Introduction: Fibrothecal group tumours are part of the gonadal stromal and sex cord tumours (GSST) and represent 1-4.7% of all organic ovarian tumours and are usually benign. We felt it important to report a case in order to share our experience in the management of fibrothecal tumours of the ovary. Observation: This was a 51-year-old postmenopausal patient who was referred to the hospital for abdominal pain in the context of an abdominal mass. A clinical examination and abdominal ultrasound concluded that the left ovarian tumour was benign. A left annecxectomy was performed and the histological examination concluded that the tumour was benign. No tumour marker was measured and no CT scan was performed. The postoperative course was simple and the patient was followed for 3 months.

The aim of the work was to describe the epidemiological, clinical and therapeutic aspects of peritonitis by gastroduodenal perforation. Patients and methods: the study was retrospective, and descriptive from January 2010 to December 2013.It was carried out in the general surgery department of the hospital of Sikasso (Mali). Patients operated for gastric and/or duodenal perforation were included. The parameters studied were frequency, age, sex, clinical aspects, and treatment, postoperative. Results: The clinical records of 54 patients were collected. Gastroduodenal perforations accounted for 9.2% of digestive perforations. The mean age of the patients was 34.8 years, with a sex ratio =9.8 at the risk of men. The notion of taking non-steroidal anti-inflammatory drugs was found in 72.2%, an ingestion of plant decoction in 18.5%, a history of gastritis in 7.4%, a history of peptic ulcer in 8%. Abdominal pain was present in all patients; vomiting in 19 patients. The average duration of evolution of the symptomatology was 36 hours. On physical examination the most frequent signs were abdominal contracture in 96.2% of patients; a disappearance of prehepatic dullness in all patients. On digital rectal examination Douglas-fir was bulging and painful in 24 patients. X-ray of the abdomen without preparation found pneumoperitonea in 77.7% of patients. An excision, suture plus omentoplasty were performed in 52 patients. Surgical follow-ups were simple in 77.9% patients, with a morbidity of 12.9% and a mortality of 9.3%. Conclusion: Peritonitis by gastroduodenal perforation is a frequent pathology in our context. It can cause a high mortality. Early diagnosis and treatment could improve outcomes.

Goiter is the most common endocrine pathology in the world [2]. Its medical treatment is based on hormone therapy, iodotherapy and its surgical treatment is based on thyroidectomy. Familial goiter appeared to be more common in the gotrigenic regions of Europe, rarer in North America. In Africa, particularly in Mali, there have been few studies on familial goiter. The purpose of this study was to report 4 cases of goiter in the same siblings with a history of goiter in the mother and paternal grandmother to review the literature.

Les fractures-décollements épiphysaires traumatiques de l’extrémité distale du fémur constituent des lésions rares [1] dont la gravité est liée aux séquelles invalidantes à type d'inégalité de longueur de membre ou le plus souvent de déviation axiale. Le traitement chirurgical vise à autoriser une mobilisation précoce du genou. La chirurgie effectuée essentiellement par la technique d’embrochage ou de vissage, à foyer ouvert ou mieux à foyer fermé peut être source de complications notamment la migration des broches, les infections, la raideur du genou, les troubles de la croissance. Le but de ce travail était de décrire leurs aspects épidémiologiques, anatomo-cliniques, thérapeutiques et évolutifs dans le service d’orthopédie et traumatologie de l’hôpital régional de Ségou qui est un hôpital de référence de la région en matière de pathologie orthopédique et traumatologique. Il s’agissait d’une étude observationnelle prospective sur 49 mois de mars 2015 à mars 2019. Elle avait concerné 32 patients présentant un décollement épiphysaire traumatique du fémur distal survenu dans un délai inférieur ou égal à 21 jours, sur un genou sain à cartilage fertile traité chirurgicalement et suivi dans le service. Le diagnostic de décollement épiphysaire traumatique du fémur distal avait été retenu grâce à l'examen clinique et complété par les clichés radiographiques du genou de face et de profil. Le traitement était chirurgical. Les résultats fonctionnels avaient été évalués selon les critères fonctionnels de la Société d’Orthopédie Traumatologie de l’Est (SOTEST). A ces critères, nous avons ajouté, l’amyotrophie. Elle a été mesurée de manière comparative par la circonférence de la cuisse à 10 cm au-dessus de la base de la patella. Trente-deux atients ont été colligés dont 24 garçons et 8 filles d’âge moyen de 12 ans avec des extrêmes de 8 ans et 16 ans. Les lésions classées selon de Salter Harris nous ont permis de noter 22% de type I (n=7), type II 72% (n=23), type III 3% (n=1), type IV 3% (n=1).Le traitement chirurgical consistait en la réduction associée à une ostéosynthèse : La fixation osseuse avait été réalisée par des broches de Kirschner de diamètre 22 en « X » dans 28 cas (type I n=7, type II n=21), par des vis spongieuses 6,5 dans 2 patients (type II n=1, Type III n=1), et par des broches de Kirschner et vis à os spongieux dans 2 cas (type II n=1, Type IV n=1). La consolidation avait été obtenue chez tous les patients dans un délai moyen de 6 semaines avec des extrêmes de 4 et 12 semaines. Le résultat fonctionnel était jugé bon dans 15% des cas et très bon dans 85%. Les fractures-décollements épiphysaires traumatiques de l’extrémité distale du fémur constituent des lésions rares dont la gravité est liée aux séquelles invalidantes à type d'inégalité de longueur de membre ou le plus souvent de déviation axiale. Le traitement chirurgical vise à reconstituer l’anatomie du fémur distal, à éviter les déplacements secondaires et à autoriser une mobilisation précoce du genou.

Appendicular mucocele is a rare appendicular tumor characterized by dilation of the appendicular lumen with linear changes in the mucosa and hypersecretion of mucus. We report a case of appendicular mucocele in a 73-year-old patient. Clinically, there was pain in the right iliac fossa, nausea, sometimes vomiting, and a poorly defined mass palpated in the right iliac fossa. The abdominal ultrasound found an appendicular plastron and the complete blood count showed leukocytosis. The patient underwent an appendectomy and the operative consequences were simple, the pathological examination of the operative specimen concluded that there was a mucinous cystadenoma of the appendix.

Clitoral cyst remains a common pathology in our regions given the persistence of traditional female genital mutilation practices. Cyst forms in the bed of clitoris carried entirely by excision. In our case the consultation was motivated by the size of the cyst, psychological trauma, discomfort and gravity caused by the tumor. Our gesture consisted of surgical exeresis with attempt to restore little lips. The operating procedures were simple and the result was satisfactory. Through this case we want to highlight the complications of excision and the treatment of clitoral scar cyst.

Moringa oleifera (LAM), arbre tropical à usages multiples, constitue de nos jours une nouvelle ressource alimentaire et économique pour les pays du Sud. Ces feuilles sont faciles d’accès et très riches en protéines, en vitamines et en minéraux. Elles sont de plus en plus utilisées dans des projets luttant contre la malnutrition. Cependant, le goût, la présentation et la qualité de ces produits demeurent une préoccupation majeure nécessitant des adaptations innovantes. L’objectif de cette étude était d’étudier la comprimabilité des poudres de feuilles de Moringa oleifera en améliorant le goût avec des matières premières locales et facilement accessibles comme la poudre de pulpe de Andansonia digitata. Les propriétés physico-chimiques et les tests pharmacotechniques ont permis de contrôler la qualité des poudres, des comprimés et orienter le choix du procédé de fabrication. Cinq (5) types de formulations (F1 à F5) ont été réalisés et les comprimés ont été fabriqués par compression après granulation par la voie sèche. Les comprimés des formulations F4 et F5 ont donné de meilleures propriétés pharmacotechniques selon les recommandations de la pharmacopée Européenne 6.0. L’association des deux poudres offre une alternative pour la fabrication de comprimés à croquer avec un goût acceptable.Mots-clés : Moringa oleifera, Adansonia digitata, poudre, comprimé, contrôle qualité.

19. INFECTION DU SITE OPÉRATOIRE (ISO) EN CHIRURGIE ORTHOPÉDIQUE A L'HOPITAL DE SIKASSO.

Introduction: The objective of this study was to determine the incidence of the operating site infection, precise the etiologies and identify the risk factors. Patients and method: This was a prospective study conducted at the department of orthopedics and traumatology at the hospital of Sikasso. The study was conducted from January 2016 to December 2018. The infection criteria were those of CDC from Atlanta. Result: This study was conducted on 140 patients including 105 men and 35 women with an average age of 40, 1 years old (3 and 88 years old). We have made 150 interventions including 131 osteosynthesis, 14 arthroplasties and 5 corrective osteotomies. The global incidence of the operating site infection was of 19, 3% with a 13, 4% incidence in surgery class I, 21, 4% in the class II, 26, 5% in the class III and 50% in the class IV. . It was superficial in 18 cases (62, 1%) and deep in 11 cases (37, 9%). The average post-surgery hospitalization duration was 13 days (5 and 70 days in extreme cases). The isolated germs were Staphylococcus aureus in 6 cases (37,5 %), Pseudomonas aeruginosa in 3 cases (18,7 %), Enterobacter cloacae in 3 cases (18,7%), Escherichia Coli in 2 cases (12,5 %,), Klebsiellapneumoniae in 1 case (6,3%) and Proteus mirabilis in 1 case (6,3%). They were 100% sensible to imipramine. 90% of the cases were resisting to ampicillin and penicillin G. Conclusion: The incidence of the ISO in our department was high. The risk factors of Operating Sites Infections were: hospitalization duration beyond 2 days, ASA Score over 2 days long, the surgical intervention class and the operation duration higher than 3 hours. Techniques are described for the continuous rearing of Busseola fusca (Fuller) (Lepidoptera: Noctuidae) on an artificial diet. The insect was reared for 20 successive generations at 25°C 85% RH and photoperiod 12:12 h. The larval stage lasted on average 27.6 days for males, and 28.5 days for females. The pupal stage lasted on average 13.4 days in both sexes, and mean total development time (egg to adult) was approximately 50 days. The mean pupation rate was 80%. Adult emergence rate was 74%, with a mean sex ratio of 1:0.9. Females laid an average of 514 eggs of which 52%, hatched. Additional experiments were conducted to develop techniques to improve oviposition by moths, and to manipulate larval, pupal and egg incubation times. Mating and hatching rates were similar with wooden-framed cages containing 35 males: 30 females and plastic, bottle cages with 4 males: 3 females used in the routine rearing. Pupation was delayed by 16 days when larvae were kept at 20°C instead of 25°C; adult emergence was delayed by 8-27 days when pupae were kept at 15°C instead of 25°C; and hatching of eggs was delayed by 3-7 days when eggs were kept at 15°C for 7-9 days instead of 25°C. Studies on larval diapause termination indicated that diapause could be manipulated to provide pupae and moths for use in the mass-rearing colony. The optimum pupation rate (70%) was obtained by watering diapause larvae daily for six days (compared to 40% pupation rate for larvae receiving no water), with pupae forming over a mean period of 12.3 days (compared to 43.9 days to pupation for larvae receiving no water).

27. A sustainable food support for non-breastfed infants: implementation and acceptability within a WHO mother-to-child HIV transmission prevention trial in Burkina Faso.

- Abstract
Objective: To provide HIV-positive mothers who opted for exclusive breastfeeding or formula feeding from birth to 6 months postpartum as a means of prevention of mother-to-child transmission (PMTCT) of HIV with a sustainable infant food support programme (FSP) from 6 to 12 months postpartum. We describe the implementation and assessment of this pilot initiative.Design: The FSP included a 6-month provision of locally produced infant fortified mix (IFM; 418 kJ/100 g of gruel) for non-breastfed infants coupled with infant-feeding and psychosocial counselling and support. Acceptability and feasibility were assessed in a subsample of sixty-eight mother-infant pairs.Setting: The FSP was developed in collaboration with local partners to support participants in a PMTCT prevention study. Formula was provided for free from 0 to 6 months postpartum. Cessation by 6 months was recommended for breastfeeding mothers.Results: The FSP was positively received and greatly encouraged breastfeeding mothers to cease by 6 months. As recommended, most infants were given milk as an additional replacement food, mainly formula subsidised by safety networks. Among daily IFM consumers, feeding practices were satisfactory overall; however, the IFM was shared within the family by more than one-third of the mothers. Cessation of IFM consumption was observed among twenty-two infants, seventeen of whom were fed milk and five neither of these.Conclusions: Without any food support most mothers would have been unable to provide appropriate replacement feeding. The food security of non-breastfed infants urgently needs to be addressed in HIV PMTCT programmes. - Abstract
Background: Food technology transfer to rural households, based on local ingredients, is a relevant and sustainable strategy to ensure better nutrition of young children. Objective. To develop an improved mush based on local ingredients and evaluate the potential for transferring its technology to rural housewives.Methods: We developed a flour-based food using Alicom software and performed laboratory trials to evaluate its actual nutritional quality. Then we recruited housewives from each of the 27 project villages and trained them in flour production and mush preparation twice daily, 6 days a week, for 26 weeks. Mush was sampled during the training session and at weeks 4, 12, and 22 and evaluated for actual flow distance and dry matter content, which served to estimate energy density and iron and zinc contents.Results: The laboratory trials reported average energy densities of 103 kcal/l00 g, iron contents of 2.6 mg/100 kcal, and zinc contents of 1.2 mg/100 kcal. The average (+/- SD) energy densities of the mush samples obtained during the training session and at weeks 4, 12, and 22 were 103.0 +/- 5.6, 103.3 +/- 5.2, 107.9 +/- 11.5, and 101.3 +/- 8.7 kcal/100 g, respectively. The average iron contents were 2.3 +/- 0.5, 2.3 +/- 0.5, 2.6 +/- 0.3, and 1.8 +/- 0.8 mg/ 100 kcal, respectively, and the average zinc contents were 1.6 +/- 0.1, 1.6 +/- 0.1, 1.7 +/- 0.1, and 1.6 +/- 0.2 mg/100 kcal.Conclusions: Developing a suitable complementary food from local ingredients and educating households in nutrition and use of local products are feasible. - Abstract
African animal trypanosomiasis (AAT) represents a significant challenge to livestock development in Africa. Reliable and practical techniques are required for the disease's early detection and management. One of the most commonly used tests for this purpose is the Enzyme-Linked Immunosorbent Assay (ELISA). This study sought to create a protocol for producing labelled antibodies from egg yolk. IgG was purified from serum from cattle that were hyperimmune to Trypanosoma brucei brucei before being used to immunise chickens. IgY antibodies were extracted from eggs, labelled with peroxidase, and tested for activity against commercial products. The results revealed that IgY levels were consistently higher than IgG levels, and the experimental antiserum had high diagnostic potential. We also calculated the ratios of commercial and individual egg yolk antisera. The findings allowed us to rank the diagnostic potential of the experimental antisera, with detection rates of 47.33 % for positive samples and 41.47 % for negative samples. Our results show that the experimental antiserum detects target antibodies with comparable accuracy and statistical significance (p < 0.001). Furthermore, the production method based on laying chickens proved to be simple, effective, and economical. This locally synthesised antiserum provides a viable alternative to expensive commercial options, paving the way for more widespread use in serodiagnosis. 32. IHR-PVS National Bridging Workshop in Cameroon: An interactive and participatory approach to engage stakeholders in the development of a One Health road map.

Introduction: Stakeholders involved in the implementation of the One Health (OH) welcome support for the operationalization of the approach and advice on how to address OH collaboration challenges. The IHR/PVS National Bridging Workshop (NBW) is an operational and outcome-oriented tool approach that allows animal health, human health and other relevant sectors to focus on their coordination. This paper describes how Cameroon leveraged on the NBW success factors to engage stakeholders in strengthening multisectoral collaboration., Methods: Stakeholder's engagement was implemented in two phases. Phase one consisted of engaging the multisectoral national task team for the preparation of the workshop. Phase two consisted of the bridging exercise itself during a three day workshop. The WOAH-WHO standardized IHR/PVS NBW toolkit was used throughout the workshop., Results: A total of 66 participants took part in the exercise. In total, 36% each came from human and animal health sectors with 23% and 5% from the environmental health and other sectors respectively. A total of 55% participants came from the national level and 39% from the regional level. The joint roadmap contained 55 activities and 13 objectives. Priority objectives were the establishment of a OH platform at all levels (62% of the vote) and building stakeholder's capacity on the OH approach (56% of the vote). A total of 67% of the activities required low or moderate cost and 87% would have a high impact on multisectoral collaboration., Conclusion: The NBW allowed consensus on operational activities to fill the gaps in coordination to build health security capacities. It enabled Cameroon to create a joint road map for enhanced multisectoral collaboration for health security. The output will be integrated in the National Action Plan for Health Security operational plan and support operational One Health activities. It would be crucial to develop global capacity assessment frameworks for environmental health, which could be included in the NBW, to incorporate interconnections with environmental sector. Postpartum family planning is the prevention of pregnancy during the 12 months following childbirth., Objective: To study the use of contraceptive methods in the postpartum period in the obstetrics gynecology department of the district hospital of the commune II of Bamako., Materials and Methods: We conducted a descriptive and analytical cross-sectional study with prospective data collection from January 1, 2019 to December 31, 2020. All women who had given birth who had chosen and benefited from a contraceptive method were included. The statistical test used was Fisher's test with a significance level set at 5%., Results: In 2 years, the contraceptive prevalence in the postpartum was 26.1%. More than 2/3 of counseling (61%) was done during prenatal consultations, 8% during the latency phase, 26% in the immediate postpartum and 5% during the postnatal visit. 34. Multidrug resistant bacteria isolated from nosocomial infections at University Teaching Hospital of Point-G, Bamako, Mali.

- Abstract
An infection is said to be nosocomial or hospital if it is absent when the patient enters the hospital and it appears and develops at least 48 h late. The objective of this study was to determine the resistance phenotypes of bacteria isolated from nosocomial infections at the University Teaching Hospital of Point G. Urine, blood, pus, skin and bronchoalveolar fluid samples were taken in different units, and bacteria isolations were performed on usual selective media such as Drigalski Colombia agar supplemented with nalidixic acid and colistin and 5% sheep blood and chocolate agar. Identifications of bacteria such as Enterobacteriaceae, Pseudomonas and acinetobacter, and Staphylococci were done using API20
E gallery, API20NE gallery and catalase/oxidase tests, and the Pastorex Staph kit respectively. The antimicrobial susceptibility testing was performed on Mueller-Hinton agar using the diffusion method. A total of 463 patients were inpatients for at least 48 h in the different units, and a nosocomial infection was notified in at least 57 patients (12.3%). A total of 65 episodes of nosocomial infections were observed in these 57 patients. Of the bacteria isolated, multidrug-resistant bacteria (MDR) represented 63.7% (n=36). These were extended-spectrum beta-lactamase (ESBL)-secreting Enterobacteriaceae (n=21), high-level cephalosporinase (n=13) and methicillin-resistant coagulase-negative Staphylococci (n=2). Despite this high number of multi-resistant bacteria isolated in this study; colistin and amikacin had very good activity on enterobacteriaceae. Background: This study aimed to investigate the effects of the supplementation of sesame flour in fermented chicken sausages ('S1' containing 800 g kg
-1 chicken fillet, 180 g kg-1 veal fat and 20 g kg-1 sesame flour and 'S2' containing 800 g kg-1 chicken fillet, 160 g kg-1 veal fat and 40 g kg-1 sesame flour) compared with control sausages (containing 800 g kg-1 chicken fillet and 200 g kg-1 veal fat) on the physico-chemical characteristics, texture, and structure during the fermentation stage., Results: The physicochemical parameters of samples belonging to the control, S1, and S2 batches were significantly affected by the addition of sesame flour and the fermentation stage. For instance: (i) the lowest protein content was observed for control samples on day 1 (61.4 ± 6.52 g kg-1 ) whereas the highest level was noted for S2 samples on day 15 (327.5 ± 22.2 g kg-1 ), and (ii) an inverse trend was observed for the fat content because the lowest content was observed for samples in the S2 batch on day 1 (129.0 ± 5.30 g kg-1 ) whereas the highest fat content was noted for samples belonging to control batch on day 15 (332.0 ± 1.29 g kg-1 ). The application of statistical methods to mid-infrared spectroscopy allowed clear discrimination between control, S1, and S2 batches. The addition of sesame flour in the recipes induced some modification in the secondary structure because β-turn levels ranged from 39.30 to 34.50, 36.76 to 34.70, and 38.93 to 34.70 for control, S1, and S2 batches, respectively, throughout the fermentation stage. Sodium dodecyl-sulfate polyacrylamide gel electrophoresis (SDS-PAGE) showed a similar protein profile pattern in the three batches on days 1 and 5, but on day 10 control and S2 batches showed the most intense degradation of myofibrillar proteins., Conclusion: The results demonstrated that mid-infrared spectroscopy coupled with chemometric tools could be used as a rapid screening tool to assess and monitor the quality of dry chicken sausages enriched with sesame flour throughout the fermentation stage. © 2022 Society of Chemical Industry., (© 2022 Society of Chemical Industry.)- Published

- Abstract
Introduction: Mastitis and abscess of the newborn breast are serious infections. The objective of our study was to describe the epidemiological and therapeutic aspects of these infections in our context., Materials and Methods: This was a retrospective study from January 2017 to December 2019. We included all children aged ≤2 months admitted for mastitis or breast abscess. The variables studied were epidemiological and therapeutic., Results: We collected 34 files, i.e., 11.3 cases/year. The sex ratio was 0.5. The mean age was 16.3 ± 6.6 days. The mean time to the consultation was 3.7 ± 2.4 days. The parents had undertaken a massage in 79.4% of cases. It was mastitis in 12 cases and abscess in 21 cases. Treatment was surgical in 26 cases., Conclusion: This affection is frequent with a female predominance. Its management must be early. The proscription of breast massage would reduce its frequency., Competing Interests: None

Introduction: Contraceptive prevalence is low in Mali. The information provided by the media men can help to increase or decrease the use of contraceptive methods., Objective: To study the knowledge, attitudes and practices of Bamako media Men on family planning., Material and Methods: We carried out a descriptive cross-sectional study with prospective data collection from June 1 to August 30, 2019. It concerned journalists and presenters of 15 radio and 6 televisions in Bamako., Results: During the 3 months, 615 media Men agreed to participate in this study. These are men and women from 36 to 45 years old in 37.2% of cases, married in 81.3% of cases. The radio with 85.5% was their main source of information on family planning. They all knew at least one contraceptive method. The best-known methods were pills (94.3%), injectables (57%) and implants (49.1%). They were in favor of the practice of family planning in 77.2% of cases, 76.9% had already used a method and 56.7% had already hosted a program on family planning., Conclusion: Media men are essential in the transmission of information. The quality of the information provided by these Media men can be influenced by their personal perceptions and attitudes. 41. Value of tube combined with real-time ultrasound-guided accurate interstitial high-dose-rate brachytherapy for post-operative pelvic side-wall recurrences of cervical cancer.

- Abstract
Purpose: Treatment of post-operative pelvic side-wall relapses of cervical cancer has always been a therapeutic challenge for radiation oncologists. Radiation dose boost to recurrent tumor by brachytherapy is necessary, but difficult to achieve. Our treatment center has successfully achieved precise transvaginal insertion of a pelvic side-wall mass, using a metal tube with real-time ultrasound guidance. This study investigates the efficacy and safety of image-guided high-dose-rate (HDR) interstitial brachytherapy (IBT) for patients with post-operative pelvic side-wall relapses., Material and Methods: Between 2018 and 2020, 36 post-operative pelvic side-wall relapses of cervical cancer patients receiving external beam radiotherapy (EBRT) combined with HDR-IBT were analyzed retrospectively. Doses per fraction ranged from 6.0 to 7.0 Gy, whereas cumulative equivalent doses in 2 Gy fractions ranged from 80 to 100 Gy. Effects of prognostic factors on local control (LC) and progression-free survival (PFS) were analyzed, and late toxicity data were evaluated., Results: A total of thirty-six patients were included, with a median follow-up of 19.3 months. The tumor response was obtained for all patients, with radiological complete remission in 20 (55.6%) patients. Two-years LC and PFS rates were 72.2% and 47.2%, respectively. Grade II rectal toxicity was observed in 5 patients (13.9%). Multivariate analyses for LC and PFS using proportional regression model were performed, in which shape of exophytic tumor was associated with a significantly better prognosis for both LC and PFS (HR = 0.071, 0.128, p = 0.005, 0.002). 90 remained associated with a significantly better prognosis for PFS (HR = 0.056, p = 0.000)., Conclusions: A metal tube placed in the vagina, under the guidance of real-time ultrasound in transvaginal IBT for pelvis masses can be accurately achieved. The shape of exophytic tumor and CTV D90 were associated with a significantly better prognosis for PFS, and the shape of exophytic tumor was also associated with a better prognosis for LC. Background: Plasmodium falciparum transmission depends on mature gametocytes that can be ingested by mosquitoes taking a blood meal on human skin. Although gametocyte skin sequestration has long been hypothesized as important contributor to efficient malaria transmission, this has never been formally tested., Methods: In naturally infected gametocyte carriers from Burkina Faso, we assessed infectivity to mosquitoes by direct skin feeding and membrane feeding. We directly quantified male and female gametocytes and asexual parasites in finger-prick and venous blood samples, skin biopsy samples, and in of mosquitoes that fed on venous blood or directly on skin. Gametocytes were visualized in skin tissue with confocal microscopy., Results: Although more mosquitoes became infected when feeding directly on skin then when feeding on venous blood (odds ratio, 2.01; 95% confidence interval, 1.21-3.33; P = .007), concentrations of gametocytes were not higher in the subdermal skin vasculature than in other blood compartments; only sparse gametocytes were observed in skin tissue., Discussion: Our data strongly suggest that there is no significant skin sequestration of P. falciparum gametocytes. Gametocyte densities in peripheral blood are thus informative for predicting onward transmission potential to mosquitoes and can be used to target and monitor malaria elimination initiatives., (© The Author(s) 2019. Introduction: Maternal mortality remains a public health problem in developing countries, including Mali. Among the factors associated with maternal mortality are sociodemographic and economicfactors, but also dysfunctions related to the health care system. It is in this context that this study was conducted in the gynecology and obstetrics department of the Nianankoro Fomba Hospital in Ségou to determine the factors associated with maternal deaths by the audit method in the Segouregion., Method: It was a cross-sectional, descriptive and retrospective study that was conducted from January 2014 to December 2015. The data were captured and analyzed on Epi Info version 2003., Results: The analysisincluded 41 auditeddeaths out of 79 maternaldeathsreportedduring the period, amaternalmortality rate of 51.90%. Direct obstetric causes accounted for the majority (51.23%). Of these causes hemorrhagewas the main with 17.08% followed by eclampsia (14.63%). 31.71% of deathswerepreventable. 44. [Operating site infection in the orthopedic surgery department at the hospital of Sikasso].

- Abstract
Introduction: The objective of this study was to determine the incidence of the operating site infection, precise the etiologies and identify the risk factors., Patients and Method: This was a prospective study conducted at the department of orthopedics and traumatology at the hospital of Sikasso. The study was conducted from January 2016 to December 2018. The infection criteria were those of CDC from Atlanta., Result: This study was conducted on 140 patients including 105 men and 35 women with an average age of 40, 1 years old (3 and 88 years old). We have made 150 interventions including 131 osteosynthesis, 14 arthroplasties and 5 corrective osteotomies. The global incidence of the operating site infection was of 19, 3% with a 13, 4% incidence in surgery class I, 21, 4% in the class II, 26, 5% in the class III and 50% in the class IV. . It was superficial in 18 cases (62, 1%) and deep in 11 cases (37, 9%). The average post-surgery hospitalization duration was 13 days (5 and 70 days in extreme cases). The isolated germs were Staphylococcus aureus in 6 cases (37,5 %), Pseudomonas aeruginosa in 3 cases (18,7 %), Enterobacter cloacae in 3 cases (18,7%), Escherichia Coli in 2 cases (12,5 %,), Klebsiellapneumoniae in 1 case (6,3% ) and Proteus mirabilis in 1 case (6,3%). They were 100% sensible to imipramine. 90% of the cases were resisting to ampicillin and penicillin G., Conclusion: The incidence of the ISO in our department was high. Objective: The objective of Our study was to describe the epidemiological, clinical, therapeutic and prognostic aspects of Nonunion of patella fractures., Patients Methods: It was a retrospective study done concerning 25 patients treated fromJanuary 2005 to December 2015 in the orthopedic trauma service CHU Bocar Sidy SALL Kati., Results: We gather together 25 cases of Nonunion of patella fractures disease among 28 caseswhich was about 89.28%. Our patients were divided into 20 men and 5 women, mean age was 36 years, ranging from ages 22 to 75 years. Posttraumatic period was 6-18 months. The initial fracture was neglected in 21 cases and treated in 4 cases. Physical examination revealed lameness in 92% of cases, atrophy of the quadriceps in 56% of cases, joint stiffness, 56% and an inter-fragmentary gap in 92% of patients. The radiographic data were consistent with pseudoarthrosis in all cases. The inter-fragmentary gap was 36 mm on average. The care of our patients was based on plugging and guy-wiring in 64 % of cases and strapping pre- and peri patella in 12% of cases. A partial patellectomy was practiced in 2 cases. A VY plasty of the quadriceps was recommended in 4 cases.The patients were reviewed after a mean of 10 months (8 months- 24 months). Functional outcomes analyzed using Bostman scores were excellent in 4 cases (16%), good in 18 cases (72%) and poor in 3 cases (12%)., Conclusion: Nonunion of the patella is mostly due to the lack of treatment or wrong treatment leads. Cyst scar post circumcision of the stump of the prepuce is pathology of which we have not found any cases in the literature. The circumcision traditional is still practiced in our regions with its attendant complications, in particular, the high section, the whole or part of the gland, infections. The case we report concerns a patient 24 years of age, having a large cyst of 8cm diameter, evolving for the past 17 years. The patient has not informed his parents that when the desire to marry has become very pressing. The consultation was motivated by the psychological trauma, the discomfort and the pain caused by the infection, and the fistulisation. The treatment was surgical with bedroom suites. Background: Worldwide, the number of emerging and re-emerging infectious diseases is increasing, highlighting the importance of global disease pathogen surveillance. Traditional population-based methods may fail to capture important events, particularly in settings with limited access to health care, such as urban informal settlements. In such environments, a mixture of surface water runoff and human feces containing pathogenic microorganisms could be used as a surveillance surrogate., Method: We conducted a temporal metagenomic analysis of urban sewage from Kibera, an urban informal settlement in Nairobi, Kenya, to detect and quantify bacterial and associated antimicrobial resistance (AMR) determinants, viral and parasitic pathogens. Data were examined in conjunction with data from ongoing clinical infectious disease surveillance., Results: A large variation of read abundances related to bacteria, viruses, and parasites of medical importance, as well as bacterial associated antimicrobial resistance genes over time were detected. Significant increased abundances were observed for a number of bacterial pathogens coinciding with higher abundances of AMR genes. Vibrio cholerae as well as rotavirus A, among other virus peaked in several weeks during the study period whereas Cryptosporidium spp. and Giardia spp, varied more over time., Conclusion: The metagenomic surveillance approach for monitoring circulating pathogens in sewage was able to detect putative pathogen and resistance loads in an urban informal settlement. Thus, valuable if generated in real time to serve as a comprehensive infectious disease agent surveillance system with the potential to guide disease prevention and treatment. - Abstract
In this paper, we present a mathematical model of malaria transmission dynamics with age structure for the vector population and a periodic biting rate of female anopheles mosquitoes. The human population is divided into two major categories: the most vulnerable called non-immune and the least vulnerable called semi-immune. By applying the theory of uniform persistence and the Floquet theory with comparison principle, we analyse the stability of the disease-free equilibrium and the behaviour of the model when the basic reproduction ratio [Formula: see text] is greater than one or less than one. At last, numerical simulations are carried out to illustrate our mathematical results.

49. [Major Causes Of Limb Amputations At The Hospital In Sikasso].

- Abstract
Introduction: In this study, we aimed to determine the major causes of limb amputations to improve its treatment., Patients and Methods: We conducted a-18 month prospective study from March 2015 to September 2016 at the Hospital of Sikasso, a hospital of second reference in Mali., Results: We carried out 50 major limb amputations including 25 men and 25 women aged 38.9 years old on average with extremes of 3 and 80 years old. The amputed limb was the upperlimb in 38 cases (76%) and the lowerlimb in 12 cases (24%). Housewives were the most represented with 18 cases (36%), followed by farmers with 15 cases (30%). The amputation was done immediately or in delayed emergency in 32 cases (54%). Trauma was the most frequent etiology with 13 cases (26%). The post-operative complications have been met in 47 cases. They have been dominated by stump pain and delayed healing in 23 cases (49%) each, suppuration in 10 cases (21%) and the stump necrosis in 3 cases (7%). The mortality rate was 8% (4 cases). We have made prosthetic apparatus for18 patients (36%)., Conclusion: The limb amputation is frequent and youths were the most involved. Adding amylase to fortified blended foods can improve energy density, and increase child's energy and nutrient intake. The efficacy of this strategy is unknown for the World Food Programme's Super Cereal Plus (SC+) and Super Cereal (SC) blends. The primary goal of this study was to investigate the increased energy intake from amylase-containing SC+ and SC compared to control porridges in Burkinabe children. Secondly, energy intake from amylase-containing porridges compared to CERELAC
® , Vitazom, and eeZeeBAR™ was studied. Thirdly, caregivers' (n = 100) porridge acceptability was investigated. The design was a randomized double-blind controlled cross-over trial studying the effect of amylase addition to SC+ and SC flours on porridge energy and nutrient intake in healthy Burkinabe children aged 12-23 (n = 80) and 24-35 months (n = 40). Amylase added to porridges increased energy density from 0.68 to 1.16 kcal/g for SC+ and from 0.66 to 1.03 kcal/g for SC porridges. Among children aged 12-23 months, mean energy intake from all porridges with amylase (135-164 kcal/meal) was significantly higher compared to control SC+ porridges (84-98 kcal/meal; model-based average). Among children aged 24-35 months, mean energy intakes were also significantly higher from all porridges with amylase added (245-288 kcal/meal) compared to control SC porridges (175-183 kcal/meal). Acceptability of the porridges among caregivers was rated neutral to good, both for amylase-added and non-amylase-containing porridges. These findings suggest that, among 12-35-month-old, adding amylase to fortified blended foods significantly increased energy and consequently nutrient intake per meal by 67% for SC+ and 47% for SC.
