In this study, the g-C3N4/CoFe2O4/Reduced graphene oxide composites were successfully synthesized by the co-precipitation method combined with mixed phase mixture. The obtained results from characterization methods such as XRD, SEM, FT-IR, EDX,… showed that the composite has a high structure and crystallinity, and spinel ferrite particles were dispersed fairly evenly onto reduced graphene oxide sheets as well as layers of g-C3N4. The existence of Co/Fe-O and Co-O-C bonds in materials was determined. The photocatalytic activity of g-C3N4/CoFe2O4/Reduced graphene oxide (GCN/CF-rGO) was estimated through the degradation of Tetracycline (TC) in aqueous solution. TC decomposition efficiency is up to 95% after 240 minutes of reaction and is higher than that of each component material. GCN/CF-rGO is a potential catalyst for effective application in TC decomposition reaction under visible light and has the ability to implement treatment in practice.

Various studies have evaluated that acerola fruit is one of the best natural sources of vitamin- C. Besides that, the fruits also contain a high level of phenolic and carotenoids. The result tested in vitro performed that acerola extract can be antioxidative, anti-inflammatory, antihyperglycemic, antitumor antigenotoxic and hepatoprotective activity. The techniques such as pasteurization, fermentation, extraction, drying, encapsulation can diversify acerola products.

Background: Mixing vaccines was proposed as a solution to tackle supply chain interruptions during the crisis of the COVID-19 pandemic. This study aimed to investigate the safety of mixing COVID-19 vaccines for a booster dose in Hanoi, Vietnam. Method: A cross-sectional study was conducted via a telephone-based interview to identify the adverse events following COVID-19 vaccination among 719 participants in Hanoi, Vietnam. Results: In total, 45.76% of participants experienced at least one adverse event following two doses of the COVID-19 vaccine. Most of the adverse events were local effects with mild symptoms such as fever, headache, muscle pain, and/or pain at the site. In general, matching two doses in the same vaccines was not associated with the adverse events as compared to mixing vaccines (OR = 1.43, 96%CI: 0.93–2.2), except matching two doses of Pfizer (OR = 2.25, 95%CI: 1.33–3.82). Conclusion: The findings of this study suggest the overall safety of mixed vaccination. In light of the vaccine shortage, mixing vaccinations for COVID-19 prevention is a good solution. Further studies with larger cohorts and investigating immunity following mixing vaccines are needed to elucidate the mechanism.

Water mint (Mentha aquatica) is well-known as a popular herbal plant in Viet Nam. Rosmarinic acid (RA) is a phenolic compound found in many genus of Mentha and has numerous biological activities. Its applications are wide from the food industry to cosmetics. The purpose of this study was to evaluate the effects of different plant parts, drying, various solvent to sample ratios (1:20, 1:30, 1:40, 1:50, 1:60, and 1:70 (w/v)) and extraction temperature (30, 50, 70, and 90°C) on RA content of water mint. Ground materials of fresh and dried leaves, fresh and dried whole plants (each for 1 g) were extracted with water. The extraction was performed on magnetic stirrer. The extract was passed through filter paper retaining the clear solution for the quantitative and RA analysis. RA content was determined by using a spectrophotometer. The dried whole plants, a solvent to sample ratio of 1:50 (w/v), extraction temperature of 70°C, and extraction time of 30 minutes were the best conditions for RA content from water mint.

Objectives To understand the perspectives of patients and rheumatologists for tapering DMARDs in RA. Methods Using semi-structured interview guides, we conducted individual interviews and focus groups with RA patients and rheumatologists, which were audiotaped and transcribed. We conducted a pragmatic thematic analysis to identify major themes, comparing and contrasting different views on DMARD tapering between patients and rheumatologists. Results We recruited 28 adult patients with RA (64% women; disease duration 1–54 y) and 23 rheumatologists (52% women). Attitudes across both groups towards tapering DMARDs were ambivalent, ranging from wary to enthusiastic. Both groups expressed concerns, particularly the inability to ‘recapture’ the same level of disease control, while also acknowledging potential positive outcomes such as reduced drug harms. Patient tapering perspectives (whether to and when) changed over time and commonly included non-biologic DMARDs. Patient preferences were influenced by lived experiences, side effects, previous tapering experiences, disease trajectory, remission duration and current life roles. Rheumatologists’ perspectives varied on timing and patient profile to initiate tapering, and were informed by both data and clinical experience. Patients expressed interest in shared decision-making (SDM) and close monitoring during tapering, with ready access to their health-care team if problems arose. Rheumatologists were generally open to tapering (not stopping), though sometimes only when requested by their patients. Conclusion The perspectives of patients and rheumatologists on tapering DMARDs in RA vary and evolve over time. Rheumatologists should periodically discuss DMARD tapering with patients as part of SDM, and ensure monitoring and flare management plans are in place.

Background: Few studies have examined pediatric rheumatologists’ approaches to treatment decision making for biologic therapy for patients with juvenile idiopathic arthritis (JIA). This study presents the qualitative research undertaken to support the development of a Best–Worst Scaling (BWS) survey for tapering in JIA. The study objectives were to (1) describe the treatment decision-making process of pediatric rheumatologists to initiate and taper biologics; and (2) select attributes for a BWS survey. Methods: Pediatric rheumatologists across Canada were recruited to participate in interviews using purposeful sampling. Interviews were conducted until saturation was achieved. Interview recordings were transcribed verbatim and transcripts were analyzed using deductive thematic analysis. Initial codes were organized into themes and subthemes using an iterative process. Attributes for the BWS survey were developed from these themes and a literature review was conducted in parallel to inform survey development. Further refinement of the attributes was done through consultation with the research team. Results: Five pediatric rheumatologists participated in the interviews. Shared decision making was part of the approach to initiating and tapering biologics in their practice. Tapering approaches differed; some pediatric rheumatologists preferred to stop biologics immediately, while others tapered by reducing dose and/or increasing the dose interval over time. A total of 14 attributes were developed for the BWS. Thirteen attributes were selected from the themes that emerged from the qualitative interviews and one attribute was included after review with the research team. Attributes related to patient characteristics included JIA subtype, time in remission, history or presence of joint damage or erosive disease, how challenging it was to achieve remission, and history of flares. Contextual attributes included accessibility of biologics and willingness to taper biologics. Conclusion: This study contributes to the limited literature on pediatric rheumatologists’ approaches to treatment decision making for biologics in JIA and identifies attributes that affect the decision to both initiate and taper. Further research is planned to implement the BWS survey to understand the importance of the attributes identified. Additional investigation is required to determine if these characteristics align with patient and parent preferences.

Patient-centered care is widely acknowledged as an important goal in healthcare delivery. Research has demonstrated that patient perceptions of patient-centred care can be linked to clinical and long-term outcomes in addition to being a useful metric for quality improvement efforts. Objectives: 1) To identify patients’ perspectives and experiences of patient-centerd care in some communes in Thua Thien Hue province; 2) To determine the association between the patients’ perspectives, experiences on patient-centered care and utilization of the health services. Methods: A cross sectional descriptive study was conducted in 313 patients living in Thua Thien Hue province. A structured questionnaire was use to investigate 4 main domains of patient-centered care: Information and Education, Relationships with doctors and other health professionals, making decisions, motivate patients about self-management. We used test ANOVA to analyze the association between patient-centered care and health care utilization among participants (p < 0.05). Results: The percentage of good patient-centered care (≥ 3.75 points) was 43.1% (Mean = 3.58 (0.57)). Patients using health services at the commune health centers had a higher experience in patient-centered care compared to district, province and central hospitals (p < 0.005). Domains “Encourage patients to take care of themselves” and “Relationship factor with doctors” were evaluated low. Supply chain network design problem is increasingly showing its importance, especially the perishable supply chain. This research develops a multi-objective mathematical model to design four-echelon intermodal multi-product perishable supply chain configuration in order to ensure a balance of the three pillars of sustainable development: economy, environment, and society. The optimization objective functions of the model are, respectively, minimizing costs, delivery time, emissions, and the supply-demand mismatch in time. The model addresses particular problems in the supply chain of fresh fruits, which is more challenging compared to other types of perishable products due to its seasonal characteristics. The study proposes a new approach that combines and standardizes the above objective functions into a single weighted objective function. The solution from the model supports the decision-making process at both strategic and tactical levels. Strategically, the model supports decisions about the location, size of facilities, product flows, and workforce level. Tactically, the decision variables provide information on harvest time, delivery time, the delivery route, and mode of transport. To demonstrate its practical applicability, the model is applied to Mekong Delta region, Vietnam, where a variety of fruit types, large yields, and high distribution demand in this region make designing a shared supply chain desirable for its overall economic, environmental, and social concerns. Moreover, sensitivity analysis regarding weights of different objectives is performed to assess possible changes in supply chain configurations. Application of this model to other perishable products, the addition of modes of transport, social policy, and uncertainty parameters may be suggested for future research.

This paper explores the maintenance of livelihoods under climate, environmental, and economic development pressures, through the case of Thang Binh District in Quang Nam Province, Vietnam. Within widespread recognition of the need to link sustainable livelihoods approaches with climate change adaptation, there is growing awareness of the importance of people-centered approaches which keep the diverse experience, capabilities, and knowledges of the most vulnerable at the heart of sustainable livelihoods thinking. In response, this paper explores the conditions for changes in modes of livelihoods in a case study area where top-down strategies for sustainable livelihoods are met with residents’ diverse experiences of vulnerability, and where climate and environmental changes shape residents’ relations with the landscape. The research is undertaken via interviews with residents, farmers/fishers, and local government officials. Our study finds that whilst government-led initiatives for sustainable livelihoods are welcomed in the locality, inflexible policies can make it challenging for the most vulnerable people to access support. Moreover, residents see the capacity to live with and respond to extreme weather events as a critical component of maintaining a sustainable livelihood. Our findings reinforce international literature, showing that ‘the poor’ are not a homogenous category, and illustrate the importance of attention to the smallest levels of government who are tasked with putting sustainable livelihoods initiatives into practice in relation to people’s daily lives.

Registries are an important platform to which persons with dementia and other cognitive impairments can contribute to research studies. Registries also provide an opportunity for patients to stay informed about current studies. Engaging patients in registry development can increase sustainability of a registry and patient retention in clinical registries. We sought the perspective of persons with dementia and their accompanying family members about their registry participation experiences, barriers and facilitators to participation, and potential avenues for improvement of registry processes such as recruitment, data collection, and knowledge translation. Two semi-structured focus groups with persons with dementia and their family members ( n = 18) were conducted and analyzed using thematic content analysis. Participants were recruited from an existing patient registry made up of patients currently being seen in a dementia assessment clinic. The main themes identified included altruistic motives with regards to registry participation; and access to and privacy of personal health information. As electronic health records are becoming more common, understanding barriers and facilitators from the perspectives of people with dementia is essential to inform the future development of cognitive condition-related registries. The results from our focus groups identified engagement strategies and solutions to overcome perceived barriers for individuals experiencing progressive cognitive decline to participate in longitudinal registry projects.

This paper elaborates the importance of considering social and cultural factors within management responses to environmental change in coastal areas. The case study taken is Xuan Thuy National Park in Nam Dinh Province, Vietnam. This is a marginalised coastal area where rising sea levels, increasing storm surges and saltwater intrusion place pressure on coastal ecosystems, yet where communities continue to rely on these same ecosystems for agriculture- and aquaculture-related livelihoods. We interview stakeholders in Xuan Thuy National Park, connecting these with a narrative review of existing research into social and environmental change in the park to understand research gaps and challenges for vulnerable coastal areas like the Nam Dinh coast. Based on our findings, we suggest that whilst the effects of a changing environment on physical health and economic activity are increasingly well understood, effects on wellbeing and social relations can be even more immediate and profound in daily living. In turn, we argue environmental management has a crucial role to play not only for ecosystem-based adaptation, but also in sustaining wellbeing and allowing culturally meaningful practices to continue – especially in coastal regions where changes can be even more intense and immediate. However, we caution that whilst techno-scientific solutions grounded in environmental management do have significant potential in reducing impacts of extreme events and slower-onset environmental changes, they must not divert attention away from structural issues that can make some people or areas more vulnerable in the first instance.

Background The prevalence of anxiety and its association with sociodemographic and clinical factors is not well characterized in those with multiple sclerosis (MS). We aimed to estimate the prevalence and examine associated factors of anxiety in persons with MS. Methods A cross-sectional analysis was conducted utilizing data from 244 participants from the Neurological Disease and Depression study. Anxiety was assessed using the Hospital Anxiety and Depression Scale (HADS). Descriptive statistics and multiple logistic regression was used to examine anxiety and associated factors. Results Nearly 30.0% of participants had anxiety according to the HADS. The most prevalent symptom of anxiety was "worrying thoughts" (26.6%). After adjustment for various confounders, depression (OR: 7.31 95% CI 3.29–16.26) was found to be associated with higher odds of anxiety, while lower odds of anxiety were associated with higher education (OR: 0.51, 95% CI 0.28–0.94). Furthermore, anxiety was strongly associated with decreased quality of life. Conclusion Anxiety represents a substantial burden for those with MS and is associated with a variety of adverse outcomes including decreased quality of life. Our results further emphasize the importance of understanding the impact of anxiety in this population.

Amylin is produced in the pancreas and the brain, and acts centrally to reduce feeding and body weight. Recent data show that amylin can act in the ventral tegmental area (VTA) to reduce palatable food intake and promote negative energy balance, but the behavioral mechanisms by which these effects occur are not fully understood. The ability of VTA amylin signaling to reduce intake of specific palatable macronutrients (fat or carbohydrate) was tested in rats in several paradigms, including one-bottle acceptance tests, two-bottle choice tests, and a free-choice diet. Data show that VTA amylin receptor activation with the amylin receptor agonist salmon calcitonin (sCT) preferentially and potently reduces intake of fat, with more variable suppression of sucrose intake. Intake of a non-nutritive sweetener is also decreased by intra-VTA administration of sCT. As several feeding-related signals that act in the mesolimbic system also impact motivated behaviors besides feeding, we tested the hypothesis that the suppressive effects of amylin signaling in the VTA extend to other motivationally relevant stimuli. Results show that intra-VTA sCT reduces water intake in response to central administration of the dipsogenic peptide angiotensin II, but has no effect on ad libitum water intake in the absence of food. Importantly, open field and social interaction studies show that VTA amylin signaling does not produce anxiety-like behaviors. Collectively, these findings reveal a novel ability of VTA amylin receptor activation to alter palatable macronutrient intake, and also demonstrate a broader role of VTA amylin signaling for the control of motivated ingestive behaviors beyond feeding.

Background:Patients with neurologic conditions commonly have depression. Online tools have the potential to improve outcomes in these patients in an efficient and accessible manner. We aimed to identify evidence-informed online tools for patients with comorbid neurologic conditions and depression.Methods:A scoping review of online tools (free, publicly available, and not requiring a facilitator) for patients with depression and epilepsy, Parkinson disease (PD), multiple sclerosis (MS), traumatic brain injury (TBI), or migraine was conducted. MEDLINE, EMBASE, PsycINFO, Cochrane Database of Systematic Reviews, and Cochrane CENTRAL Register of Controlled Trials were searched from database inception to January 2017 for all 5 neurologic conditions. Gray literature using Google and Google Scholar as well as app stores for both Android and Apple devices were searched. Self-management or self-efficacy online tools were not included unless they were specifically targeted at depression and one of the neurologic conditions and met the other eligibility criteria.Results:Only 4 online tools were identified. Of these 4 tools, 2 were web-based self-management programs for patients with migraine or MS and depression. The other 2 were mobile apps for patients with PD or TBI and depression. No online tools were found for epilepsy.Conclusions:There are limited depression tools for people with neurologic conditions that are evidence-informed, publicly available, and free. Future research should focus on the development of high-quality, evidence-based online tools targeted at neurologic patients.

The beneficial glycemic and food intake-suppressive effects of glucagon-like peptide-1 (GLP-1) have made this neuroendocrine system a leading target for pharmacological approaches to the treatment of diabetes and obesity. One strategy to increase the activity of endogenous GLP-1 is to prevent the rapid degradation of the hormone by the enzyme dipeptidyl peptidase-IV (DPP-IV). However, despite the expression of both DPP-IV and GLP-1 in the brain, and the clear importance of central GLP-1 receptor (GLP-1R) signaling for glycemic and energy balance control, the metabolic effects of central inhibition of DPP-IV activity are unclear. To test whether hindbrain DPP-IV inhibition suppresses blood glucose, feeding, and body weight gain, the effects of 4(th) intracerebroventricular (ICV) administration of the FDA-approved DPP-IV inhibitor sitagliptin were evaluated. Results indicate that hindbrain delivery of sitagliptin improves glycemic control in a GLP-1R-dependent manner, suggesting that this effect is due at least in part to increased endogenous brainstem GLP-1 activity after sitagliptin administration. Furthermore, 4(th) ICV injection of sitagliptin reduced 24h body weight gain and energy intake, with a selective suppression of high-fat diet, but not chow, intake. These data reveal a novel role for hindbrain GLP-1R activation in glycemic control and also demonstrate that DPP-IV inhibition in the caudal brainstem promotes negative energy balance.

Background: Transitions of care are high-risk situations for the manifestation of medication discrepancies and, therefore, present threats for potential patient harm. Medication discrepancies can occur at any transition within the healthcare system. Methods: Fifth-year pharmacy students assessed a best possible medication list (BPML) during a medication review (based on medication history and patient interview) in community pharmacies. They documented all discrepancies between the BPML and the latest medication prescription. Discrepancies were classified using the medication discrepancy taxonomy (MedTax) classification system and were assessed for their potential clinical and economic impact. Results: Overall, 116 patients with a mean age and medication prescription of 74 (&plusmn, 10.3) years and 10.2 (&plusmn, 4.2), respectively, were analyzed. Of the 317 discrepancies identified, the most frequent type was related to strength and/or frequency and/or number of units of dosage form and/or the total daily dose. Although, the majority of discrepancies were rated as inconsequential (55.2%) on health conditions, the remainder posed a potential moderate (43.2%) or severe impact (1.6%). In 49.5% of the discrepancies, the current patients&rsquo, medication cost less than the prescribed. Conclusion: Community pharmacies are at a favorable place to identify discrepancies and to counsel patients. To improve patient care, they should systematically perform medication reconciliation whenever prescriptions are renewed or added.

Background The evaluation of quality of care in juvenile idiopathic arthritis (JIA) is critical for advancing patient outcomes but is not currently part of routine care across all centers in Canada. The study objective is to review the current landscape of JIA quality measures and use expert panel consensus to define key performance indicators (KPIs) that are important and feasible to collect for routine monitoring in JIA care in Canada. Methods Thirty-seven candidate KPIs identified from a systematic review were reviewed for inclusion by a working group including 3 pediatric rheumatologists. A shortlist of 14 KPIs was then assessed using a 3-round modified Delphi panel based on the RAND/UCLA Appropriateness Method. Ten panelists across Canada participated based on their expertise in JIA, quality measurement, or lived experience as a parent of a child with JIA. During rounds 1 and 3, panelists rated each KPI on a 1–9 Likert scale on themes of importance, feasibility, and priority. In round 2, panelists participated in a moderated in-person discussion that resulted in minor modifications to some KPIs. KPIs with median scores of ≥ 7 on all 3 questions without disagreement were included in the framework. Results Ten KPIs met the criteria for inclusion after round 3. Five KPIs addressed patient assessments: pain, joint count, functional status, global assessment of disease activity, and the clinical Juvenile Arthritis Disease Activity Score (cJADAS). Three KPIs examined access to care: wait times for consultation, access to pediatric rheumatologists within 1 year of diagnosis, and frequency of clinical follow-up. Safety was addressed through KPIs on tuberculous screening and laboratory monitoring. KPIs examining functional status using the Childhood Health Assessment Questionnaire (CHAQ), quality of life, uveitis, and patient satisfaction were excluded due to concerns about feasibility of measurement. Conclusions The proposed KPIs build upon existing KPIs and address important processes of care that should be measured to improve the quality of JIA care. The feasibility of capturing these measures will be tested in various data sources including the Understanding Childhood Arthritis Network (UCAN) studies. Subsequent work should focus on development of meaningful outcome KPIs to drive JIA quality improvement in Canada and beyond.
