Abstract

Purpose: The service configuration with distinct child and adolescent mental health services (CAMHS) and adult mental health services (AMHS) may be a barrier to continuity of care. Because of a lack of transition policy, CAMHS clinicians have to decide whether and when a young person should transition to AMHS. This study describes which characteristics are associated with the clinicians’ advice to continue treatment at AMHS. Methods: Demographic, family, clinical, treatment, and service-use characteristics of the MILESTONE cohort of 763 young people from 39 CAMHS in Europe were assessed using multi-informant and standardized assessment tools. Logistic mixed models were fitted to assess the relationship between these characteristics and clinicians’ transition recommendations. Results: Young people with higher clinician-rated severity of psychopathology scores, with self- and parent-reported need for ongoing treatment, with lower everyday functional skills and without self-reported psychotic experiences were more likely to be recommended to continue treatment. Among those who had been recommended to continue treatment, young people who used psychotropic medication, who had been in CAMHS for more than a year, and for whom appropriate AMHS were available were more likely to be recommended to continue treatment at AMHS. Young people whose parents indicated a need for ongoing treatment were more likely to be recommended to stay in CAMHS. Conclusion: Although the decision regarding continuity of treatment was mostly determined by a small set of clinical characteristics, the recommendation to continue treatment at AMHS was mostly affected by service-use related characteristics, such as the availability of appropriate services.

Abstract

Purpose: To test reliability and reproducibility of ESASO morphologic OCT-based classification of diabetic maculopathy (DM). Methods: This is a multi-center cross-sectional study including a coordination center (CC) and 18 participating centers (PCs). After instruction on the correct use of ESASO Classification, the validation process was carried out in two consecutive stages. In the first retrospective phase, we evaluated the concordance between PCs and CC in the staging of OCT images collected during PCs’ daily activity (608 images). In a second prospective phase, we analyzed the inter-observer agreement of staging assigned by each PCs to OCT images selected by the CC (22 images). Results: The overall concordance achieved in the retrospective phase was 89.8% (Kappa = 0.83 (95% CI: 0.78–0.87); p<0.0001). In 99.5% of cases, concordance did not differ by more than one stage. In the prospective phase, PCs reached an inter-operator agreement of 93.0% (Krippendorff's Alpha = 0.953, 95% CI: 0.929–0.977, p<0.0001). Any discrepancy among the 22 images was within one stage. Conclusion: The results achieved in this study confirm that ESASO OCT-based Classification can be considered as an easy and reproducible method to stage DM during clinical practice. A diffused use of a common and validated method to describe the progression of retinal damage in DM may offer several clinical and scientific advantages.

Abstract

The aims of this study were to assess the sensitivity of positron emission tomography (PET) using [18F]fluorodeoxyglucose (18F-FDG) in the detection of uveal melanoma, and to establish the relationship between pre-operative 18F-FDG uptake and a number of pathological features of uveal melanoma. Twenty consecutive patients with a clinical diagnosis of uveal melanoma were enrolled in the study. 18F-FDG uptake was assessed in all subjects and the following parameters were assessed in 11 enucleated subjects: the mitotic index, the MIB-1 proliferating cell index, number of epithelioid cells, largest tumour diameter, tumoral necrosis and inflammatory infiltration. Tumours with a diameter less than 7.5 mm were not detected by PET, possibly because of limited spatial resolution, and only 7 of 12 tumours with a diameter greater than 7.5 mm were detected. With tumours greater than 7.5 mm in diameter, PET and 18F-FDG allow two subgroups to be distinguished: those with high and those with low glucose consumption. Apart from tumour size, 18F-FDG uptake was not related to the pathological features examined. We hypothesize that 18F-FDG uptake may be related to the ratio of viable to non-viable cells, or to the hypoxic cell fraction within the tumour.

36. Ocular hypotensive efficacy and safety of travoprost 0.004% in inadequately controlled primary open-angle glaucoma or ocular hypertension: short-term, multicenter, prospective study.

Abstract

Objective: To evaluate the intraocular pressure (IOP) lowering efficacy and safety of travoprost 0.004% in patients with open-angle glaucoma or ocular hypertension, poorly controlled with or intolerance to beta-blockers. To record the short-term effect on diastolic ocular perfusion pressure (DOPP).Research Design and Methods: One hundred and three patients with open-angle glaucoma or ocular hypertension were treated with travoprost 0.004% once daily for 90 days in an open-label, non-controlled study. Efficacy and safety were assessed at baseline, after 45 and 90 days. Clinical registry number IT0301.Main Outcome Measures: The primary outcome measure, IOP, was recorded at 10 am, 12 pm, and 4 pm at each visit. DOPP was evaluated at 10 am, at baseline and visit 3. Safety measures included adverse events, biomicroscopy, visual acuity, heart rate, and blood pressure.Results: Mean IOP was reduced from 22.2 +/- 1.7 mmHg to 16.5 +/- 2.1 after 45 days (p < 0.0001), and to 16.1 +/- 2.2 after 90 days (p < 0.0001). The DOPP increased by 5.3 +/- 6.3 mmHg after 90 days of treatment (p < 0.0001). No drug related serious adverse events were reported during the study.Conclusions: The open-label and non-comparative nature of the study represented its principal limitations. The study confirmed the efficacy and tolerability of travoprost in the treatment of open-angle glaucoma or ocular hypertension, in a subset of patients unsuccessfully treated with beta-blockers. In this study, travoprost significantly increased DOPP at short-term follow-up. Further studies to assess the effect of travoprost on DOPP are warranted. [ABSTRACT FROM AUTHOR]

Abstract

Evidences from studies on patients with overt cerebellar diseases as well as on healthy individuals suggest a possible role for the cerebellum in cognition, mood and behaviour. The aim of the present study is to review those neuroimaging studies examining the cerebellum in Bipolar Disorder (BD) and Major Depressive Disorder (MDD) and to illustrate a possible role of cerebellum in their pathophysiological mechanisms. Cellular and molecular findings from post mortem studies such as mitochondria abnormalities, brain-derived neurotrophic factor and its high affinity receptor tyrosine kinase B, transcription factor specificity protein 4, the glial fibrillary acidic protein have also been reviewed. In total 28 studies have been included in the review; among these, 12 studies were related to structural and functional neuroimaging of cerebellum in BD, 13 studies to structural and functional neuroimaging in MDD and 4 studies to cellular and molecular issues. This wealth of evidence from contemporary studies, indicating that the cerebellum (vermis in particular) is engaged in the modulation of emotional processing, provides strong support for the clinical relevance of cerebellar-limbic connections, and is in agreement with earlier clinical and electrophysiological studies that lead to the indication of the cerebellum as an "emotional pacemaker".

Abstract

To investigate corneal nerve damage and its subsequent regeneration after excimer laser photokeratectomy in rabbit eyes.Corneal photokeratectomy was performed on the right eyes of 34 pigmented rabbits, while the left eyes were used as a control. A Summit UV 200 excimer laser (Summit Technology, Waltham, MA) was used to produce a 100 microns depth phototherapeutic keratectomy (4.5 mm diameter). The rabbits were killed at 0, 1, 2, 3, 4, 5, 7, 8, 10, 15, and 20 days and 1, 2, 3, and 4 months after treatment. Corneal nerve regeneration was investigated using a monoclonal antibody directed against a 150 kD subunit of neurofilaments, in combination with acetylcholinesterase staining.At time 0, the subepithelial plexus had disappeared completely from the photoablated area. One day after treatment, some regenerating nerves sprouting within the treated area were visualized by a monoclonal antibody directed against a 150 kD subunit of neurofilaments. Acetylcholinesterase staining was detectable 7 days later. One month after surgery the subepithelial plexus had already reappeared, but was morphologically disorganized. Four months after treatment, the newly formed subepithelial plexus had considerably thickened in comparison with that of the control eye.This study shows that corneal nerve regeneration after excimer laser photoablation in rabbit eyes, although more intense, is similar to that produced after other surgical procedures involving the epithelium and the anterior stroma or one of the two.

Abstract

Background: Cataract surgery is the most common operation performed worldwide. A fixed topical corticosteroid-antibiotic combination is usually prescribed in clinical practice for 2 or more weeks to treat post surgical inflammation and prevent infection. However, this protracted schedule may increase the incidence of corticosteroid-related adverse events and notably promote antibiotic resistance. Methods: This International, multicentre, randomized, blinded-assessor, parallel-group clinical study evaluated the non-inferiority of 1-week levofloxacin/dexamethasone eye drops, followed by 1-week dexamethasone alone, vs. 2-week gold-standard tobramycin/dexamethasone (one drop QID for all schedules) to prevent and treat ocular inflammation and prevent infection after uncomplicated cataract surgery. Non-inferiority was defined as the lower limit of the 95% confidence interval (CI) around a treatment difference >–10%. The study randomized 808 patients enrolled in 53 centres (Italy, Germany, Spain and Russia). The primary endpoint was the proportion of patients without anterior chamber inflammation on day 15 defined as the end of treatment. Endophthalmitis was the key secondary endpoint. This study is registered with EudraCT code: 2018-000286-36. Results: After the end of treatment, 95.2% of the patients in the test arm vs. 94.9% of the control arm had no signs of inflammation in the anterior chamber (difference between proportions of patients = 0.028; 95% CI: −0.0275/0.0331). No case of endophthalmitis was reported. No statistically significant difference was evident in any of the other secondary endpoints. Both treatments were well tolerated. Conclusions: Non-inferiority of the new short pharmacological strategy was proven. One week of levofloxacin/dexamethasone prevents infection, ensures complete control of inflammation in almost all patients and may contain antibiotic resistance.

Abstract

Purpose The service configuration with distinct child and adolescent mental health services (CAMHS) and adult mental health services (AMHS) may be a barrier to continuity of care. Because of a lack of transition policy, CAMHS clinicians have to decide whether and when a young person should transition to AMHS. This study describes which characteristics are associated with the clinicians’ advice to continue treatment at AMHS. Methods Demographic, family, clinical, treatment, and service-use characteristics of the MILESTONE cohort of 763 young people from 39 CAMHS in Europe were assessed using multi-informant and standardized assessment tools. Logistic mixed models were fitted to assess the relationship between these characteristics and clinicians’ transition recommendations. Results Young people with higher clinician-rated severity of psychopathology scores, with self- and parent-reported need for ongoing treatment, with lower everyday functional skills and without self-reported psychotic experiences were more likely to be recommended to continue treatment. Among those who had been recommended to continue treatment, young people who used psychotropic medication, who had been in CAMHS for more than a year, and for whom appropriate AMHS were available were more likely to be recommended to continue treatment at AMHS. Young people whose parents indicated a need for ongoing treatment were more likely to be recommended to stay in CAMHS. Conclusion Although the decision regarding continuity of treatment was mostly determined by a small set of clinical characteristics, the recommendation to continue treatment at AMHS was mostly affected by service-use related characteristics, such as the availability of appropriate services.

Abstract

The authors report a case of congenital bilateral corneal opacities in which one of the eyes was enucleated because of malignant glaucoma and corneal perforation. Corneal defects and iridocorneal adhesion were found, but aphakia was the major pathologic ocular finding. The clinical picture and pathology study indicated this case as a Peters' anomaly presenting congenital aphakia.

Abstract

Background and Objective A randomized clinical trial using the argon-green (514 nm) and the double-frequency Nd:YAG (532 nm) lasers was carried out on 42 eyes with proliferative diabetic retinopathy (PDR), with the aim of assessing the long-term effects of double-frequency Nd:YAG panretinal photocoagulation (PRP). Materials and Methods Twenty-one eyes were randomized to argon laser treatment (ALT) and 21 to double-frequency Nd:YAG laser treatment (NdLT). The mean follow-up was 28.90 months (±6.13) in the ALT group and 29.57 months (±5.17) in the NdLT group. Results In the ALT group, 20 eyes (95.2%) showed regression of PDR and one eye (4.8%) enlargement of pre-existing new vessels. In the NdLT group, neovascularization regressed in 20 eyes (95.2%) and increased in one (4.8%). Conclusions The long-term efficacy of double-frequency Nd:YAG laser PRP in the treatment of PDR thus appears to be similar to that of argon-green. © 1996 Wiley-Liss, Inc.

Abstract

The aims of this study were to assess the sensitivity of positron emission tomography (PET) using [18F]fluorodeoxyglucose (18F-FDG) in the detection of uveal melanoma, and to establish the relationship between pre-operative 18F-FDG uptake and a number of pathological features of uveal melanoma. Twenty consecutive patients with a clinical diagnosis of uveal melanoma were enrolled in the study. 18F-FDG uptake was assessed in all subjects and the following parameters were assessed in 11 enucleated subjects: the mitotic index, the MIB-1 proliferating cell index, number of epithelioid cells, largest tumour diameter, tumoral necrosis and inflammatory infiltration. Tumours with a diameter less than 7.5 mm were not detected by PET, possibly because of limited spatial resolution, and only 7 of 12 tumours with a diameter greater than 7.5 mm were detected. With tumours greater than 7.5 mm in diameter, PET and 18F-FDG allow two subgroups to be distinguished: those with high and those with low glucose consumption. Apart from tumour size, 18F-FDG uptake was not related to the pathological features examined. We hypothesize that 18F-FDG uptake may be related to the ratio of viable to non-viable cells, or to the hypoxic cell fraction within the tumour.

Abstract

Aims: To emphasize that ocular ischemic syndrome (OIS) is a rare but threatening condition. It is a consequence of occlusion or dissection of internal carotid artery (ICA)., Methods: A 66-year-old caucasian male presented in the emergency room with painful vision loss in his right eye and mild neurological symptoms. He was diagnosed ocular ischemic syndrome (OIS) in ICA and ophthalmic artery occlusion due to right ICA dissection., Conclusions: It is very important to know and be able to recognise OIS as it may often be misdiagnosed or treated as a different entity. 50. The Impact of Coping Strategies and Perceived Family Support on Depressive and Anxious Symptomatology During the Coronavirus Pandemic (COVID-19) Lockdown.

Abstract

The coronavirus pandemic represents a severe global crisis, affecting physical, and psychological health. Lockdown rules imposed to counteract the rapid growth of COVID-19, mainly social restrictions, have represented a risk factor for developing depressive and anxious symptoms. The research aims are to explore the effect of coping strategies and perceived social support on depressive and anxious symptomatology during the COVID-19 pandemic. Ninety-six healthy people (46 males, mean age = 39.3; SD = 16.6) completed through on-line platform: Socio-demographic questionnaire, Coping Inventory for Stressful Situations (CISS), Multidimensional Scale of Perceived Social Support (MSPSS) and Symptom Checklist-90-Revised (SCL-90-R), 3 weeks after the imposition of lockdown restrictions. SCL-90-R Depression scores showed significant positive correlation with CISS Emotion ( r = 0.85; p = 0.001) and Avoidant ( r = 0.34; p = 0.018), a significant negative correlation with MSPSS Family support ( r = -0.43; p = 0.003). SCL-90-R Anxiety scores showed a significant positive correlation with CISS Emotion ( r = 0.72; p = 0.001) and Avoidant ( r = 0.35; p = 0.016). No significant correlations between both CISS Emotion and Avoidant scales with social support emerged. Two Multiple Linear Regression analysis were performed using, respectively, SCL-90-R Depression and Anxiety scores as dependent variables, and the CISS and MSPSS scales, age, and gender as predictors. The first regression model ( R 2 = 0.78; adjusted R 2 = 0.75) revealed CISS Emotion (β = 0.83; p = 0.001) and MSPSS Family support (β = -0.24; p = 0.004) had a predictive effect on SCL-90-R Depression scores. The second regression model ( R 2 = 0.52; adjusted R 2 = 0.472) revealed that only CISS Emotion (β = 0.71; p = 0.001) predicted the SCL-90-R Anxiety scores. In conclusion, during the COVID-19 pandemic lockdowns, coping focus on emotions seemed to increase anxious and depressive symptoms, probably due to the uncontrollable nature of the stressful event and the high emotional response. Family support which reduces the sense of loneliness had an exclusive role in mitigating depressive symptoms. These results highlight the importance of promoting psychological strategies to improve emotional regulation skills, reducing isolation from family, to prevent mood symptomatology in healthy citizens during large-scale health crises., (Copyright © 2020 Mariani, Renzi, Di Trani, Trabucchi, Danskin and Tambelli.)
