ireland since 2003, together with analyses of the results of the survey instrument from international and national perspective. Approach: A consortium of SCONUL member libraries has been using LibQUAL+® in the UK and Ireland since 2003. Over 70 UK and Irish Higher Education Libraries have implemented the survey providing a wealth of data on the quality of academic library provision. This paper examines the composition of the consortium and its implementation patterns. The results of the surveys at a national and international level are discussed with statistical analyses on users’ expectations of libraries, how these are changing over time, and how successful libraries are at meeting these expectations. Demonstrable outcomes from LibQUAL+® at a local and national level are also presented. Findings: Customer perceptions of library services within the UK have increased since 2003 indicating the results of action taken has had a positive impact. The opportunity for improvement does however remain, encouraging us to strive to further in our quest for the perfect library service. Value: This paper will be of interest to those involved in library surveys or other methods of listening to users. In particular it may assist the development of deeper understanding of library users’ expectations in a changing environment, and will also therefore be of value to all those concerned with library strategy and development.

Abstract

The lower limit of domain size resolution using microphase separation of short poly(acrylic acid) homopolymers equipped with a short fluorinated tail, posing as an antagonist ‘A block’ in pseudo AB block copolymers has been investigated. An alkyl halide initiator with a fluorocarbon chain was utilized as a first ‘A block’ in the synthesis of low molecular weight polymers (1400–4300 g mol−1) using photoinduced Cu(II)-mediated polymerization allowing for very narrow dispersity. Poly(tert-butyl acrylate) was synthesized and subsequently deprotected to give very low degrees of polymerization (N), amphiphilic polymers with low dispersity (Đ = 1.06–1.13). By exploiting the high driving force for demixing and the well-defined ‘block’ sizes, we are able to control the nanostructure in terms of domain size (down to 3.4 nm full-pitch) and morphology. This work demonstrates the simple and highly controlled synthesis of polymers to push the boundaries of the smallest achievable domain sizes obtained from polymer self-assembly.
