9. Multimodal brain imaging and vascular risk factors in classification of people with Alzheimer's dementia

Abstract

Background: Alzheimer's Disease (AD) is the most common cause of dementia in older people. To date, the gold standard for AD diagnosis is neuropathological examination, which is impractical and using stringent diagnostic criteria are the only viable option for AD diagnosis, resulting inoverlap of different pathologies in people diagnosed with AD. This study is an attempt to classify people with AD through visual interpretation of [18F] fluorodeoxy glucose positron emission tomography (FDG PET) images and to identify and differentiate typical AD from mixed subtype. Material and Methods: The current study is based on FDG PET data collected as part of two large scale Phase 3 clinical trials by Tau Therapeutics (TauRx) pharmaceuticals of a novel tau aggregation inhibitor medication, LMTXÒ in 970 AD participants. All available FDG PET images were visually classified into typical AD and mixed subtype using PMOD Alzheimer's Discrimination Tool (PALZ) and further confirmed using quantitative analysis and automated transfer learning method. Results: Participants with mixed subtype were younger and had more severe disease in comparison to those with typical AD subtype. Hypertension was significantly correlated with typical AD and White Matter Hyperintensities (WMH) were significantly associated with mixed subtype, when compared with mixed subtype and age-matched controls respectively. Left cortical hypo-metabolism and right hypo-metabolism in the cerebellum, typical of crossed cerebellar diaschisis (CCD).

Abstract

The sol gel method was used in preparing a series of A site partially substituted La1−xBaxCoO3 (x ≥ 0.1 ≤ 0.4) perovskite catalysts coded LBC1, 2, 3, and 4 and their potential as catalysts for soot oxidation were evaluated. The Brunauer–Emmett–Teller (BET), Inductively Coupled Plasma Atomic Emission Spectroscopy (ICPAES), Thermogravimetric/Differential Thermal Analysis (TGA/DTG), X-ray analysis (XRD) were used in characterizing the prepared perovskite catalyst. The result shows that at (x≥ 0.2 ≤ 0.4), there was an increase in surface area when we compare it with that of x = 0. The increase in surface area helps in increasing the catalytic performance of the catalyst. Also, when evaluating the catalytic performance of the synthesized catalysts, it was observed that doping the perovskite catalysts helped in the general improvement of the catalytic performance for soot oxidation. The best performance in this research study with a T50 of 484 °C was observed at x = 0.2 catalyst (LBC2). This shows that the non-noble perovskite catalysts prepared in this research study has the potential to replace the noble metal based catalysts used presently in the diesel automotive industry.

Abstract

The ingestion of foreign body is a very common occurrence in pediatric age group. The retrieval is an emergency procedure performed mostly by the Otolaryngologist and Gastroenterologist. We present a case of coin ingestion who reported to a tertiary care center. It was retained in the lower part of esophagus as confirmed by imaging. Subsequently during the rigid esophagoscopy it was found missing from distal esophagus. A 9 year old female child presented with accidental ingestion of coin. The imaging done immediately prior to intervention confirmed position of coin in distal esophagus. However, up on visualization with rigid esophagoscope the coin was not found in esophagus and suspected to have passed in to stomach. We used C-arm to confirm the location which has its own limitations. There have been various case reports of using c-arm for tracing lost needles and broken instruments by oro-maxillo-facial surgeons. There are no reports where it has been used to aid foreign body removal from gastro-intestinal tract. This is first case report where the use has been improvised due to lack of advanced facilities available at better and well equipped centers.The retrieval of foreign body from stomach aided by c-arm has not been reported in literature till now. The case report would add to the knowledge, options and practice at a center which does not have any super speciality facilities. [ABSTRACT FROM AUTHOR]

40. Problems and expectations of caregivers of patients with dementia

Abstract

Background: Patients with dementia (PwD) are primarily cared by a family member or their relatives in India. In managing these patients they often face a number of problems during care and sometimes they do not have any knowledge or understanding of the problems, so problems remain unattended. Objectives: Present study aimed to find out the problems faced by caregivers of dementia patients and management strategies evolved by themselves vis a vis training needs. Methods: The primary caregivers to patients with dementia admitted to the Department of Geriatric Mental Health giving consent were interviewed using a semi-structured proforma. Information regarding sociodemographic details, caregiver tasks, problems in performing care giving tasks and their own management techniques vis a vis need for training in managing the patients were gathered. Results & Discussions: A total of 21 caregivers of patients with dementia consented to participating in the study. They reported that they often confronted with a variety of problems in managing activities of daily living (ADL), and behavioral and psychological/emotional spheres of the patients during care giving. Managing toileting, bathing, feeding and dressing were the main problem areas. Caregivers depended on the management strategies evolved by themselves for providing support and managing day to day difficulties faced during care giving. The majority of the caregivers acknowledged the need for structured skills training for managing patients with dementia.

Abstract

Alzheimer's disease (AD) is the most common cause of dementia and accounts for approximately 50% to 80% of all cases of dementia. The diagnosis of probable AD is based on clinical criteria and overlapping clinical features pose a challenge to accurate diagnosis. However, neuroimaging has been included as a biomarker in various published criteria for the diagnosis of probable AD, in the absence of a confirmatory diagnostic test during life. Advances in neuroimaging techniques and their inclusion in diagnostic and research criteria for the diagnosis of AD includes the use of positron emission tomography (PET) imaging as a biomarker in various therapeutic and prognostic studies in AD. The development and application of a range of PET tracers will allow more detailed assessment of people with AD and will improve diagnostic specificity and targeted therapy of AD. The aim of this review is to summarize current evidence on PET imaging using the non-specific tracer [18F]fluorodeoxyglucose and specific tracers that target amyloid and tau pathology in people with AD.
