BACKGROUND: Understanding environmental correlates of sedentary behaviour (SB) among young people is important as such data can identify approaches to limit sedentary time. This paper estimates associations of parent-reported neighbourhood and adolescent-reported home environments with SB among adolescents aged 11-19 years from 14 countries. METHODS: In the International Physical activity and the Environment Network (IPEN) Adolescent Study (an observational, cross-sectional multi-country study), adolescents wore a triaxial accelerometer for seven days that assessed sedentary time (ST). Adolescents completed survey measures of sedentary behaviour (SB) related to recreational screen time and sitting time in motor vehicles. Parents and adolescents completed surveys assessing neighbourhood and home environments. Accelerometer based ST was available in 3,982 adolescents while survey data were available for 6,302 dyads. We estimated the total and direct effects of each environmental attribute on ST and SB. Sex of the adolescent and city/country were examined as moderators. RESULTS: The average ST in adolescents from 14 countries ranged from 7.8 to 10.5 h/day. Personal social media was the only significant correlate of total ST across both sexes. With respect to self-reported SB, adolescents accumulated an average of 3.8 h of non-school screen time per day and nearly 40 min of transport-related sitting time. Screen time was associated with all home environment variables, including social media account, as well as land use mix-diversity, traffic safety, and crime safety. Transport-related sitting time was related to land use mix-diversity, recreation facilities, walking facilities, and pedestrian infrastructure, but no home environment variables. City/country and sex were significant moderators of several associations. CONCLUSIONS: Both home and neighbourhood environment features were related to ST and SB. Having social media accounts emerged as a major contributor towards sedentarism in adolescents.

Background and purpose: Documentation of new-generation implants’ short-term performance could reassure surgeons and patients about their use, while awaiting the long-term outcome. Our aim was to compare the performance of a newer total knee arthroplasty (TKA) system with its predecessor, measured by patient-reported knee function, pain, and complication rate. Methods: We performed a multi-center, randomized, controlled trial (clinicaltrials.gov ID: NCT03073941). 314 patients with primary osteoarthritis were randomized to treatment with a Persona or NexGen cruciate-retaining TKA system and followed for 2 years. The primary outcome was measured with the patient-reported outcome (PRO) Oxford Knee Score (OKS) 2 years post-surgery. Secondary outcomes were the OKS-Activity and Participation questionnaire (OKS-APQ), Forgotten Joint Score (FJS), EQ-5D-3L, and number of complications during the study period. Responder analyses were performed using Patient Acceptable Symptom State (PASS) and Minimal Important Change (MIC) criteria. Results: Primary outcome was available from 289 patients (92%). We found no difference in adjusted mean OKS between the groups 2 years post-surgery (0.1, 95% confidence interval –1.4 to 1.7). We found no significant differences in adjusted mean of secondary PROs, PRO time-weighted averages, proportion of patients with PASS or MIC, or complications 2 years post-surgery. Conclusion: We found no difference in OKS 2 years post-surgery, or in any secondary variables analyzed including complications, between the 2 TKA systems. Background The widespread ownership of digital devices among adolescents presents challenges and opportunities in schools, particularly during recess. This study aimed to examine the association of screen use during recess with physical activity behaviours.Methods The study was based on a population-based cross-sectional study including 1347 adolescents from 28 schools (2017–2018). Physical activity behaviours were assessed using thigh-attached accelerometers, worn 24/7 for up to 6 weekdays and 2 weekend days. We examined accelerometer data for all recess periods, aligned with each adolescent’s school schedule, along with leisure-time activity from the same days. Physical behaviours were classified during recess and leisure-time behaviour (negative control). Frequency of screen use during recess was based on self-report using a 5-point ordinal scale.Results Greater frequency of screen use during recess was associated with less time engaged in physically active behaviours and more time spent sitting, consistent with dose-dependent associations. In multivariable-adjusted analysis, adolescents with no screen use during recess spent an absolute 11.1% (95% CI 5.4 to 6.8) more of their recess time being physically active compared to those with frequent use. Based on the mean recess duration observed in the sample, adolescents not using screens during recess engaged in physical activities for an average of 44.9 min per day (95% CI 42.3 to 47.6), compared with 35.1 min (95% CI 26.0 to 44.3) for frequent screen users. Leisure time activities, used as a negative control, showed no link to screen use during recess.Conclusion Increased screen use during recess was associated with lower physical activity levels. These findings suggest that regulating digital device use during recess could enhance physical activity among adolescents.

Background and purpose: In contemporary medial unicompartmental knee arthroplasty (mUKA), non-lateral patellofemoral osteoarthritis (PFOA) is not considered a contraindication. However, we still lack knowledge on the association of PFOA severity on patient reported outcome measures (PROMs) after mUKA. We aimed to examine the association between PFOA severity and PROM-score changes after mUKA. Methods: We included 549 mobile-bearing mUKAs. PFOA was graded intraoperatively as 0 = normal cartilage, 1–2 = superficial changes or < 50% of depth, and 3–4 = changes of > 50% of depth or to the bone, using the International Cartilage Repair Society (ICRS) cartilage lesion classification system. All patients completed the Oxford Knee Score (OKS), Activity and Participation Questionnaire (APQ), and Forgotten Joint Score (FJS), preoperatively and 3, 12, and 24 months postoperatively. PROM changes were compared using linear regression models adjusted for sex, age, body mass index, and preoperative PROM score. Results: We found no significant differences in OKS, FJS, and APQ change when comparing group 3–4 with group 0 at any follow-up. When comparing group 1–2 with 0 we found a statistical but not clinical significantly higher change in OKS scores at 24-month follow-up (2.5, 95% confidence interval [CI] 0.36–4.6) and in APQ scores at 24-month follow-up (10.6, CI 1.2–20.0) in favor of group 1–2. Conclusion: Severe PFOA, excluding severe lateral facet PFOA, had no negative association on PROM score development following mobile-bearing mUKA.

Some international reports, however, have advocated one-stage revision. No systematic review or meta-analysis has ever compared the risk of reinfection following one-stage and two-stage revisions for chronic infection in hip arthroplasties.Methods: The review was performed in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analysis. Relevant studies were identified using PubMed and Embase. We assessed studies that included patients with a chronic infection of a hip arthroplasty treated with either one-stage or two-stage revision and with available data on occurrence of reinfections. We performed a meta-analysis estimating absolute risk of reinfection using a random-effects model.Results: We identified 36 studies eligible for inclusion. None were randomized controlled trials or comparative studies. The patients in these studies had received either one-stage revision (n = 375) or two-stage revision (n = 929). Reinfection occurred with an estimated absolute risk of 13.1% (95% confidence interval: 10.0%–17.1%) in the one-stage cohort and 10.4% (95% confidence interval: 8.5%–12.7%) in the two-stage cohort. The methodological quality of most included studies was considered low, with insufficient data to evaluate confounding factors.Conclusions: Our results may indicate three additional reinfections per 100 reimplanted patients when performing a one-stage versus two-stage revision. However, the risk estimates were statistically imprecise and the quality of underlying data low, demonstrating the lack of clear evidence that two-stage revision is superior to one-stage revision among patients with chronically infected hip arthroplasties. This systematic review underscores the need for improvement in reporting and collection of high-quality data and for large comparative prospective studies on this issue.Keywords: infection, arthroplasty, hip replacement, one-stage, two-stage, reoperation

Uncertainty exists regarding whether including ICD-8 codes alongside ICD-10 is needed for complete disease identification. We assessed the extent of left-truncation and left-censoring in the DNPR arising from omitting ICD-8 codes.Patients and Methods: We sampled 500,000 Danes ≥ 40 years of age in 1995, 2010, and 2018. From the DNPR, we identified cardiovascular, endocrine, gastrointestinal, neurological, pulmonary, rheumatic, and urogenital diseases as well as fractures. We obtained the number of people with a disease recorded with ICD-8 codes only (ie, the ICD-8 record would be left-truncated by not using ICD-8 codes), ICD-8 plus ICD-10 codes (ie, the ICD-8 record would be left-censored by not using ICD-8 codes), and ICD-10 codes only. For each ICD group, we calculated the proportion of people with the disease relative to the total sample (ie, 500,000 people) and the total number of people with the disease across all ICD groups.Results: Overall, the left-truncation issue decreased over the years. Relative to all people with a disease, the left-truncated proportion was for example 59% in 1995 and < 2% in 2018 for diabetes mellitus; 93% in 1995, and 54% in 2018 for appendicitis. The left-truncation issue increased with age group for most diseases. The proportion of disease records left-censored by not using ICD-8 codes was generally low but highest for chronic diseases.Conclusion: The left-truncation issue diminished over sample years, particularly for chronic diseases, yet remained rather high for selected surgical diseases. The left-truncation issue increased with age group for most diseases. Centers for Teaching and Learning (CTLs) are at the forefront of many quality enhancement activities aimed at teaching and learning at universities and do not solely focus on supporting individual teacher in conducting quality teaching but are also playing a strategic role in the university. CTLs provide in other words holistic academic development. This article provides examples of how a CTL at the University of Southern Denmark has operationalised holistic academic development using Holt et al.'s points of leverage as a starting point. The leverage points are combined and exemplified to suggest a model for enhancing teaching quality which entails five levels; definitions, descriptions, documentation, evaluation and recognition of quality in teaching.

Objectives: A new school policy mandating 45 min physical activity daily during school was introduced in Denmark in 2014. We aimed to evaluate the effect of this policy on BMI in school-aged children. It was hypothesized that the school policy would decrease BMI, especially in the obese fraction of the population (90th percentile BMI). Study design: This register-based study was conducted as a natural experiment. Methods: Analyses were based on data from The National Child Health Register that contains nationwide data on height and weight from mandatory preventive health examinations completed by school nurses or medical doctors during pre-preparatory classes (0th-3rd grade) and lower secondary education (7th-9th grade). A total of 401,517 children were included in the analyses with annual repeated cross-sectional data covering the period from 2012 to 2018. The effect of the school policy was evaluated using an interrupted time series approach comparing pre- and post-policy slopes in BMI, stratified by sex and age-group. Results: In boys, no significant differences were observed in mean BMI slopes from pre-to post-policy in either age-group. In girls, post-policy slopes were significantly higher compared to pre-policy in both age-groups (0th-3rd grade: β:0·034 kg/m2, 95%-CI: (0·024; 0·043), p-value:

Aims: The aim of this study was to describe the pattern of revision indications for unicompartmental knee arthroplasty (UKA) and total knee arthroplasty (TKA) and any change to this pattern for UKA patients over the last 20 years, and to investigate potential associations to changes in surgical practice over time. Methods: All primary knee arthroplasty surgeries performed due to primary osteoarthritis and their revisions reported to the Danish Knee Arthroplasty Register from 1997 to 2017 were included. Complex surgeries were excluded. The data was linked to the National Patient Register and the Civil Registration System for comorbidity, mortality, and emigration status. TKAs were propensity score matched 4:1 to UKAs. Revision risks were compared using competing risk Cox proportional hazard regression with a shared γ frailty component. Results: Aseptic loosening (loosening) was the most common revision indication for both UKA (26.7%) and TKA (29.5%). Pain and disease progression accounted for 54.6% of the remaining UKA revisions. Infections and instability accounted for 56.1% of the remaining TKA revision. The incidence of revision due to loosening or pain decreased over the last decade, being the second and third least common indications in 2017. There was a decrease associated with fixation method for pain (hazard ratio (HR) 0.40; 95% confidence interval (CI) 0.17 to 0.94) and loosening (HR 0.29; 95% CI 0.10 to 0.81) for cementless compared to cemented, and units UKA usage for pain (HR 0.67, 95% CI 0.50 to 0.91), and loosening (HR 0.51; 95% CI 0.37 to 0.70) for high usage. Conclusion: The overall revision patterns for UKA and TKA for the last 20 years are comparable to previous published patterns. We found large changes to UKA revision patterns in the last decade, and with the current surgical practice, revision due to pain or loosening are significantly less likely. The Better Health in Late Life cohort study aims to improve our understanding of the risks and outcomes of multimorbidity in the Danish population.Methods: A randomly-selected sample of Danish residents who were 50– 65 years of age received a questionnaire and an invitation to participate in this study. Respondents completed an online survey between October 2021 and January 2022 which addressed topics that included self-assessed health, mental health, sleep, specific medical conditions, use of painkillers, diet, alcohol consumption, smoking, physical activity, and body composition. This information was linked to the Danish health and social registries (some established in 1943 and onwards) that maintain data on filled prescriptions, hospital records, socioeconomic status, and health care utilization.Results: Responses were received from 115,431 of the 301,244 residents invited to participate (38%). We excluded respondents who answered none of the questions as well as those who provided no information on sex or indicated an age other than 50– 65 years. Of the 114,283 eligible respondents, 54.8% were female, 30.3% were overweight, and 16.7% were obese. Most participants reported a weekly alcohol consumption of less than seven units and 13.3% were current smokers; 5.2% had a history of hospitalization for solid cancer, and 3.0%, 2.3%, 2.0%, and 0.9% reported chronic pulmonary disease, diabetes, stroke, and myocardial infarction, respectively. The most frequently filled prescriptions were for medications used to treat the nervous system and cardiovascular diseases (38.1% and 37.4%, respectively).Keywords: aging, epidemiology, health registries, life course epidemiology, multimorbidity, prospective cohort

In electrochemotherapy, permeabilization of the cell membrane by electric pulses increases the anti-tumour effect of chemotherapeutics. In calcium electroporation, chemotherapy is replaced by calcium chloride with obvious benefits. This study explores the effect and underlying mechanisms of calcium electroporation on basal cell carcinomas using either high- or low-frequency electroporation. Low-risk primary basal cell carcinomas were treated in local anaesthesia with intratumoral calcium chloride followed by electroporation with high (167 kHz) or low (5 kHz) frequencies. Non-complete responders were retreated after 3 months. The primary endpoint was tumour response 3 months after last calcium electroporation. Plasma membrane calcium ATPase was examined in various cell lines as plasma membrane calcium ATPase levels have been associated with calcium electroporation efficacy. Twenty-two out of 25 included patients complete the study and 7 of these (32%) achieved complete response at 3 months with no difference in efficacy between high- and low-frequency pulses. High-frequency calcium electroporation was significantly less painful (p=0.03). Plasma membrane calcium ATPase was increased 16–32-fold in basal cell carcinoma cell lines compared with 4 other cancer cell lines. Calcium electroporation for low-risk basal cell carcinomas does not fulfil the requirements of a new dermatological basal cell carcinoma treatment but may be useful as adjuvant treatment to surgery in more advanced basal cell carcinomas. The elevated PMCA levels in basal cell carcinomas may contribute to low efficacy.

Abstract Det fysiske læringsrum forstået som den bevidste brug af undervisninglokalet har med god ret fået mere og mere opmærksomhed de seneste år som et redskab til at understøtte læringen. Denne guide gennemgår indledningsvist med hvilke begrundelser interessen for læringsrummet som et redskab er fremkommet. Herefter beskrives med udgangspunkt i begrebet didaktisk rumlig kompetence råd, anvisninger og illustrationer for, hvordan brugen af det fysiske læringsrum kan gribes an. Slutteligt peges der på udviklingsmuligheder, både for at engagere sig i nyudvikling af læringsrum på et instutionelt plan og for at udvikle sine egne didaktisk rumlige kompetencer som underviser.

Background and purpose: Revisions due to periprosthetic joint infection (PJI) are underestimated in national arthroplasty registries. Our primary objective was to assess the validity in the Danish Knee Arthroplasty Register (DKR) of revisions performed due to PJI against the Healthcare-Associated Infections Database (HAIBA). The secondary aim was to describe the cumulative incidences of revision due to PJI within 1 year of primary total knee arthroplasty (TKA) according to the DKR, HAIBA, and DKR/HAIBA combined. Methods: This longitudinal observational cohort study included 56,305 primary TKAs (2010–2018), reported in both the DKR and HAIBA. In the DKR, revision performed due to PJI was based on pre- and intraoperative assessment disclosed by the surgeon immediately after surgery. In HAIBA, PJI was identified from knee-related revision procedures coinciding with 2 biopsies with identical microbiological pathogens. We calculated the sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) of revision due to PJI in the DKR (vs. HAIBA, within 1 year of TKA) with 95% confidence intervals (CI). Cumulative incidences were calculated using the Kaplan–Meier method. Results: The DKR‘s sensitivity for PJI revision was 58% (CI 53–62) and varied by TKA year (41%–68%) and prosthetic type (31% for monoblock; 63% for modular). The specificity was 99.8% (CI 99.7–99.8), PPV 64% (CI 62–72), and NPV 99.6% (CI 99.6–99.7). 80% of PJI cases not captured by the DKR were caused by non-reporting rather than misclassification. 33% of PJI cases in the DKR or HAIBA were culture-negative. Considering potential misclassifications, the best-case sensitivity was 64%. The cumulative incidences of PJI were 0.8% in the DKR, 0.9% in HAIBA, and 1.1% when combining data. Conclusion: The sensitivity of revision due to PJI in the DKR was 58%. The cumulative incidence of PJI within 1 year after TKA was highest (1.1%) when combining the DKR and HAIBA, showing that incorporating microbiology data into arthroplasty registries can enhance PJI validity.
